    Obsessive Compulsive Inventory-Child Version (OCI-CV) in a Spanish community sample of children and adolescents

    There is a growing interest in designing instruments to assess obsessive-compulsive symptoms in children. The Obsessive- Compulsive Inventory-Child Version (OCI-CV) has showed to be a valid in the evaluation of OCD in clinical and nonclinical populations. The main goal in this study was to analyze factor structure and psychometric properties of the OCI-CV in a community Spanish sample. Method: Data were collected from 914 children/adolescents with a mean age of 13.01 (SD = 1.96; Males = 51.3%). Exploratory factor analysis was carried out in order to study the internal structure of the OCI-CV Spanish version. Further, internal consistency, test-retest reliability, and convergent and discriminant validity of the total score and the factors obtained were examined. Finally, age and gender differences were also explored. Results: Exploratory factor analysis yielded a similar structure to the original OCI-CV with the following six factors: Washing/Checking, Obsession, Ordering, Doubting, Neutralizing, and Hoarding. The internal consistency was strong for the total score, but moderate for the subscales. The Spanish version of the OCI-CV showed evidences of test-retest reliability and convergent and discriminant validity. Conclusion: The Spanish version of the OCI-CV is an instrument with adequate psychometric properties to assess obsessions and compulsions in Spanish children/adolescents.Psicologí

    Manejo quirúrgico de frenillo superior: reporte de caso

    Usualmente se observa frenillos patológicos en diversos pacientes de grupos de edades distintos. La presencia de estos frenillos con una inserción inadecuada  van ha conllevar principalmente a problemas fonéticos; así como también estéticos; compromiso en la oclusión, razón por la cual es sumamente importante valorar la historia clínica del paciente para  su manejo clínico-quirúrgico adecuado a aplicar según la elección del profesional luego de la valoración de la historia clínica.Se reporta un caso clínico de un paciente de sexo masculino de 7 años de edad, que asiste a consulta con su apoderado, manifestando “problemas al hablar”. Se realizó  frenectomía superior con la finalidad de corregir el frenillo patológico. Realizamos controles post operatorio los cuales finalizaron en resultados estéticos y funcionales satisfactorios para el paciente

    Síntomas psicopatológicos en adolescentes españoles: relación con los estilos parentales percibidos y la autoestima

    The purpose of this research was to study the presence of psychopathologic symptoms in a community sample of 935 Spanish adolescents, and its relationship with perceived parental styles and the self-esteem. Numbers of males and females were 489 and 446, respectively. Age ranged from 12 to 18 years (M = 14.84, SD = 1.83). Fourteen High School centers in Murcia and Castilla-La Mancha agreed to participate in the current study. Five of them were randomly selected to collected data. The results showed age-related differences in all variables (the psychopathologic symptoms, self-esteem and perceived parental styles). On the other hand, females showed less psychopathologic symptoms and better perception of parental styles than males. Significant negative correlations were found between self-esteem, psychopathology and some positive dimensions of parental style. The main predictors of adolescent’s mental health were: self-esteem, age, gender, revelation and affect from father and psychological control of mother.El objetivo de esta investigación fue analizar la sintomatología psicopatológica evaluada con el SCL-90-R en una muestra comunitaria de 935 adolescentes españoles y su interacción con el estilo parental percibido y la autoestima. La muestra estuvo compuesta por 489 chicos y 446 chicas de edades comprendidas entre los 12 y 18 años (M = 14.84, DT = 1.83) que cursaban estudios de Educación Secundaria y Bachillerato. Catorce centros de la Región de Murcia y Castilla-La Mancha aceptaron participar en el estudio. De éstos, fueron seleccionados al azar siete teniendo en cuenta la titularidad (pública y concertada). Los resultados indicaron que la edad influía en todas las variables medidas, presentando los adolescentes de mayor edad medias más altas en síntomas psicopatológicos, menor autoestima y peor percepción de los estilos educativos parentales. Las mujeres presentaron menos niveles de sintomatología y mejor percepción de los estilos educativos. Los resultados de los análisis correlacionales confirmaron relaciones significativas inversas entre autoestima y síntomas psicopatológicos y entre algunas dimensiones positivas de los estilos educativos. Los predictores más relevantes y de mayor peso en la salud psicológica fueron: autoestima, edad, sexo, revelación y afecto del padre y control psicológico de la madre

