    ЗАСТОСУВАННЯ ЛАЗЕРОТЕРАПІЇ У ЛІКУВАННІ ХВОРИХ ІЗ НЕВРОЛОГІЧНИМИ ПРОЯВАМИ ОСТЕОХОНДРОЗУ ПОПЕРЕКОВОГО ВІДДІЛУ ХРЕБТА

    This article evaluated the efficiency of complex application of low-energy laser radiation in the infrared spectrum by scanning technique using apparatus "MEDIC - 2 K" in the rehabilitation for patients with neurological manifestations of osteochondrosis vertebralis. 33 patients with neurological manifestations of osteochondrosis vertebralis of the lumbar spine were treated: the main group 18 of persons, the control group of 15 persons. In the main group, the patients received 15 laser treatments lasting 20 minutes with four irradiation fields according to acupuncture points (V26, VB30, V40, VB34). All patients were involved in therapeutic exercises according to the standardized program and continued to follow it on their own home. The results were evaluated at the end of treatment and after 3 months. The pain syndromebefore the treatment in both groups was approximately equal: in the main group it was 64.5 + 3.09%, in the control group it was – 63.67+2.60 (p> 0.05). After 3 months, the pain continued to decrease and amounted to 21.67 + 1.14% in the main group and 30.67 + 1.75% in the control group (p <0.05). The most effective program in terms of mobilization of the lumbar spine in patients with neurological manifestations of osteochondrosis of the lumbar spine is the complex program of rehabilitation, including drug therapy, physical therapy and laser therapy sessions.В статье оценивалась эффективность комплексного применения низкоэнергетического лазерного излучения в инфракрасном спектре по сканирующей методике на аппарате“МЕДИК – 2 К”в реабилитации больных с неврологическими проявлениями остеохондроза позвоночника. Было проведено лечение 33 пациентов с неврологическими проявлениями остеохондроза поясничного отдела позвоночника: основная группа – 18 человек, контрольная – 15 человек. В основной группе больные получали 15 процедур лазеротерапии длительностью 20 минут с облучением четырех полей  согласно точкам акупунктуры (V26, VB30, V40, VB34).Все больные занимались личебной гимнастикой по стандартной программе и продолжали ее выполнять самостоятельно в домашних условиях.  Результаты оценивали в конце лечения и через 3 месяца. Болевой синдром до лечения в обеих группах был примерно одинаковый: в основной – (64,5 %+3,09), в контрольной – (63,67%+ 2,60) (p>0,05). В конце курса лечения болевой синдром снизился более значимо в основной группе по сровнению с контрольной: (41,1%+3,01) и (51,67%+3,33) соответственно (p<0,05). Через 3 месяца боли продолжали уменьшаться и составили (21,67%+1,14) в основной и (30,67%+1,75) в контрольной группе (p<0,05). Наиболее эффективной в плане восстановления подвижности в поясничном отделе позвоночника у больных с неврологическими проявлениями остеохондроза поясничного отдела позвоночника является комплексная программа реабилитации, включающая медикаментозную терапию, занятия ЛФК и лазеротерапию.У статті оцінювалася ефективність комплексного використання низькоенергетичного лазерного випромінювання в інфрачервоному спектрі за скануючою методикою на апараті “МЕДИК–2 К” у реабілітації хворих із неврологічними проявами остеохондрозу хребта. Було проведено лікування 33 пацієнтів з неврологічними проявами остеохондрозу поперекового відділу хребта: основна група – 18, контрольна – 15 осіб. В основній групі хворі отримували 15 процедур лазеротерапії тривалістю 20 хв з опроміненням чотирьох полів  згідно з точками акупунктури (V26, Vb30, V40, Vb34). Всі хворі займалися лікувальною гімнастикою за стандартною програмою і продовжували її виконувати самостійно в домашніх умовах.  Результати оцінювали в кінці лікування і через 3 місяці.  Больовий синдром до лікування в обох групах був приблизно однаковий: в основній – (64,5+3,09%), в контрольній – (63,67+ 2,60) (p>0,05). В кінці курсу лікування больовий синдром знизився значнов основній групі,порівняно з контрольною: (41,1%+3,01) і (51,67%+3,33) відповідно (p<0,05). Через 3 місяці біль продовжував зменшуватися і склав (21,67%+1,14) в основній і (30,67%+1,75) в контрольній групах (p<0,05). Найбільш ефективним в плані відновлення рухомості в поперековому відділі хребта у хворих із неврологічними проявами остеохондрозу поперекового відділу хребта є комплексна програма реабілітації, що включає медикаментозну терапію, заняття ЛФК і лазеротерапію

