A self-controlled case series to assess the effectiveness of beta blockers for heart failure in reducing hospitalisations in the elderly
Background: To determine the suitability of using the self-controlled case series design to assess improvements in health outcomes using the effectiveness of beta blockers for heart failure in reducing hospitalisations as the example. Methods: The Australian Government Department of Veterans' Affairs administrative claims database was used to undertake a self-controlled case-series in elderly patients aged 65 years or over to compare the risk of a heart failure hospitalisation during periods of being exposed and unexposed to a beta blocker. Two studies, the first using a one year period and the second using a four year period were undertaken to determine if the estimates varied due to changes in severity of heart failure over time. Results: In the one year period, 3,450 patients and in the four year period, 12, 682 patients had at least one hospitalisation for heart failure. The one year period showed a non-significant decrease in hospitalisations for heart failure 4-8 months after starting beta-blockers, (RR, 0.76; 95% CI (0.57-1.02)) and a significant decrease in the 8-12 months post-initiation of a beta blocker for heart failure (RR, 0.62; 95% CI (0.39, 0.99)). For the four year study there was an increased risk of hospitalisation less than eight months post-initiation and significant but smaller decrease in the 8-12 month window (RR, 0.90; 95% CI (0.82, 0.98)). Conclusions: The results of the one year observation period are similar to those observed in randomised clinical trials indicating that the self-controlled case-series method can be successfully applied to assess health outcomes. However, the result appears sensitive to the study periods used and further research to understand the appropriate applications of this method in pharmacoepidemiology is still required. Leishmania isoenzyme polymorphisms in Ecuador: Relationships with geographic distribution and clinical presentation
Background: Determinants of the clinical presentation of the leishmaniases are poorly understood but Leishmania species and strain differences are important. To examine the relationship between clinical presentation, species and isoenzyme polymorphisms, 56 Leishmania isolates from distinct presentations of American tegumentary leishmaniasis (ATL) from Ecuador were analyzed.
Methods: Isolates were characterized by multilocus enzyme electrophoresis for polymorphisms of 11 isoenzymes. Patients were infected in four different ecologic regions: highland and lowland jungle of the Pacific coast, Amazonian lowlands and Andean highlands.
Results: Six Leishmania species constituting 21 zymodemes were identified: L. (Viannia) panamensis (21 isolates,
7 zymodemes), L. (V.) guyanensis (7 isolates, 4 zymodemes), L. (V.) braziliensis (5 isolates, 3 zymodemes), L.
(Leishmania) mexicana (11 isolates, 4 zymodemes), L. (L.) amazonensis (10 isolates, 2 zymodemes) and L. (L.) major
(2 isolates, 1 zymodeme). L. panamensis was the species most frequently identified in the Pacific region and was
associated with several clinical variants of cutaneous disease (CL); eight cases of leishmaniasis recidiva cutis (LRC) found in the Pacific highlands were associated with 3 zymodemes of this species. Mucocutaneous leishmaniasis
found only in the Amazonian focus was associated with 3 zymodemes of L. braziliensis. The papular variant of CL,
Uta, found in the Andean highlands was related predominantly with a single zymodeme of L. mexicana.
Conclusion: Our data show a high degree of phenotypic variation within species, and some evidence for associations between specific variants of ATL (i.e. Uta and LRC) and specific Leishmania zymodemes. This study
further defines the geographic distribution of Leishmania species and clinical variants of ATL in Ecuador

The actinobacterial transcription factor RbpA binds to the principal sigma subunit of RNA polymerase
RbpA is a small non-DNA-binding transcription factor that associates with RNA polymerase holoenzyme and stimulates transcription in actinobacteria, including Streptomyces coelicolor and Mycobacterium tuberculosis. RbpA seems to show specificity for the vegetative form of RNA polymerase as opposed to alternative forms of the enzyme. Here, we explain the basis of this specificity by showing that RbpA binds directly to the principal σ subunit in these organisms, but not to more diverged alternative σ factors. Nuclear magnetic resonance spectroscopy revealed that, although differing in their requirement for structural zinc, the RbpA orthologues from S. coelicolor and M. tuberculosis share a common structural core domain, with extensive, apparently disordered, N- and C-terminal regions. The RbpA-σ interaction is mediated by the C-terminal region of RbpA and σ domain 2, and S. coelicolor RbpA mutants that are defective in binding σ are unable to stimulate transcription in vitro and are inactive in vivo. Given that RbpA is essential in M. tuberculosis and critical for growth in S. coelicolor, these data support a model in which RbpA plays a key role in the σ cycle in actinobacteria
The XMM Cluster Survey: The interplay between the brightest cluster galaxy and the intra-cluster medium via AGN feedback
Using a sample of 123 X-ray clusters and groups drawn from the XMM-Cluster
Survey first data release, we investigate the interplay between the brightest
cluster galaxy (BCG), its black hole, and the intra-cluster/group medium (ICM).
It appears that for groups and clusters with a BCG likely to host significant
AGN feedback, gas cooling dominates in those with Tx > 2 keV while AGN feedback
dominates below. This may be understood through the sub-unity exponent found in
the scaling relation we derive between the BCG mass and cluster mass over the
halo mass range 10^13 < M500 < 10^15Msol and the lack of correlation between
radio luminosity and cluster mass, such that BCG AGN in groups can have
relatively more energetic influence on the ICM. The Lx - Tx relation for
systems with the most massive BCGs, or those with BCGs co-located with the peak
of the ICM emission, is steeper than that for those with the least massive and
most offset, which instead follows self-similarity. This is evidence that a
combination of central gas cooling and powerful, well fuelled AGN causes the
departure of the ICM from pure gravitational heating, with the steepened
relation crossing self-similarity at Tx = 2 keV. Importantly, regardless of
their black hole mass, BCGs are more likely to host radio-loud AGN if they are
in a massive cluster (Tx > 2 keV) and again co-located with an effective fuel
supply of dense, cooling gas. This demonstrates that the most massive black
holes appear to know more about their host cluster than they do about their
host galaxy. The results lead us to propose a physically motivated, empirical

Multi-site investigation of strategies for the clinical implementation of CYP2D6 genotyping to guide drug prescribing
PURPOSE:
A number of institutions have clinically implemented CYP2D6 genotyping to guide drug prescribing. We compared implementation strategies of early adopters of CYP2D6 testing, barriers faced by both early adopters and institutions in the process of implementing CYP2D6 testing, and approaches taken to overcome these barriers.
METHODS:
We surveyed eight early adopters of CYP2D6 genotyping and eight institutions in the process of adoption. Data were collected on testing approaches, return of results procedures, applications of genotype results, challenges faced, and lessons learned.
RESULTS:
Among early adopters, CYP2D6 testing was most commonly ordered to assist with opioid and antidepressant prescribing. Key differences among programs included test ordering and genotyping approaches, result reporting, and clinical decision support. However, all sites tested for copy-number variation and nine common variants, and reported results in the medical record. Most sites provided automatic consultation and had designated personnel to assist with genotype-informed therapy recommendations. Primary challenges were related to stakeholder support, CYP2D6 gene complexity, phenotype assignment, and sustainability.
CONCLUSION:
There are specific challenges unique to CYP2D6 testing given the complexity of the gene and its relevance to multiple medications. Consensus lessons learned may guide those interested in pursuing similar clinical pharmacogenetic programs
