Dielectric Characterization of Breast Biopsied Tissues as Pre-Pathological Aid in Early Cancer Detection: A Blinded Feasibility Study
Dielectric characterization has significant potential in several medical applications, providing valuable insights into the electromagnetic properties of biological tissues for disease diagnosis, treatment planning, and monitoring of therapeutic interventions. This work presents the use of a custom-designed electromagnetic characterization system, based on an open-ended coaxial probe, for discriminating between benign and malignant breast tissues in a clinical setting. The probe’s development involved a well-balanced compromise between physical feasibility and its combined use with a reconstruction algorithm known as the virtual transmission line model (VTLM). Immediately following the biopsy procedure, the dielectric properties of the breast tissues were reconstructed, enabling tissue discrimination based on a rule-of-thumb using the obtained dielectric parameters. A comparative analysis was then performed by analyzing the outcomes of the dielectric investigation with respect to conventional histological results. The experimental procedure took place at Complejo Hospitalario Universitario de Toledo—Hospital Virgen de la Salud, Spain, where excised breast tissues were collected and subsequently analyzed using the dielectric characterization system. A comprehensive statistical evaluation of the probe’s performance was carried out, obtaining a sensitivity, specificity, and accuracy of 81.6%, 61.5%, and 73.4%, respectively, compared to conventional histological assessment, considered as the gold standard in this investigation
The rate of cooling of the pulsating white dwarf star G117B15A: a new asteroseismological inference of the axion mass
We employ a state-of-the-art asteroseismological model of G117-B15A, the
archetype of the H-rich atmosphere (DA) white dwarf pulsators (also known as
DAV or ZZ Ceti variables), and use the most recently measured value of the rate
of period change for the dominant mode of this pulsating star to derive a new
constraint on the mass of axion, the still conjectural non-barionic particle
considered as candidate for dark matter of the Universe. Assuming that
G117-B15A is truly represented by our asteroseismological model, and in
particular, that the period of the dominant mode is associated to a pulsation
g-mode trapped in the H envelope, we find strong indications of the existence
of extra cooling in this star, compatible with emission of axions of mass m_a
\cos^2 \beta = 17.4^{+2.3}_{-2.7} meV.Comment: 9 pages, 5 figures and 3 tables. Accepted for publication in MNRA
ATOMS (Adjustable Transobturator Male System) Is an Effective and Safe Second-Line Treatment Option for Recurrent Urinary Incontinence after Implantation of an AdVance/AdVance XP Fixed Male Sling? A Multicenter Cohort Analysis
(1) Background: This study examined outcomes of second-line ATOMS implantation after failure of the fixed male sling (FMS) AdVance/AdVance XP. (2) Methods: A retrospective multicenter cohort analysis was carried out in men implanted with an ATOMS between 2011 and 2020 after failure of an AdVance/AdVance XP. Success was assessed on the basis of objective (dryness, 0–1 pad/24 h or >20 g/24 h pad test) and subjective results (PGI-I). We performed the Wilcoxon rank sum test, Fisher’s exact test, logistic regression, and multivariate analysis. (3) Results: The study included 88 patients from 9 centers with a mean age of 71.3 years. No Clavien–Dindo > II complications occurred within the first 3 months after ATOMS implantation. A total of 10 cases (9%) required revision in the ensuing clinical course. After a mean follow-up of 42.5 months, 76.1% achieved social continence, and 56.8% used no pads at all. Mean urine leakage/24 h dropped from 422 g (3.9 pads) to 38 g (0.69 pads) and the mean ICIQ-SF decreased from 16.25 to 5.3 (p < 0.0001). PROMs (patient-reported outcome measures) showed improvement in 98.9% of cases, and 63.6% gave a “very much better” PGI-I rating. Multivariate analysis identified a lower probability of achieving maximum satisfaction for the following factors: the AdVance XP as first-line therapy (OR 0.35), a lower ICIQ-SF question 1 (OR 0.26), status post-irradiation (OR 0.14), and more severe pain prior to ATOMS implantation (OR 0.51). (4) Conclusions: Implantation of an ATOMS is an effective and safe second-line treatment option for recurrent urinary incontinence after implantation of an AdVance/AdVance XP sling. High patient satisfaction was demonstrated in a long-term follow-up
