    What is the relationship between people with dementia and their caregiver’s illness perceptions post diagnosis and the impact on help seeking behaviour?:A systematic review

    Background: As the number of people with dementia increases, more families will be affected by the daily challenges of providing effective support, given its current incurable status. Once individuals are diagnosed with dementia, the earlier they access support the more effective the outcome. However once people receive a diagnosis, how they make sense of their dementia can impact on their help seeking intentions. Exploring the illness beliefs of people with dementia and their caregivers and this relationship to help seeking may identify how best to facilitate early support. Aims: To systematically obtain and critically review relevant studies on the relationship between illness perceptions and help seeking of people with dementia and their caregivers. Method: A systematic search was conducted and included both quantitative and qualitative studies. The Initial search was conducted in October 2018, with an adjacent search conducted in April 2020. Findings: A total of 14 articles met the inclusion criteria. Conceptually the studies examined the association of illness perceptions and help seeking post diagnosis and revealed that people living with dementia and their caregivers sought help when symptoms became severe. Components of Illness perceptions revealed that lack of knowledge, cultural beliefs, complexity of the health care system, threat to independence and acceptance were identified as major factors for delaying help seeking. Conclusion: Although research interest in the area of illness perceptions and their impact on help seeking for dementia is increasing, further work is needed to understand this area, particularly regarding the influence of the relationship between the person with dementia and their caregiver

    The response of the upper ocean to solar heating. I: The mixed layer

    The results of two earlier papers on convection in the mixed layer and on the solar heating profile are here introduced into a one-dimensional model in order to investigate the following consequences of the daily cycle of solar heating in the upper ocean: 1. the daytime convection depth becomes less than the turbocline depth; 2. the convective power supply to turbulence in the mixed layer is reduced; 3. the mixed layer below the convection layer becomes stably stratified; 4. the depth of the turbocline is reduced, leaving a diurnal thermocline between it and the top of the seasonal thermocline; 5. the heat content and potential energy of the diurnal and seasonal thermoclines are increased, slowing down the subsequent nocturnal descent of the turbocline. These diurnal changes are illustrated by integrating a one-dimensional model forced by the astronomical cycle of solar heating and seasonal variation of surface meteorology derived from Bunker's climatology. The model is integrated for 18 months to show the seasonal modulation of the diurnal cycle. Nocturnal convection plays a dominant role. The convection depth closely follows the thermal compensation depth during the day when they are less than the turbocline depth. Integrating the model with a 24-hour time step leads to large errors in the seasonal variation of mixed layer temperature and depth, and in the source term of isopycnic potential vorticity. The errors are reduced by using two time steps per day, one for the daytime when convection is quenched, the other for the night when it is active. A novel parametrization based on tuning the daily equivalent solar elevation to surface temperature further reduces the error. This parametrization is used to investigate the sensitivity of the seasonal cycles of mixed layer depth and temperature to: (1) seasonality in the surface fluxes; (2) systematic changes in the net annual solar heating; (3) random changes in the seasonal cycles of solar heating induced (i) monthly and (ii) daily. The sensitivity to uncertainty in seawater turbidity is investigated in the same way. The profile of isopycnic potential vorticity subducted into the thermocline depends on the vernal correlation of mixed layer depth and density, so gyre circulation is sensitive to solar heating in spring

    Adiposity has differing associations with incident coronary heart disease and mortality in the Scottish population: cross-sectional surveys with follow-up

