    Nursing students' information literacy skills prior to and after information literacy instruction

    Information literacy has been identified by the University of Southern Queensland as an essential graduate quality. It has also been identified in the nursing literature as a requirement for nursing profession. Data were collected from Toowoomba and Fraser Coast campuses using a questionnaire to assess first-year nursing students' information literacy skills prior to and after information literacy instruction. The findings indicate that embedding such instruction into nursing courses is beneficial

    Atopic dermatitis, cutaneous steroids and cataracts in children: two case reports

    <p>Abstract</p> <p>Introduction</p> <p>Atopic dermatitis is a chronic, pruritic, eczematous skin disease mediated through an immediate (type I) hypersensitivity reaction. Posterior sub-capsular cataracts are a recognised complication of atopic dermatitis in adults; however they are rare in children. The management of atopic dermatitis is based on the exclusion of allergens, the use of emollients, and on topical corticosteroids for disease exacerbations. Cataracts may be due to atopic dermatitis but may also occur secondary to the use of corticosteroids.</p> <p>Case presentation</p> <p>We describe two children with atopic dermatitis, treated with cutaneous corticosteroids, both of whom were diagnosed with bilateral posterior sub-capsular cataracts.</p> <p>Conclusion</p> <p>These cases demonstrate that atopic dermatitis and topical corticosteroids may be associated with cataracts in children as well as adults. The cause of cataracts in atopic dermatitis is not known, however, it has been suggested that habitual tapping and rubbing of the face may play a role. Care needs to be taken when prescribing corticosteroids. Inadequate treatment of atopic dermatitis may lead to other ocular complications such as keratitis and permanent visual loss.</p

    The 2 × 2 model of perfectionism and school‐ and community‐based sport participation.

    The authors adopted the 2 × 2 model of perfectionism to examine the unique and interactive effects of two dimensions of perfectionism (personal standards perfectionism PSP and evaluative concerns perfectionism ECP) on personal and interpersonal indicators of participant experience in youth sport (enjoyment, physical self‐worth, and friendship quality). Participants (N = 219, M age = 15.12, SD = 2.02) were recruited from various school‐ and community‐based sports and completed a multi‐section questionnaire. Consideration of main and interaction effects indicated that pure PSP (high PSP/low ECP) was associated with the most positive sport experience and pure ECP (low PSP/high ECP) was associated with the least positive sport experience. The findings suggest that subtypes of perfectionism from the 2 × 2 model are predictive of differing experiences in youth sport participation.     New insights into perinatal testicular torsion

    Perinatal testicular torsion is a relatively rare event that remains unrecognized in many patients or is suspected and treated accordingly only after an avoidable loss of time. The authors report their own experience with several patients, some of them quite atypical but instructive. Missed bilateral torsion is an issue, as are partial torsion, possible antenatal signs, and late presentation. These data are discussed together with the existing literature and may help shed new light on the natural course of testicular torsion and its treatment. The most important conclusion is that a much higher index of suspicion based on clinical findings is needed for timely detection of perinatal torsion. It is the authors’ opinion that immediate surgery is mandatory not only in suspected bilateral torsions but also in cases of possible unilateral torsions. There is no place for a more fatalistic “wait-and-see” approach. Whenever possible, even necrotic testes should not be removed during surgery because some endocrine function may be retained

    Controlling for Prior Attainment Reduces the Positive Influence that Single-Gender Classroom Initiatives Exert on High School Students’ Scholastic Achievements.

    Research points to the positive impact that gender-segregated schooling and classroom initiatives exert on academic attainment. An evaluation of these studies which reveal positive effects highlights, however, that students are typically selectively assigned to single- or mixed-gender instructional settings, presenting a methodological confound. The current study controls for students’ prior attainment to appraise the efficacy of a single-gender classroom initiative implemented in a co-educational high school in the United Kingdom. Secondary data analysis (using archived data) was performed on 266 middle-ability, 11–12 year-old students’ standardized test scores in Languages (English, foreign language), STEM-related (Mathematics, Science, Information and Communication Technology), and Non-STEM subjects (art, music, drama). Ninety-eight students (54, 55% female) were taught in single-gender and 168 (69, 41% female) in mixed-gender classrooms. Students undertook identical tests irrespective of classroom type, which were graded in accordance with U.K national curriculum guidelines. Controlling for students’ prior attainment, findings indicate that students do not appear to benefit from being taught in single-gender relative to mixed-gender classrooms in Language and STEM-related subjects. Young women benefitted from being taught in mixed-gender relative to single-gender classes for Non-STEM subjects. However, when prior ability is not controlled for, the intervention appears to be effective for all school subjects, highlighting the confounding influence of selective admissions. These findings suggest that gender-segregated classroom initiatives may not bolster students’ grades. It is argued that studies that do not control for selection effects may tell us little about the effectiveness of such interventions on scholastic achievement