    Promoting higher added value to a finfish species rejected to sea

    This project aimed to the development of the research and the technology necessary to promote higher added value to fishing activity. This is to be achieved by obtaining profit from a finfish species (“Rockcod”, Patagonotothen spp.) not known to consumers and currently discarded by the EU fishing fleet operating in the South West Atlantic, in order to supply the EU seafood industry with a good quality raw material for human food manufacturing. Use of this species, caught as a by-catch in the existing fisheries targeting hakes and cephalopods, should also increase the profitability of the fleet, contribute to maintaining employment and help to counterbalance the negative effects of fishing activity and discards in the ecosystem. The main scientific-technological objectives and expected achievements were the following: - Description of the fisheries - Improved knowledge of the biology of the species - Biomass assessment - Estimation of catches and discards - Analysis of the spatial and temporal distribution of the resource.     We report on photometric and spectroscopic observations and analysis of the 2019 superoutburst of TCP J21040470+4631129. This object showed a 9 mag superoutburst with early superhumps and ordinary superhumps, which are the features of WZ Sge-type dwarf novae. Five rebrightenings were observed after the main superoutburst. The spectra during the post-superoutburst stage showed Balmer, He I, and possible sodium doublet features. The mass ratio is derived as 0.0880(9) from the period of the superhump. During the third and fifth rebrightenings, growing superhumps and superoutbursts were observed, which have never been detected during a rebrightening phase among WZ Sge-type dwarf novae with multiple rebrightenings. To induce a superoutburst during the brightening phase, the accretion disk needs to have expanded beyond the 3 : 1 resonance radius of the system again after the main superoutburst. These peculiar phenomena can be explained by the enhanced viscosity and large radius of the accretion disk suggested by the higher luminosity and the presence of late-stage superhumps during the post-superoutburst stage, plus by more mass supply from the cool mass reservoir and/or from the secondary because of the enhanced mass transfer than those of other WZ Sge-type dwarf novae.peer-reviewe

    Do Stress Responses Promote Leukemia Progression? An Animal Study Suggesting a Role for Epinephrine and Prostaglandin-E2 through Reduced NK Activity

    In leukemia patients, stress and anxiety were suggested to predict poorer prognosis. Oncological patients experience ample physiological and psychological stress, potentially leading to increased secretion of stress factors, including epinephrine, corticosteroids, and prostaglandins. Here we tested whether environmental stress and these stress factors impact survival of leukemia-challenged rats, and studied mediating mechanisms. F344 rats were administered with a miniscule dose of 60 CRNK-16 leukemia cells, and were subjected to intermittent forced swim stress or to administration of physiologically relevant doses of epinephrine, prostaglandin-E2 or corticosterone. Stress and each stress factor, and/or their combinations, doubled mortality rates when acutely applied simultaneously with, or two or six days after tumor challenge. Acute administration of the β-adrenergic blocker nadolol diminished the effects of environmental stress, without affecting baseline survival rates. Prolonged β-adrenergic blockade or COX inhibition (using etodolac) also increased baseline survival rates, possibly by blocking tumor-related or normal levels of catecholamines and prostaglandins. Searching for mediating mechanisms, we found that each of the stress factors transiently suppressed NK activity against CRNK-16 and YAC-1 lines on a per NK basis. In contrast, the direct effects of stress factors on CRNK-16 proliferation, vitality, and VEGF secretion could not explain or even contradicted the in vivo survival findings. Overall, it seems that environmental stress, epinephrine, and prostaglandins promote leukemia progression in rats, potentially through suppressing cell mediated immunity. Thus, patients with hematological malignancies, which often exhibit diminished NK activity, may benefit from extended β-blockade and COX inhibition

    Steam sauna and mother roasting in Lao PDR: practices and chemical constituents of essential oils of plant species used in postpartum recovery

