Aim of the Study: The aims of the study were to gain an understanding of midwives’ concepts of culture and to explore their strategies to incorporate cultural sensitivity into professional practice, especially in relation to Indigenous women. The study also explored factors that impact on the provision of culturally appropriate care. Method: Semi-structured interviews were conducted with thirty two midwives in three different geographical locations within New South Wales, Australia. A modified grounded theory approach was used for the data collection and preliminary analysis. Emerging findings were explored with and compared to relevant theoretical perspectives, particularly those of Foucault. Findings: The context of practice has a direct impact upon midwives’ approach to care. A woman-centred approach is often lost when midwives are working within busy hospital settings, where they lack power and control and are usually time poor. Woman-centred care is replaced by the midwives’ need to anticipate care provision and there is a tendency to take a generic or ‘recipe’ approach to care as a result. This is less likely to occur in community settings, where midwives have more time and control shifts to the woman. In hospitals, bureaucratic procedures are emphasised over the individual decisions of midwives and the woman. Conclusions and Implications: A focus on practice alone is insufficient. The context of care needs to be examined before woman-centred care can be realised. It takes courage to challenge the ‘system’, however where there is resistance, there is possibility for change for the benefit of women and their families

    Polypharmacy management by 2030: A patient safety challenge

    The SIMPATHY (Stimulating Innovation Management of Polypharmacy and Adherence in The Elderly) consortium have explored how healthcare management programmes can be implemented to improve medication safety and prevent patient harm by addressing the appropriate use of multiple medications (polypharmacy). Fundamental to these programmes is the principle that providers work in partnership with patients to enable shared decision making regarding medication, which improves patient adherence and medicines related outcomes. This report sets out the case for prioritising working together now to address inappropriate medication use over the next decade, to ensure the quality, economic and political systems are put in place to improve medication safety for patients. There are encouraging signs of the increasing recognition of these challenges, and the timeliness of this report. In March 2017, the World Health Organisation (WHO) launched a global challenge to address medication safety, with polypharmacy as a flagship element. A special interest group will be launched by the International Foundation on Integrated Care in May 2017

    Peer-mentoring for first-time mothers from areas of socio-economic disadvantage: A qualitative study within a randomised controlled trial

    <p>Abstract</p> <p>Background</p> <p>Non-professional involvement in delivering health and social care support in areas of socio-economic deprivation is considered important in attempting to reduce health inequalities. However, trials of peer mentoring programmes have yielded inconsistent evidence of benefit: difficulties in implementation have contributed to uncertainty regarding their efficacy. We aimed to explore difficulties encountered in conducting a randomised controlled trial of a peer-mentoring programme for first-time mothers in socially disadvantaged areas, in order to provide information relevant to future research and practice. This paper describes the experiences of lay-workers, women and health professionals involved in the trial.</p> <p>Methods</p> <p>Thematic analysis of semi-structured interviews with women (n = 11) who were offered peer mentor support, lay-workers (n = 11) who provided mentoring and midwives (n = 2) who supervised the programme, which provided support, from first hospital antenatal visit to one year postnatal. Planned frequency of contact was two-weekly (telephone or home visit) but was tailored to individuals' needs.</p> <p>Results</p> <p>Despite lay-workers living in the same locality, they experienced difficulty initiating contact with women and this affected their morale adversely. Despite researchers' attempts to ensure that the role of the mentor was understood clearly it appeared that this was not achieved for all participants. Mentors attempted to develop peer-mentor relationships by offering friendship and sharing personal experiences, which was appreciated by women. Mentors reported difficulties developing relationships with those who lacked interest in the programme. External influences, including family and friends, could prevent or facilitate mentoring. Time constraints in reconciling flexible mentoring arrangements with demands of other commitments posed major personal difficulties for lay-workers.</p> <p>Conclusion</p> <p>Difficulties in initiating contact, developing peer-mentor relationships and time constraints pose challenges to delivering lay-worker peer support. In developing such programmes, awareness of potential difficulties and of how professional support may help resolve these should improve uptake and optimise evaluation of their effectiveness.</p> <p>Trial Registration Number: ISRCTN55055030</p

