Factores asociados a las dificultades de conceptualización de la idea rectora en la fundamentación de los proyectos arquitectónicos

La presente investigación tuvo como objetivo determinar los factores asociados a las dificultades de conceptualización de la idea rectora en la fundamentación de los proyectos arquitectónicos por parte de los estudiantes de los talleres de diseño de la Escuela Profesional de Arquitectura de la UPAO, Filial Piura en el año 2018. Este estudio se desarrolló en el tipo no experimental y explicativo – causal con un diseño exposfacto de corte transversal (transeccional). De una población de 589 individuos, se trabajó con una muestra de 162 estudiantes participantes de los talleres de diseño I, II, III, IV, V, VI, VII, VIII y IX de la Escuela Profesional de Arquitectura de la UPAO, Filial Piura (28% del total poblacional); asimismo, se contó con la participación de docentes de talleres de diseño en la Escuela Profesional de Arquitectura y autoridades de la Escuela Profesional de Arquitectura. Los instrumentos trabajados para recolectar la información fueron: la Ficha de Autoevaluación de la Capacidad de Conceptualización de la Idea Rectora en la Fundamentación de Proyectos Arquitectónicos, el Cuestionario Estructurado de Factores Asociados a las Dificultades de Conceptualización de la Idea Rectora en la Fundamentación de Proyectos Arquitectónicos y una Entrevista Semi Estructurada. De acuerdo a los resultados, los factores asociados a las dificultades de conceptualización de la idea rectora en la fundamentación de los proyectos arquitectónicos por parte de los estudiantes de los talleres de diseño de la Escuela Profesional de Arquitectura de la UPAO, Filial Piura, son aquellos referidos a la formación profesional inicial, la motivación / vocación / compromiso docente, los objetivos y contenidos curriculares, los hábitos de investigación de los estudiantes, los tipos y estilos de aprendizaje estudiantil así como la relación docente – estudiante.The objective of this research was to determine the factors associated with the conceptualization difficulties of the guiding idea in the foundation of the architectural projects by the students of the design workshops of the Professional School of Architecture of the UPAO, Filial Piura in the 2018. This study was developed in the non-experimental and explanatory-causal type with a transverse design (transectional). From a population of 589 individuals, we worked with a sample of 162 students participating in the design workshops I, II, III, IV, V, VI, VII, VIII and IX of the Professional School of Architecture of UPAO, Filial Piura (28% of the total population); also, there was the participation of teachers of design workshops in the Professional School of Architecture and authorities of the Professional School of Architecture. The instruments used to collect the information were: the Self-Assessment Card of the Conceptualization Capacity of the Governing Idea in the Foundation of Architectural Projects, the Structured Questionnaire of Factors Associated with the Difficulties of Conceptualization of the Governing Idea in the Foundation of Architectural Projects and a Semi-Structured Interview. Análisis de la confiabilidad del sistema de molienda en una planta concentradora, basado en la criticidad

The aim of this study is to present the process of grinding the ore concentrator plant a mining company, describing the settings of your threads and performing the calculation of the milling system reliability based on the criticality. Also, the main indicators are calculated for each team maintenance grinding area. The results enable companies maintenance decisions applying the constant monitoring of its milling equipment, which are of daily use of operators, establishing the criticality of any team in general regarding the occurrence of failure.El objetivo del presente estudio es presentar el proceso de molienda en la planta concentradora de minerales de una empresa minera, describiendo las configuraciones de sus subprocesos y realizando el cálculo de la confiabilidad del sistema de molienda basado en la criticidad. Asimismo, se calculan los principales indicadores de mantenimiento por cada equipo del área de molienda. Los resultados facilitan a la empresa tomar decisiones de mantenimiento aplicando el monitoreo permanente de sus equipos de molienda, que son de uso cotidiano de los operarios, estableciendo la criticidad de cualquier equipo en general respecto a la ocurrencia de falla

IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients. Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes. Taking the pulse of Earth's tropical forests using networks of highly distributed plots

Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science. A Cohort of Patients with COVID-19 in a Major Teaching Hospital in Europe

BACKGROUND: Since the confirmation of the first patient infected with SARS-CoV-2 in Spain in January 2020, the epidemic has grown rapidly, with the greatest impact on the region of Madrid. This article describes the first 2226 adult patients with COVID-19, consecutively admitted to La Paz University Hospital in Madrid. METHODS: Our cohort included all patients consecutively hospitalized who had a final outcome (death or discharge) in a 1286-bed hospital of Madrid (Spain) from 25 February (first case admitted) to 19 April 2020. The data were manually entered into an electronic case report form, which was monitored prior to the analysis. RESULTS: We consecutively included 2226 adult patients admitted to the hospital who either died (460) or were discharged (1766). The patients’ median age was 61 years, and 51.8% were women. The most common comorbidity was arterial hypertension (41.3%), and the most common symptom on admission was fever (71.2%). The median time from disease onset to hospital admission was 6 days. The overall mortality was 20.7% and was higher in men (26.6% vs. 15.1%). Seventy-five patients with a final outcome were transferred to the intensive care unit (ICU) (3.4%). Most patients admitted to the ICU were men, and the median age was 64 years. Baseline laboratory values on admission were consistent with an impaired immune-inflammatory profile. CONCLUSIONS: We provide a description of the first large cohort of hospitalized patients with COVID-19 in Europe. Advanced age, male sex, the presence of comorbidities and abnormal laboratory values were more common among the patients with fatal outcomes
