    How well are pregnant women in Croatia informed about the oral glucose tolerance test?

    Introduction: Preanalytical errors still constitute the largest source of errors in laboratory work. Proper patient preparation and patient’s knowledge about a particular procedure affects its accuracy and reliability. We hypothesized that most of pregnant women are not well enough informed about the proper procedure for the OGTT. The aims of this study were to investigate: (i) how well pregnant women are informed about the OGTT; (ii) the most common way to inform pregnant women about OGTT and (iii) whether pregnant women’s level of knowledge about the OGTT differ regarding source of information. Materials and methods: The anonymous questionnaire was conducted across the country in 23 Croatian primary and secondary healthcare centres. The questionnaire contained 9 questions on certain demographic data and familiarity with OGTT procedure. All 343 participants filled the questionnaire before the first blood draw. Results: 42% of the participants demonstrated high and 38% adequate level of knowledge about the OGTT procedure. Majority of participants were informed about the procedure by gynaecologist (56%). The level of knowledge differed among participants with different sources of information (P = 0.030). Further analysis showed that the level of knowledge was lower in pregnant women having received information from their gynaecologist compared to pregnant women who received information from the laboratory staff. Conclusions: In general, pregnant women are familiar with OGTT procedure, main source of information about the OGTT procedure is their gynaecologist, but the level of knowledge was higher in women who received information about the OGTT procedure from the laboratory staff

    Evaluation of the BD VacutainerÂŽ RST blood collection tube for routine chemistry analytes: clinical significance of differences and stability study

    Introduction: Preanalytical variables account for most of laboratory errors. There is a wide range of factors that affect the reliability of laboratory report. Most convenient sample type for routine laboratory analysis is serum. BD Vacutainer® Rapid Serum Tube (RST) (Becton, Dickinson and Company, Franklin Lakes, NJ, USA) blood collection tube provides rapid clotting time allowing fast serum separation. Our aim was to evaluate the comparability of routine chemistry parameters in BD Vacutainer® RST blood collection tube in reference with the BD Vacutainer® Serum Separating Tubes II Advance Tube (SST) (Becton, Dickinson and Company, Franklin Lakes, NJ, USA). Materials and methods: Blood specimens were collected from 90 participants for evaluation on its results, clotting time and stability study of six routine biochemistry parameters: glucose (Glu), aspartate aminotransferase (AST), alanine aminotransferase (ALT), calcium (Ca), lactate dehidrogenase (LD) and potassium (K) measured with Olympus AU2700 analyzer (Beckman Coulter, Tokyo, Japan). The significance of the differences between samples was assessed by paired t-test or Wilcoxon Matched-Pairs Rank test after checking for normality. Results: Clotting process was significantly shorter in the RSTs compared to SSTs (2.49 min vs. 19.47 min, respectively; P < 0.001). There was a statistically significant difference between the RST and SST II tubes for glucose, calcium and LD (P < 0.001). Differences for glucose and LD were also clinically significant. Analyte stability studies showed that all analytes were stable for 24 h at 4 ˚C. Conclusions: Most results (except LD and glucose) from RST are comparable with those from SST.     Test validation, method comparison and reference range for the measurement of β-hydroxybutyrate in peripheral blood samples

    Introduction: The measurement of β-hydroxybutyrate (βOHB) concentrations is a corner stone of the diagnosis of diabetic ketoacidosis and other ketonic states. The aim of this study was to perform a validation of a peripheral blood βOHB assay (Randox) on a Roche cobas c502 analyser and to establish a βOHB reference range for the validated assay. Materials and methods: Precision, linearity and limit of detection and blank (LoD, LoB) were determined according to Clinical and Laboratory Standards Institute (CLSI) EP05-A3, EP 06-A and EP17-A2 guidelines, using commercial control material and residual patient sample pools. As method comparison, for 190 semi-quantitative measurements of urine ketones we determined the corresponding βOHB blood concentration. The reference range was based on the CLSI C28-A3 guideline, using 304 randomly selected serum samples from population based German National Cohort (GNC) study. Results: Coefficients of variation for the validated assay ranged from 1.5% for high concentrations (3.1 mmol/L) to 6.5% for low concentrations (0.1 mmol/L). Detection capacity was LoB = 0.011 mmol/L and LoD = 0.037 mmol/L. Linearity of the assay ranged from 0.10 to 3.95 mmol/L. The agreement between the semi-quantitative urine ketone test and the βOHB blood test was moderate (Kappa = 0.66). The obtained 95% serum reference range was estimated as 0.02 to 0.28 mmol/l βOHB. Conclusions: The Ranbut βOHB assay showed good precision and analytical performance. Our results confirm that βOHB measurement in peripheral blood is indeed a preferable alternative to the semi-quantitative measurement of urine ketones

