    Continuing or temporarily stopping prestroke antihypertensive medication in acute stroke: an individual patient data meta-analysis

    Over 50% of patients are already taking blood pressure–lowering therapy on hospital admission for acute stroke. An individual patient data meta-analysis from randomized controlled trials was undertaken to determine the effect of continuation versus temporarily stopping preexisting antihypertensive medication in acute stroke. Key databases were searched for trials against the following inclusion criteria: randomized design; stroke onset ≀48 hours; investigating the effect of continuation versus stopping prestroke antihypertensive medication; and follow-up of β‰₯2 weeks. Two randomized controlled trials were identified and included in this meta-analysis of individual patient data from 2860 patients with ≀48 hours of acute stroke. Risk of bias in each study was low. In adjusted logistic regression and multiple regression analyses (using random effects), we found no significant association between continuation of prestroke antihypertensive therapy (versus stopping) and risk of death or dependency at final follow-up: odds ratio 0.96 (95% confidence interval, 0.80–1.14). No significant associations were found between continuation (versus stopping) of therapy and secondary outcomes at final follow-up. Analyses for death and dependency in prespecified subgroups revealed no significant associations with continuation versus temporarily stopping therapy, with the exception of patients randomized ≀12 hours, in whom a difference favoring stopping treatment met statistical significance. We found no significant benefit with continuation of antihypertensive treatment in the acute stroke period. Therefore, there is no urgency to administer preexisting antihypertensive therapy in the first few hours or days after stroke, unless indicated for other comorbid conditions

    Treating the acute stroke patient as an emergency: current practices and future opportunities

    Developments in acute stroke therapy have followed advances in the understanding of the evolving pathophysiology in both ischaemic stroke and intracerebral haemorrhage (ICH). In ischaemic stroke, rapid reperfusion of the ischaemic penumbra with thrombolysis within 3 h of symptom onset is of proven benefit, but few patients currently receive therapy, mainly due to the short-time window and lack of stroke expertise. In ICH, a recent study indicated that a haemostatic agent can limit ongoing bleeding and improve outcomes when administered within 4 h of stroke onset. These advances in acute stroke therapy underlie the concept that β€˜time is brain’ and that urgent intervention can limit cerebral damage. Neuroprotective therapy could offer the prospect of a greater proportion of stroke patients receiving treatment, potentially before imaging and even in the ambulance setting. Virtually all stroke patients would benefit from receiving multidisciplinary care in acute stroke units

    Height and timing of growth spurt during puberty in young people living with vertically acquired HIV in Europe and Thailand.

    OBJECTIVE: The aim of this study was to describe growth during puberty in young people with vertically acquired HIV. DESIGN: Pooled data from 12 paediatric HIV cohorts in Europe and Thailand. METHODS: One thousand and ninety-four children initiating a nonnucleoside reverse transcriptase inhibitor or boosted protease inhibitor based regimen aged 1-10 years were included. Super Imposition by Translation And Rotation (SITAR) models described growth from age 8 years using three parameters (average height, timing and shape of the growth spurt), dependent on age and height-for-age z-score (HAZ) (WHO references) at antiretroviral therapy (ART) initiation. Multivariate regression explored characteristics associated with these three parameters. RESULTS: At ART initiation, median age and HAZ was 6.4 [interquartile range (IQR): 2.8, 9.0] years and -1.2 (IQR: -2.3 to -0.2), respectively. Median follow-up was 9.1 (IQR: 6.9, 11.4) years. In girls, older age and lower HAZ at ART initiation were independently associated with a growth spurt which occurred 0.41 (95% confidence interval 0.20-0.62) years later in children starting ART age 6 to 10 years compared with 1 to 2 years and 1.50 (1.21-1.78) years later in those starting with HAZ less than -3 compared with HAZ at least -1. Later growth spurts in girls resulted in continued height growth into later adolescence. In boys starting ART with HAZ less than -1, growth spurts were later in children starting ART in the oldest age group, but for HAZ at least -1, there was no association with age. Girls and boys who initiated ART with HAZ at least -1 maintained a similar height to the WHO reference mean. CONCLUSION: Stunting at ART initiation was associated with later growth spurts in girls. Children with HAZ at least -1 at ART initiation grew in height at the level expected in HIV negative children of a comparable age

