Reszinkronizációs készülékkel élő betegek fizikai aktivitásának összehasonlító vizsgálata telemetriás adatok alapján
Absztrakt:
Bevezetés: A rendszeres fizikai aktivitás egészségre vonatkozó
hatása közismert, azonban több kutatás leírta, hogy szívbetegek számára is
nélkülözhetetlen, továbbá az ülő életmódot folytató szívbetegek számára
szekunder prevenciós hatását is kimutatták. Célkitűzés: Jelen
tanulmányunk célja annak meghatározása volt, hogy a szívelégtelen betegek
esetében telemetriás úton kapott physical activity százalékos érték miként
alakítható át MET-értékre, amellyel az egyén fizikai aktivitás során mért
oxigénfogyasztása jellemezhető. Módszer: A kutatásban 17
szívelégtelen beteg vett részt, akik közül 3 nő és 14 férfi; életkor 57,35 ±
9,54 év; testtömeg 98,71 ± 9,89 kg; átlagos BMI 36,69 ± 3,67 volt. A fizikai
aktivitást jellemző két adatsor felhasználásával lineáris regressziót végeztünk,
amelynek során egy matematikai egyenletet kaptunk, így a physical activity
százalékérték MET-értékké volt konvertálható. Eredmények: A
hatperces sétateszt alatt átlag 416,6 ± 48,2 m-t tettek meg a betegek és a mért
MET értékek átlaga 1,85 ± 0,18, a MET heti átlaga 1,12 ± 0,06 volt. Jól kivehető
hogy a betegek számára még a 6MWT is egy emelkedett aktivitást jelent a
mindennapok aktivitási szintjéhez képest. Következtetés:
Módszerünk alkalmazásával a betegek állapotváltozása a reszinkronizációs
készülékekbe beépített, fizikai aktivitást mérő mozgásérzékelő adatai alapján,
magával az implantált elektronikus eszközzel telemetriásan monitorozható. Orv

Abstract:
Introduction: The effect of regular physical activity on health
is widely recognized, but several studies have shown its key importance for
heart patients. Aim: The present study aimed to define the PA %
values, and to convert them into metabolic equivalent values (MET), which
describes oxygen consumption during physical activity. Method:
A total of seventeen patients with heart disease; 3 females and 14 males; age:
57.35 yrs ± 9.54; body mass 98.71 ± 9.89 kg; average BMI 36.69 ± 3.67 were
recruited into the study. The measured values from Cardiac Resynchronisation
Therapy devices and outer accelerometers (ActiGraph GT3X+) were studied over a
7-day time period. Using the two sets of values describing physical performance,
linear regression was calculated providing a mathematical equation, thus, the
Physical Activity values in percentage were converted into MET values.
Results: During the 6-minute walk test the patients achieved an
average of 416.6 ± 48.2 m. During 6MWT the measured values averaged at 1.85 ±
0.18 MET’s, and MET values averaged at 1.12 ± 0.06 per week. It clearly shows
that this test is a challenge for the patients compared to their daily regular
physical activity levels. Conclusion: With our method, based on
the values received from the physical activity sensor implanted into the
resynchronisation devices, changes in patients’ health status could be monitored
telemetrically with the assistance from the implanted electronic device. Orv

- 2002. május 23-25. között rendezte meg a régi magyar irodalmi kutatók soron következő országos konferenciáját.
