    Evaluation des caractéristiques, morphologiques physico-chimiques et sensorielles de la purée de deux variétés de tomates locales produites à petite échelle à Man (Côte d’Ivoire)

    Cette étude découle d’un vaste projet de production sur différents substrats (hors sol, plein champs) et de valorisation de la tomate, dans la région montagneuse de Man. Aussi, ces expériences, qui s’inscrivent dans le cadre de la valorisation par la transformation des fruits de la production, ont-elles été menées dans l’optique d’évaluer les caractéristiques morphologiques, physico-chimiques et sensorielles des fruits et de la purée des deux variétés de tomates produites. Il s’agit de la tomate traditionnelle (Tmt 1), variété la plus utilisée dans les ménages ruraux de la région montagneuse de Man et la tomate (Tmt 2), d’usage commun pour les plats de salades en zone urbaine. Cette dernière se reconnaît par ses fruits de forme allongée (Cf= 1,93±0,10> 1), de couleur rouge et moins foncée (Couleur ΔE 26,5±0,00) que la première variété (Couleur ΔE 30,3±0,00). Avec des taux de matière sèche différents ; (11,93±0,49% pour la purée de tomate Tmt 1 et de 8,81±0,52% pour la purée de tomate Tmt2, les purées qui en sont produites ont des teneurs très importantes en minéraux. Ces taux élevés pourraient ainsi permettre de compenser des carences en nutriments des populations de la zone. De plus, les pH aussi bas (3,93±0,00) pour la purée de Tmt1 et 4,13±0,01 pour celle de Tmt2, confèrent une acidité élevée à ces purées et donc une meilleure conservation. Le traitement thermique appliqué au produit à 80 °C pendant 15 Minutes garantit à la purée une bonne stabilité microbiologique conforme à la norme AFNOR (NFV-08-402). L’analyse sensorielle, basée sur les critères de la couleur (rouge), le goût salé, l’acidité et la consistance révèle que la purée produite à partir de la tomate Tmt1 est la plus appréciée des dégustateurs.Mots clés : Tomate, Morphologie, purées, physico-chimique, Man, Côte d’Ivoir

    Influence De La Fermentation Sur Les Caracteristiques Physicochimiques Et Sensorielles De La Farine Composite A Base De Banane Plantain Et D’amande De Cajou

    The purpose of this study is to determine the effect of fermentation in the formulation of infant flour that meets the nutritional needs of children, using plantain and cashew kernel flours. For this purpose, cashew kernel flours (Anacardium occidentale L) fermented for 72 h and unfermented flour were incorporated into the plantain flour (Musa paradisiaca sp) at different proportions (0, 10, 15 and 20%). The biochemical, functional compositions and antinutrient levels of the formulations were determined. The results showed that the fermentation caused a more significant increase in the protein, lipid, energy and total phenolic content of the formulations in proportion to the incorporation rate. On the other hand, carbohydrate, fiber and ash levels were significantly reduced (P <0.05) as a result of the fermentation of cashew kernels compared with the incorporation of unfermented cashew flour. The study of the swelling powers and the solubility indices of the flours showed that the fermentation entails a significant decrease proportional to the level of cashew almond flour incorporated compared to those obtained with the unfermented flour. It was also found that fermentation entailed a significant reduction (P <0.05) in the content of oxalic acid and tannins in cashew kernel flour. Sensory tests allowed limiting the maximum level of incorporation of fermented almond flour to 10% and that not fermented to 15%

    Prévention du brunissement lors de l’extraction de jus de banane plantain et valorisation des résidus issus de cette extraction

    Ces expériences ont été conduites dans l’optique d’évaluer d’une part la capacité de divers traitements anti brunissement à prévenir le brunissement du jus de banane plantain et d’autre part de valoriser les résidus issus de cette extraction sous forme de confiture. Le métabisulfite de potassium (100 mg/kg) était plus efficace pour la production d’un jus de banane légèrement coloré et de coloration stable par rapport aux deux méthodes de traitement thermique (chauffage de la banane entière non épluchée à 100 °C pendant 11 min et le chauffage de la purée à 85 °C pendant 1-2 min et l’acide ascorbique (470 mg/kg). La polyphenoloxydase était plus active dans les jus clarifiés traités à l’acide ascorbique et le témoin qui brunissent tous au fil du temps. L’analyse sensorielle a révélé que tous les jus présentaient la même intensité de flaveur banane plantain. Les résidus d’extraction des jus ont été valorisés par la production de deux types de confiture (avec ou sans ajout de poudre de baobab comme source de pectine) appréciés par les dégustateurs. Par ailleurs, la confiture contenant la poudre de baobab (Adansonia digitata) aura une meilleure conservation grâce à son pH plus acide.© 2015 International Formulae Group. Potassium metabisulfite (100 m/L) was more effective for the production of a slightly colored banana juice with stable color compared to the two methods of thermal treatment (heating the whole banana unpeeled at 100 °C for 11 min and heating the mash at 85 °C for 1-2 min) and ascorbic acid (470 mg/L). The polyphenol oxidase was more active in the clarified juice treated with ascorbic acid and the control which darkened over time. Sensory analysis revealed that all the juices had the same intensity of plantain flavor. The juice processing wastes were valued by the production of two types of jam (with or without addition of baobab powder as a source of pectin) appreciated by the tasters. Moreover, jam containing baobab (A digitata) powder will have better preservation with its more acidic pH.© 2015 International Formulae Group.     Laparoscopy in management of appendicitis in high-, middle-, and low-income countries: a multicenter, prospective, cohort study.