    Measurements of long-range near-side angular correlations in sNN=5\sqrt{s_{\text{NN}}}=5TeV proton-lead collisions in the forward region

    Two-particle angular correlations are studied in proton-lead collisions at a nucleon-nucleon centre-of-mass energy of sNN=5\sqrt{s_{\text{NN}}}=5TeV, collected with the LHCb detector at the LHC. The analysis is based on data recorded in two beam configurations, in which either the direction of the proton or that of the lead ion is analysed. The correlations are measured in the laboratory system as a function of relative pseudorapidity, Δη\Delta\eta, and relative azimuthal angle, Δϕ\Delta\phi, for events in different classes of event activity and for different bins of particle transverse momentum. In high-activity events a long-range correlation on the near side, Ξ”Ο•β‰ˆ0\Delta\phi \approx 0, is observed in the pseudorapidity range 2.0<Ξ·<4.92.0<\eta<4.9. This measurement of long-range correlations on the near side in proton-lead collisions extends previous observations into the forward region up to Ξ·=4.9\eta=4.9. The correlation increases with growing event activity and is found to be more pronounced in the direction of the lead beam.     Evidence for the strangeness-changing weak decay Ξbβˆ’β†’Ξ›b0Ο€βˆ’\Xi_b^-\to\Lambda_b^0\pi^-

    Using a pppp collision data sample corresponding to an integrated luminosity of 3.0~fbβˆ’1^{-1}, collected by the LHCb detector, we present the first search for the strangeness-changing weak decay Ξbβˆ’β†’Ξ›b0Ο€βˆ’\Xi_b^-\to\Lambda_b^0\pi^-. No bb hadron decay of this type has been seen before. A signal for this decay, corresponding to a significance of 3.2 standard deviations, is reported. The relative rate is measured to be fΞbβˆ’fΞ›b0B(Ξbβˆ’β†’Ξ›b0Ο€βˆ’)=(5.7Β±1.8βˆ’0.9+0.8)Γ—10βˆ’4{{f_{\Xi_b^-}}\over{f_{\Lambda_b^0}}}{\cal{B}}(\Xi_b^-\to\Lambda_b^0\pi^-) = (5.7\pm1.8^{+0.8}_{-0.9})\times10^{-4}, where fΞbβˆ’f_{\Xi_b^-} and fΞ›b0f_{\Lambda_b^0} are the bβ†’Ξžbβˆ’b\to\Xi_b^- and bβ†’Ξ›b0b\to\Lambda_b^0 fragmentation fractions, and B(Ξbβˆ’β†’Ξ›b0Ο€βˆ’){\cal{B}}(\Xi_b^-\to\Lambda_b^0\pi^-) is the branching fraction.     Study of the production of Ξ›b0\Lambda_b^0 and Bβ€Ύ0\overline{B}^0 hadrons in pppp collisions and first measurement of the Ξ›b0β†’J/ψpKβˆ’\Lambda_b^0\rightarrow J/\psi pK^- branching fraction