    Objective: Investigation of the association of excess adiposity with three different outcomes: all-cause mortality, coronary heart disease (CHD) mortality and incident CHD. Design: Cross-sectional surveys linked to hospital admissions and death records. Subjects: 19 329 adults (aged 18–86 years) from a representative sample of the Scottish population. Measurements: Gender-stratified Cox proportional hazards models were used to estimate hazard ratios (HRs) for all-cause mortality, CHD mortality and incident CHD. Separate models incorporating the anthropometric measurements body mass index (BMI), waist circumference (WC) or waist–hip ratio (WHR) were created adjusted for age, year of survey, smoking status and alcohol consumption. Results: For both genders, BMI-defined obesity (greater than or equal to30 kg m−2) was not associated with either an increased risk of all-cause mortality or CHD mortality. However, there was an increased risk of incident CHD among the obese men (hazard ratio (HR)=1.78; 95% confidence interval=1.37–2.31) and obese women (HR=1.93; 95% confidence interval=1.44–2.59). There was a similar pattern for WC with regard to the three outcomes; for incident CHD, the HR=1.70 (1.35–2.14) for men and 1.71 (1.28–2.29) for women in the highest WC category (men greater than or equal to102 cm, women greater than or equal to88 cm), synonymous with abdominal obesity. For men, the highest category of WHR (greater than or equal to1.0) was associated with an increased risk of all-cause mortality (1.29; 1.04–1.60) and incident CHD (1.55; 1.19–2.01). Among women with a high WHR (greater than or equal to0.85) there was an increased risk of all outcomes: all-cause mortality (1.56; 1.26–1.94), CHD mortality (2.49; 1.36–4.56) and incident CHD (1.76; 1.31–2.38). Conclusions: In this study excess adiposity was associated with an increased risk of incident CHD but not necessarily death. One possibility is that modern medical intervention has contributed to improved survival of first CHD events. The future health burden of increased obesity levels may manifest as an increase in the prevalence of individuals living with CHD and its consequences

    Psychological Predictors of Anxiety and Depression in Parkinson's Disease: A Systematic Review

    Objectives - Parkinson’s disease (PD) is a neurodegenerative disorder, affecting the motor system with psychological difficulties also frequently reported. While explanations for psychological difficulties are historically situated within a biomedical framework, more recently the relevance of psychological determinants has become a research focus. This review therefore examines this relationship with the two most commonly reported psychological difficulties (anxiety and depression) in people with PD. Methods - Databases were systematically searched up to 17 December 2013, identifying twenty-four studies meeting inclusion criteria. Results - Significant predictors of heightened anxiety and depression included: increased emotion-focused coping; less problem-focused coping; lower perceived control; more dominant beliefs about PD as part of a person’s identity, and influence on life; less social support and more avoidant personality types. Conclusions - Relationships between some specific psychological predictors and depression and anxiety seem well supported. The complexity of relationships between these psychological determinants should be taken into consideration when delivering psychological interventions

    The One with the Feminist Critique: Revisiting Millennial Postfeminism with Friends

    In the aftermath of its initial broadcast run, iconic millennial sitcom Friends (NBC, 1994–2004) generated some quality scholarship interrogating its politics of gender. But as a site of analysis, it remains a curious, almost structuring absence from the central canon of the first wave of feminist criticism of postfeminist culture. This absence is curious not only considering the place of Friends at the forefront of millennial popular culture but also in light of its long-term syndication in countries across the world since that time. And it is structuring in the sense that Friends was the stage on which many of the familiar tropes of postfeminism interrogated across the body of work on it appear in retrospect to have been tried and tested. This article aims to contribute toward redressing this absence through interrogation and contextualization of the series’ negotiation of a range of structuring tropes of postfeminist media discourse, and it argues for Friends as an unacknowledged ur-text of millennial postfeminism

    An in vitro study comparing a peripherally inserted central catheter to a conventional central venous catheter: no difference in static and dynamic pressure transmission

    <p>Abstract</p> <p>Background</p> <p>Early goal directed therapy improves survival in patients with septic shock. Central venous pressure (CVP) monitoring is essential to guide adequate resuscitation. Use of peripherally inserted central catheters (PICC) is increasing, but little data exists comparing a PICC to a conventional CVP catheter. We studied the accuracy of a novel PICC to transmit static and dynamic pressures <it>in vitro</it>.</p> <p>Methods</p> <p>We designed a device to generate controlled pressures via a column of water allowing simultaneous measurements from a PICC and a standard triple lumen catheter. Digital transducers were used to obtain all pressure readings. Measurements of static pressures over a physiologic range were recorded using 5Fr and 6Fr dual lumen PICCs. Additionally, random repetitive pressure pulses were applied to the column of water to simulate physiologic intravascular pressure variations. The resultant PICC and control waveforms were recorded simultaneously.</p> <p>Results</p> <p>Six-hundred thirty measurements were made using the 5 Fr and 6 Fr PICCs. The average bias determined by Bland-Altman plot was 0.043 mmHg for 5 Fr PICC and 0.023 mmHg for 6 Fr PICC with a difference range of 1.0 to -1.0. The correlation coefficient for both catheters was 1.0 (p-value < 0.001). Dynamic pressure waveforms plotted simultaneously between PICC and control revealed equal peaks and troughs.</p> <p>Conclusion</p> <p><it>In vitro</it>, no static or dynamic pressure differences were found between the PICC and a conventional CVP catheter. Clinical studies are required to assess whether the novel PICC has bedside equivalence to conventional catheters when measuring central venous pressures.</p