    BACKGROUND: Appendicitis is the most common abdominal surgical emergency worldwide. Differences between high- and low-income settings in the availability of laparoscopic appendectomy, alternative management choices, and outcomes are poorly described. The aim was to identify variation in surgical management and outcomes of appendicitis within low-, middle-, and high-Human Development Index (HDI) countries worldwide. METHODS: This is a multicenter, international prospective cohort study. Consecutive sampling of patients undergoing emergency appendectomy over 6 months was conducted. Follow-up lasted 30 days. RESULTS: 4546 patients from 52 countries underwent appendectomy (2499 high-, 1540 middle-, and 507 low-HDI groups). Surgical site infection (SSI) rates were higher in low-HDI (OR 2.57, 95% CI 1.33-4.99, p = 0.005) but not middle-HDI countries (OR 1.38, 95% CI 0.76-2.52, p = 0.291), compared with high-HDI countries after adjustment. A laparoscopic approach was common in high-HDI countries (1693/2499, 67.7%), but infrequent in low-HDI (41/507, 8.1%) and middle-HDI (132/1540, 8.6%) groups. After accounting for case-mix, laparoscopy was still associated with fewer overall complications (OR 0.55, 95% CI 0.42-0.71, p < 0.001) and SSIs (OR 0.22, 95% CI 0.14-0.33, p < 0.001). In propensity-score matched groups within low-/middle-HDI countries, laparoscopy was still associated with fewer overall complications (OR 0.23 95% CI 0.11-0.44) and SSI (OR 0.21 95% CI 0.09-0.45). CONCLUSION: A laparoscopic approach is associated with better outcomes and availability appears to differ by country HDI. Despite the profound clinical, operational, and financial barriers to its widespread introduction, laparoscopy could significantly improve outcomes for patients in low-resource environments.     Pooled analysis of who surgical safety checklist use and mortality after emergency laparotomy

    Background: The World Health Organization (WHO) Surgical Safety Checklist has fostered safe practice for 10 years, yet its place in emergency surgery has not been assessed on a global scale. The aim of this study was to evaluate reported checklist use in emergency settings and examine the relationship with perioperative mortality in patients who had emergency laparotomy. Methods: In two multinational cohort studies, adults undergoing emergency laparotomy were compared with those having elective gastrointestinal surgery. Relationships between reported checklist use and mortality were determined using multivariable logistic regression and bootstrapped simulation. Results: Of 12 296 patients included from 76 countries, 4843 underwent emergency laparotomy. After adjusting for patient and disease factors, checklist use before emergency laparotomy was more common in countries with a high Human Development Index (HDI) (2455 of 2741, 89⋅6 per cent) compared with that in countries with a middle (753 of 1242, 60⋅6 per cent; odds ratio (OR) 0⋅17, 95 per cent c.i. 0⋅14 to 0⋅21, P &lt; 0⋅001) or low (363 of 860, 42⋅2 percent; OR 0⋅08, 0⋅07 to 0⋅10, P &lt; 0⋅001) HDI. Checklist use was less common in elective surgery than for emergency laparotomy in high-HDI countries (risk difference −9⋅4 (95 per cent c.i. −11⋅9 to −6⋅9) per cent; P &lt; 0⋅001), but the relationship was reversed in low-HDI countries (+12⋅1 (+7⋅0 to +17⋅3) per cent; P &lt; 0⋅001). In multivariable models, checklist use was associated with a lower 30-day perioperative mortality (OR 0⋅60, 0⋅50 to 0⋅73; P &lt; 0⋅001). The greatest absolute benefit was seen for emergency surgery in low-and middle-HDI countries. Conclusion: Checklist use in emergency laparotomy was associated with a significantly lower perioperative mortality rate. Checklist use in low-HDI countries was half that in high-HDI countries

    Intraperitoneal drain placement and outcomes after elective colorectal surgery: international matched, prospective, cohort study