    The product of the Ξ›b0\Lambda_b^0 (Bβ€Ύ0\overline{B}^0) differential production cross-section and the branching fraction of the decay Ξ›b0β†’J/ψpKβˆ’\Lambda_b^0\rightarrow J/\psi pK^- (Bβ€Ύ0β†’J/ψKβ€Ύβˆ—(892)0\overline{B}^0\rightarrow J/\psi\overline{K}^*(892)^0) is measured as a function of the beauty hadron transverse momentum, pTp_{\rm T}, and rapidity, yy. The kinematic region of the measurements is pT<20Β GeV/cp_{\rm T}<20~{\rm GeV}/c and 2.0<y<4.52.0<y<4.5. The measurements use a data sample corresponding to an integrated luminosity of 3Β fbβˆ’13~{\rm fb}^{-1} collected by the LHCb detector in pppp collisions at centre-of-mass energies s=7Β TeV\sqrt{s}=7~{\rm TeV} in 2011 and s=8Β TeV\sqrt{s}=8~{\rm TeV} in 2012. Based on previous LHCb results of the fragmentation fraction ratio, fΞ›B0/fdf_{\Lambda_B^0}/f_d, the branching fraction of the decay Ξ›b0β†’J/ψpKβˆ’\Lambda_b^0\rightarrow J/\psi pK^- is measured to be \begin{equation*} \mathcal{B}(\Lambda_b^0\rightarrow J/\psi pK^-)= (3.17\pm0.04\pm0.07\pm0.34^{+0.45}_{-0.28})\times10^{-4}, \end{equation*} where the first uncertainty is statistical, the second is systematic, the third is due to the uncertainty on the branching fraction of the decay Bβ€Ύ0β†’J/ψKβ€Ύβˆ—(892)0\overline{B}^0\rightarrow J/\psi\overline{K}^*(892)^0, and the fourth is due to the knowledge of fΞ›b0/fdf_{\Lambda_b^0}/f_d. The sum of the asymmetries in the production and decay between Ξ›b0\Lambda_b^0 and Ξ›β€Ύb0\overline{\Lambda}_b^0 is also measured as a function of pTp_{\rm T} and yy. The previously published branching fraction of Ξ›b0β†’J/ψpΟ€βˆ’\Lambda_b^0\rightarrow J/\psi p\pi^-, relative to that of Ξ›b0β†’J/ψpKβˆ’\Lambda_b^0\rightarrow J/\psi pK^-, is updated. The branching fractions of Ξ›b0β†’Pc+(β†’J/ψp)Kβˆ’\Lambda_b^0\rightarrow P_c^+(\rightarrow J/\psi p)K^- are determined.Comment: 29 pages, 19figures.     Search for the rare decays B0β†’J/ψγB^{0}\to J/\psi \gamma and Bs0β†’J/ψγB^{0}_{s} \to J/\psi \gamma

    A search for the rare decay of a B0B^{0} or Bs0B^{0}_{s} meson into the final state J/ψγJ/\psi\gamma is performed, using data collected by the LHCb experiment in pppp collisions at s=7\sqrt{s}=7 and 88 TeV, corresponding to an integrated luminosity of 3 fbβˆ’1^{-1}. The observed number of signal candidates is consistent with a background-only hypothesis. Branching fraction values larger than 1.7Γ—10βˆ’61.7\times 10^{-6} for the B0β†’J/ψγB^{0}\to J/\psi\gamma decay mode are excluded at 90% confidence level.     Mismatch-repair protein MSH6 is associated with Ku70 and regulates DNA double-strand break repair

    MSH6, a key component of the MSH2–MSH6 complex, plays a fundamental role in the repair of mismatched DNA bases. Herein, we report that MSH6 is a novel Ku70-interacting protein identified by yeast two-hybrid screening. Ku70 and Ku86 are two key regulatory subunits of the DNA-dependent protein kinase, which plays an essential role in repair of DNA double-strand breaks (DSBs) through the non-homologous end-joining (NEHJ) pathway. We found that association of Ku70 with MSH6 is enhanced in response to treatment with the radiomimetic drug neocarzinostatin (NCS) or ionizing radiation (IR), a potent inducer of DSBs. Furthermore, MSH6 exhibited diffuse nuclear staining in the majority of untreated cells and forms discrete nuclear foci after NCS or IR treatment. MSH6 colocalizes with Ξ³-H2AX at sites of DNA damage after NCS or IR treatment. Cells depleted of MSH6 accumulate high levels of persistent DSBs, as detected by formation of Ξ³-H2AX foci and by the comet assay. Moreover, MSH6-deficient cells were also shown to exhibit impaired NHEJ, which could be rescued by MSH6 overexpression. MSH6-deficient cells were hypersensitive to NCS- or IR-induced cell death, as revealed by a clonogenic cell-survival assay. These results suggest a potential role for MSH6 in DSB repair through upregulation of NHEJ by association with Ku70

