Changes in compliance with school-based physical activity recommendations in Spanish youth: The UP&DOWN longitudinal study
The study aimed (a) to examine changes in physical activity (PA) during the whole day, school hours, recess, and physical education classes (PEC) during a 2-year period in primary and secondary students; (b) to identify changes in the proportion of compliance with specific PA recommendations for these periods; and (c) to examine whether PA levels at baseline are associated with PA levels 2 years later. Eight hundred and fourteen (51.8% boys) children and 658 (50.1% boys) adolescents from 41 Spanish schools participated in the study. Hip-worn accelerometers were used to assess PA during different time periods. Light PA (LPA) declined during the whole day, school hours, recess (all P < 0.001, except child girls for recess), and PEC (all, P < 0.05) in children and adolescents. Moderate-to-vigorous PA (MVPA) during the whole day and recess declined in child boys (P < 0.01 and P < 0.001, respectively) and adolescent boys (P < 0.001 and P < 0.05, respectively). MVPA during PEC declined in adolescent boys (P < 0.001) and adolescent girls (all P < 0.05). The proportion of compliance with the specific PA recommendations for these periods declined (P < 0.05), except for PEC in adolescent girls. PA during the whole day at baseline was moderately associated with PA during the whole day years later (ICCs = 0.210-0.544, with one exception), but this association was lower for the school-based PA. In conclusion, time spent in MVPA and LPA during the whole day and recess declined over time in child and adolescent boys and during PEC in adolescents. These findings highlight the need to promote PA interventions in these settings
Neck circumference and clustered cardiovascular risk factors in children and adolescents: Cross-sectional study
Objective Early detection of cardiovascular disease (CVD) risk factors, such as obesity, is crucial to prevent adverse long-term effects on individuals'' health. Therefore, the aims were: (1) to explore the robustness of neck circumference (NC) as a predictor of CVD and examine its association with numerous anthropometric and body composition indices and (2) to release sex and age-specific NC cut-off values to classify youths as overweight/obese. Design Cross-sectional study. Setting 23 primary schools and 17 secondary schools from Spain. Participants 2198 students (1060 girls), grades 1-4 and 7-10. Measures Pubertal development, anthropometric and body composition indices, systolic and diastolic blood pressure (SBP and DBP, respectively), cardiorespiratory fitness, blood sampling triglycerides (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-c), low-density lipoprotein cholesterol (LDL-c), glucose and inflammatory markers. Homoeostasis model assessment (HOMA-IR) and cluster of CVD risk factors were calculated. Results NC was positively correlated with all anthropometric and body composition indices. NC was negatively associated with maximum oxygen consumption (R 2 =0.231, p<0.001 for boys; R 2 =0.018, p<0.001 for girls) and positively associated with SBP, DBP, TC/HDL-c, TG, HOMA, complement factors C-3 and C-4, leptin, adiponectin and clustered CVD risk factor in both sexes (R 2 from 0.035 to 0.353, p<0.01 for boys; R 2 from 0.024 to 0.215, p<0.001 for girls). Moreover, NC was positively associated with serum C reactive protein, LDL-c and visfatin only in boys (R 2 from 0.013 to 0.107, p<0.05). Conclusion NC is a simple, low-cost and practical screening tool of excess of upper body obesity and CVD risk factors in children and adolescents. Paediatricians can easily use it as a screening tool for overweight/obesity in children and adolescents. For this purpose, sex and age-specific thresholds to classify children and adolescents as normal weight or overweight/obese are provided
Physical and psychological paths toward less severe fibromyalgia: A structural equation model

fibromyalgia severity. Integration of physical and psychological, experienced and observed, modifiable
factors associated with fibromyalgia severity in a single model will reveal therapeutic paths toward less
severity of disease. We aimed to examine an encompassing model of determinants of fibromyalgia
severity.
Methods: This observational, population-based cross-sectional study included 569 people with
fibromyalgia. An integrative model of fibromyalgia severity was tested by using structural equation
modelling. This model included 8 factors: resilience, catastrophizing, active lifestyle, declarative
memory, subjective fitness, objective fitness, psychological distress, and physical fatigue.
Results: Two core paths were associated with reduced fibromyalgia severity: 1) a psychological path
connecting high resilience and low catastrophizing with low distress and 2) a physical path, connecting a
more active lifestyle (directly and via high objective and subjective physical fitness) with low fatigue.
