    Acupuncture in shoulder pain and functional impairment after neck dissection: A prospective randomized pilot study

    OBJECTIVES/HYPOTHESIS: The efficacy of conventional physiotherapy and antiinflammatory/analgesic drugs in the management of shoulder pain and functional disability following neck dissection is often disappointing. Acupuncture is a safe and well-tolerated method. We report the results regarding our pilot trial of acupuncture versus conventional care in the management of postoperative shoulder pain and dysfunction after neck dissection. STUDY DESIGN: Pilot study. METHODS: Patients at a tertiary university center with chronic pain or dysfunction attributed to neck dissection were randomly assigned to either weekly acupuncture or usual care (eg., physical therapy, analgesia, and/or antiinflammatory drugs) for 5 consecutive weeks. The Constant-Murley score, a composite measure of pain, function, and activities of daily living, was the primary outcome measure. As secondary end point, The Neck Dissection Impairment Index (NDII) was used to quantify site-specific, self-reported quality of life (QOL). RESULTS: After randomization, 48 patients completed the study (23 and 25 patients on acupuncture and control arms, respectively). Constant-Murley scores improved more in the acupuncture group (gain difference between groups 13.6, P < 0.01), a statistically significant improvement in site-specific QOL was also recorded at NDII (gain difference between groups 11.5, P < 0.01). CONCLUSION: Acupuncture is safe and effective; it should be introduced and offered to patients suffering from neck pain and dysfunction related to neck dissection

    Contribuciones teóricas de Sara Paín a la especificidad del campo psicopedagógico: obra e impacto en revistas nacionales

    Este estudio se propone analizar las contribuciones teóricas de Sara Paín a la especificidad del campo psicopedagógico. Específicamente, se propone explorar la producción de categorías psicopedagógicas en la obra de Paín así como determinar su impacto en artículos científicos y profesionales. Se realizó un estudio bibliográfico cualitativo de siete libros de Paín y cuantitativo de 662 artículos publicados en cuatro revistas argentinas de psicopedagogía. Los resultados acerca de la producción conceptual indicaron que la noción transversal es aprendizaje, definido como un proceso que permite la transmisión del conocimiento hacia un sujeto en devenir. Otras categorías, que manifiestan el interés de la autora en articular aportes de la psicología genética y el psicoanálisis, son:desarrollo, pensamiento, inconsciente, diagnóstico y tratamiento de los problemas de aprendizaje y estructura. Respecto al impacto de su obra, se hallaron diez artículos que citaron al menos una de las fuentes. Las implicancias son discutidas.We propose to analyze Sara Paín’s theoretical contributions to the specificity of psychopedagogical field. Specifically, we explore the production of psychopedagogical categories in Paín's work as well as we determine their impact on scientific and professional papers. We carried out a qualitative bibliographical analysis of seven books by Paín and a quantitative analysis of 662 articles published in four Argentine journals. The results about production indicated that learning is the main concept defined as a process that allows the transmission of knowledge towards a becoming subject. Other categories, which express the author's interest in articulating contributions of genetic psychology and psychoanalysis, are: development, thought, diagnosis and treatment of learning disabilities. Regarding the impact of his work on papers, 10 articles were found that cited at least one of Paín’s books. Implications are discussed.Fil: Giachetti, Estefanía.     Las malformaciones vasculares son enfermedades raras ocasionadas por errores en el desarrollo embriológico de los vasos sanguíneos. Estas lesiones, junto con los tumores vasculares, constituyen las anomalías vasculares. En el caso de las malformaciones vasculares, éstas se denominan de acuerdo al vaso que conforma la lesión y se clasifican según las características del flujo en malformaciones de bajo o alto flujo. Si bien la mayor parte son diagnosticadas según criterios clínicos, algunas lesiones requieren de estudios complementarios tanto para el establecimiento de un correcto diagnóstico como para la evaluación de su extensión. La ecografía Doppler suele ser el estudio inicial para conocer las características hemodinámicas y contribuir al diagnóstico diferencial entre un tumor y una malformación vascular. La resonancia magnética (RM) es el mejor método para evaluar la extensión de las lesiones y su relación con estructuras adyacentes. El diagnóstico preciso del tipo de malformación vascular es importante por su pronóstico y tratamiento diferentes. La Radiología Intervencionista constituye uno de los pilares fundamentales en el manejo terapéutico. Es altamente recomendable un abordaje multidisciplinario dada la complejidad de estas lesiones, particularmente en la elección de las opciones terapéuticas y en el seguimiento clínico de los pacientes.<br>Vascular malformations are rare conditions resulting from the abnormal development of blood vessels. These lesions, together with vascular tumors, represent the vascular anomalies. Vascular malformations are named according to which type of vessel is affected and are classified into high- or low-flow malformations on the basis of flow characteristics. Most vascular malformations are diagnosed according to clinical criteria. However, some require imaging studies to confirm the diagnosis or assess its extension. Doppler ultrasound is the usual initial study for hemodynamic assessment and for its contribution to differential diagnosis between vascular tumors and malformations. Magnetic resonance imaging (MRI) is the best technique for evaluating the extent of the lesions and their relationship with adjacent structures. An accurate diagnosis of the type of vascular malformation is of importance because of the different prognosis and treatment. Interventional radiology is one of the cornerstones in the therapeutic management. A multidisciplinary approach is highly recommended given the complexity of these lesions, particularly for selecting therapeutic options and for the patient clinical follow-up

    First report of predation by a small shark on the invasive rapa whelk Rapana venosa (Valenciennes, 1846) in Argentinean waters

    The rapa whelk Rapana venosa (Valenciennes, 1846) has few predators in areas outside of its native range. Here we report the first incidence of predation by a small shark Mustelus schmitti Springer, 1939 on the rapa whelk in coastal waters of Argentina.     Probabilistic Tsunami Hazard Analysis: Multiple Sources and Global Applications

    International audienceApplying probabilistic methods to infrequent but devastating natural events is intrinsicallychallenging. For tsunami analyses, a suite of geophysical assessments should be in principle evaluatedbecause of the different causes generating tsunamis (earthquakes, landslides, volcanic activity,meteorological events, and asteroid impacts) with varying mean recurrence rates. Probabilistic TsunamiHazard Analyses (PTHAs) are conducted in different areas of the world at global, regional, and local scaleswith the aim of understanding tsunami hazard to inform tsunami risk reduction activities. PTHAs enhanceknowledge of the potential tsunamigenic threat by estimating the probability of exceeding specificlevels of tsunami intensity metrics (e.g., run-up or maximum inundation heights) within a certain period oftime (exposure time) at given locations (target sites); these estimates can be summarized in hazard mapsor hazard curves. This discussion presents a broad overview of PTHA, including (i) sources and mechanismsof tsunami generation, emphasizing the variety and complexity of the tsunami sources and their generationmechanisms, (ii) developments in modeling the propagation and impact of tsunami waves, and (iii)statistical procedures for tsunami hazard estimates that include the associated epistemic and aleatoricuncertainties. Key elements in understanding the potential tsunami hazard are discussed, in light of therapid development of PTHA methods during the last decade and the globally distributed applications,including the importance of considering multiple sources, their relative intensities, probabilities ofoccurrence, and uncertainties in an integrated and consistent probabilistic framework
