    Efecto de la renta sobre la incidencia acumulada de COVID-19: un estudio ecológico

    Objetivo analisar a relação entre renda per capita e incidência acumulada de COVID-19 nos bairros do município do Rio de Janeiro. Método estudo ecológico utilizando os bairros como unidades de análise. Foram calculadas as taxas de incidência acumulada por 100 mil habitantes e a mediana de possíveis variáveis confundidoras (sexo, raça/cor e idade). A análise múltipla contou com técnica de regressão quantílica, estimando-se os coeficientes de regressão da variável renda a cada cinco percentis no intervalo entre os percentis dez e 90, a fim de verificar a relação entre renda e incidência. Resultados a taxa municipal foi de 36,58 casos novos por 100 mil habitantes. Em geral, as maiores taxas da doença foram observadas nas regiões de elevada renda. A análise múltipla coadunou com esta observação, já que a renda per capita apresentou efeito em todos os percentis analisados, tendo coeficiente de regressão mediano de 0,02 (p-valor &lt;0,001; R2 32,93), ou seja, que a cada unidade de incidência elevam-se R0,02 na renda per capita do bairro. Conclusão as taxas de incidência acumulada de COVID-19 são influenciadas pela renda do bairro de residência dos casos, sugerindo que o acesso aos exames esteja ocorrendo de modo desigual.Objetivo analizar la relación entre la renta per cápita y la incidencia acumulada de COVID-19 en los barrios del municipio de Rio de Janeiro. El análisis múltiple contó con la técnica de regresión cuantílica, estimándose los coeficientes de regresión de la variable renta por cada cinco percentiles en el intervalo entre los percentiles 10 y 90, con el fin de verificar la relación entre la renta alquiler y la incidencia. Resultados la tasa municipal fue de 36,58 casos nuevos por 100 000 habitantes. En general, se observaron las mayores tasas de la enfermedad en las regiones de renta alta. El análisis múltiple coincidió con esta observación, ya que la renta per cápita tuvo efecto en todos los percentiles analizados, con un coeficiente de regresión medio de 0,02 (valor de p <0,001; R2 32,93). Es decir, para cada unidad de incidencia aumenta R0,02 la renta per cápita del barrio. Conclusión las tasas de incidencia acumuladas de COVID-19 se ven influidas por la renta del barrio de residencia de los casos, lo que sugiere que el acceso a los exámenes se produce de forma desigual.Objective to analyze the relationship between per capita income and the cumulative incidence of COVID-19 in the neighborhoods of the city of Rio de Janeiro, RJ, Brazil. Method an ecological study using neighborhoods as units of analysis. Multiple analysis included quantile regression, estimating the regression coefficients of the variable income for every five percentiles from the 10th to 90th percentiles to verify the relationship between income and incidence. Results the city’s rate was 36.58 new cases per 100,000 inhabitants. In general, the highest rates were observed in the wealthiest regions. Multiple analysis was consistent with this observation since the per capita income affected all percentiles analyzed, with a median regression coefficient of 0.02 (p-value &lt;0.001; R2 32.93). That is, there is an increase of R$ 0.02 in the neighborhood’s per capita income for every unit of incidence.     O presente trabalho tem por objetivo identificar a relevância da usualidade da auditoria interna como auxílio direto na detecção e prevenção de possíveis erros e fraudes dentro das organizações. Com o alto índice de usabilidade, a auditoria interna passou a ser um instrumento de controle, no qual os administradores estão utilizando para verificação da eficiência e eficácia das atividades operacionais da empresa. Quanto à metodologia, trata-se de um estudo de caráter qualitativo de forma transversal. A pesquisa deu andamento por meio de uma entrevista contendo onze questões abertas direcionada aos gestores de duas empresas do setor de comercio varejista de material de construção. Com a análise dos dados, foi possível identificar que a utilização da auditoria interna fornece inúmeras qualidades ao serviço interno da empresa, pois sua utilização se tornou uma ferramenta de gestão altamente confiável no meio empresarial. Devido a isso, a auditoria interna sendo utilizada de forma adequada, fornece inúmeras melhorias internas, desenvolvimento dos índices, entrega de informações integras e tempestivas para as tomadas de decisões, além de evitar a ocorrência de erros, fraudes e falhas. Dessa forma, os objetivos traçados foram alcançados. Em relação aos resultados, pode-se afirmar que a auditoria serve de auxílio direto na detecção de erros e fraudes, pois a mesma gera auxílios internos para que possa atingir os objetivos traçados e sem contar que a sua implantação resulta na melhoria contínua dos processos produtivos, na capacidade administrativa em obter decisões mercadológicas, no controle operacional da gestão e instalação e manutenção de controles internos

