    Fatores de estância hospitalar prolongada numa instituição de saúde de Medellín

    Objectives: The objective of this research project was to identify associated factors of extendedhospital stay, in a medium and high-complexity health care institution in Medellin.Materials and methods: An analytic, quantitative, cross-sectional study was made, with asimple random sample of patients (n=291) from the consolidate records of extended hospitalstay in the institutions subject of study. For the bivariate analysis, the x2 squared was usedin SPSS, the P values generated were evaluated with a confidence level of 95% and a marginof error of 5%, with a significant value or alpha value of 0.05 from the 291 records analyzed.Results: A significant statistical association was found between prolonged hospital stay andhome care (p= 0,028), with procedures (p= 0.046) and diagnostic aids (p= 0.38). Conclusions:The hospital stay of a patient, does not only depend on their clinical condition, but alsoon the correlation with other care actors such as the institution, provider, and service networkObjetivos: El objetivo de este proyecto de investigación se basó en identificar los factores asociados a la estancia hospitalaria prolongada en una institución de mediana y alta complejidad de Medellín. Materiales y métodos: Se realizó un estudio analítico, cuantitativo, de corte transversal; con una selección aleatoria simple de pacientes registrados en el consolidado de estancia hospitalaria prolongada en la institución objeto de estudio, donde (n =291) pacientes; para el análisis bivariado se utilizó x2 cuadrado en SPSS, los valores P arrojados fueron evaluados con un nivel de confianza del 95% y un margen de error del 5%, dando un nivel de significancia o de valor alfa de 0.05 de los 291 registros analizados. Resultados: se encontró asociación estadísticamente significativa entre estancia hospitalaria prolongada y atención domiciliaria (p= 0,028); procedimientos: (p= 0.046) y ayudas diagnósticas: (p= 0,38). Conclusiones: La estancia hospitalaria de un paciente, no solo depende de su condición clínica; depende de una interrelación entre otros actores en su atención, institución, prestador y red de servicios.Objetivo: identificar os fatores associados à estância hospitalar prologada numa instituição de mediana e alta complexidade de Medellín.  Materiais e métodos: estudo analítico, quantitativo, transversal, com uma seleção aleatória simples de 291 pacientes registrados no consolidado de estância hospitalar prolongada numa instituição de saúde. Resultados: encontrou-se associação significativa entre a estância hospitalar prolongada e a assistência domiciliar (p=0,028), com procedimentos (p=0,046) e ajudas diagnosticas (p=0,038). Conclusões: a estância hospitalar de um paciente não só depende da sua condição clínica, e sim da interação com outros atores no seu atendimento como a instituição, o prestador e rede de serviços

    RESUMEN: El presente trabajo está enfocado en la elaboración de un manual de procedimientos de control interno de inventario para mejorar la rentabilidad de la empresa Baterías de Centro América, siendo su actividad económica principal la compra y venta de repuestos para vehículos y motocicletas; El control del inventario es un elemento muy importante para el desarrollo de las empresas; un mal control, puede ser el culpable de generar clientes descontentos por el no cumplimiento de la demanda, además de ocasionar problemas financieros que pueden llevar a la empresa a la quiebra. El Capítulo I. Contiene los antecedentes del fenómeno objeto de estudio, situación problemática, enunciado del problema, delimitación del problema, justificación, y los objetivos de la investigación. El Capítulo III, contiene la metodología que se utilizara en la investigación realizada, el cual comprende el tipo de estudio, la población, las técnicas e instrumentos utilizados en la investigación y los procedimientos de análisis e interpretación de resultados. En el Capítulo IV, se analizan los resultados obtenidos mediante el instrumento utilizado en la investigación y se hace la tabulación respectiva gráficamente. ABSTRACT: The present work is focused on the elaboration of a manual of internal inventory control procedures to improve the profitability of the company Batteries of Central America, being its main economic activity the purchase and sale of spare parts for vehicles and motorcycles; Inventory control is a very important element for the development of companies; poor control can be to blame for generating dissatisfied customers due to non-compliance with the demand, in addition to causing financial problems that can lead the company to bankruptcy. Chapter I. Contains the background of the phenomenon under study, problematic situation, statement of the problem, delimitation of the problem, justification, and the objectives of the investigation. Chapter III contains the methodology that will be used in the research carried out, which includes the type of study, the population, the techniques and instruments used in the research and the procedures for analysis and interpretation of results.     "Mycobacterium tuberculosis employs a number of strategies to subvert host signaling events, leading to its persistence within macrophages. Upon infection, Mycobacterium bovis BCG induce the expression of suppressor of cytokine signaling 3 (socs3), in a Toll-like receptor 2 (TLR2)-Notch1-dependent manner. Purified phosphatidyl inositol di-mannosides (a TLR2 agonist) act as an inducer for the Notch1-socs3 pathway. This prompted us to analyze other TLR2 agonists seeking for additional molecules that may affect this pathway. We found that lipoprotein LprA, as well as glycolipids lipomannan (LM), and mannose-capped lipoarabinomannan (ManLAM) treatment of murine macrophages resulted in stimulation of notch1 and socs3 transcription.

