    The meaning of touch the prostate for man: the nurse in health promotion

    Objectives: To identify the meaning for the men on the clinical examination of digital touch Prostate cancer early detection, to characterize the cause of the deficit in the search for preventive examination and health services by men and discuss the nurse's role in promoting men's health. Method: Qualitative descriptive study with field research guided by semi-structured form. Results: The meanings attributed to the digital touch of the prostate were embarrassment, discomfort, stigma and important. The information and access are tied to socioeconomic factors of the participants. Conclusion: Nurses should assist in health education and comprehensive health care by standardizing the information for different socioeconomic groups, minimizing the stigma and embarrassments, highlighting the importance of self care for man, to improve the search for health services, screening exams and prevention

    Patrones de desove de peces de arrecife comercialmente importantes (Lutjanidae y Serranidae) en el Atlántico tropical suroccidental

    Although information on the spawning seasons of commercially important snappers (Lutjanidae) and groupers (Serranidae, subfamily Epinephelinae) is available for the north and central west Atlantic, there is little information for the tropical western South Atlantic (Brazil). As a consequence, there are few fishery regulations in this entire region that take into consideration such information. In this study, we characterized the reproductive cycles of three Epinephelinae serranids (Epinephelus morio, Mycteroperca bonaci and Cephalopholis fulva) and five lutjanids (Lutjanus synagris, L. jocu, L. analis, Ocyurus chrysurus and Rhomboplites aurorubens) that occur in the Abrolhos Bank, Brazil, the largest reef complex in the tropical western South Atlantic. A total of 3528 gonads were collected from May 2005 to October 2007. Temporal variability in spawning patterns was evaluated using the Gonadosomatic Index (GSI) and macroscopic analyses. The peak of reproductive activity for the three grouper species occurred between July and August. Snappers exhibited two peaks of reproductive activity, the more intense of which occurred between September and October. The other peak occurred between February and March, with the exception of the deep-dwelling species R. aurorubens, which only reproduced between February and March. Seasonal patterns were consistent over the two consecutive years that we studied, and these patterns are equivalent to those observed for the Northern Hemisphere, as are the sizes at maturity and at the time of sexual change (for the three protogynous hermaphrodite groupers). Fisheries management in this region could be significantly improved by a combination of catch and effort limitations being imposed during spawning seasons and by overall size limitations, particularly considering that most of the fish caught are generally below size at maturity. The proposed buffer zones for the marine areas that are presently protected in Abrolhos represent an opportunity for implementing both spatial (e.g. setting no-take zones that cover spawning sites) and temporal (e.g. seasonal closures during spawning peaks) management tools, using a participatory approach at the regional scale.A pesar de que se tiene un buen conocimiento sobre la época de freza de los pargos (Lutjanidae) y los meros (Serranidae, subfamilia Epinephelinae) en las zonas norte y central del Atlántico occidental, la información disponible del Atlántico suroccidental es escasa. Como consecuencia, en esta zona aún existen pocas regulaciones pesqueras que tengan en cuenta esta información. En este estudio se caracterizan los ciclos reproductores de tres serránidos Epinephelinae (Epinephelus morio, Mycteroperca bonaci and Cephalopholis fulva) y cinco pargos (Lutjanus synagris, L. jocu, L. analis, Ocyurus chrysurus and Rhomboplites aurorubens) en el Banco de Abrolhos, el mayor complejo de arrecifes del Atlántico suroccidental. Se examinaron un total de 3528 gónadas entre mayo de 2005 y octubre de 2007. La variabilidad temporal de las pautas de puesta se evaluó mediante el Índice Gonadosomático (GSI) y análisis microscópicos. Para los tres meros el máximo en la actividad reproductora se produce entre julio y agosto. Los lutjánidos presentan dos picos de actividad bien diferenciados, siendo el más intenso el que tiene lugar entre septiembre y octubre, mientras que el menor se produce entre febrero y marzo, con la excepción de R. aurorubens, una especie de profundidad, que sólo se reproduce entre febrero y marzo. Las pautas estacionales en la reproducción son consistentes en los dos años consecutivos y, junto a la edad de primera madurez y la edad de inversión sexual (los tres meros son hermafroditas proterogínicos), son equivalentes a las que se producen en el hemisferio septentrional. La gestión pesquera en esta región podría mejorar significativamente mediante una combinación de limitaciones en las capturas y en el esfuerzo durante las épocas de puesta, así como limitando la talla mínima de captura, considerando que la mayoría de individuos capturados actualmente son inmaduros. Las zonas de amortiguación de las áreas marinas actualmente protegidas en Abrolhos representan una buena oportunidad para implementar herramientas de gestión tanto espaciales (e.g. estableciendo zonas de exclusión pesquera en los lugares de freza) como temporales (e.g. prohibiendo la pesca durante la época de reproducción), utilizando una aproximación participativa a una escala regional