Surgical management of enchondroma with joint transfer: a case report
The enchondromas are the most common benign bone tumor in the hands, mainly in young patients and its treatment is surgical in most cases due to its possible complications avoided through different procedures such as joint transfer, which allows to maintain a good quality of life for the patient. We present the case of a 42-year-old woman who came to the clinic for blunt trauma in the right hand where she was incidentally diagnosed with enchondroma on the third finger of the proximal interphalangeal joint of the right hand. Articular transfer of the proximal interphalangeal joint was performed. Second toe on the right, at 6 weeks corroborates correct evolution with bone consolidation of the articular transfer grade III and donor zone grade II, without infection complications, the correct recovery of the extensor movement range of this joint improving so their state of life should be the main objective of these procedures
The Millennium Run Observatory: First Light
Simulations of galaxy evolution aim to capture our current understanding as
well as to make predictions for testing by future experiments. Simulations and
observations are often compared in an indirect fashion: physical quantities are
estimated from the data and compared to models. However, many applications can
benefit from a more direct approach, where the observing process is also
simulated and the models are seen fully from the observer's perspective. To
facilitate this, we have developed the Millennium Run Observatory (MRObs), a
theoretical virtual observatory which uses virtual telescopes to `observe'
semi-analytic galaxy formation models based on the suite of Millennium Run dark
matter simulations. The MRObs produces data that can be processed and analyzed
using the standard software packages developed for real observations. At
present, we produce images in forty filters from the rest-frame UV to IR for
two stellar population synthesis models, three different models of IGM
absorption, and two cosmologies (WMAP1/7). Galaxy distributions for a large
number of mock lightcones can be `observed' using models of major ground- and
space-based telescopes. The data include lightcone catalogues linked to
structural properties of galaxies, pre-observation model images, mock telescope
images, and Source Extractor products that can all be traced back to the higher
level dark matter, semi-analytic galaxy, and lightcone catalogues available in
the Millennium database. Here, we describe our methods and announce a first
public release of simulated surveys (e.g., SDSS, CFHT-LS, GOODS, GOODS/ERS,
CANDELS, and HUDF). The MRObs browser, an online tool, further facilitates
exploration of the simulated data. We demonstrate the benefits of a direct
approach through a number of example applications (galaxy number counts in

Efficacy and safety of a hexanic extract of Serenoa repens (Permixon (R)) for the treatment of lower urinary tract symptoms associated with benign prostatic hyperplasia (LUTS/BPH): systematic review and meta-analysis of randomised controlled trials and observational studies
Objectives To comprehensively evaluate the efficacy and safety of the hexanic extract of Serenoa repens (HESr, Permixon (R); Pierre Fabre Medicament, Castres, France), at a dose of 320 mg daily, as monotherapy for the treatment of lower urinary tract symptoms associated with benign prostatic hyperplasia (LUTS/BPH). Materials and methods We conducted a systematic review and meta-analysis of randomised controlled trials (RCTs) and prospective observational studies in patients with LUTS/BPH identified through searches in Medline, Web of Knowledge (Institute for Scientific Information), Scopus, the Cochrane Library, and bibliographic references up to March 2017. Articles studying S. repens extracts other than Permixon were excluded. Data were collected on International Prostate Symptom Score (IPSS), maximum urinary flow rate (Qmax), nocturia, quality of life, prostate volume, sexual function, and adverse drug reactions (ADRs). Data obtained from RCTs and observational studies were analysed jointly and separately using a random effects model. A sub-group analysis was performed of studies that included patients on longer-term treatment (= 1 year). Results Data from 27 studies (15 RCTs and 12 observational studies) were included for meta-analysis (total N = 5 800). Compared with placebo, the HESr was associated with 0.64 (95% confidence interval [CI] -0.98 to -0.31) fewer voids/ night (P < 0.001) and an additional mean increase in Q(max) of 2.75 mL/s (95% CI 0.57 to 4.93; P = 0.01). When compared with a-blockers, the HESr showed similar improvements on IPSS (weighted mean difference [WMD] 0.57, 95% CI -0.27 to 1.42; P = 0.18) and a comparable increase in Q(max) to tamsulosin (WMD -0.02, 95% CI -0.71 to 0.66; P = 0.95). Efficacy assessed using the IPSS was similar after 6 months of treatment between the HESr and 5a-reductase inhibitors (5ARIs). Analysis of all available published data for the HESr showed a mean improvement in IPSS from baseline of -5.73 points (95% CI -6.91 to -4.54; P < 0.001). HESr did not negatively affect sexual function and no clinically relevant effect was observed on prostate-specific antigen. Prostate volume decreased slightly. Similar efficacy results were seen in patients treated for = 1 year (n = 447). The HESr had a favourable safety profile, with gastrointestinal disorders being the most frequent ADR (mean incidence of 3.8%). Conclusion The present meta-analysis, which includes all available RCTs and observational studies, shows that the HESr (Permixon) reduced nocturia and improved Q(max) compared with placebo and had a similar efficacy to tamsulosin and short-term 5ARI in relieving LUTS. HESr (Permixon) appears to be an efficacious and well-tolerated therapeutic option for the longterm medical treatment of LUTS/BPH
The onion universe: all sky lightcone simulations in spherical shells
Galaxy surveys provide a large-scale view of the universe that typically has
a limited line-of-sight or redshift resolution. The lack of radial accuracy in
these surveys can be modelled by picturing the universe as a set of concentric
radial shells of finite width around the observer, i.e, an onion-like
structure. We present a new N-body simulation with 2048^3 particles developed
at the Marenostrum supercomputer with the GADGET-2 code. Using the lightcone
output we build a set of angular maps that mimic this onion-like representation
of the universe. The onion maps are a highly compressed version of the raw data
(i.e. a factor >1000 smaller size for arcminute resolution maps) and they
provide a new and powerful tool to exploit large scale structure observations.
We introduce two basic applications of these maps that are especially useful
for constraning dark energy properties: the baryon acoustic oscillations (BAO)
in the galaxy power spectrum and all-sky maps of the weak lensing distortion.
Using the weak lensing maps, we measure the convergence power spectra and
compare it to halo fit predictions. As a further application, we compute the
variance and higher-order moments of the maps. We show that sampling variance
on scales of few degrees is quite large, resulting in a significant (25% at 10
arminute scales) bias in the variance. We caution that current lensing surveys
such as the COSMOS HST should take into account this bias and extra sampling
error in their clustering analyses and inferred cosmological parameter
constraints. Finally, we test the importance of projection effects in the weak
lensing mass reconstruction. On the mean, the mass calibration works well but
it exhibits a large non-Gaussian scatter what could induce a large bias in the

Work like a Doc: a comparison of regulations on residents' working hours in 14 high-income countries
Background: Medical residents work long, continuous hours. Working in conditions of extreme fatigue has adverse effects on the quality and safety of care, and on residents' quality of life. Many countries have attempted to regulate residents’ work hours. Objectives: We aimed to review residents’ work hours regulations in different countries with an emphasis on night shifts. Methods: Standardized qualitative data on residents’ working hours were collected with the assistance of experts from 14 high-income countries through a questionnaire. An international comparative analysis was performed. Results: All countries reviewed limit the weekly working hours; North-American countries limit to 60–80 h, European countries limit to 48 h. In most countries, residents work 24 or 26 consecutive hours, but the number of long overnight shifts varies, ranging from two to ten. Many European countries face difficulties in complying with the weekly hour limit and allow opt-out contracts to exceed it. Conclusions: In the countries analyzed, residents still work long hours. Attempts to limit the shift length or the weekly working hours resulted in modest improvements in residents’ quality of life with mixed effects on quality of care and residents’ education