    Evidence of cross gene regulation of some virulence factors of Porphyromonas gingivalis by Streptococcus intermedius

    Periodontal disease has been associated with poor dental care, which promotes the accumulation of bacteria and the development of diseases of the mouth. Porphyromonas gingivalis are anaerobic Gramnegative bacteria found in the subgingival plaque. They are largely responsible for chronic periodontal disease. Streptococcus intermedius is a Gram positive coccus found in the supragingival plaque. The objective of the present work was to evaluate the expression of several virulence genes of P. gingivalis in a mixed culture with S. intermedius using qPCR and heterologous microarrays. P. gingivalis ATCC 33277 and W83 and S. intermedius ATCC 27335 strains were cultured and total RNA was extracted using the High Pure RNA isolation kit. Oligodeoxynucleotides were designed to make multiple comparisons with organisms. Microarray was performed to identify gene expression. To quantify gene expression, cDNA samples from three different P. gingivalis:S. intermedius ratios were diluted 10-1, 10-2 and 10-3. The microarray experiment indicated that in P. gingivalis, 29 genes were upregulated. The putative function of upregulated genes was the biosynthesis of different metabolic pathways.     Arginine methylation of REF/ALY promotes efficient handover of mRNA to TAP/NXF1

    The REF/ALY mRNA export adaptor binds TAP/NXF1 via an arginine-rich region, which overlaps with its RNA-binding domain. When TAP binds a REF:RNA complex, it triggers transfer of the RNA from REF to TAP. Here, we have examined the effects of arginine methylation on the activities of the REF protein in mRNA export. We have mapped the arginine methylation sites of REF using mass spectrometry and find that several arginines within the TAP and RNA binding domains are methylated in vivo. However, arginine methylation has no effect on the REF:TAP interaction. Instead, arginine methylation reduces the RNA-binding activity of REF in vitro and in vivo. The reduced RNA-binding activity of REF in its methylated state is essential for efficient displacement of RNA from REF by TAP in vivo. Therefore, arginine methylation fine-tunes the RNA-binding activity of REF such that the RNA–protein interaction can be readily disrupted by export factors further down the pathway

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Background: Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. // Methods: We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. // Findings: We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in low-income countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. // Interpretation: Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between low-income, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    The genomic substrate for adaptive radiation in African cichlid fish

    Cichlid fishes are famous for large, diverse and replicated adaptive radiations in the Great Lakes of East Africa. To understand the molecular mechanisms underlying cichlid phenotypic diversity, we sequenced the genomes and transcriptomes of five lineages of African cichlids: the Nile tilapia (Oreochromis niloticus), an ancestral lineage with low diversity; and four members of the East African lineage: Neolamprologus brichardi/pulcher (older radiation, Lake Tanganyika), Metriaclima zebra (recent radiation, Lake Malawi), Pundamilia nyererei (very recent radiation, Lake Victoria), and Astatotilapia burtoni (riverine species around Lake Tanganyika). We found an excess of gene duplications in the East African lineage compared to tilapia and other teleosts, an abundance of non-coding element divergence, accelerated coding sequence evolution, expression divergence associated with transposable element insertions, and regulation by novel microRNAs. In addition, we analysed sequence data from sixty individuals representing six closely related species from Lake Victoria, and show genome-wide diversifying selection on coding and regulatory variants, some of which were recruited from ancient polymorphisms. We conclude that a number of molecular mechanisms shaped East African cichlid genomes, and that amassing of standing variation during periods of relaxed purifying selection may have been important in facilitating subsequent evolutionary diversification