Religió, retorika, nemzettudat: ez a három kulcsfogalom jelölte ki az ülésszak témáját. Mivel a reneszánsz és a barokk idején mind a vallási, mind a nemzeti tudat retorikusán
szerkesztett szövegekben testesült meg, fontos feladatnak látszott számbavenni azokat az elméleti szabályegyütteseket,
amelyek ekkor hatottak. Vajon volt-e összefüggés a vallásos közösségtudatot
kifejező formakincs és a korai nemzeti identitástudat szövegalkotó szokásai között? Miként került kapcsolatba
a hitéleti szenvedélyek retorikus megjelenítése és a tételes tanközvetítés nyelvi normája a nemzeti önismeret
formálódó alakzataival? Milyen műfajok és stilisztikai variációk, milyen antik tradíciók alakították a korai újkor retorikus
eljárásait? Ilyen kérdésekre kereste a választ az ülésszak, amely megemlékezett az egy évtizede elhunyt kiemelkedő tudósról, Klaniczay Tibor akadémikusról,
a kutatócsoport alapítójáról, tudományszakunk kiemelkedő praeceptoráról. Kötetünk az ülésszak előadásait adja közre, a három kulcsfogalom lehetséges összefüggéseit keresve, s remélve azt, hogy ilyen módon is ösztönzést adhat a korszak irodalmi műveltségének árnyaltabb megismeréséhez.Az Olvasóhoz, 9-14. The soluble guanylate cyclase activator cinaciguat prevents cardiac dysfunction in a rat model of type-1 diabetes mellitus
BACKGROUND: Diabetes mellitus (DM) leads to the development of diabetic cardiomyopathy, which is associated with altered nitric oxide (NO)-soluble guanylate cyclase (sGC)-cyclic guanosine monophosphate (cGMP) signalling. Cardioprotective effects of elevated intracellular cGMP-levels have been described in different heart diseases. In the current study we aimed at investigating the effects of pharmacological activation of sGC in diabetic cardiomyopathy. METHODS: Type-1 DM was induced in rats by streptozotocin. Animals were treated either with the sGC activator cinaciguat (10 mg/kg/day) or with placebo orally for 8 weeks. Left ventricular (LV) pressure-volume (P-V) analysis was used to assess cardiac performance. Additionally, gene expression (qRT-PCR) and protein expression analysis (western blot) were performed. Cardiac structure, markers of fibrotic remodelling and DNA damage were examined by histology, immunohistochemistry and TUNEL assay, respectively. RESULTS: DM was associated with deteriorated cGMP signalling in the myocardium (elevated phosphodiesterase-5 expression, lower cGMP-level and impaired PKG activity). Cardiomyocyte hypertrophy, fibrotic remodelling and DNA fragmentation were present in DM that was associated with impaired LV contractility (preload recruitable stroke work (PRSW): 49.5 +/- 3.3 vs. 83.0 +/- 5.5 mmHg, P < 0.05) and diastolic function (time constant of LV pressure decay (Tau): 17.3 +/- 0.8 vs. 10.3 +/- 0.3 ms, P < 0.05). Cinaciguat treatment effectively prevented DM related molecular, histological alterations and significantly improved systolic (PRSW: 66.8 +/- 3.6 mmHg) and diastolic (Tau: 14.9 +/- 0.6 ms) function. CONCLUSIONS: Cinaciguat prevented structural, molecular alterations and improved cardiac performance of the diabetic heart. Pharmacological activation of sGC might represent a new therapy approach for diabetic cardiomyopathy
Anti-calmodulins and Tricyclic Adjuvants in Pain Therapy Block the TRPV1 Channel
Ca2+-loaded calmodulin normally inhibits multiple Ca2+-channels upon dangerous elevation of intracellular Ca2+ and protects cells from Ca2+-cytotoxicity, so blocking of calmodulin should theoretically lead to uncontrolled elevation of intracellular Ca2+. Paradoxically, classical anti-psychotic, anti-calmodulin drugs were noted here to inhibit Ca2+-uptake via the vanilloid inducible Ca2+-channel/inflamatory pain receptor 1 (TRPV1), which suggests that calmodulin inhibitors may block pore formation and Ca2+ entry. Functional assays on TRPV1 expressing cells support direct, dose-dependent inhibition of vanilloid-induced 45Ca2+-uptake at µM concentrations: calmidazolium (broad range)≥trifluoperazine (narrow range)>chlorpromazine/amitriptyline>fluphenazine>>W-7 and W-13 (only partially). Most likely a short acidic domain at the pore loop of the channel orifice functions as binding site either for Ca2+ or anti-calmodulin drugs. Camstatin, a selective peptide blocker of calmodulin, inhibits vanilloid-induced Ca2+-uptake in intact TRPV1+ cells, and suggests an extracellular site of inhibition. TRPV1+, inflammatory pain-conferring nociceptive neurons from sensory ganglia, were blocked by various anti-psychotic and anti-calmodulin drugs. Among them, calmidazolium, the most effective calmodulin agonist, blocked Ca2+-entry by a non-competitive kinetics, affecting the TRPV1 at a different site than the vanilloid binding pocket. Data suggest that various calmodulin antagonists dock to an extracellular site, not found in other Ca2+-channels. Calmodulin antagonist-evoked inhibition of TRPV1 and NMDA receptors/Ca2+-channels was validated by microiontophoresis of calmidazolium to laminectomised rat monitored with extracellular single unit recordings in vivo. These unexpected findings may explain empirically noted efficacy of clinical pain adjuvant therapy that justify efforts to develop hits into painkillers, selective to sensory Ca2+-channels but not affecting motoneurons
