    Extradigit Glomus Tumor Causing Abdominal Pain -A Case Report-

    Glomus tumors are small vascular tumors that are usually benign and rarely occur. They originate from glomus bodies and present in the reticular dermis. They are clinically distinguished by their small size and their ability to cause extreme pain. Most of these tumors are subungually located. However, atypical locations of the tumors sometimes cause misdiagnosis, particularly when the lesion is rarely reported. Therefore, we report a case of glomus tumor which presented with chronic abdominal pain, found in the abdominal wall that has never been reported before

    An evaluation of the integration of non-traditional learning tools into a community based breast and cervical cancer education program: The witness project of Buffalo

    BACKGROUND: Breast and cervical cancer continue to represent major health challenges for African American women. among Caucasian women. The underlying reasons for this disparity are multifactorial and include lack of education and awareness of screening and early detection. Traditional educational methods have enjoyed varied success in the African American community and spawned development of novel educational approaches. Community based education programs employing a variety of educational models have been introduced. Successful programs must train and provide lay community members with the tools necessary to deliver strong educational programs. METHODS: The Witness Project is a theory-based, breast and cervical cancer educational program, delivered by African American women, that stresses the importance of early detection and screening to improve survival and teaches women how to perform breast self examination. Implementing this program in the Buffalo Witness Project of Buffalo required several modifications in the curriculum, integration of non-traditional learning tools and focused training in clinical study participation. The educational approaches utilized included repetition, modeling, building comprehension, reinforcement, hands on learning, a social story on breast health for African American women, and role play conversations about breast and cervical health and support. RESULTS: Incorporating non-traditional educational approaches into the Witness Project training resulted in a 79% improvement in the number of women who mastered the didactic information. A seventy-two percent study participation rate was achieved by educating the community organizations that hosted Witness Project programs about the informed consent process and study participation. CONCLUSION: Incorporating non-traditional educational approaches into community outreach programs increases training success as well as community participation

    Short-term acclimation in adults does not predict offspring acclimation potential to hypoxia

    Abstract The prevalence of hypoxic areas in coastal waters is predicted to increase and lead to reduced biodiversity. While the adult stages of many estuarine invertebrates can cope with short periods of hypoxia, it remains unclear whether that ability is present if animals are bred and reared under chronic hypoxia. We firstly investigated the effect of moderate, short-term environmental hypoxia (40% air saturation for one week) on metabolic performance in adults of an estuarine amphipod, and the fitness consequences of prolonged exposure. We then reared the offspring of hypoxia-exposed parents under hypoxia, and assessed their oxyregulatory ability under declining oxygen tensions as juveniles and adults. Adults from the parental generation were able to acclimate their metabolism to hypoxia after one week, employing mechanisms typically associated with prolonged exposure. Their progeny, however, did not develop the adult pattern of respiratory regulation when reared under chronic hypoxia, but instead exhibited a poorer oxyregulatory ability than their parents. We conclude that species apparently hypoxia-tolerant when tested in short-term experiments, could be physiologically compromised as adults if they develop under hypoxia. Consequently, we propose that the increased prevalence of hypoxia in coastal regions will have marked effects in some species currently considered hypoxia tolerant

    The seasonal nitrogen cycle in Wilkinson Basin, Gulf of Maine, as estimated by 1-D biological model optimization