    HIV-1 drug resistance mutations emerging on darunavir therapy in PI-naive and -experienced patients in the UK

    \ua9 The Author 2016. Background: Darunavir is considered to have a high genetic barrier to resistance. Most darunavir-associated drug resistance mutations (DRMs) have been identified through correlation of baseline genotype with virological response in clinical trials. However, there is little information on DRMs that are directly selected by darunavir in clinical settings. Objectives: We examined darunavir DRMs emerging in clinical practice in the UK. Patients and methods: Baseline and post-exposure protease genotypes were compared for individuals in the UK Collaborative HIV Cohort Study who had received darunavir; analyses were stratified for PI history. A selection analysis was used to compare the evolution of subtype B proteases in darunavir recipients and matched PInaive controls. Results: Of 6918 people who had received darunavir, 386 had resistance tests pre- and post-exposure. Overall, 2.8% (11/386) of these participants developed emergent darunavir DRMs. The prevalence of baseline DRMs was 1.0% (2/198) among PI-naive participants and 13.8% (26/188) among PI-experienced participants. Emergent DRMs developed in 2.0% of the PI-naive group (4 mutations) and 3.7% of the PI-experienced group (12 mutations). Codon 77 was positively selected in the PI-naive darunavir cases, but not in the control group. Conclusions: Our findings suggest that although emergent darunavir resistance is rare, it may be more common among PI-experienced patients than those who are PI-naive. Further investigation is required to explore whether codon 77 is a novel site involved in darunavir susceptibility

    Virological failure and development of new resistance mutations according to CD4 count at combination antiretroviral therapy initiation

    Objectives: No randomized controlled trials have yet reported an individual patient benefit of initiating combination antiretroviral therapy (cART) at CD4 counts > 350 cells/ÎŒL. It is hypothesized that earlier initiation of cART in asymptomatic and otherwise healthy individuals may lead to poorer adherence and subsequently higher rates of resistance development. Methods: In a large cohort of HIV-positive individuals, we investigated the emergence of new resistance mutations upon virological treatment failure according to the CD4 count at the initiation of cART. Results: Of 7918 included individuals, 6514 (82.3%), 996 (12.6%) and 408 (5.2%) started cART with a CD4 count ≀ 350, 351-499 and ≄ 500 cells/ÎŒL, respectively. Virological rebound occurred while on cART in 488 (7.5%), 46 (4.6%) and 30 (7.4%) with a baseline CD4 count ≀ 350, 351-499 and ≄ 500 cells/ÎŒL, respectively. Only four (13.0%) individuals with a baseline CD4 count > 350 cells/ÎŒL in receipt of a resistance test at viral load rebound were found to have developed new resistance mutations. This compared to 107 (41.2%) of those with virological failure who had initiated cART with a CD4 count < 350 cells/ÎŒL. Conclusions: We found no evidence of increased rates of resistance development when cART was initiated at CD4 counts above 350 cells/ÎŒL. HIV Medicin

    Information literacy and its application in nursing education

    Information literacy has been embedded by the university into the first year nursing curriculum. Embedding this literacy will not necessarily ensure the nursing graduates will apply this skill to the provision of high quality evidence-based health care. For this to happen information literacy skills gained in the classroom must contribute to sound decision making based on best practice evidence. This paper discusses the findings of a three phases research project designed to (i) determine the information literacy skills, confidence and problem solving abilities of students entering the university's Bachelor of Nursing Program; (ii) determine if information literacy skills, confidence and problem solving abilities improve as a result of embedding information literacy instruction into a nursing course; and iii) ascertain whether there are any differences in information literacy skills, confidence and problem solving abilities based on the students demographic information. Data were collected in two sequential semesters using a questionnaire administered to the students. The response rates in semester one and two were 45 and 56 per cent respectively. Student confidence and awareness regarding information literacy is positively affected by learning experiences from semester one to semester two. Students indicated that they need both specific and regular instruction to adequately retain learning. Overall the study suggests that embedding information literacy instruction into the first year, first semester nursing program is beneficial. By the second semester the information literacy confidence and awareness of students increased as a result of intra-curricular instruction, however, problem solving skills need to be improved