Human keratinocytes are vanilloid resistant
BACKGROUND: Use of capsaicin or resiniferatoxin (RTX) as analgesics is an attractive therapeutic option. RTX opens the cation channel inflammatory pain/vanilloid receptor type 1 (TRPV1) permanently and selectively removes nociceptive neurons by Ca(2+)-cytotoxicity. Paradoxically, not only nociceptors, but non-neuronal cells, including keratinocytes express full length TRPV1 mRNA, while patient dogs and experimental animals that underwent topical treatment or anatomically targeted molecular surgery have shown neither obvious behavioral, nor pathological side effects. METHODS: To address this paradox, we assessed the vanilloid sensitivity of the HaCaT human keratinocyte cell line and primary keratinocytes from skin biopsies. RESULTS: Although both cell types express TRPV1 mRNA, neither responded to vanilloids with Ca(2+)-cytotoxicity. Only ectopic overproduction of TRPV1 rendered HaCaT cells sensitive to low doses (1-50 nM) of vanilloids. The TRPV1-mediated and non-receptor specific Ca(2+)-cytotoxicity ([RTX]>15 microM) could clearly be distinguished, thus keratinocytes were indeed resistant to vanilloid-induced, TRPV1-mediated Ca(2+)-entry. Having a wider therapeutic window than capsaicin, RTX was effective in subnanomolar range, but even micromolar concentrations could not kill human keratinocytes. Keratinocytes showed orders of magnitudes lower TRPV1 mRNA level than sensory ganglions, the bona fide therapeutic targets in human pain management. In addition to TRPV1, TRPV1b, a dominant negative splice variant was also noted in keratinocytes. CONCLUSION: TRPV1B expression, together with low TRPV1 expression, may explain the vanilloid paradox: even genuinely TRPV1 mRNA positive cells can be spared with therapeutic (up to micromolar) doses of RTX. This additional safety information might be useful for planning future human clinical trials
Hodgkin-lymphoma-derived cells show high sensitivity to dactinomycin and paclitaxel.
Depending on stage and risk factors, up to 30% of patients with advanced Hodgkin
lymphoma (HL) progress or relapse. Patients with pleural effusions have a
particularly poor prognosis and this stage of HL is regularly resistant to
chemotherapy. All currently available HL cell lines are derived from late stage
HL patients. In the present study we measured the sensitivity of these HL lines
against the 26 most frequently used cytostatic drugs. We used a novel fluorescent
short-term survival assay where the cell was incubated with the drugs for 4 days.
The precise number of differentially stained live and dead cells was determined
using a custom-built automated laser confocal fluorescent microscope. We found
that HL cells, independently of their origin, showed very similar sensitivity
patterns for several of the drugs. All HL cell lines were highly sensitive to
dactinomycin, paclitaxel and etoposide. Our data suggest that the inclusion of
dactinomycin and paclitaxel into chemotherapy protocols against late stage
Hodgkin lymphoma with pleural effusion may be justified
Is it Necessary to Perform Sentinel Lymph Node Biopsy in Thin Melanoma? A Retrospective Single Center Analysis
Sentinel lymph node biopsy (SLNB) is a standard procedure for regional lymph node staging and still has the most important prognostic value for the outcome of patients with thin melanoma. In addition to ulceration, SLNB had to be considered even for a single mitotic figure in thin (<1 mm) melanoma according to AJCC7th guideline, therefore, a retrospective review was conducted involving 403 pT1 melanoma patients. Among them, 152 patients suffered from pT1b ulcerated or mitotic rate ≥ 1/ mm2 melanomas according to the AJCC7th staging system. SLNB was performed in 78 cases, of which nine (11.5%) showed SLN positivity. From them, interestingly, we found a relatively high positive sentinel rate (6/78-8%) in the case of thin primary melanomas ˂0.8 mm. Moreover, the presence of regression increased the probability of sentinel positivity by 5.796 fold. After reassessing pT stage based on the new AJCC8th, 37 pT1b cases were reordered into pT1a category. There was no significant relation between other characteristics examined (age, gender, Breslow, Clark level, and mitosis index) and sentinel node positivity. Based on our data, we suggest that mitotic rate alone is not a sufficiently powerful predictor of SLN status in thin melanomas. If strict histopathological definition criteria are applied, regression might be an additional adverse feature that aids in identifying T1 patients most likely to be SLN-positive. After reassessing of pT1b cases according to AJCC8th regression proved to be independent prognostic factor on sentinel lymph node positivity. Our results propose that sentinel lymph node biopsy might also be considered at patients with regressive thin (˂0.8 mm) melanomas
