    Caracterização geológica e petrológica preliminar do vulcanismo ácido da região de Campos Novos, Santa Catarina

    Geological mapping on a scale of 1:50,000 was carried out in an area of 1800 km2, located in the central part of the Videira and Barracão Sheets in the Campos Novos, Celso Ramos and Anita Garibaldi municipalities, Santa Catarina. Five basic flows followed by six acidic units, similar to ignimbrites, were recognized and comprise a 500- to 600- m thick pile. Petrographic and major element geochemical data reveal the tholeitic character of the association and its relationship with low-titanium basalts and Palmas-type acidic rocks. Preliminary petrogenetic study suggests that the acid rocks originated by partial melting of a gabbroic parent which may be the intrusive equivalent of the basic flows of the study area.Trabalhos de mapeamento geológico em escala 1:50.000 foram realizados numa área de 1800 km2, localizada na região centro-sul do estado de Santa Catarina, cobrindo parte das folhas de Videira e Barracão, nos municípios de Campos Novos, Celso Ramos e Anita Garibaldi. Foram reconhecidos 5 derrames básicos sobrepostos por 6 unidades ácidas do tipo ignimbrito, perfazendo um pacote com espessura de 500 a 600 metros. Estudos petrográficos e litoquímicos de elementos maiores revelam o caráter toleítico da associação e sua correlação com os basaltos baixo-titânio e ácidas tipo Palmas. Um ensaio petrogenético preliminar sugere a origem das rochas ácidas por fusão parcial de uma fonte gabróica que pode ser o correspondente intrusivo dos derrames básicos registrados na área estudada

    Análise de colecistectomias videolaparoscópicas no Hospital de Clínicas de Porto Alegre

    OBJECTIVE: Laparoscopic cholecystectomy (LC) is the treatment of choice for cholelithiasis. Our objective was to assess the results of LC in comparison with previous data published in 1994.PATIENTS AND METHODS: From 1992 to 1999, 2,300 patients were submitted to surgery at the Hospital de Clínicas de Porto Alegre. The medical records of 1,540 of these patients were assessed retrospectively. The variables evaluated were preoperative diagnosis, duration of hospital stay and of surgery, intra- and postoperative complications, conversion rate to open surgery, and anatomicopathological examination of the gallbladder.RESULTS: The most common preoperative diagnosis was of symptomatic gallstones (92%); the average hospital stay was of 3.6 ± 6 days (median of 2 days); the average duration of surgery was of 89.5 ± 38 minutes; the most frequent intraoperative complications were gallbladder rupture (7.3%), calculi in peritoneum (0.8%), and iatrogenic bile duct injury (0.2%); and the postoperative complications reported were wound infection (1.3%), retained stones (0.6%), and biliary peritonitis (0.5%). The conversion rate to open procedure was of 2.5%, and reoperation was necessary in 1.8% of cases. There was only one death (0.06%).CONCLUSION: In comparison to data from a previous experience, there has been an improvement in LC results, but additional technical improvements can still be made. OBJETIVO: A colecistectomia videolaparoscópica (CVL) é o tratamento preferencial da colelitíase. O objetivo deste trabalho é avaliar os resultados da CVL comparando com a experiência inicial relatada em 1994.PACIENTES E MÉTODOS: De 1992 a 1999 foram operados 2.300 pacientes no Hospital de Clínicas de Porto Alegre. Deste total foram revisados retrospectivamente 1.540 prontuários. As variáveis analisadas foram a indicação cirúrgica, o tempo de internação hospitalar e cirúrgico, as complicações trans e pós-operatórias, a taxa de conversão para cirurgia aberta e o exame anatomopatológico da vesícula biliar.RESULTADOS: A principal indicação de cirurgia foi a colelitíase sintomática (92%). O tempo de internação hospitalar foi 3,6 ± 6 dias e a mediana de 2 dias e o tempo cirúrgico médio de 89,5 ± 38 minutos. As principais complicações intra-operatórias foram a perfuração da vesícula biliar (7,3%), a queda de cálculos na cavidade (0,8%) e lesão iatrogênica de via biliar (0,2%). No pós-operatório, as complicações mais freqüentes foram a infecção de ferida operatória (1,3%), a coledocolitíase residual (0,6%) e o coleperitônio (0,5%). A taxa de conversão foi de 2,5% e de reoperação de 1,8%. Houve apenas um óbito (0,06%).CONCLUSÕES: Em relação à experiência inicial, a CVL evoluiu muito, mas ainda pode ser aprimorada tecnicamente. &nbsp

