Evaluation of a community based oral rehydration programme in rural Bangladesh
Oral rehydration therapy is a proven method for preventing mortality from dehydration due to diarrhoea. In rural Bangladesh a community based programme, called the Oral Therapy Extension Programme (OTEP), has been teaching mothers how to make and use an ORT solution using local household salt (lobon) and unrefined brown sugar (gur). The programme has been organised by a national non-governmental organization called the Bangladesh Rural Advancement Committee (BRAC). This programme plans to visit 16 million households throughout the country by 1990 and 5 million households had been visited by December 1985. The results from an earlier evaluation of the First Phase of the programme were encouraging. A further more detailed evaluation, which concentrated on determining the usage and safety of homemade lobon-gur solution, was undertaken in 1984 and this thesis presents the findings. The literature has been reviewed on the problem of diarrhoeal diseases and on the discovery of ORT and its applications. Methodological issues in undertaking the evaluation of such programmes are then considered. This evaluation focussed on usage and safety and was carried out using two main approaches, firstly through an indepth village case study and then by using a household sample survey. The cultural perceptions and practices regarding diarrhoea were studied in the village case study, which revealed that to the population there were four types of illnesses which all had the symptoms of diarrhoea. The sample survey found the usage of the ORT ranged from 2% to 55% of episodes, depending on how usage is defined. The ability of mothers to prepare the ORT solution was also studied and this showed that the ability, although originally good, has declined over time after teaching. The policy and programme implications of these results are discussed and recommendations are presented
Do side-effects reduce compliance to iron supplementation? A study of daily- and weekly-dose regimens in pregnancy.
Side-effects of iron supplementation lead to poor compliance. A weekly-dose schedule of iron supplementation rather than a daily-dose regimen has been suggested to produce fewer side-effects, thereby achieving a higher compliance. This study compared side-effects of iron supplementation and their impact on compliance among pregnant women in Bangladesh. These women were assigned to receive either weekly doses of 2 x 60 mg iron (one tablet each Friday morning and evening) or a daily dose of 1 x 60 mg iron. Fifty antenatal care centres were randomly assigned to prescribe either a weekly- or a daily-supplementation regimen (86 women in each group). Side-effects were assessed by recall after one month of supplementation and used for predicting compliance in the second and third months of supplementation. Compliance was monitored using a pill bottle equipped with an electronic counting device that recorded date and time whenever the pill bottle was opened. Of five gastrointestinal side-effects (heartburn, nausea, vomiting, diarrhoea, or constipation) assessed, vomiting occurred more frequently in the weekly group (21%) than in the daily group (11%, p<0.05). Compliance (ratio between observed and recommended tablet intake) was significantly higher in the weekly-supplementation regimen (93%) than in the daily-supplementation regimen (61%, p<0.05). Overall, gastrointestinal side-effects were not significantly associated with compliance. However, the presence of nausea and/or vomiting reduced compliance in both the regimens-but only among women from the lower socioeconomic group. In conclusion, weekly supplementation of iron in pregnancy had a higher compliance compared to daily supplementation of iron despite a higher frequency of side-effects. The findings support the view that gastrointestinal side-effects generally have a limited influence on compliance, at least in the dose ranges studied. Efforts to further reduce side-effects of iron supplementation may not be a successful strategy for improving compliance and effectiveness of antenatal iron supplementation
Robust Concept Erasure via Kernelized Rate-Distortion Maximization
Distributed representations provide a vector space that captures meaningful
relationships between data instances. The distributed nature of these
representations, however, entangles together multiple attributes or concepts of
data instances (e.g., the topic or sentiment of a text, characteristics of the
author (age, gender, etc), etc). Recent work has proposed the task of concept
erasure, in which rather than making a concept predictable, the goal is to
remove an attribute from distributed representations while retaining other
information from the original representation space as much as possible. In this
paper, we propose a new distance metric learning-based objective, the
Kernelized Rate-Distortion Maximizer (KRaM), for performing concept erasure.
KRaM fits a transformation of representations to match a specified distance
measure (defined by a labeled concept to erase) using a modified
rate-distortion function. Specifically, KRaM's objective function aims to make
instances with similar concept labels dissimilar in the learned representation
space while retaining other information. We find that optimizing KRaM
effectively erases various types of concepts: categorical, continuous, and
vector-valued variables from data representations across diverse domains. We
also provide a theoretical analysis of several properties of KRaM's objective.
To assess the quality of the learned representations, we propose an alignment
score to evaluate their similarity with the original representation space.
Additionally, we conduct experiments to showcase KRaM's efficacy in various
settings, from erasing binary gender variables in word embeddings to
vector-valued variables in GPT-3 representations.Comment: NeurIPS 202
Unsupervised Opinion Summarization Using Approximate Geodesics
Opinion summarization is the task of creating summaries capturing popular
opinions from user reviews. In this paper, we introduce Geodesic Summarizer
(GeoSumm), a novel system to perform unsupervised extractive opinion
summarization. GeoSumm involves an encoder-decoder based representation
learning model, that generates representations of text as a distribution over
latent semantic units. GeoSumm generates these representations by performing
dictionary learning over pre-trained text representations at multiple decoder
layers. We then use these representations to quantify the relevance of review
sentences using a novel approximate geodesic distance based scoring mechanism.