    Bodies Moving and Being Moved: Mapping affect in Christian Nold's Bio Mapping

    In A History of Spaces (2004), John Pickles observes that one of the less well-known representational norms of mapping is its focus on ‘natural and physical objects rather than developing universal conventions dealing with symbol, affect and movement.’ New media artist Christian Nold's work has dealt explicitly with two of these cartographic blindspots, grafting new and old technologies that both, in different ways, create bodily traces – the GPS trace of movement and the GSR (galvanic skin response) trace of arousal, often taken as an index of emotional response. Although Nold's socially engaged practice can be placed within the ‘locative media’ genre it also taps into the technological imaginaries around physiological sensors and intimate data. This paper considers Nold's Bio Mapping (2004-) projects in the context of his longstanding concern with social collectives and public space as a field of social relations. Looking at particular maps from Nold's Bio Mapping project, it considers the implications of blending the traces of the body's internal states with the traces produced by locomotive movement, and the relationship between the individuals thus traced and the collectives that Nold seeks to represent. Concurrent with Nold's practice there has been a wave of interest in affect and emotion (and the distinction between them) within the humanities. This paper brings Nold's work into contact with the Deleuzian/Spinozan concept of affect employed in one strand of this writing, drawing in particular on the work of Brian Massumi. Rather than using theory to simply illustrate Nold's practice, it follows the implications of Deleuze's cartographic model of individuation, the logic of which ultimately problematises the very distinction between the two bodily phenomena traced by Nold's device

    Biopharmaceutical considerations in paediatrics with a view to the evaluation of orally administered drug products – a PEARRL review.

    Objectives: In this review, the current biopharmaceutical approaches for evaluation of oral formulation performance in paediatrics are discussed. Key findings: The paediatric gastrointestinal (GI) tract undergoes numerous morphological and physiological changes throughout its development and growth. Some physiological parameters are yet to be investigated, limiting the use of the existing in vitro biopharmaceutical tools to predict the in vivo performance of paediatric formulations. Meals and frequencies of their administration evolve during childhood and affect oral drug absorption. Furthermore, the establishment of a paediatric Biopharmaceutics Classification System (pBCS), based on the adult Biopharmaceutics Classification System (BCS), requires criteria adjustments. The usefulness of computational simulation and modeling for extrapolation of adult data to paediatrics has been confirmed as a tool for predicting drug formulation performance. Despite the great number of successful physiologically based pharmacokinetic models to simulate drug disposition, the simulation of drug absorption from the GI tract is a complicating issue in paediatric populations. Summary: The biopharmaceutics tools for investigation of oral drug absorption in paediatrics need further development, refinement and validation. A combination of in vitro and in silico methods could compensate for the uncertainties accompanying each method on its own

    Prevalence and architecture of de novo mutations in developmental disorders.

    The genomes of individuals with severe, undiagnosed developmental disorders are enriched in damaging de novo mutations (DNMs) in developmentally important genes. Here we have sequenced the exomes of 4,293 families containing individuals with developmental disorders, and meta-analysed these data with data from another 3,287 individuals with similar disorders. We show that the most important factors influencing the diagnostic yield of DNMs are the sex of the affected individual, the relatedness of their parents, whether close relatives are affected and the parental ages. We identified 94 genes enriched in damaging DNMs, including 14 that previously lacked compelling evidence of involvement in developmental disorders. We have also characterized the phenotypic diversity among these disorders. We estimate that 42% of our cohort carry pathogenic DNMs in coding sequences; approximately half of these DNMs disrupt gene function and the remainder result in altered protein function. We estimate that developmental disorders caused by DNMs have an average prevalence of 1 in 213 to 1 in 448 births, depending on parental age. Given current global demographics, this equates to almost 400,000 children born per year

    Heterozygous Variants in KMT2E Cause a Spectrum of Neurodevelopmental Disorders and Epilepsy.