    A comparative study of Ural Owl Strix uralensis breeding season diet within its European breeding range, derived from nest box monitoring schemes

    ABSTRACT Capsule: Voles are the main prey of the Ural Owl Strix uralensis in Europe, with larger prey and higher prey diversity being positively associated with owl breeding performance. Aims: To assess the breeding diet and its influence on the breeding performance of the Ural Owl across a north–south gradient of its European range using nest box monitoring data. Methods: Comparable monitoring of nest boxes in different biogeographical regions of Europe (Finland, Latvia, Slovenia) and diet analysis from nest samples to assess the taxonomic and trait influence of prey on owl breeding performance in different environments. Results: High plasticity in the Ural Owl hunting behaviour under different prey availability conditions resulted in significant differences between regions and years. Voles formed the highest proportion of the diet in all studied regions. Owl brood size was positively associated by higher proportions of voles and mice in the diet, and with increasing proportions of seasonally available larger prey and consequently prey diversity. Brood size was negatively associated with the proportion of non-mammalian and predominantly forest-living prey. Conclusions: The study highlighted the importance of comparative studies of raptor ecology across their geographical ranges in different environmental conditions to reveal undiscovered patterns, which may go undetected when conducting studies at the regional scale only.Peer reviewe

    Rola nacięcia krocza w profilaktyce uszkodzeń podczas porodów pochwowych – wyniki z dwóch ośrodków europejskich

    Objectives: There is an ongoing debate regarding the routine versus restrictive use of episiotomy. The study aim was to investigate if episiotomy during vaginal deliveries can reduce both, the number and severity of genital lacerations. Material and methods: The study included all women who gave vaginal birth at AOU. “G. Martino” Messina (n=382) and the Clinic for Ob/Gyn Clinical Center of Serbia, Belgrade (n=4221) during 2011. Lacerations during birth were recorded and divided according to location and severity. Women with lacerations were subdivided into two groups: with or without medio-lateral episiotomy. We assessed potential risk factors for laceration: maternal age, parity, use of labor stimulants and epidural analgesia, participation in antenatal classes, fetal presentation, neonatal birth weight, and duration of the second stage of labor. Results: Older women had higher grade perineum or combined lacerations. Children with higher birth weight in occipito-posterior presentation caused higher grade lacerations. Performance of episiotomy was connected with fewer perineum and labial lacerations. There were no differences in laceration grade between patients with and without episiotomy. Assessed parameters proved to be good discriminating factors between lacerations sites. According to logistic regression, laceration site was the most important risk factor for laceration grade. Combined lacerations had the highest grade. Conclusions: Episiotomy can significantly reduce the number of genital lacerations, but it does not influence laceration grade. Advanced maternal age, higher parity, occipito-posterior presentation and fetal macrosomia can cause lacerations during vaginal birth. Therefore, we suggest analysis of maternal and fetal factors to prevent widespread genital lacerations.Cel: Trwa debata w prawie właściwego stosowania nacięcia krocza: regularne kontra ograniczone. Celem badania była ocena czy nacięcie krocza podczas porodu pochwowego może zredukować zarówno liczbę jak i ciężkość uszkodzeń krocza. Materiał i metoda: Do badania włączono wszystkie kobiety, które w 2011 roku urodziły drogą pochwową w ośrodku w AOU „G.Martino” Messina (n=382) i w Klinice Położniczo-Ginekologicznej w Serbii, w Belgradzie (n=4221). Uszkodzenia krocza podczas porodu zostały podzielone względem lokalizacji i ciężkości. Kobiety z uszkodzeniami podzielono na dwie podgrupy: z nacięciem i bez nacięcia pośrodkowo-bocznego krocza. Oceniono możliwe czynniki ryzyka uszkodzeń krocza: wiek matki, rodność, użycie stymulacji porodu, znieczulenie zewnątrzoponowe, uczestnictwo w szkole rodzenia, położenie płodu, masa urodzeniowa noworodka, czas trwania drugiej fazy porodu. Wyniki: Starsze kobiety miały wyższy stopień uszkodzenia krocza i bardziej złożone pęknięcia. Urodzenie dziecka z większą masą urodzeniową w ułożeniu potylicowym-tylnym powodowało wyższy stopień pęknięć krocza. Nacięcie krocza wiązało się z mniejszą ilością pęknięć krocza i warg sromowych. Nie zanotowano różnic w stopniu uszkodzenia krocza pomiędzy pacjentkami z i bez nacięcia krocza. Oceniane czynniki są przydatne w różnicowaniu miejsca uszkodzenia. Na podstawie regresji logistycznej, miejsce pęknięcia było najważniejszym czynnikiem ryzyka stopnia uszkodzenia krocza. Złożone uszkodzenia miały najwyższy stopień. Wnioski: Nacięcie krocza istotnie zmniejszało liczbę uszkodzeń krocza, lecz nie wpływało na ich stopień. Zaawansowany wiek matki, wyższa rodność, ułożenie potylicowe-tylne i makrosomia płodu mogą powodować pęknięcia krocza w trakcie porodu. Sugerujemy analizę matczynych i płodowych czynników ryzyka celem zapobiegania szerokim uszkodzeniom krocza w trakcie porodu

