    Impacto de la estimulación subtalámica a largo plazo sobre la situación cognitiva de los pacientes con enfermedad de Parkinson avanzada

    Objetivo El objetivo es evaluar los efectos de la estimulación cerebral profunda del núcleo subtalámico bilateral (STN-DBS) sobre el estado cognitivo de los pacientes con enfermedad de Parkinson 5 años después de la cirugía. Materiales y métodos En este estudio prospectivo se incluyeron 50 pacientes con enfermedad de Parkinson (62,5% hombres, edad media 62,2 ± 8,2 años y duración de la enfermedad 14,1 ± 6,3 años) sometidos a STN-DBS. Todos los pacientes fueron evaluados preoperatoriamente y un año después de la cirugía, y 40 pacientes fueron seguidos hasta 5 años. En cada visita se realizaron las siguientes evaluaciones neuropsicológicas: Mini-Mental State Examination, Mattis Dementia Rating Scale (MDRS), test de secuencias números-letras de WAIS III-LN, Prueba de dibujo de reloj, Prueba de aprendizaje verbal auditivo Rey, la Prueba de retención visual de Benton, la Prueba de juicio de orientación de línea de Benton, la fluidez verbal fonética y semántica, la Prueba Stroop y la Escala de clasificación de depresión de Montgomery-Asberg. Resultados Anualmente se observaron reducciones en la puntación de Mini-Mental State Examination (–0,89%), Prueba del dibujo de reloj (–2,61%) y MDRS (–1,72%), fueron más marcados tanto para la fluidez verbal fonética (–13,28%) como semántica (–12,40%). Para la Prueba de aprendizaje verbal auditivo Rey observamos un deterioro en la capacidad de recuerdo diferido (–10,12%) un año después de la cirugía. A los 5 años la mayor parte del deterioro se produjo en la fluidez verbal, con reducciones adicionales de 16,10% y 16,60% para la fluidez verbal semántica y fonética, respectivamente. Se observó un empeoramiento más moderado del recuerdo inmediato (–16,87%), WAIS III-LN (–16,67%) y de la prueba de orientación lineal de Benton (–11,56%). Discusión La STN-DBS no condujo a deterioro cognitivo global a los 5 años de la cirugía. Hubo un deterioro significativo en la función verbal desde el primer año de la cirugía. El deterioro de la capacidad de aprendizaje y de las funciones visuoespaciales podría atribuirse al propio proceso degenerativo de la enfermedad.This study was partially funded by research grant INT-BC2016-1 from Biocruces Bizkaia Health Research Institute

    The COMT Val158 Met polymorphism as an associated risk factor for Alzheimer disease and mild cognitive impairment in APOE 4 carriers

    <p>Abstract</p> <p>Background</p> <p>The aim of this study is to examine the influence of the <it>catechol-O-methyltranferase (COMT) </it>gene (polymorphism Val158 Met) as a risk factor for Alzheimer's disease (AD) and mild cognitive impairment of amnesic type (MCI), and its synergistic effect with the <it>apolipoprotein E gene (APOE)</it>.</p> <p>A total of 223 MCI patients, 345 AD and 253 healthy controls were analyzed. Clinical criteria and neuropsychological tests were used to establish diagnostic groups.</p> <p>The DNA Bank of the University of the Basque Country (UPV-EHU) (Spain) determined <it>COMT </it>Val158 Met and <it>APOE </it>genotypes using real time polymerase chain reaction (rtPCR) and polymerase chain reaction (PCR), and restriction fragment length polymorphism (RFLPs), respectively. Multinomial logistic regression models were used to determine the risk of AD and MCI.</p> <p>Results</p> <p>Neither <it>COMT </it>alleles nor genotypes were independent risk factors for AD or MCI. The high activity genotypes (GG and AG) showed a synergistic effect with <it>APOE ε4 </it>allele, increasing the risk of AD (OR = 5.96, 95%CI 2.74-12.94, p < 0.001 and OR = 6.71, 95%CI 3.36-13.41, p < 0.001 respectivily). In AD patients this effect was greater in women.</p> <p>In MCI patients such as synergistic effect was only found between AG and <it>APOE ε4 </it>allele (OR = 3.21 95%CI 1.56-6.63, p = 0.02) and was greater in men (OR = 5.88 95%CI 1.69-20.42, p < 0.01).</p> <p>Conclusion</p> <p><it>COMT </it>(Val158 Met) polymorphism is not an independent risk factor for AD or MCI, but shows a synergistic effect with <it>APOE ε4 </it>allele that proves greater in women with AD.</p

