    The structure of the title steroid [alternative name: 3,6- diacetoxy-5-methyl-19-norcholest-9(10)-ene], C31H50O4, confirms the generally accepted mechanism for the rearrangement of a cholestan-5�-ol derivative reported a century ago by Westphalen. The methyl group at position 10 of the starting material migrates to position 5 in the steroidal nucleus, while a 9 bond is formed, as indicated by the C C bond length of 1.347 (4)

    2-Aminobenzoxazole-appended coumarins as potent and selective inhibitors of tumour-associated carbonic anhydrases

    We have carried out the design, synthesis, and evaluation of a small library of 2-aminobenzoxazole-appended coumarins as novel inhibitors of tumour-related CAs IX and XII. Substituents on C-3 and/or C-4 positions of the coumarin scaffold, and on the benzoxazole moiety, together with the length of the linker connecting both units were modified to obtain useful structure-activity relationships. CA inhibition studies revealed a good selectivity towards tumour-associated CAs IX and XII (Ki within the mid-nanomolar range in most of the cases) in comparison with CAs I, II, IV, and VII (Ki > 10 µM); CA IX was found to be slightly more sensitive towards structural changes. Docking calculations suggested that the coumarin scaffold might act as a prodrug, binding to the CAs in its hydrolysed form, which is in turn obtained due to the esterase activity of CAs. An increase of the tether length and of the substituents steric hindrance was found to be detrimental to in vitro antiproliferative activities.     Tuning the activity of iminosugars: novel N-alkylated deoxynojirimycin derivatives as strong BuChE inhibitors

    We have designed unprecedented cholinesterase inhibitors based on 1-deoxynojirimycin as potential anti-Alzheimer’s agents. Compounds are comprised of three key structural motifs: the iminosugar, for interaction with cholinesterase catalytic anionic site (CAS); a hydrocarbon tether with variable lengths, and a fragment derived from 2-phenylethanol for promoting interactions with peripheral anionic site (PAS). Title compounds exhibited good selectivity towards BuChE, strongly depending on the substitution pattern and the length of the tether. The lead compounds were found to be strong mixed inhibitors of BuChE (IC50 = 1.8 and 1.9 µM). The presumptive binding mode of the lead compound was analysed using molecular docking simulations, revealing H-bond interactions with the catalytic subsite (His438) and CAS (Trp82 and Glu197) and van der Waals interactions with PAS (Thr284, Pro285, Asn289). They also lacked significant antiproliferative activity against tumour and non-tumour cells at 100 µM, making them promising new agents for tackling Alzheimer’s disease through the cholinergic approach.Dirección General de Investigación de España.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Taking the pulse of Earth's tropical forests using networks of highly distributed plots

    Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science.     Synthesis and Evaluation of Pyrimidine Steroids as Antiproliferative Agents

    A small and focused library of steroidal non-fused and fused pyrimidines was prepared from pregnenolone acetate and diosgenin, respectively. The key step was the cycloaddition reaction of nitrogen-containing 1,3-binucleophiles with the steroidal &alpha;,&beta;-unsaturated ketone. Urea, thiourea and guanidine reacted in a similar manner and afforded the steroidal pyrimidines in good yields. The antiproliferative tests against human tumor cell lines gave GI50 values in the micromolar range and had no effect on healthy fibroblasts. Additional experiments indicated that the compounds did not act as P-glycoprotein substrates, thus avoiding the rise of drug resistance. The fused steroidal pyrimidinethione was selected as drug lead for further testing due to its strong antiproliferative activities within the low micromolar range

    Valoración clínica y angiográfica de la reestenosis del stent coronario convencional

    Introduction: Ischemic heart disease is the leading cause of death in Cuba. Coronary angioplasty with stenting is an excellent treatment option, but restenosis overshadows its prognosis and is more common with the use of bare metal stents.Objective: To characterize restenosis after coronary bare-metal stent from the clinical and angiographic points of view.Method: A descriptive cross-sectional study was conducted in 59 patients with a diagnosis of restenosis after coronary bare-metal stent confirmed by angiography in the Interventional Cardiology Unit of Cardiocentro Ernesto Che Guevara of Villa Clara, Cuba, from February 2010 to April 2012. The variables analyzed were age, sex, coronary risk factors, previous history of angina or heart attack, vessel involved, type of restenosis and treated lesion, vessel diameter and lesion length.Results: Predominance of male patients (76.27 %), aged 60-69 years (40.66 %). The most common risk factors were hypertension (76.27 %), smoking (37.28 %) and type II diabetes mellitus (33.89 %). The most affected vessel was the left anterior descending (28.81 %), arterial diameters were predominantly ≤ 2.5 mm (54.2%) and lesion length > 20 mm (65, 97). Type B injuries (52.5) and focal restenosis (57.62 %) were more frequent.Conclusions: Patients with restenosis were predominantly male, between 60-69 years old, with a history of hypertension, smoking and diabetes mellitus type II. Left anterior descending disease, diameter ≤ 2.5 mm, lesion length > 20 mm, type B coronary lesion and focal restenosis were more frequent.Introducción: La cardiopatía isquémica es la principal causa de muerte en Cuba. La angioplastia coronaria con stent es una excelente opción terapéutica, pero la reestenosis ensombrece su pronóstico y es mucho más frecuente con el uso de stents convencionales.Objetivo: Caracterizar clínica y angiográficamente la reestenosis del stent coronario convencional.Método: Se realizó un estudio descriptivo y transversal en los 59 pacientes con diagnóstico de reestenosis del stent coronario convencional confirmado por angiografía en la Unidad de Cardiología Intervencionista del Cardiocentro “Ernesto Che Guevara” de Villa Clara, Cuba, durante el período febrero 2010 – abril 2012. Las variables analizadas fueron edad, sexo, factores de riesgo coronario, historia previa de angina o infarto, vaso afectado, tipo de reestenosis y de lesión tratada, diámetro del vaso y longitud de la lesión.Resultados: Predominaron los pacientes del sexo masculino (76,3 %), con edades comprendidas entre 60-69 años (40,7 %). Los factores de riesgo más frecuentes fueron la hipertensión arterial (76,3 %), el hábito de fumar (37,3 %) y la diabetes mellitus tipo II (33,9 %). El vaso más afectado fue la descendente anterior (59,3 %), los diámetros arteriales eran predominantemente ≤ 2,5 mm (54,2 %) y la longitud de las lesiones > 20 mm (66,1 %). Las lesiones tipo B (52,5 %) y la reestenosis focal (57,6 %) fueron más frecuentes.Conclusiones: Los pacientes con reestenosis eran predominantemente hombres, entre 60-69 años de edad, con antecedentes de hipertensión arterial, tabaquismo y diabetes mellitus tipo II. La enfermedad de la descendente anterior, el diámetro del vaso ≤ 2,5 mm, la longitud de las lesiones > 20 mm, la lesión coronaria tipo B y la reestenosis focal fueron más frecuentes