    An innovative tailored instructional design for computer programming courses in engineering

    Industry 4.0 and 5.0 topics are emerging fields and have seen rising demand recently. There is a critical need, on the other hand, for improved methods of instructing programming languages since a growing lack of student motivation during the pandemic has had a deleterious influence on the education of programmers. In this context, online/hybrid computer programming courses must be addressed with innovative solutions to support the field with well-educated professionals. In this paper, we present a case study to propose an innovative tailored instructional design for the online/hybrid learning environments for programming courses in engineering faculties. To develop the instructional design, the Kemp Instructional Design Model was followed. The instructional design is a result of the main outputs of the RECOM “Redesigning Introductory Computer Programming Using Innovative Online Modules” project, which aims to bridge the gap between the existing course design in programming courses and the needs of "Covid” and “post-Covid” generation students

    Alterations in osteoclast function and phenotype induced by different inhibitors of bone resorption - implications for osteoclast quality

    <p>Abstract</p> <p>Background</p> <p>Normal osteoclasts resorb bone by secretion of acid and proteases. Recent studies of patients with loss of function mutations affecting either of these processes have indicated a divergence in osteoclastic phenotypes. These difference in osteoclast phenotypes may directly or indirectly have secondary effects on bone remodeling, a process which is of importance for the pathogenesis of both osteoporosis and osteoarthritis. We treated human osteoclasts with different inhibitors and characterized their resulting function.</p> <p>Methods</p> <p>Human CD14 + monocytes were differentiated into mature osteoclasts using RANKL and M-CSF. The osteoclasts were cultured on bone in the presence or absence of various inhibitors: Inhibitors of acidification (bafilomycin A1, diphyllin, ethoxyzolamide), inhibitors of proteolysis (E64, GM6001), or a bisphosphonate (ibandronate). Osteoclast numbers and bone resorption were monitored by measurements of TRACP activity, the release of calcium, CTX-I and ICTP, as well as by counting resorption pits.</p> <p>Results</p> <p>All inhibitors of acidification were equally potent with respect to inhibition of both organic and inorganic resorption. In contrast, inhibition of proteolysis by E64 potently reduced organic resorption, but only modestly suppressed inorganic resorption. GM6001 alone did not greatly affect bone resorption. However, when GM6001 and E64 were combined, a complete abrogation of organic bone resorption was observed, without a great effect on inorganic resorption. Ibandronate abrogated both organic and inorganic resorption at all concentrations tested [0.3-100 ÎźM], however, this treatment dramatically reduced TRACP activity.</p> <p>Conclusions</p> <p>We present evidence highlighting important differences with respect to osteoclast function, when comparing the different types of osteoclast inhibitors. Each class of osteoclast inhibitors will lead to different alterations in osteoclast quality, which secondarily may lead to different bone qualities.</p

