    Degradação fisica das áreas de entorno do reservatório da usina hidrelétrica Luiz Gonzaga/PE BRASIL

    Avaliaram-se as relações entre as propriedades físicas dos solos com erosão laminar e desbarracamento marginal em área próximas a Usina Hidrelétrica Luiz Gonzaga. Foram elencadas oito áreas A2, A3, A4, A11, A12, A13, A14 e A16, sendo o solo classificado como Neossolo Quartzarênico (RQ). Os solos das áreas experimentais e dos desbarrancamentos caracterizam-se por apresentar altos teores de areia, baixas quantidades de argila e silte. A intensidade de erosão laminar nas áreas experimentais foi classificada como nula a pequena e nos desbarrancamento marginais a erosão foi classificada como nula e pequena (A2, A4, A12, A13, A16) e média (A3, A11). O coeficiente de correlação entre argila e areia fina (r=0,50, p<0,05) foi significativo (Tabela 7). Em geral, foi constatado baixo grau de correlação linear entre a matéria orgânica e argila (r=0,45, p<0,05), matéria orgânica e areia fina (r=0,39, p<0,05) e argila e silte (r=0,30, p<0,05). Dessarte, os atributos físicos das unidades ambientais imprimem fragilidade, sobretudo no que tange a suscetibilidade à erosão e que aportam para o assoreamento do reservatório a longo prazo. Técnicas de bioengenharia são necessárias para atalhar o comprometimento da vida útil do reservatório

    Arcuate AgRP, but not POMC neurons, modulate paraventricular CRF synthesis and release in response to fasting

    Background: The activation of the hypothalamic–pituitary–adrenal (HPA) axis is essential for metabolic adaptation in response to fasting. However, the neurocircuitry connecting changes in the peripheral energy stores to the activity of hypothalamic paraventricular corticotrophin-releasing factor (CRFPVN) neurons, the master controller of the HPA axis activity, is not completely understood. Our main goal was to determine if hypothalamic arcuate nucleus (ARC) POMC and AgRP neurons can communicate fasting-induced changes in peripheral energy stores, associated to a fall in plasma leptin levels, to CRFPVN neurons to modulate the HPA axis activity in mice. Results: We observed increased plasma corticosterone levels associate with increased CRFPVN mRNA expression and increased CRFPVN neuronal activity in 36 h fasted mice. These responses were associated with a fall in plasma leptin levels and changes in the mRNA expression of Agrp and Pomc in the ARC. Fasting-induced decrease in plasma leptin partially modulated these responses through a change in the activity of ARC neurons. The chemogenetic activation of POMCARC by DREADDs did not affect fasting-induced activation of the HPA axis. DREADDs inhibition of AgRPARC neurons reduced the content of CRFPVN and increased its accumulation in the median eminence but had no effect on corticosterone secretion induced by fasting. Conclusion: Our data indicate that AgRPARC neurons are part of the neurocircuitry involved in the coupling of PVNCRF activity to changes in peripheral energy stores induced by prolonged fasting.Fil: Alves Fernandes, Alan Carlos.     Perfuração coronariana com tamponamento cardíaco: relato de caso

    Relato de caso clínico de paciente do sexo feminino, com 75 anos, hipertensa e ex-tabagista, evoluindo com angina estável e cansaço aos esforços menores que os habituais. Em cintilografia do miocárdio se evidenciou lesão isquêmica de aproximadamente 15% do miocárdio do ventrículo esquerdo. Foi admitida de forma eletiva em hospital terciário especializado em cardiologia clínica, para realizar angioplastia percutânea eletiva de lesão tipo C em Cx. Em procedimento, realizou-se a passagem de 2 guias através de lesão de 50% em terço proximal e 95% em terço médio de Cx e pré dilatação das lesões com balões. Posicionados 2 stents em overlapping, com perfuração Tipo III de Ellis no terço médio do vaso. Imediatamente, foi administrada Protamina 50 mg e realizado balonamento a baixa pressão na topografia da lesão em 3 momentos diferentes por até 30 minutos. Todavia, houve persistência de vazamento, optando-se pela passagem de segundo guia no terço distal do vaso e realizada técnica do cateter duplo-guia com implante de endoprótese a nível do vazamento, novamente não havendo sucesso na estabilização da lesão. Após 60 minutos, paciente apresentou sinais clínicos de instabilidade hemodinâmica e sintomas adrenérgicos, sendo evidenciada trombose difusa em Cx e presença de derrame pericádico associado a continuidade do extravasamento ao controle angiográfico. Imediatamente aventada a possibilidade de tamponamento cardíaco e realizado pericardiocentese com melhora imediata, porém com manutenção do extravasamento de contraste pela PC, necessitando de abordagem cirúrgica de emergência em sequência. Submetida a cirurgia de revascularização miocárdica com enxerto de veia safena para Cx e rafia de perfuração de ventriculo esquerdo. Evoluiu sem complicações significativas no pós-operatório, recebendo alta após 5 dias melhorada, assintomática, em ritmo sinusal

    PNEUMOLOGIA E SUAS EVIDÊNCIAS CLÍNICAS COM ÊNFASE NA DPOC: REVISÃO DE LITERATURA

    Chronic obstructive pulmonary disease (COPD) is considered the fourth cause of death in Brazil, a worrying scenario considering that only 10% of those diagnosed receive adequate treatment. The present review study sought to evaluate new advances in the management of COPD exacerbations, documented through clinical and randomized studies. This is an integrative review research carried out using the PubMed database, which took into account the following inclusion criteria: clinical trials and controlled and randomized tests; articles published in the last year; that had full text available and that addressed the management of COPD exacerbations. Xiyanping has been found to have the ability to inhibit the inflammatory response throughout the body and provide a significant synergistic effect when combined with antibiotics, resulting in improved lung function in elderly people with acute exacerbation of COPD. Furthermore, a reduction in airway reactivity and an increase in secretion clearance were observed. Patients with COPD who have high baseline FENO levels and have not been previously treated may benefit from the use of salmeterol/fluticasone instead of tiotropium, indicating that high FENO levels may predict a favorable response to ICS/LABA treatment. Finally, acupuncture has been shown to be an effective and safe adjunctive approach in the treatment of acute exacerbations of COPD, with patients reporting significant improvements.A doença pulmonar obstrutiva crônica (DPOC) é considerada a quarta causa de morte no Brasil, um cenário preocupante tendo em vista que apenas 10% dos que possuem o diagnóstico realizam o tratamento adequado. O presente estudo de revisão buscou avaliar novos avanços para o manejo das exacerbações da DPOC, documentados por meio de estudos clínicos e randomizados. Trata-se de uma pesquisa de revisão integrativa realizada por meio da base de dados PubMed, que levou em consideração os seguintes critérios de inclusão: ensaios clínicos e testes controlados e aleatórios; artigos publicados no último ano; que possuíam texto completo disponível e que abordassem acerca do manejo das exacerbações da DPOC. Verificou-se que o Xiyanping possui a capacidade de inibir a resposta inflamatória em todo o corpo e proporcionar um efeito sinérgico significativo quando combinado com antibióticos, resultando na melhora da função pulmonar em idosos com exacerbação aguda da DPOC. Além disso, foi observada uma redução na reatividade das vias aéreas e um aumento na eliminação de secreções. Pacientes com DPOC que apresentam altos níveis basais de FENO e não haviam sido tratados anteriormente podem se beneficiar do uso de salmeterol/fluticasona em vez de tiotrópio, indicando que altos níveis de FENO podem predizer uma resposta favorável ao tratamento com ICS/LABA. Por último, a acupuntura demonstrou ser uma abordagem adjuvante eficaz e segura no tratamento de exacerbações agudas da DPOC, com relatos de melhorias significativas por parte dos pacientes

    Phylogeography of the neotropical Anopheles triannulatus complex (Diptera: Culicidae) supports deep structure and complex patterns

