    Herschel/HIFI observations of IRC+10216: water vapor in the inner envelope of a carbon-rich AGB star

    We report the results of observations of ten rotational transitions of water vapor toward the carbon-rich AGB (asymptotic giant branch) star IRC+10216 (CW Leonis), carried out with Herschel's HIFI instrument. Each transition was securely detected by means of observations using the dual beam switch mode of HIFI. The measured line ratios imply that water vapor is present in the inner outflow at small distances (few x 1.E+14 cm) from the star, confirming recent results reported by Decin et al. from observations with Herschel's PACS and SPIRE instruments. This finding definitively rules out the hypothesis that the observed water results from the vaporization of small icy objects in circular orbits. The origin of water within the dense C-rich envelope of IRC+10216 remains poorly understood. We derive upper limits on the H2-17O/H2-16O and H2-18O/H2-16O isotopic abundance ratios of ~ 5.E-3 (3 sigma), providing additional constraints on models for the origin of the water vapor in IRC+10216.Comment: Accepted for publication in the Astrophysical Journal Letter

    Distribution and kinematics of the HCN(3-2) emission down to the innermost region in the envelope of the O-rich star W Hya

    We report high angular resolution observations of the HCN (3-2) line emission in the circumstellar envelope of the O-rich star W Hya with the Submillimeter Array. The proximity of this star allows us to image its molecular envelope with a spatial resolution of just ~40 AU, corresponding to about 10 times the stellar diameter. We resolve the HCN (3-2) emission and find that it is centrally peaked and has a roughly spherically symmetrical distribution. This shows that HCN is formed in the innermost region of the envelope (within ~10 stellar radii), which is consistent with predictions from pulsation-driven shock chemistry models, and rules out the scenario in which HCN forms through photochemical reactions in the outer envelope. Our model suggests that the envelope decreases steeply in temperature and increases smoothly in velocity with radius, inconsistent with the standard model for mass-loss driven by radiative pressure on dust grains. We detect a velocity gradient of ~5 km/s in the NW--SE direction over the central 40 AU. This velocity gradient is reminescent of that seen in OH maser lines, and could be caused by the rotation of the envelope or by a weak bipolar outflow.Comment: accepted for publication in ApJ

    Multi-criteria manufacturability indices for ranking high-concentration monoclonal antibody formulations

The need for high-concentration formulations for subcutaneous delivery of therapeutic monoclonal antibodies (mAbs) can present manufacturability challenges for the final ultrafiltration/diafiltration (UF/DF) step. Viscosity levels and the propensity to aggregate are key considerations for high-concentration formulations. This work presents novel frameworks for deriving a set of manufacturability indices related to viscosity and thermostability to rank high-concentration mAb formulation conditions in terms of their ease of manufacture. This is illustrated by analyzing published high-throughput biophysical screening data that explores the influence of different formulation conditions (pH, ions, and excipients) on the solution viscosity and product thermostability. A decision tree classification method, CART (Classification and Regression Tree) is used to identify the critical formulation conditions that influence the viscosity and thermostability. In this work, three different multi-criteria data analysis frameworks were investigated to derive manufacturability indices from analysis of the stress maps and the process conditions experienced in the final UF/DF step. Polynomial regression techniques were used to transform the experimental data into a set of stress maps that show viscosity and thermostability as functions of the formulation conditions. A mathematical filtrate flux model was used to capture the time profiles of protein concentration and flux decay behavior during UF/DF.     Probing the mass-loss history of AGB and red supergiant stars from CO rotational line profiles - II. CO line survey of evolved stars: derivation of mass-loss rate formulae

    We aim to (1) set up simple and general analytical expressions to estimate mass-loss rates of evolved stars, and (2) from those calculate estimates for the mass-loss rates of asymptotic giant branch (AGB), red supergiant (RSG), and yellow hypergiant stars in our galactic sample. Rotationally excited lines of CO are a very robust diagnostic in the study of circumstellar envelopes (CSEs). When sampling different layers of the CSE, observations of these molecular lines lead to detailed profiles of kinetic temperature, expansion velocity, and density. A state-of-the-art, nonlocal thermal equilibrium, and co-moving frame radiative transfer code that predicts CO line intensities in the CSEs of late-type stars is used in deriving relations between stellar and molecular-line parameters, on the one hand, and mass-loss rate, on the other. We present analytical expressions for estimating the mass-loss rates of evolved stellar objects for 8 rotational transitions of the CO molecule, apply them to our extensive CO data set covering 47 stars, and compare our results to those of previous studies. Our expressions account for line saturation and resolving of the envelope, thereby allowing accurate determination of very high mass-loss rates. We argue that, for estimates based on a single rotational line, the CO(2-1) transition provides the most reliable mass-loss rate. The mass-loss rates calculated for the AGB stars range from 4x10^-8 Msun/yr up to 8x10^-5 Msun/yr. For RSGs they reach values between 2x10^-7 Msun/yr and 3x10^-4 Msun/yr. The estimates for the set of CO transitions allow time variability to be identified in the mass-loss rate. Possible mass-loss-rate variability is traced for 7 of the sample stars. We find a clear relation between the pulsation periods of the AGB stars and their derived mass-loss rates, with a levelling off at approx. 3x10^-5 Msun/yr for periods exceeding 850 days.Comment: Accepted for publication by Astronomy and Astrophysics, 24 pages + 28 pages appendix, 20 figure