    SURGERY SUSPENSION AND THE PROCESS OF COMMUNICATION

    Objetivo: Identificar nas produções científicas nacionais os motivos referente a suspensão de cirurgias e discutir suas causas frente o processo de comunicação dos profissionais. Métodos: Trata-se de uma revisão bibliográfica nas bases de dados da BVS, no período de 1990 a 2010. Resultados: A maior taxa de suspensão correspondeu a 33,0 % enquanto a menor taxa ficou em 5,1%. As principais causas dos cancelamentos foram referentes ao próprio cliente, quer seja por falta de condições clínicas, quer seja pelo não comparecimento para internação. Conclusão: Os dados da pesquisa apontam que o processo de comunicação é muitas das vezes é falho e poderia ser passível de intervenção dos profissionais de saúde.     Recomendações para oxigenoterapia domiciliar prolongada da Sociedade Brasileira de Pneumologia e Tisiologia (2022)

    Some chronic respiratory diseases can cause hypoxemia and, in such cases, long-term home oxygen therapy (LTOT) is indicated as a treatment option primarily to improve patient quality of life and life expectancy. Home oxygen has been used for more than 70 years, and support for LTOT is based on two studies from the 1980s that demonstrated that oxygen use improves survival in patients with COPD. There is evidence that LTOT has other beneficial effects such as improved cognitive function, improved exercise capacity, and reduced hospitalizations. LTOT is indicated in other respiratory diseases that cause hypoxemia, on the basis of the same criteria as those used for COPD. There has been an increase in the use of LTOT, probably because of increased life expectancy and a higher prevalence of chronic respiratory diseases, as well as greater availability of LTOT in the health care system. The first Brazilian Thoracic Association consensus statement on LTOT was published in 2000. Twenty-two years later, we present this updated version. This document is a nonsystematic review of the literature, conducted by pulmonologists who evaluated scientific evidence and international guidelines on LTOT in the various diseases that cause hypoxemia and in specific situations (i.e., exercise, sleep, and air travel). These recommendations, produced with a view to clinical practice, contain several charts with information on indications for LTOT, oxygen sources, accessories, strategies for improved efficiency and effectiveness, and recommendations for the safe use of LTOT, as well as a LTOT prescribing model.Algumas doenças respiratórias crônicas podem evoluir com hipoxemia e, nessas situações, a oxigenoterapia domiciliar prolongada (ODP) está indicada como opção terapêutica com o objetivo principal de melhorar a qualidade e a expectativa de vida desses pacientes. O oxigênio domiciliar é usado há mais de 70 anos, e a ODP tem como base dois estudos da década de oitenta que demonstraram que o uso de oxigênio melhora a sobrevida de pacientes com DPOC. Existem evidências de que a ODP tem outros efeitos benéficos como melhora da função cognitiva e da capacidade de exercício e redução de hospitalizações. A ODP está indicada para outras doenças respiratórias que cursam com hipoxemia, segundo os mesmos critérios estabelecidos para a DPOC. Tem sido observado aumento no uso da ODP provavelmente pela maior expectativa de vida, maior prevalência de doenças respiratórias crônicas e maior disponibilidade de ODP no sistema de saúde. O primeiro consenso sobre ODP da Sociedade Brasileira de Pneumologia e Tisiologia foi publicado em 2000; após 22 anos, apresentamos esta versão atualizada. Este documento é uma revisão não sistemática da literatura, realizada por pneumologistas que avaliaram evidências científicas e diretrizes internacionais sobre ODP nas diversas doenças que cursam com hipoxemia e em situações específicas (exercício, sono e viagens aéreas). Estas recomendações, tendo em vista a prática clínica, oferecem diversos quadros com informações sobre indicações, fontes de oxigênio, acessórios e estratégias para melhor eficiência, efetividade e uso seguro da ODP, assim como um modelo para sua prescrição