    Subaqueous basaltic magmatic explosions trigger phreatomagmatism: a case study from Askja, Iceland

    Sequences of basaltic pillow lavas that transition upwards with systematic gradation from pillow fragment breccias to fluidal bomb-bearing breccia to bomb-bearing lapilli tuffs are common at Askja volcano, Iceland. Based on the detailed textural investigation of three of these sequences, we argue that they record temporally continuous transition from effusive to explosive products that were erupted from and deposited at or near a single subaqueous vent. The recognition of such sequences is important as they provide evidence for controls on the onset of explosive activity in subaqueous environments. Such investigations are complicated by the interplay of magmatic gas expansion, phreatomagmatic and mechanical granulation fragmentation mechanisms in the subaqueous eruptive environment. All of the sequences studied at Askja have textural, componentry and sedimentological characteristics suggestive of a close genetic and spatial relationship between the pillow lavas and all of the overlying glassy clastic deposits. The identification of magma fragmentation signatures in pyroclasts was accomplished through detailed textural studies of pyroclasts within the full range of grain sizes of a given deposit i.e. bomb/blocks, lapilli and fine ash. These textural characteristics were compared and evaluated as discriminators of fragmentation in pyroclastic deposits. The presence of angular vitric clasts within the breccia and lapilli tuff displaying fragile glassy projections indicates little or no postdepositional textural modification. A shift in vesicle and clast textures between the pillow lavas and the large concentration of fluidal bombs in the breccia indicate that the phreatomagmatic explosions were initially triggered by magmatic vesiculation. The initial magmatic gas expansion may have been triggered by depressurization caused by the drainage of the ice-confined lake surrounding Askja. The Fuel Coolant Interactions (FCI) of the more efficient phreatomagmatic explosion was enabled by the increase in the surface area to volume ratio of the fluidal bombs in the water, producing a premix of magma and water. The onset and increasing influence of phreatomagmatic fragmentation is preserved in the presence of very fine blocky ash particles and diminished presence of larger particles such as fluidal bombs. The textural, sedimentological and environmental characteristics of these deposits suggest that phreatomagmatic explosions can be triggered by initial magmatic gas expansion, but that it is likely one of many mechanisms for triggering such explosions

    Internal Wave Turbulence Near a Texel Beach

    A summer bather entering a calm sea from the beach may sense alternating warm and cold water. This can be felt when moving forward into the sea (‘vertically homogeneous’ and ‘horizontally different’), but also when standing still between one’s feet and body (‘vertically different’). On a calm summer-day, an array of high-precision sensors has measured fast temperature-changes up to 1°C near a Texel-island (NL) beach. The measurements show that sensed variations are in fact internal waves, fronts and turbulence, supported in part by vertical stable stratification in density (temperature). Such motions are common in the deep ocean, but generally not in shallow seas where turbulent mixing is expected strong enough to homogenize. The internal beach-waves have amplitudes ten-times larger than those of the small surface wind waves. Quantifying their turbulent mixing gives diffusivity estimates of 10−4–10−3 m2 s−1, which are larger than found in open-ocean but smaller than wave breaking above deep sloping topography

    Does the stock market gender stereotype corporate boards? Evidence from the market's reaction to directors' trades

The definitive version is available at http://onlinelibrary.wiley.com/Attitudes towards male and female managers within organizations are well documented, but how the stock market perceives their relative capabilities is less studied. Recent evidence documents a negative short-run market reaction to the appointment of female chief executive officers and suggests that female executives are less informed than their male counterparts about future corporate performance. These results appear to dispute the stock market value of having women on corporate boards. However, such short-run market reactions may retain a ‘gender bias’, reflecting the prevalence of negative stereotypes, where the market reacts to ‘beliefs’ rather than ‘performance’. This study tests for such bias by examining the stock market reaction to directors' trades in their own companies' shares, by measuring both the short-run and longer-term returns after the directors' trades. Allowing for firm and trade effects, some evidence is found that, in the longer term, markets recognize that female executives' trades are informative about future corporate performance, although initially markets underestimate these effects. This has important implications for research that has attempted to assess the value of board diversity by examining only short-run stock market responses