    End-diastolic wall thickness as a predictor of recovery of function in myocardial hibernation Relation to rest-redistribution Tl-201 tomography and dobutamine stress echocardiography

    AbstractOBJECTIVESThe study assessed whether end-diastolic wall thickness (EDWT), measured with echocardiography, is an important marker of myocardial viability in patients with suspected myocardial hibernation, and it compared this index to currently established diagnostic modalities of dobutamine stress echocardiography (DSE) and rest-redistribution thallium-201 (Tl-201) scintigraphy.BACKGROUNDBecause myocardial necrosis is associated with myocardial thinning, preserved EDWT may provide a simple index of myocardial viability that is readily available from the resting echocardiogram.METHODSAccordingly, 45 patients with stable coronary artery disease and ventricular dysfunction underwent rest 2D echocardiograms, DSE and rest-redistribution Tl-201 tomography before revascularization and a repeat resting echocardiogram ≄2 months later.RESULTSGlobal wall motion score index decreased from 2.38 ± 0.73 to 1.94 ± 0.82 after revascularization (p < 0.001). Thirty-eight percent of severely dysfunctional segments recovered resting function. Compared to segments without recovery of resting function, those with recovery had greater EDWT (0.94 ± 0.18 cm vs. 0.67 ± 0.22 cm, p ≀ 0.0001) and a higher Tl-201 uptake (78 ± 13% vs. 59 ± 21%; p < 0.0001). An EDWT >0.6 cm had a sensitivity of 94% and specificity of 48% for recovery of function. Similarly, a Tl-201 maximal uptake of ≄60% had a sensitivity of 91% and specificity of 50%. Receiver operating characteristic curves for prediction of recovery of regional and global function were similar for EDWT and maximum Tl-201 uptake. Combination of EDWT and any contractile reserve during DSE for recovery of regional function improved the specificity to 77% without a significant loss in sensitivity (88%).CONCLUSIONSEnd-diastolic wall thickness is an important marker of myocardial viability in patients with suspected hibernation, and it can predict recovery of function similar to Tl-201 scintigraphy. Importantly, a simple measurement of EDWT ≀0.6 cm virtually excludes the potential for recovery of function and is a valuable adjunct to DSE in the assessment of myocardial viability

    A new interdisciplinary treatment strategy versus usual medical care for the treatment of subacromial impingement syndrome: a randomized controlled trial

    BACKGROUND: Subacromial impingement syndrome (SIS) is the most frequently recorded shoulder disorder. When conservative treatment of SIS fails, a subacromial decompression is warranted. However, the best moment of referral for surgery is not well defined. Both early and late referrals have disadvantages – unnecessary operations and smaller improvements in shoulder function, respectively. This paper describes the design of a new interdisciplinary treatment strategy for SIS (TRANSIT), which comprises rules to treat SIS in primary care and a well-defined moment of referral for surgery. METHODS/DESIGN: The effectiveness of an arthroscopic subacromial decompression versus usual medical care will be evaluated in a randomized controlled trial (RCT). Patients are eligible for inclusion when experiencing a recurrence of SIS within one year after a first episode of SIS which was successfully treated with a subacromial corticosteroid injection. After inclusion they will receive injection treatment again by their general practitioner. When, after this treatment, there is a second recurrence within a year post-injection, the participants will be randomized to either an arthroscopic subacromial decompression (intervention group) or continuation of usual medical care (control group). The latter will be performed by a general practitioner according to the Dutch National Guidelines for Shoulder Problems. At inclusion, at randomization and three, six and 12 months post-randomization an outcome assessment will take place. The primary outcome measure is the patient-reported Shoulder Disability Questionnaire. The secondary outcome measures include both disease-specific and generic measures, and an economic evaluation. Treatment effects will be compared for all measurement points by using a GLM repeated measures analyses. DISCUSSION: The rationale and design of an RCT comparing arthroscopic subacromial decompression with usual medical care for subacromial impingement syndrome are presented. The results of this study will improve insight into the best moment of referral for surgery for SIS

    The inner wind of IRC+10216 revisited: New exotic chemistry and diagnostic for dust condensation in carbon stars

