Prolonged Isolated Soluble Dietary Fibre Supplementation in Overweight and Obese Patients: A Systematic Review with Meta-Analysis of Randomised Controlled Trials.
The prevalence of overweight and obesity is rising rapidly, currently affecting 1.9 billion adults worldwide. Prebiotic dietary fibre supplementation is a promising approach to improve weight loss and reduce metabolic complications in overweight and obese subjects due to modifications of the microbiota composition and function. Previous systematic reviews and meta-analyses addressing similar questions revealed discordant evidence and/or are outdated. We searched MEDLINE, Embase, Google Scholar, and forward and backward citations for randomised controlled trials (RCTs) with isolated soluble dietary fibre supplementation for at least 12 weeks in overweight and obese patients measuring body weight, published through April 2022. We expressed the results as mean differences (MDs) using the random-effects model of the metafor package in R and assessed risk of bias using the Cochrane RoB2 tool. We conducted the study according to the PRISMA guidelines and registered the protocol on PROSPERO (CRD42022295246). The participants with dietary fibre supplementation showed a significantly higher reduction in body weight (MD -1.25 kg, 95% CI -2.24, -0.25; 27 RCTs; 1428 participants) accompanied by a significant decrease in BMI, waist circumference, fasting blood insulin, and HOMA-IR compared to the control group. Certainty of evidence was high, paving the way for the implementation of isolated soluble dietary fibre supplementation into clinical practice
Review of road surface photometry methods and devices – Proposal for new measurement geometries
Specifications concerning road lighting and photometry of road surfaces were established more than 50 years ago. Road lighting design and road marking visibility were developed for vehicle driving. The observation distance defined by standards corresponds to interurban applications; however, within Europe these areas do not tend to be lit. The objective of the SURFACE project is to propose new geometries for the photometric characterisation of pavements, both adapted to different urban travel modes and new lighting technologies. This article reviews the available guidelines, standards, measuring devices and literature regarding geometries and road lighting applications, and presents the project SURFACE analysis and proposal. The SURFACE consortium recommends adding several new angles for different driving conditions and road users; 2.29 degrees for urban environments and consistency with road marking standard, and 1 degrees for extra-urban environment and consistency with previous geometries. A 5 degrees angle, corresponding to 17-m viewing distance, could be an interesting compromise, suitable for urban driving at low speed, cycling and for scooters. The angles of 10 degrees and 20 degrees are under consideration for describing the boundary between diffuse and specular behaviour
Management of Hyperglycemia in Hospitalized Patients Receiving Parenteral Nutrition.
Almost half of inpatients on parenteral nutrition experience hyperglycemia, which increases the risk of complications and mortality. The blood glucose target for hospitalized patients on parenteral nutrition is 7.8 to 10.0 mmol/L (140 to 180 mg/dL). For patients with diabetes, the same parenteral nutrition formulae as for patients without diabetes can be used, as long as blood glucose levels can be adequately controlled using insulin. Insulin can be delivered via the subcutaneous or intravenous route or, alternatively, added to parenteral nutrition admixtures. Combining parenteral with enteral and oral nutrition can improve glycemic control in patients with sufficient endogenous insulin stores. Intravenous insulin infusion is the preferred route of insulin delivery in critical care as doses can be rapidly adjusted to altered requirements. For stable patients, insulin can be added directly to the parenteral nutrition bag. If parenteral nutrition is infused continuously over 24 hours, the subcutaneous injection of a long-acting insulin combined with correctional bolus insulin may be adequate. A Gaze-Driven Evolutionary Algorithm to Study Aesthetic Evaluation of Visual Symmetry
Empirical work has shown that people like visual symmetry. We used a gaze-driven evolutionary algorithm technique to answer three questions about symmetry preference. First, do people automatically evaluate symmetry without explicit instruction? Second, is perfect symmetry the best stimulus, or do people prefer a degree of imperfection? Third, does initial preference for symmetry diminish after familiarity sets in? Stimuli were generated as phenotypes from an algorithmic genotype, with genes for symmetry (coded as deviation from a symmetrical template, deviation–symmetry, DS gene) and orientation (0° to 90°, orientation, ORI gene). An eye tracker identified phenotypes that were good at attracting and retaining the gaze of the observer. Resulting fitness scores determined the genotypes that passed to the next generation. We recorded changes to the distribution of DS and ORI genes over 20 generations. When participants looked for symmetry, there was an increase in high-symmetry genes. When participants looked for the patterns they preferred, there was a smaller increase in symmetry, indicating that people tolerated some imperfection. Conversely, there was no increase in symmetry during free viewing, and no effect of familiarity or orientation. This work demonstrates the viability of the evolutionary algorithm approach as a quantitative measure of aesthetic preference
Attitudes and expectations of patients on home parenteral nutrition towards eHealth: A multicenter survey.