    Despite current guidelines, intraperitoneal drain placement after elective colorectal surgery remains widespread. Drains were not associated with earlier detection of intraperitoneal collections, but were associated with prolonged hospital stay and increased risk of surgical-site infections.Background Many surgeons routinely place intraperitoneal drains after elective colorectal surgery. However, enhanced recovery after surgery guidelines recommend against their routine use owing to a lack of clear clinical benefit. This study aimed to describe international variation in intraperitoneal drain placement and the safety of this practice. Methods COMPASS (COMPlicAted intra-abdominal collectionS after colorectal Surgery) was a prospective, international, cohort study which enrolled consecutive adults undergoing elective colorectal surgery (February to March 2020). The primary outcome was the rate of intraperitoneal drain placement. Secondary outcomes included: rate and time to diagnosis of postoperative intraperitoneal collections; rate of surgical site infections (SSIs); time to discharge; and 30-day major postoperative complications (Clavien-Dindo grade at least III). After propensity score matching, multivariable logistic regression and Cox proportional hazards regression were used to estimate the independent association of the secondary outcomes with drain placement. Results Overall, 1805 patients from 22 countries were included (798 women, 44.2 per cent; median age 67.0 years). The drain insertion rate was 51.9 per cent (937 patients). After matching, drains were not associated with reduced rates (odds ratio (OR) 1.33, 95 per cent c.i. 0.79 to 2.23; P = 0.287) or earlier detection (hazard ratio (HR) 0.87, 0.33 to 2.31; P = 0.780) of collections. Although not associated with worse major postoperative complications (OR 1.09, 0.68 to 1.75; P = 0.709), drains were associated with delayed hospital discharge (HR 0.58, 0.52 to 0.66; P &lt; 0.001) and an increased risk of SSIs (OR 2.47, 1.50 to 4.05; P &lt; 0.001). Conclusion Intraperitoneal drain placement after elective colorectal surgery is not associated with earlier detection of postoperative collections, but prolongs hospital stay and increases SSI risk

    Global economic burden of unmet surgical need for appendicitis

    Background: There is a substantial gap in provision of adequate surgical care in many low-and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods: Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results: Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion: For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially

    Search for the jet-induced diffusion wake in the quark-gluon plasma via measurements of jet-track correlations in photon-jet events in Pb+Pb collisions at sNN=5.02TeV with the ATLAS detector

    This paper presents a measurement of jet-track correlations in photon-jet events, using 1.72 nb^(-1) of Pb+Pb data at root(sNN) = 5.02 TeV recorded with the ATLAS detector at the LHC. Events with energetic photon-jet pairs are selected, where the photon and jet are approximately back-to-back in azimuth. The angular correlation between jets and charged-particle tracks with transverse momentum (pT) in the range 0.5-2.0 GeV in the hemisphere opposite to the jet, | phi(jet,track)| &gt; pi/2, is measured as a function of their relative pseudorapidity difference, | eta(jet,track)|. In central Pb+Pb collisions, these correlations are predicted to be sensitive to the diffusion wake in the quark-gluon plasma resulting from the lost energy of high-pT partons traversing the plasma, with a characteristic modification as a function of | eta(jet,track)|. The correlations are examined with different selections on the jet-to-photon pT ratio to select events with different degrees of energy loss. No diffusion wake signal is observed within the current sensitivity and upper limits at 95% confidence level on the diffusion wake amplitude are reported

    Configuration, Performance, and Commissioning of the ATLAS b-jet Triggers for the 2022 and 2023 LHC data-taking periods

    In 2022 and 2023, the Large Hadron Collider produced approximately two billion hadronic interactions each second from bunches of protons that collide at a rate of 40 MHz. The ATLAS trigger system is used to reduce this rate to a few kHz for recording. Selections based on hadronic jets, their energy, and event topology reduce the rate to O(10) kHz while maintaining high efficiencies for important signatures resulting in b-quarks, but to reach the desired recording rate of hundreds of Hz, additional real-time selections based on the identification of jets containing b-hadrons (b-jets) are employed to achieve low thresholds on the jet transverse momentum at the High-Level Trigger. The configuration, commissioning, and performance of the real-time ATLAS b-jet identification algorithms for the early LHC Run 3 collision data are presented. These recent developments provide substantial gains in signal efficiency for critical signatures; for the Standard Model production of Higgs boson pairs, a 50% improvement in selection efficiency is observed in final states with four b-quarks or two b-quarks and two hadronically decaying tau-leptons

    Search for vector-like leptons coupling to first- and second-generation Standard Model leptons in pp collisions at s \sqrt{s} = 13 TeV with the ATLAS detector

    A search for pair production of vector-like leptons coupling to first- and secondgeneration Standard Model leptons is presented. The search is based on a dataset of proton-proton collisions at root s = 13 TeV recorded with the ATLAS detector during Run 2 of the Large Hadron Collider, corresponding to an integrated luminosity of 140fb-1. Events are categorised depending on the flavour and multiplicity of leptons (electrons or muons), as well as on the scores of a deep neural network targeting particular signal topologies according to the decay modes of the vector-like leptons. In each of the signal regions, the scalar sum of the transverse momentum of the leptons and the missing transverse momentum is analysed. The main background processes are estimated using dedicated control regions in a simultaneous fit with the signal regions to data. No significant excess above the Standard Model background expectation is observed and limits are set at 95% confidence level on the production cross-sections of vector-like electrons and muons as a function of the vector-like lepton mass, separately for SU(2) doublet and singlet scenarios. The resulting mass lower limits are 1220GeV (1270GeV) and 320GeV (400GeV) for vector-like electrons (muons) in the doublet and singlet scenarios, respectively
