Condutas legais e o comportamento na relação profissional/paciente utilizados pelos cirurgiões-dentistas atuantes na área de Ortodontia / Legal conduct and behavior in the professional/patient relationship used by dentists working in Orthodontics

Com a promulgação da Lei 8.078/90, o Direito do Consumidor ganhou fundamental relevância. Neste sentido, os cirurgiões-dentistas começaram a desenvolver relações profissionais de consumo com seus pacientes, onde a odontologia classifica-se como prestadora de serviços e o paciente o consumidor. Nem todos os profissionais fazem uso de contrato odontológico, porém, em sua grande maioria demonstram ter uma preocupação com a utilização de contratos nos procedimentos adotados. Esta revisão de literatura tem como objetivo principal o esclarecimento do tópico ‘responsabilidade civil de um profissional liberal, mais especificamente o Cirurgião-dentista/ortodontista, das sanções a que ele está sujeito no exercício de sua profissão perante à legislação atual, havendo breves comentários também do código de defesa do consumidor. Este código coloca o Cirurgião-Dentista como aquele que deve provar que se utilizou de todo o conhecimento e técnica para a execução de seu trabalho. Conclui-se então que os profissionais demonstram ter preocupação com processos jurídicos para prevenir complicações, no qual os cirurgiões- dentistas devem agir com cautela. Tão logo, é importante que sejam registrados todos os dados dos pacientes para respaldo físico e evitar possíveis problemas judiciais, bem como obterem total conhecimento das leis que regem o Direito do Consumidor

Investigating the role of symptom valorisation in tuberculosis patient delay in urban areas in Portugal

TB incidence has been decreasing in Portugal, but median patient delay (PD) has risen. Symptom valorisation may determine PD by influencing help-seeking behaviour. We aimed to analyse the association between symptom valorisation and PD, while characterising individuals who disregarded their symptoms. Methods: A cross-sectional study was conducted among TB patients in Lisbon and Oporto in 2019 - 2021. Subjects who delayed seeking care because they did not value their symptoms or thought these would go away on their own were considered to have disregarded their symptoms. PD was categorised using a 21-day cut-off, and a 30-day cut-off for sensitivity analysis. We estimated the effect of symptom valorisation on PD through a directed acyclic graph. Then, a multivariable regression analysis characterised patients that disregarded their symptoms, adjusting for relevant variables. We fitted Poisson regression models to estimate crude and adjusted prevalence ratios (PR). Results: The study included 75 patients. Median PD was 25 days (IQR 11.5-63.5), and 56.0% of participants had PD exceeding 21 days. Symptom disregard was reported by 38.7% of patients. Patients who did not value their symptoms had higher prevalence of PD exceeding 21 days compared to those who valued their symptoms [PR 1.59 (95% CI 1.05-2.42)]. The sensitivity analysis showed consistent point estimates but wider confidence intervals [PR 1.39 (95% CI 0.77-2.55)]. Being a smoker was a risk factor for symptom disregard [PR 2.35 (95% CI 1.14-4.82)], while living in Oporto [PR 0.35 (95% CI 0.16-0.75)] and having higher household incomes [PR 0.39 (95% CI 0.17-0.94)] were protective factors. Conclusions: These findings emphasise the importance of symptom valorisation in timely TB diagnosis. Patients who did not value their symptoms had longer PD, indicating a need for interventions to improve symptom recognition. Intersetorialidade e política pública: os resíduos sólidos urbanos em Castanhal - PA / Intersectoriality and public policy: urban solid waste in Castanhal - PA

O objetivo desta pesquisa é analisar a intersetorialidade das secretariais vinculadas ao saneamento básico no município de Castanhal-PA e a problemática do vazadouro a céu aberto. A obtenção dos dados primários referentes intersetorialidade, necessários para a realização da pesquisa, ocorreu por meio de visitas às secretarias direta e indiretamente envolvidos no saneamento básico do município para a aplicação de formulários semiestruturados: Secretaria Municipal de Meio Ambiente (SEMMA), Secretaria Municipal de Obras (SEMOB), Vigilância Sanitária (VISA), Secretaria de Saúde  (SSA), Secretaria de Planejamento Urbano (SPU), para levantamento de informações quanto a gestão e gerenciamento de RS em Castanhal-PA. Os dados obtidos indicaram, dentre as secretarias amostradas, apenas a SEMOB tem conhecimento do tempo de funcionamento do vazadouro. Todas as secretarias estão cientes da ilegalidade na utilização da área, que se dá pela ausência de licenciamento e forma inadequada de disposição final dos RSU. Além disso, não há conhecimento por 100% da amostragem sobre a delimitação da área e nem esclarecimentos sobre a expansão e utilização de novas áreas, demostrando que este procedimento é aleatório e ausente de critérios técnicos, dificultando uma ação das secretarias. Um dos principais fatores que corrobora para este entrave é a ausência do Plano Municipal de Gestão Integrada dos RSU (PMGIRS). Os dados obtidos junto às secretarias amostradas indicaram que não existem cooperativas, empresas recicladoras ou organizações de catadores em parceria com as secretarias para atuação na área do vazadouro a céu aberto. Também não há elaboração, acompanhamento, avaliação e atualização do Plano Diretor municipal. A problemática inerente a intersetorialidade no município de Castanhal-PA, está diretamente relacionada a atuação específica de cada secretaria sem articulação em conjunto com as demais. Tal fato é justificado pela fragmentação e distanciamento entre as unidades amostradas, o que dificulta o processo intersetorial e atenua o problema relacionados aos RS no município, elevando os gatos públicos e promovendo entraves socioambientais sem controles.

