    Techniques are described herein for improving wireless determinism by enhancing the Access Point (AP) join and client association processes. The AP join process may be enhanced by considering several factors to improve the determinism of wireless service. These factors include reliability, controller availability, network availability, and value-added services. Similarly, the client association process may also be enhanced by considering these factors to select a better AP / Service Set Identifier (SSID)

    Evaluation of burnout in medical interns: an institutional study

    Background: Burnout is a psychological syndrome characterised as state of emotional exhaustion, negative attitude towards the recipient of care (depersonalisation) and a feeling of low accomplishments in human service professionals. However, there is very limited literature about Burnout syndrome among medical interns from India. Thus, this study aimed to found out the prevalence of burnout syndrome among medical interns and to find any gender differences between male and female interns.Methods: The study was a cross sectional study with a convenient sample of 100 medical Interns. Subjects filled a pretested semi-structured questionnaire, which consisted of demographic data and measures of assessment. Burnout was assessed by application of Masalach Burnout Inventory. Definition of burnout was taken as high score (>27 points) on Emotional exhaustion subscale, along with high scores (>10 points) on depersonalization subscale and Low score (<33 points) on personal accomplishment subscale.Results: Mean age of the sample was 23.9 years. 48 % were males and 52% were females. Burnout syndrome was found to be present in 22% subjects. 34% scored High on Emotional exhaustion subscale, while 46% scored High on depersonalization subscale and 77% scored Low on personal accomplishment subscale. Burnout status and subscales scores did not vary significantly with gender.Conclusions: Considering the higher ratings on various domains of Burnout among medical interns there is a need for targeted action and further research for a healthy workforce development in field of Medical care

    NODE AND SERVICE DISCOVERY IN WIRELESS LOCAL AREA NETWORK CONTROLLER CLUSTER ARCHITECTURES USING HYPERLEDGER

    In the wireless cluster deployments, node discovery is a challenge. Further, it can be more challenging to discover the services running in such deployments. There are existing methods which use client side discovery and server side discovery that need a centralized Service Registry to maintain all available service instances. Presented herein are techniques that provide for the utilization of a private blockchain and HyperLedger in order to discover nodes and their services in wireless cluster deployments. Techniques of this proposal may provide for de‑centralizing node and service discovery in wireless cluster deployments without compromising authentication and security aspects. In one example, when a node comes up, it can authenticate itself with a Blockchain provider to be recognized as a legitimate node for a deployment. The node and services associated therewith would be added to the Ledger. The Ledger can be made available to all nodes in the deployment, which allows members to learn the node and service details and further communicate with the respective nodes for various services

    Evaluation of Physiological Parameters in Response to Endurance Exercise of Zanskar Ponies Adapted to High Altitude of Ladakh Region

    Zanskar pony, a native horse breed of Ladakh mainly used for transportation in Trans-Himalayan region of India, is well adapted to high altitude hypobaric hypoxia environment. Due to extreme conditions of the Ladakh region, better endurance of these ponies under hypoxic and extreme cold conditions is of utmost concern for their recruitment in Indian Army. In the present study, 12 young trained Zanskar ponies were evaluated during endurance exercise at an altitude of 3292 meter above mean sea level. The animals were subjected to carriage transport with 65-70Kg load or riding on a track of 5-6 Km. Physiological parameters viz., pulse rate (PR), heart rate (HR), rectal temperature (RT), respiratory rate (RR) and oxygen saturation (SaO2) were recorded in Zanskar ponies during pre-exercise (T0), post- exercise (T1) and post recovery (T2, 2 hours post resting) stages. Results showed marked increase in PR, HR, RR and RT post exercise time points. The mean values of PR increased from 49.83±4.62 to 73.67±21.54 per minute, HR from 48±13.60 to 75±15.82 beats/min, RR from 37.83±9.70 to 57.67±13.48 per min and RT from 99.62±0.34 101.04±0.53 °F from pre stress to post endurance stress. The mean SaO2 level reduced significantly (88.58±6.75 at T0 versus 64.00±18.70 at T1 and 54.42±14.79 at T2) post exercise. This indicated limited availability of arterial oxygen for tissues which could be vital factor for adverse change in some of physio-biochemical parameters. Though the trend of physiological response was similar for all the 12 animals, still variation at individual animal level was observed during endurance stress. In future, some of these physiological parameters along with biochemical and molecular parameters could be evaluated as potential biomarkers in selecting ponies with superior endurance trait specifically under hypoxic conditions

    Sequence Characterisation and Genotyping of Allelic Variants of Beta Casein Gene Establishes Native Cattle of Ladakh to be a Natural Resource for A2 Milk

