    STUDIES ON THROMBOLYTIC AND ANTI-HELMINTIC ACTIVITY OF Cocccinea grandis

    Traditional system of medicine consists of large number of plants with various medicinal and pharmacological values and hence represents a priceless tank of new bio active molecules. Coccinea grandis belongs to the family Cucurbitaceae, is a rapidly growing, perennial climber or trailing vine. Traditionally different parts of this plant namely roots, leaves, and fruits are used in folklore medicine for several purposes like jaundice, diabetes, wound healing, ulcers, stomach ache, skin disease, fever, asthma, cough. Development of anthelmintic resistance and high cost of conventional anthelmintic drugs led to the evaluation of medicinal plants as an alternative source of anthelmintics.  As no scientific data was available on this plant hence, the present study was carried out for the investigation of antihelmintic activity and thrombolytic activity of bark of Coccinea grandis. Antihelmintic activity was done on pheritima posthuma. The results obtained were compared with standard albendazole and control groups respectively, the parameters evaluated are time of paralysis and time of death. On the other hand thrombolytic activity was done by using aqueous extract of Coccinea grandis bark. Thrombi or emboli can lodge in a blood vessel and block the flow of blood in that location depriving tissues of normal blood flow and oxygen. This can result in damage, destruction (infarction), or even death of the tissues (necrosis) in that area. Streptokinase is an antigenic thrombolytic agent used for the treatment of acute myocardial infarction, served as standard. This study was conducted on human blood sample and compared with standard and control groups respectively. The parameters studied are percentage of clot lysis. From the present study we found that various extracts of this plants showed significant thrombolytic and antihelmintic properties.Key words: thrombolysis, helminthes, coccinea, albendazole, streptokinase

    Truncated hemoglobin, HbN, is post-translationally modified in Mycobacterium tuberculosis and modulates host-pathogen interactions during intracellular infection

    Mycobacterium tuberculosis (Mtb) is a phenomenally successful human pathogen having evolved mechanisms that allow it to survive within the hazardous environment of macrophages and establish long term, persistent infection in the host against the control of cell-mediated immunity. One such mechanism is mediated by the truncated hemoglobin, HbN, of Mtb that displays a potent O2-dependent nitric oxide dioxygenase activity and protects its host from the toxicity of macrophage-generated nitric oxide (NO). Here we demonstrate for the first time that HbN is post-translationally modified by glycosylation in Mtb and remains localized on the cell membrane and the cell wall. The glycan linkage in the HbN was identified as mannose. The elevated expression of HbN in Mtb and M. smegmatis facilitated their entry within the macrophages as compared with isogenic control cells, and mutation in the glycan linkage of HbN disrupted this effect. Additionally, HbN-expressing cells exhibited higher survival within the THP-1 and mouse peritoneal macrophages, simultaneously increasing the intracellular level of proinflammatory cytokines IL-6 and TNF-α and suppressing the expression of co-stimulatory surface markers CD80 and CD86. These results, thus, suggest the involvement of HbN in modulating the host-pathogen interactions and immune system of the host apart from protecting the bacilli from nitrosative stress inside the activated macrophages, consequently driving cells toward increased infectivity and intracellular survival

    Captive breeding of a near threatened fish, pengba Osteobrama belangeri (Valenciennes, 1844) using three different inducing agents

    Farm reared pengba, Osteobrama belangeri were induced to spawn in captivity during August, 2012 by injecting three different synthetic hormones, Ovaprim, Ovatide and Gonopro-FH. Single dose (1 ml kg-1 body weight) of each hormone was administered and results were recorded. Spawning was observed within 8 h after injection. Hatching of eggs were observed after 22±2 h of incubation at 27±1OC. The mean fertilization rate was 84.05±0.36% for Ovaprim, 79.17±3.95% for Ovatide and 84.85±0.89% for Gonopro-FH treated fish. The mean hatching rate was 84.69±1.73% with Ovaprim, 75.01±1.92% with Ovatide and 86.52±0.88% with Gonopro-FH. Gonopro-FH and Ovaprim gave 5.67 and 4.88% higher fertilization rate as well as 11.5 and 9.69% more hatching rate of eggs respectively as compared to Ovatide. Ovaprim and Gonopro-FH were found to be more effective in induced breeding of O. belangeri

    Preparation and evaluation of azithromycin binary solid dispersions using various polyethylene glycols for the improvement of the drug solubility and dissolution rate

    ABSTRACT Azithromycin is a water-insoluble drug, with a very low bioavailability. In order to increase the solubility and dissolution rate, and consequently increase the bioavailability of poorly-soluble drugs (such as azithromycin), various techniques can be applied. One of such techniques is "solid dispersion". This technique is frequently used to improve the dissolution rate of poorly water-soluble compounds. Owing to its low solubility and dissolution rate, azithromycin does not have a suitable bioavailability. Therefore, the main purpose of this investigation was to increase the solubility and dissolution rate of azithromycin by preparing its solid dispersion, using different Polyethylene glycols (PEG). Preparations of solid dispersions and physical mixtures of azithromycin were made using PEG 4000, 6000, 8000, 12000 and 20000 in various ratios, based on the solvent evaporation method. From the studied drug release profile, it was discovered that the dissolution rate of the physical mixture, as the well as the solid dispersions, were higher than those of the drug alone. There was no chemical incompatibility between the drug and polymer from the observed Infrared (IR) spectra. Drug-polymer interactions were also investigated using Differential Scanning Calorimetry (DSC), Powder X-Ray Diffraction (PXRD) and Scanning Election Microscopy (SEM). In conclusion, the dissolution rate and solubility of azithromycin were found to improve significantly, using hydrophilic carriers, especially PEG 6000

