    Instrumentos de avaliação sensorial pós-acidente vascular encefálico (AVE) descritos em português: uma revisão sistemática

    Most survivors of stroke present, among other consequences, a sensory deficit. To assess the impact of this deficit on the activity and the effect of sensory reeducation protocols, it is important to use objective instruments. The objective of this systematic review was to analyze which sensory evaluation tools for post-stroke patients are available in Brazilian Portuguese and describe its features and/or properties. A search was conducted in the electronic databases SCIELO, LILACS, PUBMED, CINAHL, MEDLINE via OVDI, and EMBASE. The relevant studies were analyzed as to accessibility, objectivity of the score and measurement properties, adding articles through manual search when necessary. The search resulted in 96 studies, reduced to five eligible ones. A study was added through the list of references and the manual search was used for complementation. Among the seven reviewed articles, only three sensory evaluation tools are available in Portuguese: moving touch pressure (MTP), sensitivity domain of Fugl-Meyer Scale (FMS), and Nottingham Sensory Assessment (NSA). In addition, although the reliability of the FMS and of the NSA may be considered appropriate, the other properties of measurement need to be evaluated in future studies.Gran parte de los supervivientes de accidente vascular encefálico (AVE) presentan, además de otras secuelas, alguno déficit sensorial. Para evaluar el impacto de ese déficit en la actividad y el efecto de protocolos de reeducación sensorial es importante utilizar instrumentos objetivos. Los objetivos de esta revisión sistemática son analizar cuales instrumentos de evaluación sensorial para pacientes pos-AVE están disponibles en portugués brasileño y describir sus características y/o propiedades. Se realizó una búsqueda en las bases de datos electrónicos SciELO, LILACS, PubMed, CINAHL, MEDLINE vía OVDI y Embase. Los estudios relevantes fueron analizados en relación a la accesibilidad, objetividad de la puntuación y propiedades de medida, añadiéndose artículos mediante búsqueda manual cuando necesario. Las búsquedas resultaron en 96 estudios, reducidos a cinco estudios elegibles. Se agregó un estudio mediante la lista de referencias, y se utilizó la búsqueda manual para la complementación. Entre los siete artículos analizados, hay solamente tres instrumentos de evaluación sensorial disponibles en portugués: Moving touch pressure (MTP), dominio sensibilidad de la Escala de Fugl-Meyer (EFM) y Evaluación Sensorial de Nottingham (ESN). Además, aunque la confiabilidad de la EFM y de la ESN es considerada adecuada, las demás propiedades de medida necesitan ser evaluadas en estudios futuros.Grande parte dos sobreviventes de acidente vascular encefálico (AVE) apresenta, além de outras sequelas, algum déficit sensorial.     Es imposible pensar que el desarrollo de habilidades de pensamiento científico puedan realizarse sin un contexto problematizado que lo contenga, por ello en este trabajo se propone una intervención educativa entre docentes investigadores de la Facultad de Ciencias Exactas, Físicas y Naturales de la Universidad Nacional de Córdoba, docentes y alumnos de 5º año del establecimiento educativo IPEM 360 con especialización en ciencias naturales y habitantes de un asentamiento urbano marginal; para propiciar prácticas que impulsen la dimensión comunitaria de las personas como parte de su proyecto personal.     Utilidad de las fórmulas CDK-EPI y MDRD-4 para estimar el índice de filtración glomerular en pacientes con patología glomerular

    More than 40 formulas have been developed to estimate the glomerular filtration rate (GFR) involving creatinine concentration (Cr) as well as demographic and anthropometric variables. Modification of Diet in Renal Disease (MDRD-4) is the most recommended one. However, several studies state that Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation improves the results obtained with MDRD-4. Our aim was to compare the results of the estimated GFR (eGFR) obtained with both formulas in patients with glomerular pathology. We studied 32 individuals with glomerulopathies, age (mean±SD): 34±8 years old, 24 females (F) and 8 males (M). Cr was assessed by an automated method. Results were expressed as mean±SD. eGFR (ml/min) with CKD-EPI and MDRD-4 were 102±41 and 105±51 respectively; no significant differences were obtained between both formulas (p>0.05). Correlation coefficient between both equations was highly significant (r=0.93; p0.05). We conclude that both formulas could be used indistinctly as estimators of GFR in patients with glomerular pathology. For GFR values > normal values, eGFR with both equations would tend to a poorer correlation, then stratification and comparison of these values with those obtained from Cr clearance are suggested in order to determine which equation would be the best analytical predictor in patients with glomerulopathy and glomerular hyperfiltration.Fil: Pezzarini, Eleonora.     Genetic associations at 53 loci highlight cell types and biological pathways relevant for kidney function.