    Dietary reference values for sodium

    Following a request from the European Commission, the EFSA Panel&nbsp;on Nutrition, Novel Foods and Food Allergens (NDA) derived dietary reference values (DRVs) for sodium. Evidence from balance studies on sodium and on the relationship between sodium intake and health outcomes, in particular cardiovascular disease (CVD)-related endpoints and bone health, was reviewed. The data were not sufficient to enable an average requirement (AR) or population reference intake (PRI) to be derived. However, by integrating the available evidence and associated uncertainties, the Panel&nbsp;considers that a sodium intake of 2.0&nbsp;g/day represents a level of sodium for which there is sufficient confidence in a reduced risk of CVD in the general adult population. In addition, a sodium intake of 2.0&nbsp;g/day is likely to allow most of the general adult population to maintain sodium balance. Therefore, the Panel&nbsp;considers that 2.0&nbsp;g sodium/day is a safe and adequate intake for the general EU population of adults. The same value applies to pregnant and lactating women. Sodium intakes that are considered safe and adequate for children are extrapolated from the value for adults, adjusting for their respective energy requirement and including a growth factor, and are as follows: 1.1&nbsp;g/day for children aged 1\u20133&nbsp;years, 1.3&nbsp;g/day for children aged 4\u20136&nbsp;years, 1.7&nbsp;g/day for children aged 7\u201310&nbsp;years and 2.0&nbsp;g/day for children aged 11\u201317&nbsp;years, respectively. For infants aged 7\u201311&nbsp;months, an Adequate Intake (AI) of 0.2&nbsp;g/day is proposed based on upwards extrapolation of the estimated sodium intake in exclusively breast-fed infants aged 0\u20136&nbsp;months

    Impact of renal impairment on atrial fibrillation: ESC-EHRA EORP-AF Long-Term General Registry

    Background: Atrial fibrillation (AF) and renal impairment share a bidirectional relationship with important pathophysiological interactions. We evaluated the impact of renal impairment in a contemporary cohort of patients with AF. Methods: We utilised the ESC-EHRA EORP-AF Long-Term General Registry. Outcomes were analysed according to renal function by CKD-EPI equation. The primary endpoint was a composite of thromboembolism, major bleeding, acute coronary syndrome and all-cause death. Secondary endpoints were each of these separately including ischaemic stroke, haemorrhagic event, intracranial haemorrhage, cardiovascular death and hospital admission. Results: A total of 9306 patients were included. The distribution of patients with no, mild, moderate and severe renal impairment at baseline were 16.9%, 49.3%, 30% and 3.8%, respectively. AF patients with impaired renal function were older, more likely to be females, had worse cardiac imaging parameters and multiple comorbidities. Among patients with an indication for anticoagulation, prescription of these agents was reduced in those with severe renal impairment, p&nbsp;&lt;.001. Over 24&nbsp;months, impaired renal function was associated with significantly greater incidence of the primary composite outcome and all secondary outcomes. Multivariable Cox regression analysis demonstrated an inverse relationship between eGFR and the primary outcome (HR 1.07 [95% CI, 1.01–1.14] per 10&nbsp;ml/min/1.73&nbsp;m2 decrease), that was most notable in patients with eGFR &lt;30&nbsp;ml/min/1.73&nbsp;m2 (HR 2.21 [95% CI, 1.23–3.99] compared to eGFR ≥90&nbsp;ml/min/1.73&nbsp;m2). Conclusion: A significant proportion of patients with AF suffer from concomitant renal impairment which impacts their overall management. Furthermore, renal impairment is an independent predictor of major adverse events including thromboembolism, major bleeding, acute coronary syndrome and all-cause death in patients with AF