    Bovine milk is regarded as nature's perfect food due to presence of vital nutrients. However some peptides are generated after proteolytic digestion of β-casein that have opioid properties and may increase the risk of chronic diseases. There are 13 genetic variants of bovine beta-casein; out of these A1 and A2 are the most common in dairy cattle breeds. The A1 and A2 variants differ only at position 67, which is histidine in A1 or proline in A2 milk. Earlier published reports have indicated that A1 β casein could be responsible for several health disorders like diabetes, coronary heart disease etc. while A2 β-casein is generally considered safe for human consumption. In the present study, an effort was made to sequence characterize β casein gene and identify allelic distribution of A1A2 alleles in native cattle of Ladakh region adapted to high altitude and low oxygen condition. The data showed 2 non-synonymous variations in coding region, while 5’UTR was completely conserved. The 3’UTR showed 2 more variations in Ladakhi samples. Further, the genotyping in 85 Ladakhi cattle for A1A2 alleles revealed that in Ladakhi cattle, A2 allele is predominantly present as reported for some of the other Indian breeds. The frequency of A2 allele was 0.90 and frequency of A2A2 genotype was found to be 0.79 in Ladakhi cattle. The present data strongly indicate that local cattle of Ladakh with higher frequency of A2 allele and A2A2 genotype is natural resource for A2 milk.  Systematic efforts should be made for long term conservation and genetic improvement of this invaluable genetic resource of Ladakh

    Global burden of 369 diseases and injuries in 204 countries and territories, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019

    Background: In an era of shifting global agendas and expanded emphasis on non-communicable diseases and injuries along with communicable diseases, sound evidence on trends by cause at the national level is essential. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) provides a systematic scientific assessment of published, publicly available, and contributed data on incidence, prevalence, and mortality for a mutually exclusive and collectively exhaustive list of diseases and injuries. Methods: GBD estimates incidence, prevalence, mortality, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs) due to 369 diseases and injuries, for two sexes, and for 204 countries and territories. Input data were extracted from censuses, household surveys, civil registration and vital statistics, disease registries, health service use, air pollution monitors, satellite imaging, disease notifications, and other sources. Cause-specific death rates and cause fractions were calculated using the Cause of Death Ensemble model and spatiotemporal Gaussian process regression. Cause-specific deaths were adjusted to match the total all-cause deaths calculated as part of the GBD population, fertility, and mortality estimates. Deaths were multiplied by standard life expectancy at each age to calculate YLLs. A Bayesian meta-regression modelling tool, DisMod-MR 2.1, was used to ensure consistency between incidence, prevalence, remission, excess mortality, and cause-specific mortality for most causes. Prevalence estimates were multiplied by disability weights for mutually exclusive sequelae of diseases and injuries to calculate YLDs. We considered results in the context of the Socio-demographic Index (SDI), a composite indicator of income per capita, years of schooling, and fertility rate in females younger than 25 years. Uncertainty intervals (UIs) were generated for every metric using the 25th and 975th ordered 1000 draw values of the posterior distribution. Findings: Global health has steadily improved over the past 30 years as measured by age-standardised DALY rates. After taking into account population growth and ageing, the absolute number of DALYs has remained stable. Since 2010, the pace of decline in global age-standardised DALY rates has accelerated in age groups younger than 50 years compared with the 1990–2010 time period, with the greatest annualised rate of decline occurring in the 0–9-year age group. Six infectious diseases were among the top ten causes of DALYs in children younger than 10 years in 2019: lower respiratory infections (ranked second), diarrhoeal diseases (third), malaria (fifth), meningitis (sixth), whooping cough (ninth), and sexually transmitted infections (which, in this age group, is fully accounted for by congenital syphilis; ranked tenth). In adolescents aged 10–24 years, three injury causes were among the top causes of DALYs: road injuries (ranked first), self-harm (third), and interpersonal violence (fifth). Five of the causes that were in the top ten for ages 10–24 years were also in the top ten in the 25–49-year age group: road injuries (ranked first), HIV/AIDS (second), low back pain (fourth), headache disorders (fifth), and depressive disorders (sixth). In 2019, ischaemic heart disease and stroke were the top-ranked causes of DALYs in both the 50–74-year and 75-years-and-older age groups. Since 1990, there has been a marked shift towards a greater proportion of burden due to YLDs from non-communicable diseases and injuries. In 2019, there were 11 countries where non-communicable disease and injury YLDs constituted more than half of all disease burden. Decreases in age-standardised DALY rates have accelerated over the past decade in countries at the lower end of the SDI range, while improvements have started to stagnate or even reverse in countries with higher SDI. Interpretation: As disability becomes an increasingly large component of disease burden and a larger component of health expenditure, greater research and developm nt investment is needed to identify new, more effective intervention strategies. With a rapidly ageing global population, the demands on health services to deal with disabling outcomes, which increase with age, will require policy makers to anticipate these changes. The mix of universal and more geographically specific influences on health reinforces the need for regular reporting on population health in detail and by underlying cause to help decision makers to identify success stories of disease control to emulate, as well as opportunities to improve.