    Pesquisa Nacional de Saúde 2019: histórico, métodos e perspectivas

    This article presents the history and construction of the National Health Survey (PNS) 2019, a household survey conducted in partnership with the Brazilian Institute of Geography and Statistics. The objective was to provide the country with information on the determinants, conditions and health needs of the Brazilian population. The expected sample was 108,525 households, considering a non-response rate of 20%. The questionnaire had three parts: (i) regarding the household; (ii) to all residents of the household, focusing on the collection of socioeconomic and health information; and (iii) aimed at the selected resident (15 years or more) for whom lifestyles, chronic diseases, violence, among other topics were investigated, and anthropometric measures (sub-sample) were measured. The PNS information will serve as a basis for the (re)formulation of health policies, as well as support for existing actions and programs of the Unified Health System.Este artigo apresenta o histórico e a construção da Pesquisa Nacional de Saúde (PNS) 2019, inquérito de base domiciliar realizado em parceria com a fundação Instituto Brasileiro de Geografia e Estatística. O objetivo da PNS 2019 foi dotar o país de informações sobre os determinantes, condicionantes e necessidades de saúde da população brasileira. A amostra prevista foi de 108.525 domicílios particulares, considerando-se uma taxa de não resposta de 20%. Seu questionário continha três partes, orientadas para (i) o domicílio, (ii) todos os moradores do domicílio, com enfoque na coleta de informações socioeconômicas e de saúde, e (iii) o morador selecionado (idade ≥15 anos), sobre o qual investigou-se estilos de vida, doenças crônicas, violências, entre outros temas, e aferiu-se medidas antropométricas (subamostra). As informações da PNS 2019 servirão de base para a (re)formulação de políticas de saúde e subsídio a ações e programas existentes do Sistema Único de Saúde

    O esgotamento sanitário é um processo essencial à manutenção da saúde pública da população de um município. Nesse sentido, a falta de tratamento do esgoto resulta em problemas como o surgimento de doenças endêmicas e contaminação de recursos naturais. O objetivo desta pesquisa foi verificar e analisar a destinação e disposição final dos resíduos, gases e rejeitos oriundos do tratamento de efluentes das estações de tratamento de esgoto - ETE do município de Paragominas-PA. O método aplicado foi o dedutivo, pois, com tal processo de raciocínio é possível, partindo de uma ou mais premissas a obtenção de uma conclusão imprescindível e evidente. Esta é uma pesquisa de caráter exploratório, com aplicação de formulário semiestruturado para obtenção de respostas esclarecedoras sobre a quantidade, tipo e disposição/destinação final de resíduos gerados e retirados das estações de tratamento de esgoto em funcionamento no município de Paragominas-PA. A análise dos dados obtidos indicou a presenta em maiores proporções de efluentes, lodos e resíduos sólidos. Os resíduos sólidos juntamente com o lodo são dispostos no aterro controlado municipal, os gases passam por filtros e são dispostos na atmosfera, já os efluentes são lançados em um corpo hídrico receptor, após condições adequadas. Entretanto não se tem dados quantitativos precisos e com proposições para mitigação dos impactos a partir do quantitativo de gases, efluentes e resíduos sólidos gerados

    Early and Late Pathogenic Events of Newborn Mice Encephalitis Experimentally Induced by Itacaiunas and Curionópolis Bracorhabdoviruses Infection

    In previous reports we proposed a new genus for Rhabdoviridae and described neurotropic preference and gross neuropathology in newborn albino Swiss mice after Curionopolis and Itacaiunas infections. In the present report a time-course study of experimental encephalitis induced by Itacaiunas and Curionopolis virus was conducted both in vivo and in vitro to investigate cellular targets and the sequence of neuroinvasion. We also investigate, after intranasal inoculation, clinical signs, histopathology and apoptosis in correlation with viral immunolabeling at different time points. Curionopolis and Itacaiunas viral antigens were first detected in the parenchyma of olfactory pathways at 2 and 3 days post-inoculation (dpi) and the first clinical signs were observed at 4 and 8 dpi, respectively. After Curionopolis infection, the mortality rate was 100% between 5 and 6 dpi, and 35% between 8 and 15 dpi after Itacaiunas infection. We identified CNS mice cell types both in vivo and in vitro and the temporal sequence of neuroanatomical olfactory areas infected by Itacaiunas and Curionopolis virus. Distinct virulences were reflected in the neuropathological changes including TUNEL immunolabeling and cytopathic effects, more intense and precocious after intracerebral or in vitro inoculations of Curionopolis than after Itacaiunas virus. In vitro studies revealed neuronal but not astrocyte or microglial cytopathic effects at 2 dpi, with monolayer destruction occurring at 5 and 7 dpi with Curionopolis and Itacaiunas virus, respectively. Ultrastructural changes included virus budding associated with interstitial and perivascular edema, endothelial hypertrophy, a reduced and/or collapsed small vessel luminal area, thickening of the capillary basement membrane, and presence of phagocytosed apoptotic bodies. Glial cells with viral budding similar to oligodendrocytes were infected with Itacaiunas virus but not with Curionopolis virus. Thus, Curionopolis and Itacaiunas viruses share many pathological and clinical features present in other rhabdoviruses but distinct virulence and glial targets in newborn albino Swiss mice brain

    Elective Cancer Surgery in COVID-19-Free Surgical Pathways During the SARS-CoV-2 Pandemic: An International, Multicenter, Comparative Cohort Study.

    PURPOSE: As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19-free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS: This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19-free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS: Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19-free surgical pathways. Patients who underwent surgery within COVID-19-free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19-free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score-matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19-free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION: Within available resources, dedicated COVID-19-free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks
