    Physical activity intervention for elderly patients with reduced physical performance after acute coronary syndrome (HULK study): Rationale and design of a randomized clinical trial

    Background: Reduced physical performance and impaired mobility are common in elderly patients after acute coronary syndrome (ACS) and they represent independent risk factors for disability, morbidity, hospital readmission and mortality. Regular physical exercise represents a means for improving functional capacity. Nevertheless, its clinical benefit has been less investigated in elderly patients in the early phase after ACS. The HULK trial aims to investigate the clinical benefit of an early, tailored low-cost physical activity intervention in comparison to standard of care in elderly ACS patients with reduced physical performance. Design: HULK is an investigator-initiated, prospective multicenter randomized controlled trial (NCT03021044). After successful management of the ACS acute phase and uneventful first 1 month, elderly (≥70 years) patients showing reduced physical performance are randomized (1:1 ratio) to either standard of care or physical activity intervention. Reduced physical performance is defined as a short physical performance battery (SPPB) score of 4-9. The early, tailored, low-cost physical intervention includes 4 sessions of physical activity with a supervisor and an home-based program of physical exercise. The chosen primary endpoint is the 6-month SPPB value. Secondary endpoints briefly include quality of life, on-treatment platelet reactivity, some laboratory data and clinical adverse events. To demonstrate an increase of at least one SPPB point in the experimental arm, a sample size of 226 patients is needed. Conclusions: The HULK study will test the hypothesis that an early, tailored low-cost physical activity intervention improves physical performance, quality of life, frailty status and outcome in elderly ACS patients with reduced physical performance

    Understanding the faint red galaxy population using large-scale clustering measurements from SDSS DR7

    We use data from the SDSS to investigate the evolution of the large-scale galaxy bias as a function of luminosity for red galaxies. We carefully consider correlation functions of galaxies selected from both photometric and spectroscopic data, and cross-correlations between them, to obtain multiple measurements of the large-scale bias. We find, for our most robust analyses, a strong increase in bias with luminosity for the most luminous galaxies, an intermediate regime where bias does not evolve strongly over a range of two magnitudes in galaxy luminosity, and no evidence for an upturn in bias for fainter red galaxies. Previous work has found an increase in bias to low luminosities that has been widely interpreted as being caused by a strong preference for red dwarf galaxies to be satellites in the most massive halos. We can recover such an upturn in bias to faint luminosities if we push our measurements to small scales, and include galaxy clustering measurements along the line-of-sight, where we expect non-linear effects to be the strongest. The results that we expect to be most robust suggest that the low luminosity population of red galaxies is not dominated by satellite galaxies occupying the most massive haloes.Comment: Matches version accepted by MNRA

    Ameliorating Systematic Uncertainties in the Angular Clustering of Galaxies: A Study using SDSS-III

    We investigate the effects of potential sources of systematic error on the angular and photometric redshift, z_phot, distributions of a sample of redshift 0.4 < z < 0.7 massive galaxies whose selection matches that of the Baryon Oscillation Spectroscopic Survey (BOSS) constant mass sample. Utilizing over 112,778 BOSS spectra as a training sample, we produce a photometric redshift catalog for the galaxies in the SDSS DR8 imaging area that, after masking, covers nearly one quarter of the sky (9,913 square degrees). We investigate fluctuations in the number density of objects in this sample as a function of Galactic extinction, seeing, stellar density, sky background, airmass, photometric offset, and North/South Galactic hemisphere. We find that the presence of stars of comparable magnitudes to our galaxies (which are not traditionally masked) effectively remove area. Failing to correct for such stars can produce systematic errors on the measured angular auto-correlation function, w, that are larger than its statistical uncertainty. We describe how one can effectively mask for the presence of the stars, without removing any galaxies from the sample, and minimize the systematic error. Additionally, we apply two separate methods that can be used to correct the systematic errors imparted by any parameter that can be turned into a map on the sky. We find that failing to properly account for varying sky background introduces a systematic error on w. We measure w, in four z_phot slices of width 0.05 between 0.45 < z_phot < 0.65 and find that the measurements, after correcting for the systematic effects of stars and sky background, are generally consistent with a generic LambdaCDM model, at scales up to 60 degrees. At scales greater than 3 degrees and z_phot > 0.5, the magnitude of the corrections we apply are greater than the statistical uncertainty in w.Comment: Accepted by MNRA

    Redshift measurement and spectral classification for eBOSS galaxies with the redmonster software

    We describe the redmonster automated redshift measurement and spectral classification software designed for the extended Baryon Oscillation Spectroscopic Survey (eBOSS) of the Sloan Digital Sky Survey IV (SDSS-IV). We describe the algorithms, the template standard and requirements, and the newly developed galaxy templates to be used on eBOSS spectra. We present results from testing on early data from eBOSS, where we have found a 90.5% automated redshift and spectral classification success rate for the luminous red galaxy sample (redshifts 0.6 ≲ z ≲ 1.0). The redmonster performance meets the eBOSS cosmology requirements for redshift classification and catastrophic failures and represents a significant improvement over the previous pipeline. We describe the empirical processes used to determine the optimum number of additive polynomial terms in our models and an acceptable ΔXr2 threshold for declaring statistical confidence. Statistical errors on redshift measurement due to photon shot noise are assessed, and we find typical values of a few tens of km s-1. An investigation of redshift differences in repeat observations scaled by error estimates yields a distribution with a Gaussian mean and standard deviation of μ ∼ 0.01 and σ ∼ 0.65, respectively, suggesting the reported statistical redshift uncertainties are over-estimated by ∼54%. We assess the effects of object magnitude, signal-to-noise ratio, fiber number, and fiber head location on the pipeline's redshift success rate.     Lateglacial and Holocene climate and environmental change in the northeastern Mediterranean region: Diatom evidence from Lake Dojran (Republic of Macedonia/Greece)

    The juncture between the west-east and north-south contrasting Holocene climatic domains across the Mediterranean is complex and poorly understood. Diatom analysis of Lake Dojran (Republic of Macedonia/Greece) provides a new insight into lake levels and trophic status during the Lateglacial and Holocene periods in the northeastern Mediterranean. Following a very shallow or even desiccated state at the core base at ca. 12,500 cal yr BP, indicated by sedimentological and hydro-acoustic data, diatoms indicate lake infilling, from a shallow state with abundant benthos to a plankton-dominated relatively high lake level and eutrophic state thereafter. Diatom-inferred shallowing between ca. 12,400 - 12,000 cal yr BP and a very low lake level and eutrophic, oligosaline state between ca. 12,000 - 11,500 cal yr BP provide strong evidence for Younger Dryas aridity. The earliest Holocene (ca. 11,500 - 10,700 cal yr BP) was characterised by a high lake level, followed by a lake-level reduction and increased trophic level between ca. 10,700-8,500 cal yr BP. The lake was relatively deep and exhibited peak Holocene trophic level between ca. 8,500-3,000 cal yr BP, becoming shallow thereafter. The diatom data provide more robust evidence and strengthen previous lake-level interpretation based on sedimentological and geochemical data during the earliest, mid and late Holocene, and also clarify previous uncertainty in interpretation of Lateglacial and early-Holocene lake-level change. Our results are also important in disentangling regional climate effects from local catchment dynamics during the Holocene, and to this end we exploit extant regional palynological evidence for vegetation change in the highlands and lowlands. The importance of seasonality in driving Holocene climate change is assessed by reference to the summer and winter latitudinal temperature gradient (LTG) model of Davis and Brewer (2009). We suggest that increased precipitation drove the high lake level during the earliest Holocene. The early- Holocene low lake level and relatively high trophic state may result climatically from high seasonality of precipitation and locally from limited, nutrient-rich catchment runoff. We argue that the mid- Holocene relatively deep and eutrophic state was driven mainly by local vegetation succession and associated changes in catchment processes, rather than showing a close relationship to climate change. The late-Holocene shallow state may have been influenced by a temperature-induced increase in evaporative concentration, but was coupled with clear evidence for intensified human impact. This study improves understanding of Lateglacial and Holocene climate change in the northeastern Mediterranean, suggests the important role of the LTG on moisture availability during the Holocene, and clarifies the influence of catchment processes on palaeohydrology

    Exploring cosmic homogeneity with the BOSS DR12 galaxy sample

    In this study, we probe the transition to cosmic homogeneity in the Large Scale Structure (LSS) of the Universe using the CMASS galaxy sample of BOSS spectroscopic survey which covers the largest effective volume to date, 3 h-3 Gpc3 at 0.43 ≤ z ≤ 0.7. We study the scaled counts-in-spheres, N(2.97 for r>RH, we find RH = (63.3±0.7) h-1 Mpc, in agreement at the percentage level with the predictions of the ΛCDM model RH=62.0 h-1 Mpc. Thanks to the large cosmic depth of the survey, we investigate the redshift evolution of the transition to homogeneity scale and find agreement with the ΛCDM prediction. Finally, we find that 2 is compatible with 3 at scales larger than 300 h-1 Mpc in all redshift bins. These results consolidate the Cosmological Principle and represent a precise consistency test of the ΛCDM model.PostprintPeer reviewe

    TBCRC 019: A phase II trial of nanoparticle albumin-bound paclitaxel with or without the anti-death receptor 5 monoclonal antibody tigatuzumab in patients with triple negative breast cancer

    Purpose: Tigatuzumab (TIG), an agonistic anti-DR5 antibody, triggers apoptosis in DR5+ human tumor cells without crosslinking. TIG has strong in vitro/in vivo activity against basal-like breast cancer cells enhanced by chemotherapy agents. This study evaluates activity of TIG and chemotherapy in patients with metastatic triple-negative breast cancer (TNBC). Experimental Design: Randomized 2:1 phase II trial of albumin-bound paclitaxel (nab-PAC) ± TIG in patients with TNBC stratified by prior chemotherapy. Patients received nab-PAC weekly × 3 ± TIG every other week, every 28 days. Primary objective was within-arm objective response rate (ORR). Secondary objectives were safety, progression-free survival (PFS), clinical benefit, and TIG immunogenicity. Metastatic research biopsies were required. Results: Among 64 patients (60 treated; TIG/nab-PAC n = 39 and nab-PAC n = 21), there were 3 complete remissions (CR), 8 partial remissions (PR; 1 almost CR), 11 stable diseases (SD), and 17 progressive diseases (PD) in the TIG/nab-PAC arm (ORR, 28%), and no CRs, 8 PRs, 4 SDs, and 9 PDs in the nab-PAC arm (ORR, 38%). There was a numerical increase in CRs and several patients had prolonged PFS (1,025+, 781, 672, 460, 334) in the TIG/nab-PAC arm. Grade 3 toxicities were 28% and 29%, respectively, with no grade 4–5. Exploratory analysis suggests an association of ROCK1 gene pathway activation with efficacy in the TIG/nab-PAC arm. Conclusions: ORR and PFS were similar in both. Preclinical activity of TIG in basal-like breast cancer and prolonged PFS in few patients in the combination arm support further investigation of anti-DR5 agents. ROCK pathway activation merits further evaluation