    Effect of viral storm in patients admitted to intensive care units with severe COVID-19 in Spain: a multicentre, prospective, cohort study

    Background: The contribution of the virus to the pathogenesis of severe COVID-19 is still unclear. We aimed to evaluate associations between viral RNA load in plasma and host response, complications, and deaths in critically ill patients with COVID-19. Methods: We did a prospective cohort study across 23 hospitals in Spain. We included patients aged 18 years or older with laboratory-confirmed SARS-CoV-2 infection who were admitted to an intensive care unit between March 16, 2020, and Feb 27, 2021. RNA of the SARS-CoV-2 nucleocapsid region 1 (N1) was quantified in plasma samples collected from patients in the first 48 h following admission, using digital PCR. Patients were grouped on the basis of N1 quantity: VIR-N1-Zero ([removed]2747 N1 copies per mL). The primary outcome was all-cause death within 90 days after admission. We evaluated odds ratios (ORs) for the primary outcome between groups using a logistic regression analysis. Findings: 1068 patients met the inclusion criteria, of whom 117 had insufficient plasma samples and 115 had key information missing. 836 patients were included in the analysis, of whom 403 (48%) were in the VIR-N1-Low group, 283 (34%) were in the VIR-N1-Storm group, and 150 (18%) were in the VIR-N1-Zero group. Overall, patients in the VIR-N1-Storm group had the most severe disease: 266 (94%) of 283 patients received invasive mechanical ventilation (IMV), 116 (41%) developed acute kidney injury, 180 (65%) had secondary infections, and 148 (52%) died within 90 days. Patients in the VIR-N1-Zero group had the least severe disease: 81 (54%) of 150 received IMV, 34 (23%) developed acute kidney injury, 47 (32%) had secondary infections, and 26 (17%) died within 90 days (OR for death 0·30, 95% CI 0·16–0·55; p<0·0001, compared with the VIR-N1-Storm group). 106 (26%) of 403 patients in the VIR-N1-Low group died within 90 days (OR for death 0·39, 95% CI 0·26–0·57; p[removed]11 página

    Transcriptome-wide association study of breast cancer risk by estrogen-receptor status

    Previous transcriptome-wide association studies (TWAS) have identified breast cancer risk genes by integrating data from expression quantitative loci and genome-wide association studies (GWAS), but analyses of breast cancer subtype-specific associations have been limited. In this study, we conducted a TWAS using gene expression data from GTEx and summary statistics from the hitherto largest GWAS meta-analysis conducted for breast cancer overall, and by estrogen receptor subtypes (ER+ and ER-). We further compared associations with ER+ and ER- subtypes, using a case-only TWAS approach. We also conducted multigene conditional analyses in regions with multiple TWAS associations. Two genes, STXBP4 and HIST2H2BA, were specifically associated with ER+ but not with ER- breast cancer. We further identified 30 TWAS-significant genes associated with overall breast cancer risk, including four that were not identified in previous studies. Conditional analyses identified single independent breast-cancer gene in three of six regions harboring multiple TWAS-significant genes. Our study provides new information on breast cancer genetics and biology, particularly about genomic differences between ER+ and ER- breast cancer.Peer reviewe

    ¿Cómo evaluar e intervenir sobre la disfemia? Un análisis de percepciones de especialistas en logopedia

    La tartamudez se define como un trastorno de la fluidez del habla que afecta a la comunicación. Este trastorno, resulta complejo y puede suponer un importante desafío para muchos logopedas y otros profesionales. Hasta el momento, no abundan las investigaciones focalizadas acerca de cómo evaluar e intervenir en la tartamudez desde la perspectiva de los profesionales en logopedia. Precisamente por ello, el objetivo del presente estudio es conocer cuáles son los principales métodos de evaluación e intervención para la tartamudez que utilizan los logopedas en el contexto español y la satisfacción que tienen con éstos. Para ello, se administró una encuesta en la que participaron un total de 123 logopedas y, posteriormente, se efectuó un análisis cuantitativo de los datos obtenidos. Los resultados permitieron conocer los diferentes métodos de evaluación e intervención más ampliamente utilizados en el tratamiento de la tartamudez, aunque se han observado ciertas dificultades para alcanzar un consenso en la utilización de los mismos. Además, los logopedas reclaman la necesidad de confeccionar nuevos instrumentos de evaluación y de una mayor colaboración por parte de los centros escolares. Por tanto, resultaría primordial adoptar un enfoque holístico tratando de enseñar a los sujetos con tartamudez estrategias para afrontar la fluidez y, a la vez, aprender a convivir y manejar el trastorno