    COVID-19 trajectories among 57 million adults in England: a cohort study using electronic health records

    BACKGROUND: Updatable estimates of COVID-19 onset, progression, and trajectories underpin pandemic mitigation efforts. To identify and characterise disease trajectories, we aimed to define and validate ten COVID-19 phenotypes from nationwide linked electronic health records (EHR) using an extensible framework. METHODS: In this cohort study, we used eight linked National Health Service (NHS) datasets for people in England alive on Jan 23, 2020. Data on COVID-19 testing, vaccination, primary and secondary care records, and death registrations were collected until Nov 30, 2021. We defined ten COVID-19 phenotypes reflecting clinically relevant stages of disease severity and encompassing five categories: positive SARS-CoV-2 test, primary care diagnosis, hospital admission, ventilation modality (four phenotypes), and death (three phenotypes). We constructed patient trajectories illustrating transition frequency and duration between phenotypes. Analyses were stratified by pandemic waves and vaccination status. FINDINGS: Among 57 032 174 individuals included in the cohort, 13 990 423 COVID-19 events were identified in 7 244 925 individuals, equating to an infection rate of 12·7% during the study period. Of 7 244 925 individuals, 460 737 (6·4%) were admitted to hospital and 158 020 (2·2%) died. Of 460 737 individuals who were admitted to hospital, 48 847 (10·6%) were admitted to the intensive care unit (ICU), 69 090 (15·0%) received non-invasive ventilation, and 25 928 (5·6%) received invasive ventilation. Among 384 135 patients who were admitted to hospital but did not require ventilation, mortality was higher in wave 1 (23 485 [30·4%] of 77 202 patients) than wave 2 (44 220 [23·1%] of 191 528 patients), but remained unchanged for patients admitted to the ICU. Mortality was highest among patients who received ventilatory support outside of the ICU in wave 1 (2569 [50·7%] of 5063 patients). 15 486 (9·8%) of 158 020 COVID-19-related deaths occurred within 28 days of the first COVID-19 event without a COVID-19 diagnoses on the death certificate. 10 884 (6·9%) of 158 020 deaths were identified exclusively from mortality data with no previous COVID-19 phenotype recorded. We observed longer patient trajectories in wave 2 than wave 1. INTERPRETATION: Our analyses illustrate the wide spectrum of disease trajectories as shown by differences in incidence, survival, and clinical pathways. We have provided a modular analytical framework that can be used to monitor the impact of the pandemic and generate evidence of clinical and policy relevance using multiple EHR sources.     Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Skeletal Muscle Haemangioma: A Cause for Chronic Pain about the Knee: A Case Report

    Skeletal muscle haemangiomas are uncommon soft tissue tumors; more than 90% are misdiagnosed initially. They present as chronic pain and swelling in a muscle with or without a history of trauma. Plain X-rays, bone scans, computerized tomography (CT) studies, and angiography studies may not always be specific for this tumor. Diagnostic ultrasound is an appropriate initial imaging modality for suspected haemangioma, although magnetic resonance imaging is the investigation of choice. Many treatment modalities for the symptomatic haemangiomas are available of which surgical excision is the most preferred. We present an unusual case of pain, swelling, and restriction of movements in the right knee following an episode of trauma in a 12-year-old boy who was being followed for 1 year by a general practioner and later referred to us. The patient was diagnosed to have intramuscular cavernous haemangioma in the vastus medialis by us for which he was treated by surgical excision and followed for 1 year and found to have no recurrence.     As per the statement by World Health Organization, Infertility is presence of a medical condition of the couple with inability in getting conceived for 1 year or more without usage of any contraception. In the present situation, the incidence of Infertility has increased and it is viewed as a major concern with affecting about 15% to 20% of the couples. A detailed systematic examination of history, investigative protocol is necessary for proper management of infertility. The patients were also advised for blood hormonal analysis tests such as Follicle Stimulation Hormone (FSH), Testosterone, Luteinizing Hormone (LH), Prolactin. Methylene tetrahydrofolate reductase (MTHFR) and methionine synthase reductase (MTRR) are the key enzymes in folate metabolism. The enzyme activities of MTHFR and MTRR are influenced by gene polymorphisms. So the polymorphisms of MTHFR and MTRR may be a potential risk factor for male infertility. The present study was done in 100 males, 50 diabetic infertile and 50 non-diabetic fertile men. Semen samples were obtained and analyzed as per the instructions given in the WHO manual for semen analysis [10].     World Health Organization has stated Infertility as a medical condition in which an individual has the inability to conceive even after one year of regular unprotected active sexual intercourse. Presently, Infertility has been a major condition, which is multifactorial in nature and is supposed to have been affected approximately about 15% to 20% of the couples who are desperate for pregnancy. The contributory factors for male infertility may be of different factors comprising of complex pathogenesis, change in lifestyle, other organic diseases, genetic factors, environmental risk factors and their interactions. The patients with subnormal semen parameters are advised for blood hormonal analysis tests as these hormonal tests are more reliable and valid in the further proper management of the infertility [11]. These hormonal tests include Follicle Stimulation Hormone (FSH), Testosterone, Luteinizing Hormone (LH), Prolactin. This cross section study was done in 100 males, 50 diabetic infertile men and 50 non-diabetic fertile men. Semen samples were obtained by masturbation after 2–7 days of sexual abstinence.