Pretende-se caracterizar a prestação de cuidados biomédicos em Angola durante a atividade da Companhia de Diamantes de Angola. Uma análise comparativa de políticas e práticas de saúde pública de vários atores coloniais, como os serviços de saúde da Companhia, sua congénere do Estado e outras empresas coloniais, revelará diferenças de investimento na saúde, isto é, instalações e pessoal de saúde, e tratamentos. Este escrutínio bem como as condições de vida iluminarão o carácter idiossincrático e central dos serviços de saúde da Companhia em termos de morbimortalidade em Angola, e a centralidade destes para as representações de um império cuidador

ATLANTIC EPIPHYTES: a data set of vascular and non-vascular epiphyte plants and lichens from the Atlantic Forest

Epiphytes are hyper-diverse and one of the frequently undervalued life forms in plant surveys and biodiversity inventories. Epiphytes of the Atlantic Forest, one of the most endangered ecosystems in the world, have high endemism and radiated recently in the Pliocene. We aimed to (1) compile an extensive Atlantic Forest data set on vascular, non-vascular plants (including hemiepiphytes), and lichen epiphyte species occurrence and abundance; (2) describe the epiphyte distribution in the Atlantic Forest, in order to indicate future sampling efforts. Our work presents the first epiphyte data set with information on abundance and occurrence of epiphyte phorophyte species. All data compiled here come from three main sources provided by the authors: published sources (comprising peer-reviewed articles, books, and theses), unpublished data, and herbarium data. We compiled a data set composed of 2,095 species, from 89,270 holo/hemiepiphyte records, in the Atlantic Forest of Brazil, Argentina, Paraguay, and Uruguay, recorded from 1824 to early 2018. Most of the records were from qualitative data (occurrence only, 88%), well distributed throughout the Atlantic Forest. For quantitative records, the most common sampling method was individual trees (71%), followed by plot sampling (19%), and transect sampling (10%). Angiosperms (81%) were the most frequently registered group, and Bromeliaceae and Orchidaceae were the families with the greatest number of records (27,272 and 21,945, respectively). Ferns and Lycophytes presented fewer records than Angiosperms, and Polypodiaceae were the most recorded family, and more concentrated in the Southern and Southeastern regions. Data on non-vascular plants and lichens were scarce, with a few disjunct records concentrated in the Northeastern region of the Atlantic Forest. For all non-vascular plant records, Lejeuneaceae, a family of liverworts, was the most recorded family. We hope that our effort to organize scattered epiphyte data help advance the knowledge of epiphyte ecology, as well as our understanding of macroecological and biogeographical patterns in the Atlantic Forest. Taking the pulse of Earth's tropical forests using networks of highly distributed plots

Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science. Portuguese recommendations for the use of methotrexate in the treatment of rheumatoid arthritis

Objectives:To develop Portuguese evidence-based recommendations for the use of methotrexate (MTX) in daily clinical practice in rheumatic disorders. Methods:The Portuguese project was integrated in the multinational 3E Initiative (Evidence, Expertise, Exchange) 2007-2008 where a total of 751 rheumatologists from 17 countries have participated. Ten clinical questions concerning the use of MTX in rheumatic diseases were formulated and the Portuguese group added three more questions. A systematic literature search in Medline, Embase, Cochrane Library and 2005-2007 ACR/EULAR meeting abstracts was conducted. Selected articles were systematically reviewed and the evidence was appraised according to the Oxford Levels of Evidence. In Portugal, a national meeting was held in Obidos on February 15 th and 16 th, 2008, involving 50 rheumatologists who discussed and voted by Dephi method the recommendations. Finally, the agreement among the rheumatologists and the potential impact on their clinical practice was assessed. Results: Thirteen national key recommendations on the use of MTX were formulated: work-up before starting MTX, optimal dosage and route of administration, use of folic acid, monitoring, management of hepatotoxicity, long-term safety, mono versus combination therapy, management in the perioperative period, during infections, before/during pregnancy and after clinical remission, screening and treatment of tuberculosis and the role of MTX as a steroid-sparing agent in rheumatic diseases. Discussion: The Portuguese recommendations for the use of MTX in daily clinical practice were developed, which are evidence-based and supported by a panel of 50 rheumatologists, enhancing their validity and practical use.