We use the relevance scores to identify popular opinions in order to compose
general and aspect-specific summaries. Our proposed model, GeoSumm, achieves
state-of-the-art performance on three opinion summarization datasets. We
perform additional experiments to analyze the functioning of our model and
showcase the generalization ability of {\X} across different domains.Comment: Findings of EMNLP 202
The role of mothers-in-law in antenatal care decision-making in Nepal: a qualitative study
Background
Antenatal care (ANC) has been recognised as a way to improve health outcomes for pregnant women and their babies. However, only 29% of pregnant women receive the recommended four antenatal visits in Nepal but reasons for such low utilisation are poorly understood. As in many
countries of South Asia, mothers-in-law play a crucial role in the decisions around accessing health care facilities and providers. This paper aims to explore the mother-in-law’s role in (a) her daughter-in-law’s ANC uptake; and (b) the decision-making process about using ANC services in Nepal.
Methods
In-depth interviews were conducted with 30 purposively selected antenatal or postnatal mothers (half users, half non-users of ANC), 10 husbands and 10 mothers-in-law in two different (urban and rural) communities.
Results
Our findings suggest that mothers-in-law sometime have a positive influence, for example when encouraging women to seek ANC, but more often it is negative. Like many rural women of their generation, all mothers-in-law in this study were illiterate and most had not used ANC themselves. The main factors leading mothers-in-law not to support/ encourage ANC check ups were expectations regarding pregnant women fulfilling their household duties, perceptions that ANC was not beneficial based largely on their own past experiences, the scarcity of resources
under their control and power relations between mothers-in-law and daughters-in-law. Individual knowledge and social class of the mothers-in-law of users and non-users differed significantly, which is likely to have had an effect on their perceptions of the benefits of ANC.
Conclusion
Mothers-in-law have a strong influence on the uptake of ANC in Nepal. Understanding their role is important if we are to design and target effective community-based health promotion interventions. Health promotion and educational interventions to improve the use of ANC should target women, husbands and family members, particularly mothers-in-law where they control access to family resources
Unlocking community capability through promotion of self-help for health: experience from Chakaria, Bangladesh
Background
People’s participation in health, enshrined in the 1978 Alma Ata declaration, seeks to tap into community capability for better health and empowerment. One mechanism to promote participation in health is through participatory action research (PAR) methods. Beginning in 1994, the Bangladeshi research organization ICDDR,B implemented a project “self-help for health,” to work with existing rural self-help organizations (SHOs). SHOs are organizations formed by villagers for their well-being through their own initiatives without external material help. This paper describes the project’s implementation, impact, and reflective learnings.
Methods
Following a self-help conceptual framework and PAR, the project focused on building the capacity of SHOs and their members through training on organizational issues, imparting health literacy, and supporting participatory planning and monitoring. Quarterly activity reports and process documentation were the main sources of qualitative data used for this paper, enabling documentation of changes in organizational issues, as well as the number and nature of initiatives taken by the SHOs in the intervention area. Health and demographic surveillance system (HDSS) data from intervention and comparison areas since 1999 allowed assessment of changes in health indicators over time.
Results
Villagers and members of the SHOs actively participated in the self-help activities. SHO functionality increased in the intervention area, in terms of improved organizational processes and planned health activities. These included most notably in convening more regular meetings, identifying community needs, developing and implementing action plans, and monitoring progress and impact. Between 1999 and 2015, while decreases in infant mortality and increases in utilization of at least one antenatal care visit occurred similarly in intervention and comparison areas, increases in immunization, skilled birth attendance, facility deliveries and sanitary latrines were substantially more in intervention than comparison areas.
Conclusion
Building community capability by working with pre-existing SHOs, encouraging them to place health on their agendas, strengthening their functioning and implementation of health activities led to sustained improvements in utilization of services for over 20 years. Key elements underpinning success include efforts to build and maintain trust, ensuring social inclusion in project activities, and balancing demands for material resources with flexibility to be responsive to community needs
Broad targeting of resistance to apoptosis in cancer
Apoptosis or programmed cell death is natural way of removing aged cells from the body. Most of the anti-cancer therapies trigger apoptosis induction and related cell death networks to eliminate malignant cells. However, in cancer, de-regulated apoptotic signaling, particularly the activation of an anti-apoptotic systems, allows cancer cells to escape this program leading to uncontrolled proliferation resulting in tumor survival, therapeutic resistance and recurrence of cancer. This resistance is a complicated phenomenon that emanates from the interactions of various molecules and signaling pathways. In this comprehensive review we discuss the various factors contributing to apoptosis resistance in cancers. The key resistance targets that are discussed include (1) Bcl-2 and Mcl-1 proteins; (2) autophagy processes; (3) necrosis and necroptosis; (4) heat shock protein signaling; (5) the proteasome pathway; (6) epigenetic mechanisms; and (7) aberrant nuclear export signaling. The shortcomings of current therapeutic modalities are highlighted and a broad spectrum strategy using approaches including (a) gossypol; (b) epigallocatechin-3-gallate; (c) UMI-77 (d) triptolide and (e) selinexor that can be used to overcome cell death resistance is presented. This review provides a roadmap for the design of successful anti-cancer strategies that overcome resistance to apoptosis for better therapeutic outcome in patients with cancer