    Cardiovascular abnormalities in patients with oral cleft: a clinical-electrocardiographic-echocardiographic study

    OBJECTIVES: The present study aims to describe the clinical, electrocardiographic, and echocardiographic cardiological findings in a group of patients with oral clefts. METHODS: This is a prospective cross-sectional study on 70 children (age range from 13 days to 19 years) with oral clefts who attended the multidisciplinary program of a university hospital from March 2013 to September 2014. The patients were evaluated by a pediatric cardiologist and underwent detailed anamnesis, physical examination, electrocardiogram, and echocardiogram. RESULTS: Sixty percent of the patients were male; 55.7% presented with cleft lip and palate, and 40.0% presented with health complaints. Comorbidities were found in 44.3%. Relevant pregnancy, neonatal, family and personal antecedents were present in 55.7%, 27.1%, 67.2%, and 24.3% of the patients, respectively. Regarding the antecedents, 15.2% of the patients presented with a cardiac murmur, 49.0% with a familial risk of developing plurimetabolic syndrome, and 6% with family antecedents of rheumatic fever. Electrocardiographic evaluation showed one case of atrioventricular block. Echocardiograms were abnormal in 35.7% of the exams, including 5 cases of mitral valve prolapse — one of which was diagnosed with rheumatic heart disease. CONCLUSION: The finding of a family risk of developing plurimetabolic syndrome and a diagnosis of rheumatic heart disease indicates that patients with oral clefts may be more prone to developing acquired heart disease. Thus, our findings highlight the importance of anamnesis and methodological triangulation (clinical-electrocardiographic-echocardiographic) in the investigation of patients with oral clefts and emphasize that cardiological follow-up to evaluate acquired and/or rhythm heart diseases is necessary. This strategy permits comorbidity prevention and individualized planned treatment

    Detection of bacterial samples on the aquatic ecosystems adjacent to Saquarema Lagoon – Rio de Janeiro

    Introduction: Saquarema Lagoon (RJ) has a high ecological and economic value owing to its multiple uses. The population’s constant growth increases the amount of sewage containing bacteria and antimicrobial drugs that are discharged to the environment. Objectives: to detect Gram negative bacilli able to colonize or infect humans and animals and determine their antimicrobial resistance profiles. Methodology:samples were collected in the city centre in April 2010 and at Jaconé (Lagoon’s most preserved site) in February 2011. The total and thermo tolerant coliforms were determined and the isolation of samples was made using agar media containing  32cg/ mL of cephalotin. All samples were tested for antimicrobial susceptibility (AST) and on 16 samples, plasmid DNA was extracted. Results: different Gram negative bacteria were detected, such as: Enterobacter spp, Citrobacter freundii, Klebsiella pneumoniae and Pseudomonas aeruginosa. The coliform results showed that the water quality is proper for Human recreation. AST results demonstrated the existence of bacteria resistant to antimicrobial drugs frequently used in the community. It was possible to detected high molecular weight plasmids and nine samples (56,25%) showed at least one plasmid DNA electrophoresis band. Conclusions: there were not detected resistant samples to antimicrobial drugs normally used in hospital settings, which may possibly refute the idea of a contamination by nosocomial and/or veterinary sewage discharge

O tempo vivido na Europa e no contexto global tem vindo a acentuar a urgência das prioridades quantificadas, com um discurso dominante onde há menos política (pessoas) e mais representação económica (coisas). O correlato entre pessoas e coisas é, como sabemos, o dinheiro, ou trabalho reificado. A crise europeia, em torno da dívida soberana e dos maiores orçamentos do mundo, da capacidade da sua gestão na linguagem dura dos mercados e das taxas de juro veio modificar os objetivos imediatos da Europa, que em 2000 eram ambiciosos — “a sociedade mais competitiva do mundo em 2010” — para uma estratégia de emergência, agora chamada horizonte 2020. Este é o panorama ideal para colocar o ensino artístico em risco. Os fóruns internacionais passaram a valorizar os resultados da educação em rankings e sondagens de aproveitamento, cuja principal estratégia e preocupação é a mensurabilidade e comparabilidade, como são exemplo os relatórios PISA: avaliam-se em todos os países, as competências em Ciências, Matemática e Língua Materna.     Durvalumab Plus Carboplatin/Paclitaxel Followed by Maintenance Durvalumab With or Without Olaparib as First-Line Treatment for Advanced Endometrial Cancer: The Phase III DUO-E Trial

