Comienza con un breve estado de la cuestión, seguido por la presentación de las líneas temáticas y estilísticas tratadas en los diferentes artículos. Planteado como una síntesis del aporte original y sumamente relevante de esta sección de la revista Cauce, no solo destaca las nuevas miradas de los investigadores implicados, sino que adelanta líneas de investigación novedosas para futuras reflexiones. Finalmente, incluye el testimonio invalorable del propio autor, que proporciona claves de lectura y pistas de indagación útiles para los investigadores de su obra.The following text is intended as an introduction to the “ dossier ” on the figure and narrative production of the Peruvian-Spanish writer Fernando Iwasaki. It begins with a short status of the issue, followed by the presentation of thematic and stylistic lines treated in the different articles. Conceived as a synthesis of the original contribution of this section of Cauce Review, this introduction not only highlights new approaches from the researchers involved, it also opens novel lines of research for future consideration. Finally, this introduction contains the invaluable testimony of the author himself, which provides key reading and clues of useful research

    Percepción del estrés en los estudiantes de Enfermería ante sus prácticas clínicas

    Objective: This research aims to establish what the Nursing students’ main stress sources are during their practice period, according to their gender, grade and age. Methodology. It is a descriptive transversal study. The sample was composed by 45 Nursing students, 30 women and 15 men, at the University of Murcia in 2010/2011. The information gathering tool was KEZKAK questionnaire. Results. The ignorance facing a determined clinical situation and the risk of damaging the patient are the Nursing students’ two main stress sources. Men often suffer from a bigger stress than women; nevertheless, as well as the age increases and the students pass grades, the stress level is smaller. Conclusions. Nursing students’ stress determines their formation within their clinical practices period. Thus, it is necessary that the theoretical learning that supports these practices considers theses needs that own Nursing students require.OBJETIVO. Esta investigación pretende conocer cuáles son las principales fuentes de estrés durante el periodo de prácticas de los estudiantes de Enfermería de la Universidad de Murcia en función de su sexo, curso y edad. METODOLOGÍA. Se trata de un estudio transversal de corte descriptiva. La muestra estuvo compuesta por un total de 45 alumnos, 30 mujeres y 15 hombres, de Enfermería de la Universidad de Murcia del curso académico 2010/2011. El instrumento de recogida de la información fue cuestionario KEKZAK. RESULTADOS. El desconocimiento ante una determinada situación clínica y el riesgo de dañar al paciente son las dos principales fuentes estresoras para los estudiantes de enfermería. Los hombres suelen sufrir un mayor estrés que sus compañeras; no obstante, a medida que aumenta la edad y se avanza de curso el grado de estrés es cada vez menor. CONCLUSIONES. El estrés de los estudiantes de enfermería marca su formación durante el período de prácticas clínicas. Por tanto, se hace necesario que el aprendizaje teórico que sustenta a las prácticas atienda a estas necesidades que los propios estudiantes de enfermería demandan

    Dens invaginatus is a developmental disorder that is caused by an invagination of the inner epithelium of the enamel organ, inside dental papilla, during the early stages of morphodifferentiation of multifactorial etiology. A case of a 9 years-old female patient is presented, during the clinical examination it was observed fistula and the right central incisor with macrodontia, the patient referred no history of pain or trauma. Radiographically, it was observed an invagination that crosses the root forming two apical foramina, so that was diagnosed as type III dens invaginatus with pulp necrosisEl dens invaginatus es una alteración del desarrollo que se produce por una invaginación del epitelio interno del órgano del esmalte, dentro de la papila dental, durante los estadios tempranos de la morfodiferenciación, de etiología multifactorial. Se reporta el caso clínico de una paciente de 9 años de edad, en el examen clínico se observó fístula, el incisivo central derecho con macrodoncia y un cíngulo exagerado, la paciente no refería antecedentes de dolor ni de traumatismos.     The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Perspectivas sobre el futuro de la narrativa hispánica: Ensayos y testimonios

    ¿Cuáles han sido y son las orientaciones más significativas de la literatura hispánica contemporánea? ¿Cuáles serían las más «cargadas de futuro»? En ellas, ¿cuál sería la parte de los valores perennes y la de nuevos valores? Reflexionar sobre estas preguntas, que remiten, por una parte, a la cuestión general de la contemporaneidad y, por otra, a la dialéctica problemática entre presente, pasado y futuro, es el objeto de este volumen, donde se propone una visión fundamentada, y apoyada en la consideración de 126 escritores, del presente y del futuro de la narrativa en lengua española. Los quince ensayos de hispanistas pertenecientes a diversas universidades europeas aquí recogidos ambicionan poner de relieve tanto las continuidades como las rupturas y las “emergencias”; siendo todas ellas ponderadas e interpretadas con un enfoque diacrónico y comparatista que relativice las supuestas novedades. Por otra parte, se abordan y problematizan los numerosos cambios contextuales que se vienen produciendo, en especial, la competencia renovada de la literatura con la cultura de la imagen (piénsese en la ciberliteratura y en la blogosfera).