    Reduced glomerular filtration rate defines chronic kidney disease and is associated with cardiovascular and all-cause mortality. We conducted a meta-analysis of genome-wide association studies for estimated glomerular filtration rate (eGFR), combining data across 133,413 individuals with replication in up to 42,166 individuals. We identify 24 new and confirm 29 previously identified loci. Of these 53 loci, 19 associate with eGFR among individuals with diabetes. Using bioinformatics, we show that identified genes at eGFR loci are enriched for expression in kidney tissues and in pathways relevant for kidney development and transmembrane transporter activity, kidney structure, and regulation of glucose metabolism. Chromatin state mapping and DNase I hypersensitivity analyses across adult tissues demonstrate preferential mapping of associated variants to regulatory regions in kidney but not extra-renal tissues. These findings suggest that genetic determinants of eGFR are mediated largely through direct effects within the kidney and highlight important cell types and biological pathways

    Antiphospholipid and antioangiogenic activity in females with recurrent miscarriage and antiphospholipid syndrome

    Background: Antiphospholipid syndrome is an autoimmune disease characterized by thrombosis, fetal losses and thrombocytopenia associated to antiphospholipid antibodies. They are directed to phospholipids, such as cardiolipins (anticardiolipin) and lupus anticoagulant or to complexes formed by phospholipids and protein cofactors, such as b2 glycoprotein 1 (a-b2GP1) and annexin V (a-annexin V). These auto-antibodies may be considered as a family of antibodies involved in thrombotic events and antiphospholipid activity. On the other hand, some proangiogenic factors are involved in the normal development of placental vasculature, such as the vascular endothelial growth factor. Overexpression of vascular endothelial growth factor receptor in its soluble form (sVEGFR-1) has been associated to a higher antiangiogenic activity. Our aim was to analyse the association between anticardiolipin, lupus anticoagulant, a-b2GP1, a-annexin V and sVEGFR-1 with recurrent miscarriage before week 10 of gestation in females with antiphospholipid syndrome. Methods: We studied 24 females (primary or secondary antiphospholipid syndrome), who were divided into two groups: females with recurrent miscarriage before week 10 of gestation (M; n=12) and females with no history of fetal loss (NM; n=12). Anticardiolipin, a-b2GP1, a-annexin V and sVEGF-R1 concentrations were assessed by ELISA, while lupus anticoagulant was assessed by screening and confirmatory tests. Results: A significant association was observed between the number of positive biomarkers and the belonging group (P<0.05). Besides, a positive result for lupus anticoagulant and a-b2GP1 was found to be significantly associated to the M group (P<0.05). Conclusions: Lupus anticoagulant and a-b2GP1 may be implicated in pregnancies complicated by recurrent miscarriage in females with antiphospholipid syndrome.Fil: Pelusa, Héctor Fabián.     Cross-cultural adaptation of the Chedoke Arm and Hand Activity Inventory (CAHAI)

    Introduction: The Chedoke Arm and Hand Activity Inventory (CAHAI) was developed in English and French (original version in both languages) for evaluation of the level of the upper limb activity in subjects with hemiparesis after stroke. Objective: To translate and cross-culturally adapt the manual of application and scoring of CAHAI to Portuguese-Brazil. Method: The process included six steps: the translation process with two independent translation; merging of the two translation; layout, typography and grammar review; two independent backtranslations; meeting with the Committee of Experts, and sending to the author of the original version, and pre-testing of the version CAHAI-Brazil (raters: n=5; subjects: n=4). Results: The CAHAI-Brasil version had satisfactory results in the translation and adaptation, and appropriate index of agreement among raters (kappa between 0,76 and 1,00). Some expressions in the manual and some of the materials used for the test had to be adapted to Brazilian culture. Conclusion: This study show the CAHAI-Brazil version was successfully translated and adapted

    Alteration of the microRNA-122 regulatory network in rat models of hepatotoxicity

    MicroRNAs are small RNA molecules that post-transcriptionally regulate gene expression. MicroRNA-122 is the most abundant and specific liver microRNA. Hepatotoxicity involves a significant alteration of liver gene expression. The aim of this work was to evaluate the microRNA-122 regulatory network in models of hepatotoxicity induced by thioacetamide or carbon tetrachloride. We report that the toxins decreased the expression of microRNA-122, which corresponded with an increase in two target genes: Cyclin G1 and the cationic amino acid transporter CAT-1. We found a decreased expression of its precursor, pri-microRNA-122, and of the transcription factors that specifically bind its promoter: CCAAT/enhancer-binding protein alpha, and members of the hepatocyte nuclear factor family. Therefore, microRNA-122 expression levels are under transcriptional control during hepatotoxicity. We propose that the changes observed are associated with the liver response to cope with the injury caused by the hepatotoxins, likely through a cell proliferation process to repair the damaged tissue.Fil: Lardizábal, María Noelia.
