    Decay of escherichia coli in soil following the application of biosolids to agricultural land

    The decay of Escherichia coli in a sandy loam soil, amended with enhanced and conventionally treated biosolids, was investigated in a field experiment following spring and autumn applications of sewage sludge. Control soils, without the application of biosolids, were also examined to determine the background indigenous populations of E. coli which are present in the environment. The survival of indigenous E. coli and populations of E. coli applied to soil in biosolids, is assessed in relation to environmental factors influencing pathogen-decay processes in soil

    Comfort vs risk: a grounded theory about female adolescent behaviour in the sun.

    AIMS AND OBJECTIVES: To generate a grounded theory about female adolescent behaviour in the sun. BACKGROUND: Nurses have key roles in health promotion and skin cancer prevention. Adolescents' resistance to sun safety messages and their vulnerability to sunburn are of concern internationally. Understanding why young women do as they do in the sun may enhance skin cancer prevention, but their behaviour has not been explained before in the UK. DESIGN: The study incorporated a qualitative grounded theory design using the approach of Glaser. METHODS: Qualitative data were gleaned from group and one-to-one, semi-structured interviews with 20 female participants aged 14-17, research memos and literature. Sampling was purposive and theoretical. Data collection, analysis and theory generation occurred concurrently. Data were analysed using the constant comparative method. Data collection ended when a substantive theory had been generated. RESULTS: Data analysis revealed five categories of findings: fitting in, being myself, being physically comfortable, slipping up and being comfortable (the core category). The theory generated around the core explains how young women direct their sun-related activities towards meeting their physical and psychosocial comfort needs. CONCLUSIONS: A contribution of this research is the grounded theory explaining the behaviour of young women in the sun. Further, the theory challenges assumptions that female adolescents necessarily take risks; it explains their sun-related activities in terms of comfort. The theory extends findings from other researchers' descriptive qualitative studies and also appears to apply to young people in countries other than the UK. RELEVANCE TO CLINICAL PRACTICE: Understanding the sun-related activity of young women in terms of physical and psychosocial comfort may help nurses to develop new approaches to skin cancer prevention. These could complement existing messages and humanise health promotion

    Telecare motivational interviewing for diabetes patient education and support : a randomised controlled trial based in primary care comparing nurse and peer supporter delivery

    Background: There is increasing interest in developing peer-led and 'expert patient'-type interventions, particularly to meet the support and informational needs of those with long term conditions, leading to improved clinical outcomes, and pressure relief on mainstream health services. There is also increasing interest in telephone support, due to its greater accessibility and potential availability than face to face provided support. The evidence base for peer telephone interventions is relatively weak, although such services are widely available as support lines provided by user groups and other charitable services. Methods/Design: In a 3-arm RCT, participants are allocated to either an intervention group with Telecare service provided by a Diabetes Specialist Nurse (DSN), an intervention group with service provided by a peer supporter (also living with diabetes), or a control group receiving routine care only. All supporters underwent a 2-day training in motivational interviewing, empowerment and active listening skills to provide telephone support over a period of up to 6 months to adults with poorly controlled type 2 diabetes who had been recommended a change in diabetes management (i.e. medication and/or lifestyle changes) by their general practitioner (GP). The primary outcome is self-efficacy; secondary outcomes include HbA1c, total and HDL cholesterol, blood pressure, body mass index, and adherence to treatment. 375 participants (125 in each arm) were sought from GP practices across West Midlands, to detect a difference in self-efficacy scores with an effect size of 0.35, 80% power, and 5% significance level. Adults living with type 2 diabetes, with an HbA1c > 8% and not taking insulin were initially eligible. A protocol change 10 months into the recruitment resulted in a change of eligibility by reducing HbA1c to > 7.4%. Several qualitative studies are being conducted alongside the main RCT to describe patient, telecare supporter and practice nurse experience of the trial. Discussion and implications of the research: With its focus on self-management and telephone peer support, the intervention being trialled has the potential to support improved self-efficacy and patient experience, improved clinical outcomes and a reduction in diabetes-related complications

    Mapping natural capital: optimising the use of national scale datasets

    Understanding the spatial distribution of specific environmental variables and the interdependencies of these variables is crucial for managing the environment in a sustainable way. Here we discuss two methods of mapping – a Geographical Information System classification-based approach and a statistical model-based approach. If detailed, spatially comprehensive covariate datasets exist to complement the ecological-response data, then using a statistical model-based analysis provides the potential for greater understanding of underlying relationships, as well as the uncertainty in the spatial predictions. Further, the model-based approach facilitates scenario testing. Although similar methods are already adopted in species distribution modeling, the flexibility of the model framework used is rarely exploited to go beyond modeling occupancy or suitability for a single species, into modeling complex derived metrics such as community composition and indicators of natural capital. As an example, we assess the potential benefits of the statistical model-based approach to mapping natural capital through the use of two national survey datasets; The Centre for Ecology and Hydrology (CEH) Land Cover Map (LCM) and the British Geological Survey’s (BGS) Parent Material Model (PMM), to predict national soil microbial community distributions based on data from a sample of > 1000 soils covering Great Britain. The results are mapped and compared against a more traditional, land classification-based approach. The comparison shows that, although the maps look broadly similar, the model-based approach provides better overall spatial prediction, and the contribution of individual model terms (along with their uncertainty) are far easier to understand and interpret, whilst also facilitating any scenario testing. We therefore both recommend the use of spatial statistical modelling techniques to map natural capital and anticipate that they will become more prominent over the forthcoming years

    This paper will describe the developing relationship between Nottinghamshire Fire and Rescue Services and the two Higher Education Institutions in Nottingham. It will chronicle how a very traditional relationship has been transformed, initially by a simple consultancy project, into a much closer working relationship characterised by a much richer variety of collaborative projects. It demonstrates the potential mutual benefits that greater trust and reciprocity between the institutions can bring to both academia and to practice and the impact it has already had on curriculum development, teaching and learning in Nottingham

    Predicting the cost of the consequences of a large nuclear accident in the UK

    Nuclear accidents have the potential to lead to significant off-site effects that require actions to minimise the radiological impacts on people. Such countermeasures may include sheltering, evacuation, restrictions on the sale of locally-grown food, and long-term relocation of the population amongst others. Countries with nuclear facilities draw up emergency preparedness plans, and put in place such provisions as distributing instructions and iodine prophylaxis to the local population. Their plans are applied in simulated exercises on a regular basis. The costs associated with emergency preparedness and the safety provisions to reduce the likelihood of an accident, and/or mitigate the consequences, are justified on the basis of the health risks and accident costs averted. There is, of course, only limited actual experience to indicate the likely costs so that much of the costing of accidents is based on calculations. This paper reviews the methodologies used, in particular the approach that has been developed in the UK, to appraise the costs of a hypothetical nuclear accident. Results of analysing a hypothetical nuclear accident at a fictitious reactor site within the United Kingdom are discussed in relation to the accidents at Three Mile Island 2, Chernobyl and Fukushima Dai-ichi
