    Environmental and lifestyle risk factors of breast cancer in Malta-a retrospective case-control study

    The funding for this research was obtained as part of IMaGenX – and ItaliaMalta co-financed EU project Operational Programme 2007–2013.AIM AND BACKGROUND: Environmental exposures are known to play a role in the development of cancer, including breast cancer. There are known associations of breast cancer with environmental factors such as sunlight exposure, diet and exercise and alcohol consumption as well as physiological factors. This study examines the prevalence of risk factors for breast cancer related to dietary intake, environment and lifestyle in the female population of Malta. Malta has had little research in this area, and therefore an exploratory study was carried out. METHODS: A retrospective case-control design was applied. Two hundred cases and 403 controls were included. Both cases and controls were subjects without a known family history for breast cancer. Controls were age-matched to cases in an age-decade category roughly at a 2:1 ratio. Interviews were carried out face-to-face using a questionnaire designed by Maltese and Sicilian researchers, encompassing various factors including diet, lifestyle, physiological factors and medical history. Breast cancer risk was then analysed using both univariate and multivariate analyses. For factors having a metric scale, the Mann-Whitney test was used to compare mean scores, while for categorical factors, the chi-square test was used to compare percentages between the case and control groups. Statistical modelling was carried out using binary logistic regression to relate the likelihood of breast cancer to over 50 risk/protective factors analysed collectively. RESULTS: Univariate analysis showed around 20 parameters of interest, 14 of which were statistically significant at a 0.05 level of significance. Logistic regression analysis identified 11 predictors of interest that were statistically significant. Tomato, coffee and canned meat consumption were associated with lower likelihood of breast cancer (OR = 0.988, 0.901, 0.892, respectively), whereas beans and cabbage consumption and low sodium salt were positively associated with breast cancer (OR = 1.045, 1.834, 1.028, respectively). Premenopausal status was associated with a lower risk of breast cancer compared to postmenopausal status (OR = 0.067). Not having experienced myocardial infarction was associated with lower odds of breast cancer (OR = 0.331). Increased height was also found to have a strong association with risk of breast cancer, with the odds of having breast cancer increasing for every centimetre increase in height (OR = 1.048). In terms of quantity, odds of having breast cancer were lower in those exposed to sunlight (OR = 0.891). The odds of having breast cancer were also lower in those not using the oral contraceptive pill (OR = 0.454). CONCLUSIONS: Various factors in this exploratory study were found to be associated with development of breast cancer. While causal conclusions cannot be made, tomato consumption is of particular interest, as these results corroborate findings found in other studies. A negative association of breast cancer with sunlight exposure and oral contraceptive pill use corroborates findings in other studies. Other associations with dietary intake can be explained by dietary changes. More robust studies in this area, including possible longitudinal studies, are warranted.peer-reviewe

    Small molecule receptor tyrosine kinase inhibitor of platelet-derived growth factor signaling (SU9518) modifies radiation response in fibroblasts and endothelial cells

    BACKGROUND: Several small receptor tyrosine kinase inhibitors (RTKI) have entered clinical cancer trials alone and in combination with radiotherapy or chemotherapy. The inhibitory spectrum of these compounds is often not restricted to a single target. For example Imatinib/Gleevec (primarily a bcr/abl kinase inhibitor) or SU11248 (mainly a VEGFR inhibitor) are also potent inhibitors of PDGFR and other kinases. We showed previously that PDGF signaling inhibition attenuates radiation-induced lung fibrosis in a mouse model. Here we investigate effects of SU9518, a PDGFR inhibitor combined with ionizing radiation in human primary fibroblasts and endothelial cells in vitro, with a view on utilizing RTKI for antifibrotic therapy. METHODS: Protein levels of PDGFR-α/-β and phosphorylated PDGFR in fibroblasts were analyzed using western and immunocytochemistry assays. Functional proliferation and clonogenic assays were performed (i) to assess PDGFR-mediated survival and proliferation in fibroblasts and endothelial cells after SU9518 (small molecule inhibitor of PDGF receptor tyrosine kinase); (ii) to test the potency und selectivity of the PDGF RTK inhibitor after stimulation with PDGF isoforms (-AB, -AA, -BB) and VEGF+bFGF. In order to simulate in vivo conditions and to understand the role of radiation-induced paracrine PDGF secretion, co-culture models consisting of fibroblasts and endothelial cells were employed. RESULTS: In fibroblasts, radiation markedly activated PDGF signaling as detected by enhanced PDGFR phosphorylation which was potently inhibited by SU9518. In fibroblast clonogenic assay, SU9518 reduced PDGF stimulated fibroblast survival by 57%. Likewise, SU9518 potently inhibited fibroblast and endothelial cell proliferation. In the co-culture model, radiation of endothelial cells and fibroblast cells substantially stimulated proliferation of non irradiated fibroblasts and vice versa. Importantly, the RTK inhibitor significantly inhibited this paracrine radiation-induced fibroblast and endothelial cell activation. CONCLUSION: Radiation-induced autocrine and paracrine PDGF signaling plays an important role in fibroblast and endothelial cell proliferation. SU9518, a PDGFR tyrosine kinase inhibitor, reduces radiation-induced fibroblast and endothelial cell activation. This may explain therapeutic anticancer effects of Imatinib/Gleevec, and at the same time it could open a way of attenuating radiation-induced fibrosis

