Introduction: The purpose of this study was to comparatively assess the two techniques of cleft palate repair i.e. Kriens intravelar veloplasty (IVV) and modified Furlow Palatoplasty (MFP) for post-operative fistula formation, wound dehiscence at suture line, nasal regurgitation, velopharyngeal insufficiency, soft palate lengthening and speech.Method: This prospective study was conducted on 60 patients having primary cleft palate.  They were assigned either to IVV group or MFP group randomly so that both the groups consisted of 30 patients each. The two groups were operated under general anesthesia. Measurements at the time of operation were made with the help of soft ruler and Castroviejo caliper. Follow up of patient's was done 1 week, 1 month, 3 month, 6 months and complication is present was noted. Five year post operatively speech was recorded and assessed by the speech language pathologist. Post-operative Nasoendoscopy was also performed to assess the velopharyngeal insufficiency.Result: The MFP group showed more percentage elongation of the soft palate and less incidence of post-operative palatal fistula formation than IVV group. Total speech scores were superior in MFP patients but the differences were less robust. Velopharyngeal incompetence was present in both groups but was less severe in MFP group than the IVV group. Conclusion: The MFP group showed comparatively superior results than the IVV group but required an increased surgical time. Therefore MFP can be used as an alternative technique for cleft palate repair

Recurrent pregnancy loss (RPL) is a significant clinical problem that may occur before the 20th week of gestation. There is no general consensus on how many consecutive abortions are considered as RPL. The goal of this study is to investigate the correlation between recurrent miscarriage (RM) and common polymorphisms in angiotensin-converting enzyme (ACE), plasminogen activator inhibitor 1 (PAI-1) and Methylenetetrahydrofolate Reductase (MTHFR) genes among women experiencing RM. The literature existing in different population was searched and based on these finding we conclude that polymorphism in either one of these genes may increase chances of miscarriage. KEYWORDS: Polymorphism; Recurrent pregnancy; Plasminogen activator inhibitor 1

Demonstrating the asymmetry of the Indian Ocean Dipole response in regional earth system model of CORDEX-SA

An accurate representation of the Indian Ocean Dipole (IOD) is crucial for the reliable projection of Indian summer monsoon rainfall, making it necessary to improve the understanding of the response of the IOD in the warming climate. For the first time, a high-resolution regional earth system model (RESM) over the CORDEX-SA domain is used to investigate the IOD characteristics. The model performance is evaluated in simulating the IOD and associated mechanism. RESM shows a good resemblance in simulating IOD phases (positive and negative). However, the systematic discrepancy is observed in magnitude. Additionally, RESM well represented the positive IOD's inter-event variability. For example, the stronger event dominated by significant cold anomalies over Sumatra with enhanced westward-extended while a moderate event shows weak cooling confined to the region of Sumatra. Additionally, RESM shows potential to distinguish the ENSO and non-ENSO years with more remarkable skill in representing the spatial pattern of SST over IOD region during non-ESNO years than ENSO years. The RESM realistically simulated the IOD amplitude with greater skill than CMIP5/6 models reported in the earlier studies, indicating reliability towards the projection of the Indian summer monsoon. The weaker IOD-ENSO relationship is caused by producing the more significant number of IOD during non-ENSO years. Despite this reliable fidelity, IOD's slightly earlier peak is driven by the early establishment of low-level equatorial easterly wind. This study provided valuable insight into the IOD's different phases, responsible forcings, and limitations of the RESM in accounting for the role of internal climate variability that can be useful for further improvement in the model physics

Objective: GenoType MTBDRplus line probe assay (LPA) is developed for performing drug susceptibility testing (DST) for Rifampicin (RIF) and isoniazid in sputum specimens from smear-positive pulmonary tuberculosis (TB) patients and revised national TB control Programme (RNTCP) has endorsed LPA for the diagnosis of multi drug resistant TB (MDR-TB). This study was conducted to assess the potential utility of LPA for MDR-TB patient management. Methods: MDR-TB suspects under RNTCP PMDT criteria C referred from different districts in Delhi state were included in the study January 2013 toDecember 2014. Sputum specimens found acid-fast bacilli positive by fluorescent microscopy were processed for LPA.Results: Out of 3062 specimens, 2055 (67.1%) MDR-TB suspects were read as positive and specimens from 1007 (32.9%) suspects were read as negative in sputum smear microscopy. Out of 2019 specimens valid LPA results, 1427 were found to be pan-sensitive, 280 were MDR-TB, 40 were RIF monoresistant, 183 were Isoniazid (INH) monoresistant, and 89 specimens were found negative for Mycobacterium tuberculosis. Conclusion: Routine use of LPA can substantially reduce the time to diagnosis of RIF and/or INH-resistant TB and can hence potentially enable earlier commencement of appropriate drug therapy and thereby facilitate prevention of further transmission of drug resistant strains. Keywords: Multi drug resistant tuberculosis, Line probe assay, Rifampicin, Isoniazid

Azimuthal anisotropy of charged jet production in root s(NN)=2.76 TeV Pb-Pb collisions

    Get PDF
We present measurements of the azimuthal dependence of charged jet production in central and semi-central root s(NN) = 2.76 TeV Pb-Pb collisions with respect to the second harmonic event plane, quantified as nu(ch)(2) (jet). Jet finding is performed employing the anti-k(T) algorithm with a resolution parameter R = 0.2 using charged tracks from the ALICE tracking system. The contribution of the azimuthal anisotropy of the underlying event is taken into account event-by-event. The remaining (statistical) region-to-region fluctuations are removed on an ensemble basis by unfolding the jet spectra for different event plane orientations independently. Significant non-zero nu(ch)(2) (jet) is observed in semi-central collisions (30-50% centrality) for 20 <p(T)(ch) (jet) <90 GeV/c. The azimuthal dependence of the charged jet production is similar to the dependence observed for jets comprising both charged and neutral fragments, and compatible with measurements of the nu(2) of single charged particles at high p(T). Good agreement between the data and predictions from JEWEL, an event generator simulating parton shower evolution in the presence of a dense QCD medium, is found in semi-central collisions.