    Prevalencia de sintomatología dolorosa osteomuscular en un hospital del Valle del Cauca, Colombia

    Objective: To determine the prevalence of painful musculoskeletal symptoms, as well as interference with labor. Methodology: Descriptive, cross-sectional study carried out on 100 health workers in a Hospital del Valle del Cauca, who were administered the Cornell Musculoskeletal Symptomatology Questionnaire (CMDQ) adapted to the Spanish language. The obtained data were statistically analyzed. Results: 86% of the subjects were women, the age range between 19-58 years, 64% have more than two years working in the institution. The most prevalent musculoskeletal symptoms in the last week were: lower back 55%, neck 43%, upper back 41%, the trunk being the most affected body segment. 25% of the participants reported interference in the work and the lower back was the one that interfered the most with 55%. Conclusions: The implementation of epidemiological surveillance systems is recommended for the prior detection of musculoskeletal disorders (DME) and the application of prevention strategies for the benefit of worker health.Objetivo: Determinar la prevalencia de sintomatología dolorosa osteomuscular, así como la interferencia en la labor. Metodología: Estudio descriptivo, de corte transversal realizado en 100 trabajadores del área de la salud de un Hospital del Valle del Cauca, a quienes se les aplicó el cuestionario de sintomatología músculo esquelética de Cornell (CMDQ) adaptado al idioma español. Los datos obtenidos fueron analizados estadísticamente. Resultados: El 86% de los sujetos fueron mujeres, el rango de edad entre 19-58 años, el 64% tiene más de dos años trabajando en la institución. Los síntomas osteomusculares más prevalentes en la última semana fueron: espalda baja 55%, cuello 43%, espalda alta 41%, siendo el segmento corporal más afectado el tronco. El 25% de los participantes refirió interferencia en la labor y la espalda baja fue la que más interfirió en la labor con 55%. Conclusiones: Se recomienda la implementación de sistemas de vigilancia epidemiológica para la detección previa de desórdenes músculo esqueléticos (DME) y la aplicación de estrategias de prevención en beneficio de la salud del trabajador

    Considerations and consequences of allowing DNA sequence data as types of fungal taxa

    Nomenclatural type definitions are one of the most important concepts in biological nomenclature. Being physical objects that can be re-studied by other researchers, types permanently link taxonomy (an artificial agreement to classify biological diversity) with nomenclature (an artificial agreement to name biological diversity). Two proposals to amend the International Code of Nomenclature for algae, fungi, and plants (ICN), allowing DNA sequences alone (of any region and extent) to serve as types of taxon names for voucherless fungi (mainly putative taxa from environmental DNA sequences), have been submitted to be voted on at the 11th International Mycological Congress (Puerto Rico, July 2018). We consider various genetic processes affecting the distribution of alleles among taxa and find that alleles may not consistently and uniquely represent the species within which they are contained. Should the proposals be accepted, the meaning of nomenclatural types would change in a fundamental way from physical objects as sources of data to the data themselves. Such changes are conducive to irreproducible science, the potential typification on artefactual data, and massive creation of names with low information content, ultimately causing nomenclatural instability and unnecessary work for future researchers that would stall future explorations of fungal diversity. We conclude that the acceptance of DNA sequences alone as types of names of taxa, under the terms used in the current proposals, is unnecessary and would not solve the problem of naming putative taxa known only from DNA sequences in a scientifically defensible way. As an alternative, we highlight the use of formulas for naming putative taxa (candidate taxa) that do not require any modification of the ICN.Peer reviewe

    Las crisis financieras recientes han incrementado de manera significativa la complejidad de la política económica. Hoy día es necesario estar preparado para enfrentar huracanes financieros que surgen de lugares inesperados y que se expanden alrededor del mundo como reguero de pólvora. Para peor, estas crisis producen un profundo desconcierto entre inversores y analistas —incluyendo al propio Fondo Monetario Internacional—. La presente obra contiene un conjunto importante de artículos de alto calibre profesional que se enfocan sobre los nuevos desafíos que enfrenta la política económica en estas circunstancias. Los artículos estudian las características de los flujos de capitales, tanto en su totalidad como en su composición (capital de cartera, inversión directa, etc.) y su interrelación con el mercado de commodities (café y petróleo, en particular). Esto se complementa con un estudio cuidadoso de las vulnerabilidades financieras y un detallado análisis del papel de todos estos factores en la economía colombiana. El libro se convierte en un verdadero pionero en esta literatura y debería transformarse en referencia obligada para todos aquellos que quieran entender los nuevos fenómenos y desarrollar herramientas para prevenir sus peores consecuencias o paliar sus costos en economías emergentes, como la colombiana