    Aims. We model the chemistry of the inner wind of the carbon star IRC+10216 and consider the effect of periodic shocks induced by the stellar pulsation on the gas to follow the non-equilibrium chemistry in the shocked gas layers. We consider a very complete set of chemical families, including hydrocarbons and aromatics, hydrides, halogens and phosphorous-bearing species. Derived abundances are compared to the latest observational data from large surveys and Herschel. Results. The shocks induce a non-equilibrium chemistry in the dust formation zone of IRC+10216 where the collision destruction of CO in the post-shock gas triggers the formation of O-bearing species (H2O, SiO). Most of the modelled abundances agree very well with the latest values derived from Herschel data on IRC+10216. Hydrides form a family of abundant species that are expelled into the intermediate envelope. In particular, HF traps all the atomic fluorine in the dust formation zone. Halogens are also abundant and their chemistry is independent of the C/O ratio of the star. Therefore, HCl and other Cl-bearing species should also be present in the inner wind of O-rich AGB or supergiant stars. We identify a specific region ranging from 2.5 R* to 4 R*, where polycyclic aromatic hydrocarbons form and grow. The estimated carbon dust-to-gas mass ratio derived from the mass of aromatics ranges from 1.2 x 10^(-3) to 5.8 x 10^{-3} and agrees well with existing observational values. The aromatic formation region is located outside hot layers where SiC2 is produced as a bi-product of silicon carbide dust synthesis. Finally, we predict that some molecular lines will show flux variation with pulsation phase and time (e.g., H2O) while other species will not (e.g., CO). These variations merely reflect the non-equilibrium chemistry that destroys and reforms molecules over a pulsation period in the shocked gas of the dust formation zone.Comment: 15 pages, 10 figures.     A molecular line survey of the extreme carbon star CRL 3068 at millimeter wavelengths

    We present the results of a molecular line survey of the extreme carbon star CRL 3068. The observations were carried out with the Arizona Radio Observatory (ARO) 12m telescope and the Heinrich Hertz Submillimeter Telescope (SMT) at the 2mm and 1.3mm atmospheric windows. The observations cover the frequency bands from 130--162 GHz and 219.5--267.5 GHz. The typical sensitivities achieved are T_R<15 mK and T_R<7 mK for the ARO 12m and SMT, respectively. Seventy two individual emission features belonging to 23 molecular species and isotopologues were detected. Only three faint lines remain unidentified. The species c-C3H, CH3CN, SiC2, and the isotopologues, C17O and C18O, HC15N, HN13C, C33S, C34S, 13CS, 29SiS, and 30SiS are detected in this object for the first time. Rotational diagram analysis is carried out to determine the column densities and excitation temperatures. The isotopic ratios of the elements C, N, O, S, and Si have also been estimated. The results are consistent with stellar CNO processing and suggest that CRL 3068 is more carbon rich than IRC+10216 and CIT 6. It is also shown that the chemical composition in CRL 3068 is somewhat different from that in IRC+10216 with a more extensive synthesis of cyclic and long-chain molecules in CRL 3068.     Differences in isoprene and monoterpene emissions from cold-tolerant eucalypt species grown in the UK

    The UK may be required to expand its bioenergy production in order to make a significant contribution towards the delivery of its ‘net zero’ greenhouse gas emissions target by 2050. However, some trees grown for bioenergy are emitters of volatile organic compounds (VOCs), including isoprene and terpenes, precursors in the formation of tropospheric ozone, an atmospheric pollutant, which require assessment to understand any consequent impacts on air quality. In this initial scoping study, VOC emission rates were quantified under UK climate conditions for the first time from four species of eucalypts suitable for growing as short-rotation forest for bioenergy. An additional previously characterised eucalypt species was included for comparison. Measurements were undertaken using a dynamic chamber sampling system on 2-3 year-old trees grown under ambient conditions. Average emission rates for isoprene, normalised to 30 °C and 1000 ÎŒmol m−2 s−1 PAR, ranged between 1.3 ÎŒg C gdw−1 h−1 to 10 ÎŒg C gdw−1 h−1. All the eucalypt species measured were categorised as ‘medium’ isoprene emitters (1–10 ÎŒg C gdw−1 h−1). Total normalised monoterpene emission rates were of similar order of magnitude to isoprene or approximately one order of magnitude lower. The composition of the monoterpene emissions differed between the species and major compounds included eucalyptol, α-pinene, limonene and ÎČ-cis-ocimene. The emission rates presented here contribute the first data for further studies to quantify the potential impact on UK atmospheric composition, if there were widespread planting of eucalypts in the UK for bioenergy purposes

    General practitioners apply the usual care for shoulder complaints better than expected – analysis of videotaped consultations

    BACKGROUND: The education and activation program (EAP) is a newly developed intervention to prevent the development of chronic shoulder complaints (SCs). Trained general practitioners (GPs) administer the EAP. The EAP addresses inadequate cognitions and maladaptive behavior related to the SCs. The effect of the EAP is evaluated in a randomized clinical trial. The aim of the present study is to use videotaped consultations to study (1) the performance of trained GPs administering the EAP and (2) the presence of key features of the EAP already embedded in usual care (UC). METHODS: Five trained GPs were videotaped while treating a standardized patient with EAP. Additionally, five GPs administering UC were videotaped. Two blinded observers evaluated the videotapes in relation to key features of the EAP which were scored on the EAP checklist. RESULTS: The mean total score on the EAP checklist was 4.7 (SD = 2.9) for the UC group and 7.1 (SD = 2.1) for the EAP group. Neither group reached a score higher than 8, which was considered to reflect an acceptable number of key EAP features. CONCLUSION: Our comparison of the presence of key features of EAP shows that the UC and EAP groups differed less than was expected. GPs in the UC group performed above expectation, with a mean total score of 4.7. Moreover, the low number of key features present in the EAP group may very well have led to a reduced effectiveness of the EAP.