BACKGROUND & AIMS
Advances in technology enable patients on home parenteral nutrition (HPN) to manage their treatment more independently and safely. eHealth is a promising application of electronic means in healthcare, aimed at improving and simplifying processes and connecting the different parties involved. A thorough understanding of the attitudes and expectations of patients on HPN towards eHealth is a prerequisite for a successful implementation. However, to the best of our knowledge, such a survey preceding the implementation of HPN specific eHealth care has never been conducted. The objective of this preliminary survey is the acquisition of insights on the attitudes and expectations of patients on HPN towards eHealth. Resulting findings then serve as the basis for the design of an eHealth platform to facilitate communication among those involved in HPN care, improve the HPN management, and safeguard and monitor the treatment.
METHODS
We conducted a survey on the attitudes and expectations of patients towards an envisioned eHealth platform for HPN. Patients were recruited from large Swiss hospitals by their treating physician or directly by the research team. The surveys were conducted between September 2020 and October 2021 by structured personal interviews based on a questionnaire.
RESULTS
We included 35 patients on HPN (21 [60%] females) treated in ambulant care of 4 hospitals. They had a median (interquartile range) age of 55 (18) years and a median (interquartile range) duration of parenteral nutrition of 1.3 (3.1) years. Most patients (n = 30, 86%) were equipped with a smartphone, tablet, or computer and 22 (63%) used apps and rated themselves as proficient with the corresponding digital device. A majority of patients rated the following aspects and features of the platform as important: Data collection and storage (n = 29, 83%), checklists for PN, catheter, and infusion pump handling (n = 28, 80%), video instructions (n = 27, 77%), and videoconferencing with physicians (n = 25, 71%). Most patients (n = 26, 74%) were willing to enter data into the platform themselves. The type of data to be entered should be defined on an individual basis.
CONCLUSIONS
Patients on HPN are open to videoconference consultations and using an eHealth platform. Two-thirds have the necessary technical skills including suitable digital devices for an eHealth care. We identified key features of an eHealth platform to improve HPN management
Explicit processing of verbal and spatial features during letter-location binding modulates oscillatory activity of a fronto-parietal network.