    We delineate a KMT2E-related neurodevelopmental disorder on the basis of 38 individuals in 36 families. This study includes 31 distinct heterozygous variants in KMT2E (28 ascertained from Matchmaker Exchange and three previously reported), and four individuals with chromosome 7q22.2-22.23 microdeletions encompassing KMT2E (one previously reported). Almost all variants occurred de novo, and most were truncating. Most affected individuals with protein-truncating variants presented with mild intellectual disability. One-quarter of individuals met criteria for autism. Additional common features include macrocephaly, hypotonia, functional gastrointestinal abnormalities, and a subtle facial gestalt. Epilepsy was present in about one-fifth of individuals with truncating variants and was responsive to treatment with anti-epileptic medications in almost all. More than 70% of the individuals were male, and expressivity was variable by sex; epilepsy was more common in females and autism more common in males. The four individuals with microdeletions encompassing KMT2E generally presented similarly to those with truncating variants, but the degree of developmental delay was greater. The group of four individuals with missense variants in KMT2E presented with the most severe developmental delays. Epilepsy was present in all individuals with missense variants, often manifesting as treatment-resistant infantile epileptic encephalopathy. Microcephaly was also common in this group. Haploinsufficiency versus gain-of-function or dominant-negative effects specific to these missense variants in KMT2E might explain this divergence in phenotype, but requires independent validation. Disruptive variants in KMT2E are an under-recognized cause of neurodevelopmental abnormalities

    Bi-allelic Loss-of-Function CACNA1B Mutations in Progressive Epilepsy-Dyskinesia.

    The occurrence of non-epileptic hyperkinetic movements in the context of developmental epileptic encephalopathies is an increasingly recognized phenomenon. Identification of causative mutations provides an important insight into common pathogenic mechanisms that cause both seizures and abnormal motor control. We report bi-allelic loss-of-function CACNA1B variants in six children from three unrelated families whose affected members present with a complex and progressive neurological syndrome. All affected individuals presented with epileptic encephalopathy, severe neurodevelopmental delay (often with regression), and a hyperkinetic movement disorder. Additional neurological features included postnatal microcephaly and hypotonia. Five children died in childhood or adolescence (mean age of death: 9 years), mainly as a result of secondary respiratory complications. CACNA1B encodes the pore-forming subunit of the pre-synaptic neuronal voltage-gated calcium channel Cav2.2/N-type, crucial for SNARE-mediated neurotransmission, particularly in the early postnatal period. Bi-allelic loss-of-function variants in CACNA1B are predicted to cause disruption of Ca2+ influx, leading to impaired synaptic neurotransmission. The resultant effect on neuronal function is likely to be important in the development of involuntary movements and epilepsy.     Investigation of hospital discharge cases and SARS-CoV-2 introduction into Lothian care homes

    Background The first epidemic wave of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in Scotland resulted in high case numbers and mortality in care homes. In Lothian, over one-third of care homes reported an outbreak, while there was limited testing of hospital patients discharged to care homes. Aim To investigate patients discharged from hospitals as a source of SARS-CoV-2 introduction into care homes during the first epidemic wave. Methods A clinical review was performed for all patients discharges from hospitals to care homes from 1st March 2020 to 31st May 2020. Episodes were ruled out based on coronavirus disease 2019 (COVID-19) test history, clinical assessment at discharge, whole-genome sequencing (WGS) data and an infectious period of 14 days. Clinical samples were processed for WGS, and consensus genomes generated were used for analysis using Cluster Investigation and Virus Epidemiological Tool software. Patient timelines were obtained using electronic hospital records. Findings In total, 787 patients discharged from hospitals to care homes were identified. Of these, 776 (99%) were ruled out for subsequent introduction of SARS-CoV-2 into care homes. However, for 10 episodes, the results were inconclusive as there was low genomic diversity in consensus genomes or no sequencing data were available. Only one discharge episode had a genomic, time and location link to positive cases during hospital admission, leading to 10 positive cases in their care home. Conclusion The majority of patients discharged from hospitals were ruled out for introduction of SARS-CoV-2 into care homes, highlighting the importance of screening all new admissions when faced with a novel emerging virus and no available vaccine