Additional interconnecting paths especially suggested a connection from the psychological to physical
path. Our model explained 83% of the fibromyalgia severity.
Conclusions: The present model integrated the complexity of mutually influencing factors of fibromyalgia
severity, which may help to better understand the disease. It emphasised the importance of: 1) physical
factors and psychological factors and their interconnections, 2) patients’ experiences and clinical
measurements, and 3) positive and negative signs such as physical fitness and distress. Future
longitudinal and experimental research should aim at testing the causal direction of the associations in
the model as well as the clinical implications suggested by the model. For instance, to reduce fatigue,
exercise should enhance not only objective fitness but also fitness-related perceptions. Reducing distress

Acute retroperitoneal bleeding due to inferior mesenteric artery aneurysm: Case report
<p>Abstract</p> <p>Background</p> <p>Visceral artery aneurysms (VAA), although uncommon, are increasingly being detected. We describe a case of spontaneous retroperitoneal hemorrhage from a ruptured IMA aneurysm associated with stenosis of the superior mesenteric artery (SMA) and celiac trunk, successfully treated with surgery.</p> <p>Methods</p> <p>A 65-year-old man presented with abdominal pain and hypovolemic shock. Abdominal CT scan showed an aneurysm of the inferior mesenteric artery with retroperitoneal hematoma. In addition, an obstructive disease of the superior mesenteric artery and celiac axis was observed.</p> <p>Results</p> <p>Upon emergency laparotomy a ruptured inferior mesenteric artery aneurysm was detected. The aneurysm was excised and the artery reconstructed by end-to-end anastomosis.</p> <p>Conclusions</p> <p>This report discusses the etiology, presentation, diagnosis and case management of inferior mesenteric artery aneurysms.</p
Objective and subjective measures of physical functioning in women with fibromyalgia: what type of measure is associated most clearly with subjective well-being?
To find modifiable factors that are related to subjective well-being would be valuable for improving
interventions in fibromyalgia. Physical activity, sedentary behaviour, and physical fitness may represent potential
areas to optimize treatment regimens. In fibromyalgia, there is a discordance between clinical observations and
patient-reported outcomes (objective and subjective assessments). Therefore, the present study aims at analyzing
the associations of objective and subjective evaluations of physical activity, sedentary behaviour, and physical
fitness with subjective well-being and determine if and how objective and subjective associations differ In the most conservative multivariate analysis, we found independent associations of the objective
measures of physical activity with positive affect and life satisfaction and sedentary behaviour with
positive affect. No such relationship was seen with subjective measures of the same behaviours.
Moreover, we observed that objective and subjective physical fitness evaluations were independent of
each other related to subjective well-being. Independent associations of the objectivemeasures (but not the subjective assessments) of physical
activity with positive affect and life satisfaction, and of sedentary behaviour with positive affect were observed.
However, objective measures and subjective appraisals of physical fitness appear to be independently related to

PRODIGE -- Envelope to disk with NOEMA I. A 3000 au streamer feeding a Class I protostar
Context. In the past few years, there has been a rise in the detection of
streamers, asymmetric flows of material directed toward the protostellar disk
with material from outside the star's natal core. It is unclear how they affect
the process of mass accretion, in particular beyond the Class 0 phase. Aims. We
investigate the gas kinematics around Per-emb-50, a Class I source in the
crowded star-forming region NGC 1333. Our goal is to study how the mass infall
proceeds from envelope to disk scales in this source. Results. We discover a
streamer delivering material toward Per-emb-50 in HCO and CO
emission. The streamer's emission can be well described by the analytic
solutions for an infalling parcel of gas along a streamline with conserved
angular momentum, both in the image plane and along the line of sight
velocities. The streamer has a mean infall rate of M yr, times higher than the current accretion rate of the
protostar. SO and SO emission reveal asymmetric infall motions in the inner
envelope, additional to the streamer around Per-emb-50. Furthermore, the
presence of SO could mark the impact zone of the infalling material.