The definitive version was published in Journal of Marine Systems 78 (2009): 77-93, doi:10.1016/j.jmarsys.2009.04.001.The objective of this study was to fit a simple ecosystem model to climatological nitrogen cycle data in the Gulf of Maine, in order to calibrate the biological model for use in future 3-D modelling studies. First depth-dependent monthly climatologies of nitrate, ammonium, chlorophyll, zooplankton, detritus and primary production data from Wilkinson Basin, Gulf of Maine, were created. A 6-box nitrogen-based ecosystem model was objectively fitted to the data through parameter optimization. Optimization was based on weighted least squares with model-data misfits nondi- mensionalized by assigned uncertainties in the monthly climatological estimates. These uncertainties were estimated as the standard deviations of the raw data from the 6-meter and monthly bin averages. On average the model fits the monthly means almost within their assigned uncertainties. Several statistics are examined to assess model-data misfit. Pattern statistics such as the correlation coefficient lack practical significance when data errors are large relative to the signal, as in this application. Thus Taylor diagrams were not found to be useful. The RMSE and model bias normalized by the data error were found to be the most useful skill metrics as they indicate whether the model fits the data within its estimated error. The optimal simulated nitrogen cycle budgets are presented, as an estimate of the seasonal nitrogen cycle in Wilkinson Basin, and discussed in context of the available data.Wilkinson Basin has spring and fall phytoplankton blooms, and strong summer stratification with a deep chlorophyll maximum near 21 m, just above the nitracline. The mean euphotic zone depth is estimated to be 25 m, relatively constant with season. The model estimates annual primary production as 176 g C m−2 yr−1, annual new production as 71 g C m−2 yr−1 and sinking PON fluxes of 9.7 and 4.7 g N m−2 yr−1 at 24 and 198 m respectively.     Microbial neuraminidase induces a moderate and transient myelin vacuolation independent of complement system activation

    Aims: Some central nervous system pathogens express neuraminidase (NA) on their surfaces. In the rat brain, a single intracerebroventricular (ICV) injection of NA induces myelin vacuolation in axonal tracts. Here, we explore the nature, the time course, and the role of the complement system in this damage. Methods: The spatiotemporal analysis of myelin vacuolation was performed by optical and electron microscopy. Myelin basic protein-positive area and oligodendrocyte transcription factor (Olig2)-positive cells were quantified in the damaged bundles. Neuronal death in the affected axonal tracts was assessed by Fluoro-Jade B and anti-caspase-3 staining. To evaluate the role of the complement, membrane attack complex (MAC) deposition on damaged bundles was analyzed using anti-C5b9. Rats ICV injected with the anaphylatoxin C5a were studied for myelin damage. In addition, NA-induced vacuolation was studied in rats with different degrees of complement inhibition: normal rats treated with anti-C5-blocking antibody and C6-deficient rats. Results: The stria medullaris, the optic chiasm, and the fimbria were the most consistently damaged axonal tracts. Vacuolation peaked 7 days after NA injection and reverted by day 15. Olig2+ cell number in the damaged tracts was unaltered, and neurodegeneration associated with myelin alterations was not detected. MAC was absent on damaged axonal tracts, as revealed by C5b9 immunostaining. Rats ICV injected with the anaphylatoxin C5a displayed no myelin injury. When the complement system was experimentally or constitutively inhibited, NA-induced myelin vacuolation was similar to that observed in normal rats. Conclusion: Microbial NA induces a moderate and transient myelin vacuolation that is not caused either by neuroinflammation or complement system activation

    Accounting for prediction uncertainty when inferring subsurface fault slip

    This study lays the groundwork for a new generation of earthquake source models based on a general formalism that rigorously quantifies and incorporates the impact of uncertainties in fault slip inverse problems. We distinguish two sources of uncertainty when considering the discrepancy between data and forward model predictions. The first class of error is induced by imperfect measurements and is often referred to as observational error. The second source of uncertainty is generally neglected and corresponds to the prediction error, that is the uncertainty due to imperfect forward modelling. Yet the prediction error can be shown to scale approximately with the size of earthquakes and thus can dwarf the observational error, particularly for large events. Both sources of uncertainty can be formulated using the misfit covariance matrix, C_χ, which combines a covariance matrix for observation errors, C_d and a covariance matrix for prediction errors, C_p, associated with inaccurate model predictions. We develop a physically based stochastic forward model to treat the model prediction uncertainty and show how C_p can be constructed to explicitly account for some of the inaccuracies in the earth model. Based on a first-order perturbation approach, our formalism relates C_p to uncertainties on the elastic parameters of different regions (e.g. crust, mantle, etc.). We demonstrate the importance of including C_p using a simple example of an infinite strike-slip fault in the quasi-static approximation. In this toy model, we treat only uncertainties in the 1-D depth distribution of the shear modulus. We discuss how this can be extended to general 3-D cases and applied to other parameters (e.g. fault geometry) using our formalism for C_p. The improved modelling of C_p is expected to lead to more reliable images of the earthquake rupture, that are more resistant to overfitting of data and include more realistic estimates of uncertainty on inferred model parameters