    Abstract Background The molecular phylogenetic relationships and population structure of the species of the Anopheles triannulatus complex: Anopheles triannulatus s.s., Anopheles halophylus and the putative species Anopheles triannulatus C were investigated. Methods\ud \ud The mitochondrial COI gene, the nuclear white gene and rDNA ITS2 of samples that include the known geographic distribution of these taxa were analyzed. Phylogenetic analyses were performed using Bayesian inference, Maximum parsimony and Maximum likelihood approaches. Results Each data set analyzed septely yielded a different topology but none provided evidence for the seption of An. halophylus and An. triannulatus C, consistent with the hypothesis that the two are undergoing incipient speciation. The phylogenetic analyses of the white gene found three main clades, whereas the statistical parsimony network detected only a single metapopulation of Anopheles triannulatus s.l. Seven COI lineages were detected by phylogenetic and network analysis. In contrast, the network, but not the phylogenetic analyses, strongly supported three ITS2 groups. Combined data analyses provided the best resolution of the trees, with two major clades, Amazonian (clade I) and trans-Andean + Amazon Delta (clade II). Clade I consists of multiple subclades: An. halophylus + An. triannulatus C; trans-Andean Venezuela; central Amazonia + central Bolivia; Atlantic coastal lowland; and Amazon delta. Clade II includes three subclades: Panama; cis-Andean Colombia; and cis-Venezuela. The Amazon delta specimens are in both clades, likely indicating local sympatry. Spatial and molecular variance analyses detected nine groups, corroborating some of subclades obtained in the combined data analysis. Conclusion Combination of the three molecular markers provided the best resolution for differentiation within An. triannulatus s.s. and An. halophylus and C. The latest two species seem to be very closely related and the analyses performed were not conclusive regarding species differentiation. Further studies including new molecular markers would be desirable to solve this species status question. Besides, results of the study indicate a trans-Andean origin for An. triannulatus s.l.     Subacute Cognitive Impairment in Individuals With Mild and Moderate COVID-19: A Case Series

    Background: Previous reported neurologic sequelae associated with SARS-CoV-2 infection have mainly been confined to hospital-based patients in which viral detection was restricted to nasal/throat swabs or to IgM/IgG peripheral blood serology. Here we describe seven cases from Brazil of outpatients with previous mild or moderate COVID-19 who developed subacute cognitive disturbances. Methods: From June 1 to August 15, 2020, seven individuals 18 to 60 years old, with confirmed mild/moderate COVID-19 and findings consistent with encephalopathy who were observed >7 days after respiratory symptom initiation, were screened for cognitive dysfunction. Paired sera and CSF were tested for SARS-CoV-2 (IgA, IgG ELISA, and RT-PCR). Serum and intrathecal antibody dynamics were evaluated with oligoclonal bands and IgG index. Cognitive dysfunction was assessed by the Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), and the Clock Drawing Test (CDT). Results: All but one of our patients were female, and the mean age was 42.6 years. Neurologic symptoms were first reported a median of 16 days (IQR 15–33) after initial COVID-19 symptoms. All patients had headache and altered behavior. Cognitive dysfunction was observed mainly in phonemic verbal fluency (MoCA) with a median of six words/min (IQR 5.25–10.75) and altered visuospatial construction with a median of four points (IQR 4–9) (CDT). CSF pleocytosis was not detected, and only one patient was positive for SARS-Co Conclusions: A subacute cognitive syndrome suggestive of SARS-CoV-2-initiated damage to cortico-subcortical associative pathways that could not be attributed solely to inflammation and hypoxia was present in seven individuals with mild/moderate COVID-19

    Skin color and severe maternal outcomes: evidence from the brazilian network for surveillance of severe maternal morbidity