Conclusions. The streamer delivers sufficient mass to sustain the protostellar
accretion rate and might produce an accretion burst, which would explain the
protostar's high luminosity with respect to other Class I sources. Our results
highlight the importance of late infall for protostellar evolution: streamers
might provide a significant amount of mass for stellar accretion after the

Fatigue in Women with Fibromyalgia: A Gene-Physical Activity Interaction Study
Fatigue is a cardinal symptom in fibromyalgia. Fatigue is assumed to be the result of genetic susceptibility and environmental factors. We aimed at examining the role of genetic susceptibility for fatigue in southern Spanish women with fibromyalgia, by looking at single nucleotide polymorphisms in 34 fibromyalgia candidate-genes, at the interactions between genes, and at the gene-physical activity interactions. We extracted DNA from saliva of 276 fibromyalgia women to analyze gene-polymorphisms. Accelerometers registered physical activity and sedentary behavior. Fatigue was assessed with the Multidimensional Fatigue Inventory. Based on the Bonferroni's and False Discovery Rate values, we found that the genotype of the rs4453709 polymorphism (sodium channel protein type 9 subunit alpha, SCN9A, gene) was related to reduced motivation (AT carriers showed the highest reduced motivation) and reduced activity (AA carriers showed the lowest reduced activity). Carriers of the heterozygous genotype of the rs1801133 (methylene tetrahydrofolate reductase, MTHFR, gene) or rs4597545 (SCN9A gene) polymorphisms who were physically active reported lower scores on fatigue compared to their inactive counterparts. Highly sedentary carriers of the homozygous genotype of the rs7607967 polymorphism (AA/GG genotype; SCN9A gene) presented more reduced activity (a dimension of fatigue) than those with lower levels of sedentary behavior. Collectively, findings from the present study suggest that the contribution of genetics and gene-physical activity interaction to fatigue in fibromyalgia is modest
Microarray profiling of mononuclear peripheral blood cells identifies novle candidate genes related to chemoradiation response in rectal cancer
Preoperative chemoradiation significantly improves oncological outcome in locally advanced rectal cancer. However there is no effective method of predicting tumor response to chemoradiation in these patients. Peripheral blood mononuclear cells have emerged recently as pathology markers of cancer and other diseases, making possible their use as therapy predictors. Furthermore, the importance of the immune response in radiosensivity of solid organs led us to hypothesized that microarray gene expression profiling of peripheral blood mononuclear cells could identify patients with response to chemoradiation in rectal cancer. Thirty five 35 patients with locally advanced rectal cancer were recruited initially to perform the study. Peripheral blood samples were obtained before neaodjuvant treatment. RNA was extracted and purified to obtain cDNA and cRNA for hybridization of microarrays included in Human WG CodeLink bioarrays. Quantitative real time PCR was used to validate microarray experiment data. Results were correlated with pathological response, according to Mandard´s criteria and final UICC Stage (patients with tumor regression grade 1–2 and downstaging being defined as responders and patients with grade 3–5 and no downstaging as non-responders). Twenty seven out of 35 patients were finally included in the study. We performed a multiple t-test using Significance Analysis of Microarrays, to find those genes differing significantly in expression, between responders (n = 11) and non-responders (n = 16) to CRT. The differently expressed genes were: BC 035656.1, CIR, PRDM2, CAPG, FALZ, HLA-DPB2, NUPL2, and ZFP36. The measurement of FALZ (p = 0.029) gene expression level determined by qRT-PCR, showed statistically significant differences between the two groups. Gene expression profiling reveals novel genes in peripheral blood samples of mononuclear cells that could predict responders and non-responders to chemoradiation in patients with locally advanced rectal cancer. Moreover, our investigation added further evidence to the importance of mononuclear cells’ mediated response in the neoadjuvant treatment of rectal cancer.This investigation was supported by the Fundación Investigación Biomédica Mutua Madrileña. Single hadron response measurement and calorimeter jet energy scale uncertainty with the ATLAS detector at the LHC
The uncertainty on the calorimeter energy response to jets of particles is
derived for the ATLAS experiment at the Large Hadron Collider (LHC). First, the
calorimeter response to single isolated charged hadrons is measured and
compared to the Monte Carlo simulation using proton-proton collisions at
centre-of-mass energies of sqrt(s) = 900 GeV and 7 TeV collected during 2009
and 2010. Then, using the decay of K_s and Lambda particles, the calorimeter
response to specific types of particles (positively and negatively charged
pions, protons, and anti-protons) is measured and compared to the Monte Carlo
predictions. Finally, the jet energy scale uncertainty is determined by
propagating the response uncertainty for single charged and neutral particles
to jets. The response uncertainty is 2-5% for central isolated hadrons and 1-3%