    PURPOSE Immunotherapy and chemotherapy combinations have shown activity in endometrial cancer, with greater benefit in mismatch repair (MMR)-deficient (dMMR) than MMR-proficient (pMMR) disease. Adding a poly(ADP-ribose) polymerase inhibitor may improve outcomes, especially in pMMR disease. METHODS This phase III, global, double-blind, placebo-controlled trial randomly assigned eligible patients with newly diagnosed advanced or recurrent endometrial cancer 1:1:1 to: carboplatin/paclitaxel plus durvalumab placebo followed by placebo maintenance (control arm); carboplatin/paclitaxel plus durvalumab followed by maintenance durvalumab plus olaparib placebo (durvalumab arm); or carboplatin/paclitaxel plus durvalumab followed by maintenance durvalumab plus olaparib (durvalumab + olaparib arm). The primary end points were progression-free survival (PFS) in the durvalumab arm versus control and the durvalumab + olaparib arm versus control. RESULTS Seven hundred eighteen patients were randomly assigned. In the intention-to-treat population, statistically significant PFS benefit was observed in the durvalumab (hazard ratio [HR], 0.71 [95% CI, 0.57 to 0.89]; P = .003) and durvalumab + olaparib arms (HR, 0.55 [95% CI, 0.43 to 0.69]; P < .0001) versus control. Prespecified, exploratory subgroup analyses showed PFS benefit in dMMR (HR [durvalumab v control], 0.42 [95% CI, 0.22 to 0.80]; HR [durvalumab + olaparib v control], 0.41 [95% CI, 0.21 to 0.75]) and pMMR subgroups (HR [durvalumab v control], 0.77 [95% CI, 0.60 to 0.97]; HR [durvalumab + olaparib v control] 0.57; [95% CI, 0.44 to 0.73]); and in PD-L1-positive subgroups (HR [durvalumab v control], 0.63 [95% CI, 0.48 to 0.83]; HR [durvalumab + olaparib v control], 0.42 [95% CI, 0.31 to 0.57]). Interim overall survival results (maturity approximately 28%) were supportive of the primary outcomes (durvalumab v control: HR, 0.77 [95% CI, 0.56 to 1.07]; P = .120; durvalumab + olaparib v control: HR, 0.59 [95% CI, 0.42 to 0.83]; P = .003). The safety profiles of the experimental arms were generally consistent with individual agents. CONCLUSION Carboplatin/paclitaxel plus durvalumab followed by maintenance durvalumab with or without olaparib demonstrated a statistically significant and clinically meaningful PFS benefit in patients with advanced or recurrent endometrial cancer

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.     Search for dark photons in Higgs boson production via vector boson fusion in proton-proton collisions at √s = 13 TeV

    A search is presented for a Higgs boson that is produced via vector boson fusion and that decays to an undetected particle and an isolated photon. The search is performed by the CMS collaboration at the LHC, using a data set corresponding to an integrated luminosity of 130 fb−1, recorded at a center-of-mass energy of 13 TeV in 2016–2018. No significant excess of events above the expectation from the standard model background is found. The results are interpreted in the context of a theoretical model in which the undetected particle is a massless dark photon. An upper limit is set on the product of the cross section for production via vector boson fusion and the branching fraction for such a Higgs boson decay, as a function of the Higgs boson mass. For a Higgs boson mass of 125 GeV, assuming the standard model production rates, the observed (expected) 95% confidence level upper limit on the branching fraction is 3.5 (2.8)%. This is the first search for such decays in the vector boson fusion channel. Combination with a previous search for Higgs bosons produced in association with a Z boson results in an observed (expected) upper limit on the branching fraction of 2.9 (2.1)% at 95% confidence level