    Taking into account the probable role that race/skin color may have for determining outcomes in maternal health, the objective of this study was to assess whether maternal race/skin color is a predictor of severe maternal morbidity. This is a secondary analysis of the Brazilian Network for Surveillance of Severe Maternal Morbidity, a national multicenter cross-sectional study of 27 Brazilian referral maternity hospitals. A prospective surveillance was performed to identify cases of maternal death (MD), maternal near miss (MNM) events, and potentially life-threatening conditions (PLTC), according to standard WHO definition and criteria. Among 9,555 women with severe maternal morbidity, data on race/skin color was available for 7,139 women, who were further divided into two groups: 4,108 nonwhite women (2,253 black and 1,855 from other races/skin color) and 3,031 white women. Indicators of severe maternal morbidity according to WHO definition are shown by skin color group. Adjusted Prevalence Ratios (PRadj - 95%CI) for Severe Maternal Outcome (SMO=MNM+MD) were estimated according to sociodemographic/obstetric characteristics, pregnancy outcomes, and perinatal results considering race. Results. Among 7,139 women with severe maternal morbidity evaluated, 90.5% were classified as PLTC, 8.5% as MNM, and 1.6% as MD. There was a significantly higher prevalence of MNM and MD among white women. MNMR (maternal near miss ratio) was 9.37 per thousand live births (LB). SMOR (severe maternal outcome ratio) was 11.08 per 1000 LB, and MMR (maternal mortality ratio) was 170.4 per 100,000 LB. Maternal mortality to maternal near miss ratio was 1 to 5.2, irrespective of maternal skin color. Hypertension, the main cause of maternal complications, affected mostly nonwhite women. Hemorrhage, the second more common cause of maternal complication, predominated among white women. Nonwhite skin color was associated with a reduced risk of SMO in multivariate analysis. Nonwhite skin color was associated with a lower risk for severe maternal outcomes.     The state of health in the European Union (EU-27) in 2019: a systematic analysis for the Global Burden of Disease study 2019

    Background: The European Union (EU) faces many health-related challenges. Burden of diseases information and the resulting trends over time are essential for health planning. This paper reports estimates of disease burden in the EU and individual 27 EU countries in 2019, and compares them with those in 2010.Methods: We used the Global Burden of Disease 2019 study estimates and 95% uncertainty intervals for the whole EU and each country to evaluate age-standardised death, years of life lost (YLLs), years lived with disability (YLDs) and disability-adjusted life years (DALYs) rates for Level 2 causes, as well as life expectancy and healthy life expectancy (HALE).Results:In 2019, the age-standardised death and DALY rates in the EU were 465.8 deaths and 20,251.0 DALYs per 100,000 inhabitants, respectively. Between 2010 and 2019, there were significant decreases in age-standardised death and YLL rates across EU countries. However, YLD rates remained mainly unchanged. The largest decreases in age-standardised DALY rates were observed for "HIV/AIDS and sexually transmitted diseases" and "transport injuries" (each -19%). "Diabetes and kidney diseases" showed a significant increase for age-standardised DALY rates across the EU (3.5%). In addition, "mental disorders" showed an increasing age-standardised YLL rate (14.5%).Conclusions: There was a clear trend towards improvement in the overall health status of the EU but with differences between countries. EU health policymakers need to address the burden of diseases, paying specific attention to causes such as mental disorders. There are many opportunities for mutual learning among otherwise similar countries with different patterns of disease

    Photography-based taxonomy is inadequate, unnecessary, and potentially harmful for biological sciences

    The question whether taxonomic descriptions naming new animal species without type specimen(s) deposited in collections should be accepted for publication by scientific journals and allowed by the Code has already been discussed in Zootaxa (Dubois & Nemésio 2007; Donegan 2008, 2009; Nemésio 2009a–b; Dubois 2009; Gentile & Snell 2009; Minelli 2009; Cianferoni & Bartolozzi 2016; Amorim et al. 2016). This question was again raised in a letter supported by 35 signatories published in the journal Nature (Pape et al. 2016) on 15 September 2016. On 25 September 2016, the following rebuttal (strictly limited to 300 words as per the editorial rules of Nature) was submitted to Nature, which on 18 October 2016 refused to publish it. As we think this problem is a very important one for zoological taxonomy, this text is published here exactly as submitted to Nature, followed by the list of the 493 taxonomists and collection-based researchers who signed it in the short time span from 20 September to 6 October 2016