    <p>Abstract</p> <p>Background</p> <p>Fundamental in traditional postpartum recovery in Lao PDR is the use of hotbeds, mother roasting, steam sauna and steam baths. During these treatments medicinal plants play a crucial role, but little has been published about how the treatments are carried out precisely, which species are used, the medicinal properties of these species, and the medicinal efficacy of their chemical constituents.</p> <p>Methods</p> <p>Sixty-five interviews, in 15 rural villages, with women of 4 different ethnic groups were conducted to survey confinement rituals, and postpartum plant use and salience. Essential oils from the main species used were extracted using steam distillation and the main chemical constituents characterized using gas chromatography-mass spectrometry (GC-MS).</p> <p>Results</p> <p>A total of 10 different species were used by three or more of the ethnic groups included in this study. All species were used in steam sauna and bath, but only 3 species were used in hotbed and mother roasting. Essential oils of <it>Amomum villosum, Amomum microcarpum </it>and <it>Blumea balsamifera </it>were found to contain significant amounts of the following terpenes: β-pinene, camphor, bornyl acetate, borneol, linalool, D-limonene, fenchone, terpinen-4-ol and α-terpinene.</p> <p>Conclusions</p> <p>Many of these terpenes have documented antimicrobial and analgesic properties, and some have also synergistic interactions with other terpenes. The mode of application in hotbed and mother roasting differs from the documented mechanisms of action of these terpenes. Plants in these two practices are likely to serve mainly hygienic purposes, by segregating the mother from infection sources such as beds, mats, stools, cloth and towels. Steam sauna medicinal plant use through inhalation of essential oils vapors can possibly have medicinal efficacy, but is unlikely to alleviate the ailments commonly encountered during postpartum convalescence. Steam sauna medicinal plant use through dermal condensation of essential oils, and steam bath cleansing of the perineal area is possibly a pragmatic use of the reported medicinal plants, as terpene constituents have documented antimicrobial, analgesic and anti-inflammatory properties.</p

    Transcriptome dynamics of a broad host-range cyanophage and its hosts

    Cyanobacteria are highly abundant in the oceans and are constantly exposed to lytic viruses. The T4-like cyanomyoviruses are abundant in the marine environment and have broad host-ranges relative to other cyanophages. It is currently unknown whether broad host-range phages specifically tailor their infection program for each host, or employ the same program irrespective of the host infected. Also unknown is how different hosts respond to infection by the same phage. Here we used microarray and RNA-seq analyses to investigate the interaction between the Syn9 T4-like cyanophage and three phylogenetically, ecologically and genomically distinct marine Synechococcus strains: WH7803, WH8102 and WH8109. Strikingly, Syn9 led a nearly identical infection and transcriptional program in all three hosts. Different to previous assumptions for T4-like cyanophages, three temporally regulated gene expression classes were observed. Furthermore, a novel regulatory element controlled early-gene transcription, and host-like promoters drove middle gene transcription, different to the regulatory paradigm for T4. Similar results were found for the P-TIM40 phage during infection of Prochlorococcus NATL2A. Moreover, genomic and metagenomic analyses indicate that these regulatory elements are abundant and conserved among T4-like cyanophages. In contrast to the near-identical transcriptional program employed by Syn9, host responses to infection involved host-specific genes primarily located in hypervariable genomic islands, substantiating islands as a major axis of phage-cyanobacteria interactions. Our findings suggest that the ability of broad host-range phages to infect multiple hosts is more likely dependent on the effectiveness of host defense strategies than on differential tailoring of the infection process by the phage

    Effect of the COVID-19 pandemic on surgery for indeterminate thyroid nodules (THYCOVID): a retrospective, international, multicentre, cross-sectional study