    One origin for metallo-Ξ²-lactamase activity, or two? An investigation assessing a diverse set of reconstructed ancestral sequences based on a sample of phylogenetic trees

    This work was supported by BBSRC (grant BB/F016778/1)Bacteria use metallo-Ξ²-lactamase enzymes to hydrolyse lactam rings found in many antibiotics, rendering them ineffective. Metallo-Ξ²-lactamase activity is thought to be polyphyletic, having arisen on more than one occasion within a single functionally diverse homologous superfamily. Since discovery of multiple origins of enzymatic activity conferring antibiotic resistance has broad implications for the continued clinical use of antibiotics, we test the hypothesis of polyphyly further; if lactamase function has arisen twice independently, the most recent common ancestor (MRCA) is not expected to possess lactam-hydrolysing activity. Two major problems present themselves. Firstly, even with a perfectly known phylogeny, ancestral sequence reconstruction is error prone. Secondly, the phylogeny is not known, and in fact reconstructing a single, unambiguous phylogeny for the superfamily has proven impossible. To obtain a more statistical view of the strength of evidence for or against MRCA lactamase function, we reconstructed a sample of 98 MRCAs of the metallo-Ξ²-lactamases, each based on a different tree in a bootstrap sample of reconstructed phylogenies. InterPro sequence signatures and homology modelling were then used to assess our sample of MRCAs for lactamase functionality. Only 5 % of these models conform to our criteria for metallo-Ξ²-lactamase functionality, suggesting that the ancestor was unlikely to have been a metallo-Ξ²-lactamase. On the other hand, given that ancestral proteins may have had metallo-Ξ²-lactamase functionality with variation in sequence and structural properties compared with extant enzymes, our criteria are conservative, estimating a lower bound of evidence for metallo-Ξ²-lactamase functionality but not an upper bound.Publisher PDFPeer reviewe

    Breaking Functional Connectivity into Components: A Novel Approach Using an Individual-Based Model, and First Outcomes

    Landscape connectivity is a key factor determining the viability of populations in fragmented landscapes. Predicting β€˜functional connectivity’, namely whether a patch or a landscape functions as connected from the perspective of a focal species, poses various challenges. First, empirical data on the movement behaviour of species is often scarce. Second, animal-landscape interactions are bound to yield complex patterns. Lastly, functional connectivity involves various components that are rarely assessed separately. We introduce the spatially explicit, individual-based model FunCon as means to distinguish between components of functional connectivity and to assess how each of them affects the sensitivity of species and communities to landscape structures. We then present the results of exploratory simulations over six landscapes of different fragmentation levels and across a range of hypothetical bird species that differ in their response to habitat edges. i) Our results demonstrate that estimations of functional connectivity depend not only on the response of species to edges (avoidance versus penetration into the matrix), the movement mode investigated (home range movements versus dispersal), and the way in which the matrix is being crossed (random walk versus gap crossing), but also on the choice of connectivity measure (in this case, the model output examined). ii) We further show a strong effect of the mortality scenario applied, indicating that movement decisions that do not fully match the mortality risks are likely to reduce connectivity and enhance sensitivity to fragmentation. iii) Despite these complexities, some consistent patterns emerged. For instance, the ranking order of landscapes in terms of functional connectivity was mostly consistent across the entire range of hypothetical species, indicating that simple landscape indices can potentially serve as valuable surrogates for functional connectivity. Yet such simplifications must be carefully evaluated in terms of the components of functional connectivity they actually predict