    On the Location of the Gamma-ray Emission in the 2008 Outburst in the BL Lacertae Object AO 0235+164 through Observations across the Electromagnetic Spectrum

    We present observations of a major outburst at centimeter, millimeter, optical, X-ray, and gamma-ray wavelengths of the BL Lacertae object AO 0235+164. We analyze the timing of multi-waveband variations in the flux and linear polarization, as well as changes in Very Long Baseline Array (VLBA) images at 7mm with 0.15 milliarcsecond resolution. The association of the events at different wavebands is confirmed at high statistical significance by probability arguments and Monte-Carlo simulations. A series of sharp peaks in optical linear polarization, as well as a pronounced maximum in the 7 mm polarization of a superluminal jet knot, indicate rapid fluctuations in the degree of ordering of the magnetic field. These results lead us to conclude that the outburst occurred in the jet both in the quasi-stationary "core" and in the superluminal knot, both parsecs downstream of the supermassive black hole. We interpret the outburst as a consequence of the propagation of a disturbance, elongated along the line of sight by light-travel time delays, that passes through a standing recollimation shock in the core and propagates down the jet to create the superluminal knot. The multi-wavelength light curves vary together on long time-scales (months/years), but the correspondence is poorer on shorter time-scales. This, as well as the variability of the polarization and the dual location of the outburst, agrees with the expectations of a multi-zone emission model in which turbulence plays a major role in modulating the synchrotron and inverse Compton fluxes.Comment: Accepted for Publication in the Astrophysical Journal Letters. 7 pages (including 5 figures).     Energy distribution of individual quasars from far-UV to X-rays: I. Intrinsic UV hardness and dust opacities

    Using Chandra and HST archival data, we have studied the individual Spectral Energy Distribution (SED) of 11 quasars. All UV spectra show a spectral break around 1100A. 5 X-ray spectra show the presence of a ``soft excess'' and 7 spectra showed an intrinsic absorption. We found that for most quasars a simple extrapolation of the far-UV powerlaw into the X-ray domain generally lies below the X-ray data and that the big blue bump and the soft X-ray excess do not share a common physical origin. We explore the issue of whether the observed SED might be dust absorbed in the far and near-UV. We fit the UV break, assuming a powerlaw that is absorbed by cubic nanodiamond dust grains. We then explore the possibility of a universal SED (with a unique spectral index) by including further absorption from SMC-like extinction. Using this approach, satisfactory fits to the spectra can be obtained. The hydrogen column densities required by either nanodiamonds or amorphous dust models are all consistent, except for one object, with the columns deduced by our X-ray analysis, provided that the C depletion is ~0.6. Because dust absorption implies a flux recovery in the extreme UV (<700A), our modeling opens the possibility that the intrinsic quasar SED is much harder and more luminous in the extreme UV than inferred from the near-UV data, as required by photoionization models of the broad emission line region.     The Eighteenth Data Release of the Sloan Digital Sky Surveys: Targeting and First Spectra from SDSS-V

    The eighteenth data release of the Sloan Digital Sky Surveys (SDSS) is the first one for SDSS-V, the fifth generation of the survey. SDSS-V comprises three primary scientific programs, or "Mappers": Milky Way Mapper (MWM), Black Hole Mapper (BHM), and Local Volume Mapper (LVM). This data release contains extensive targeting information for the two multi-object spectroscopy programs (MWM and BHM), including input catalogs and selection functions for their numerous scientific objectives. We describe the production of the targeting databases and their calibration- and scientifically-focused components. DR18 also includes ~25,000 new SDSS spectra and supplemental information for X-ray sources identified by eROSITA in its eFEDS field. We present updates to some of the SDSS software pipelines and preview changes anticipated for DR19. We also describe three value-added catalogs (VACs) based on SDSS-IV data that have been published since DR17, and one VAC based on the SDSS-V data in the eFEDS field.Comment: Accepted to ApJ

    Juxtaposing BTE and ATE – on the role of the European insurance industry in funding civil litigation

    One of the ways in which legal services are financed, and indeed shaped, is through private insurance arrangement. Two contrasting types of legal expenses insurance contracts (LEI) seem to dominate in Europe: before the event (BTE) and after the event (ATE) legal expenses insurance. Notwithstanding institutional differences between different legal systems, BTE and ATE insurance arrangements may be instrumental if government policy is geared towards strengthening a market-oriented system of financing access to justice for individuals and business. At the same time, emphasizing the role of a private industry as a keeper of the gates to justice raises issues of accountability and transparency, not readily reconcilable with demands of competition. Moreover, multiple actors (clients, lawyers, courts, insurers) are involved, causing behavioural dynamics which are not easily predicted or influenced. Against this background, this paper looks into BTE and ATE arrangements by analysing the particularities of BTE and ATE arrangements currently available in some European jurisdictions and by painting a picture of their respective markets and legal contexts. This allows for some reflection on the performance of BTE and ATE providers as both financiers and keepers. Two issues emerge from the analysis that are worthy of some further reflection. Firstly, there is the problematic long-term sustainability of some ATE products. Secondly, the challenges faced by policymakers that would like to nudge consumers into voluntarily taking out BTE LEI