The present study investigated the binding of verbal and spatial features in immediate memory. In a recent study, we demonstrated incidental and asymmetrical letter-location binding effects when participants attended to letter features (but not when they attended to location features) that were associated with greater oscillatory activity over prefrontal and posterior regions during the retention period. We were interested to investigate whether the patterns of brain activity associated with the incidental binding of letters and locations observed when only the verbal feature is attended differ from those reflecting the binding resulting from the controlled/explicit processing of both verbal and spatial features. To achieve this, neural activity was recorded using magnetoencephalography (MEG) while participants performed two working memory tasks. Both tasks were identical in terms of their perceptual characteristics and only differed with respect to the task instructions. One of the tasks required participants to process both letters and locations. In the other, participants were instructed to memorize only the letters, regardless of their location. Time–frequency representation of MEG data based on the wavelet transform of the signals was calculated on a single trial basis during the maintenance period of both tasks. Critically, despite equivalent behavioural binding effects in both tasks, single and dual feature encoding relied on different neuroanatomical and neural oscillatory correlates. We propose that enhanced activation of an anterior–posterior dorsal network observed in the task requiring the processing of both features reflects the necessity for allocating greater resources to intentionally process verbal and spatial features in this task
Advances in prevention and therapy of neonatal dairy calf diarrhoea : a systematical review with emphasis on colostrum management and fluid therapy
Neonatal calf diarrhoea remains the most common cause of morbidity and mortality in preweaned dairy calves worldwide. This complex disease can be triggered by both infectious and non-infectious causes. The four most important enteropathogens leading to neonatal dairy calf diarrhoea are Escherichia coli, rota-and coronavirus, and Cryptosporidium parvum. Besides treating diarrhoeic neonatal dairy calves, the veterinarian is the most obvious person to advise the dairy farmer on prevention and treatment of this disease. This review deals with prevention and treatment of neonatal dairy calf diarrhoea focusing on the importance of a good colostrum management and a correct fluid therapy
Zerstörungsfreie Bestimmung von Cholesterol und Collagen in arteriosklerotischen Plaques mittels NIR-Spektroskopie
Eine der häufigsten Todesursachen in den Industrieländern ist nach wie vor der Herzinfarkt infolge sklerotischer Veränderungen der Arterien. Moderne bildgebende Verfahren (Ultraschalltechniken, tomographische Verfahren) ermöglichen die Darstellung der Lumenveränderungen in den Arterien, können jedoch die chemische Zusammensetzung einer arteriosclerotischen Ablagerung (Plaque) nicht wiedergeben. Die Kenntnis der Plaquezusammensetzung wäre jedoch ein wichtiges Kriterium für die Einschätzung der Plaquevulnerabilität und könnte somit therapeutische Entscheidungen unterstützen.
Ziel unserer Untersuchungen war es nachzuweisen, ob mittels Nahinfrarot-Spektroskopie die Bestimmung von Komponenten der Arterienwand (Cholesterol, Kollagen) möglich ist.
Untersucht wurden 82 verschiedene Arterienareale autoptisch gewonnener humaner Aorten und Modellmischungen. Die NIR-Spektren wurden mit einem FT-Spektrophotometer und drei unterschiedlichen Fasersonden (1.: optisches Fenster d=4mm, 2.: d=1mm, 3.: d=400µm mit rechtwinklig zur Faserrichtung austretendem Meßstrahl) aufgenommen. Die chemische Referenzanalytik erfolgte mittels HPLC, die chemometrische Auswertung mit dem PLS (partial least square) Model. Weiterhin wurde die Eindringtiefe des Meßstrahles in das Gewebe untersucht.
Unsere Untersuchungen zeigen, daß die NIR-Spektroskopie ein geeignetes Arbeitsmittel für die quantitative Bestimmung von Arterienwandbestandteilen ist. Für die Realisierung von in-vivo-Untersuchungen mit einem entsprechendem NIR-Katheter müssen jedoch noch viele technische Details geklärt werden

Measuring category intuitiveness in unconstrained categorization tasks
What makes a category seem natural or intuitive? In this paper, an unsupervised categorization task was employed to examine observer agreement concerning the categorization of nine different stimulus sets. The stimulus sets were designed to capture different intuitions about classification structure. The main empirical index of category intuitiveness was the frequency of the preferred classification, for different stimulus sets. With 169 participants, and a within participants design, with some stimulus sets the most frequent classification was produced over 50 times and with others not more than two or three times. The main empirical finding was that cluster tightness was more important in determining category intuitiveness, than cluster separation. The results were considered in relation to the following models of unsupervised categorization: DIVA, the rational model, the simplicity model, SUSTAIN, an Unsupervised version of the Generalized Context Model (UGCM), and a simple geometric model based on similarity. DIVA, the geometric approach, SUSTAIN, and the UGCM provided good, though not perfect, fits. Overall, the present work highlights several theoretical and practical issues regarding unsupervised categorization and reveals weaknesses in some of the corresponding formal models