    Background Since its outbreak in early 2020, the COVID-19 pandemic has diverted resources from non-urgent and elective procedures, leading to diagnosis and treatment delays, with an increased number of neoplasms at advanced stages worldwide. The aims of this study were to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic; and to evaluate whether delays in surgery led to an increased occurrence of aggressive tumours.Methods In this retrospective, international, cross-sectional study, centres were invited to participate in June 22, 2022; each centre joining the study was asked to provide data from medical records on all surgical thyroidectomies consecutively performed from Jan 1, 2019, to Dec 31, 2021. Patients with indeterminate thyroid nodules were divided into three groups according to when they underwent surgery: from Jan 1, 2019, to Feb 29, 2020 (global prepandemic phase), from March 1, 2020, to May 31, 2021 (pandemic escalation phase), and from June 1 to Dec 31, 2021 (pandemic decrease phase). The main outcomes were, for each phase, the number of surgeries for indeterminate thyroid nodules, and in patients with a postoperative diagnosis of thyroid cancers, the occurrence of tumours larger than 10 mm, extrathyroidal extension, lymph node metastases, vascular invasion, distant metastases, and tumours at high risk of structural disease recurrence. Univariate analysis was used to compare the probability of aggressive thyroid features between the first and third study phases. The study was registered on ClinicalTrials.gov, NCT05178186.Findings Data from 157 centres (n=49 countries) on 87 467 patients who underwent surgery for benign and malignant thyroid disease were collected, of whom 22 974 patients (18 052 [78 center dot 6%] female patients and 4922 [21 center dot 4%] male patients) received surgery for indeterminate thyroid nodules. We observed a significant reduction in surgery for indeterminate thyroid nodules during the pandemic escalation phase (median monthly surgeries per centre, 1 center dot 4 [IQR 0 center dot 6-3 center dot 4]) compared with the prepandemic phase (2 center dot 0 [0 center dot 9-3 center dot 7]; p&lt;0 center dot 0001) and pandemic decrease phase (2 center dot 3 [1 center dot 0-5 center dot 0]; p&lt;0 center dot 0001). Compared with the prepandemic phase, in the pandemic decrease phase we observed an increased occurrence of thyroid tumours larger than 10 mm (2554 [69 center dot 0%] of 3704 vs 1515 [71 center dot 5%] of 2119; OR 1 center dot 1 [95% CI 1 center dot 0-1 center dot 3]; p=0 center dot 042), lymph node metastases (343 [9 center dot 3%] vs 264 [12 center dot 5%]; OR 1 center dot 4 [1 center dot 2-1 center dot 7]; p=0 center dot 0001), and tumours at high risk of structural disease recurrence (203 [5 center dot 7%] of 3584 vs 155 [7 center dot 7%] of 2006; OR 1 center dot 4 [1 center dot 1-1 center dot 7]; p=0 center dot 0039).Interpretation Our study suggests that the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic period could have led to an increased occurrence of aggressive thyroid tumours. However, other compelling hypotheses, including increased selection of patients with aggressive malignancies during this period, should be considered. We suggest that surgery for indeterminate thyroid nodules should no longer be postponed even in future instances of pandemic escalation.Funding None.Copyright (c) 2023 Published by Elsevier Ltd.     Laparoscopy in management of appendicitis in high-, middle-, and low-income countries: a multicenter, prospective, cohort study.

    BACKGROUND: Appendicitis is the most common abdominal surgical emergency worldwide. Differences between high- and low-income settings in the availability of laparoscopic appendectomy, alternative management choices, and outcomes are poorly described. The aim was to identify variation in surgical management and outcomes of appendicitis within low-, middle-, and high-Human Development Index (HDI) countries worldwide. METHODS: This is a multicenter, international prospective cohort study. Consecutive sampling of patients undergoing emergency appendectomy over 6 months was conducted. Follow-up lasted 30 days. RESULTS: 4546 patients from 52 countries underwent appendectomy (2499 high-, 1540 middle-, and 507 low-HDI groups). Surgical site infection (SSI) rates were higher in low-HDI (OR 2.57, 95% CI 1.33-4.99, p = 0.005) but not middle-HDI countries (OR 1.38, 95% CI 0.76-2.52, p = 0.291), compared with high-HDI countries after adjustment. A laparoscopic approach was common in high-HDI countries (1693/2499, 67.7%), but infrequent in low-HDI (41/507, 8.1%) and middle-HDI (132/1540, 8.6%) groups. After accounting for case-mix, laparoscopy was still associated with fewer overall complications (OR 0.55, 95% CI 0.42-0.71, p < 0.001) and SSIs (OR 0.22, 95% CI 0.14-0.33, p < 0.001). In propensity-score matched groups within low-/middle-HDI countries, laparoscopy was still associated with fewer overall complications (OR 0.23 95% CI 0.11-0.44) and SSI (OR 0.21 95% CI 0.09-0.45). CONCLUSION: A laparoscopic approach is associated with better outcomes and availability appears to differ by country HDI. Despite the profound clinical, operational, and financial barriers to its widespread introduction, laparoscopy could significantly improve outcomes for patients in low-resource environments.     The First Post-Kepler Brightness Dips of KIC 8462852

    We present a photometric detection of the first brightness dips of the unique variable star KIC 8462852 since the end of the Kepler space mission in 2013 May. Our regular photometric surveillance started in October 2015, and a sequence of dipping began in 2017 May continuing on through the end of 2017, when the star was no longer visible from Earth. We distinguish four main 1-2.5% dips, named "Elsie," "Celeste," "Skara Brae," and "Angkor", which persist on timescales from several days to weeks. Our main results so far are: (i) there are no apparent changes of the stellar spectrum or polarization during the dips; (ii) the multiband photometry of the dips shows differential reddening favoring non-grey extinction. Therefore, our data are inconsistent with dip models that invoke optically thick material, but rather they are in-line with predictions for an occulter consisting primarily of ordinary dust, where much of the material must be optically thin with a size scale <<1um, and may also be consistent with models invoking variations intrinsic to the stellar photosphere. Notably, our data do not place constraints on the color of the longer-term "secular" dimming, which may be caused by independent processes, or probe different regimes of a single process