    Performance of the CMS Cathode Strip Chambers with Cosmic Rays

    The Cathode Strip Chambers (CSCs) constitute the primary muon tracking device in the CMS endcaps. Their performance has been evaluated using data taken during a cosmic ray run in fall 2008. Measured noise levels are low, with the number of noisy channels well below 1%. Coordinate resolution was measured for all types of chambers, and fall in the range 47 microns to 243 microns. The efficiencies for local charged track triggers, for hit and for segments reconstruction were measured, and are above 99%. The timing resolution per layer is approximately 5 ns

    Laterite as a low-cost adsorbent in a sustainable decentralized filtration system to remove arsenic from groundwater in Vietnam

    © 2019 Elsevier B.V. In the Red River Delta, Vietnam, arsenic (As) contamination of groundwater is a serious problem where more than seventeen million people are affected. Millions of people in this area are unable to access clean water from the existing centralized water treatment systems. They also cannot afford to buy expensive household water filters. Similar dangerous situations exist in many other countries and for this reason there is an urgent need to develop a cost-effective decentralized filtration system using new low-cost adsorbents for removing arsenic. In this study, seven locally available low-cost materials were tested for arsenic removal by conducting batch adsorption experiments. Of these materials, a natural laterite (48.7% Fe2O3 and 18.2% Al2O3) from Thach That (NLTT) was deemed the most suitable adsorbent based on arsenic removal performance, local availability, stability/low risk and cost (US$ 0.10/kg). Results demonstrated that the adsorption process was less dependent on the solution pH from 2.0 to 10. The coexisting anions competed with As(III) and As(V) in the order, phosphate > silicate > bicarbonate > sulphate > chloride. The adsorption process reached a fast equilibrium at approximately 120–360 min, depending on the initial arsenic concentrations. The Langmuir maximum adsorption capacities of NLTT at 30 °C were 512 μg/g for As(III) and 580 μg/g for As(V), respectively. Thermodynamic study conducted at 10 °C, 30 °C, and 50 °C suggested that the adsorption process of As(III) and As(V) was spontaneous and endothermic in nature. A water filtration system packed with NLTT was tested in a childcare centre in the most disadvantaged community in Ha Nam province, Vietnam, to determine arsenic removal performance in an operation lasting six months. Findings showed that the system reduced total arsenic concentration in groundwater from 122 to 237 μg/L to below the Vietnam drinking water standard of 10 μg/L

    This study developed a layered double hydroxides (Mn/Mg/Fe-LDH) material through a simple co-precipitation method. The Mn/Mg/Fe-LDH oxidized arsenite [As(III)] ions into arsenate [As(V)] anions. The As(III) and oxidized As(V) were then adsorbed on Mn/Mg/Fe-LDH. The adsorption process of arseniate [As(V)] oxyanions by Mn/Mg/Fe-LDH was simultaneously conducted for comparison. Characterization results indicated that (i) the best Mg/Mn/Fe molar ratio was 1/1/1, (ii) the Mn/Mg/Fe-LDH structure was similar to that of hydrotalcite, (iii) the Mn/Mg/Fe-LDH possessed a positively charged surface (pHIEP of 10.15) and low Brunauer-Emmett-Teller surface area (SBET = 75.2 m2/g), and (iv) Fe2+/Fe3+ and Mn2+/Mn3+/Mn4+ coexisted in Mn/Mg/Fe-LDH. The As(III) adsorption process by Mn/Mg/Fe-LDH was similar to that of As(V) under different experimental conditions (initial solutions pH, coexisting foreign anions, contact times, initial As concentrations, temperatures, and desorbing agents). The Langmuir maximum adsorption capacity of Mn/Mg/Fe-LDH to As(III) (56.1 mg/g) was higher than that of As(V) (32.2 mg/g) at pH 7.0 and 25 °C. X-ray photoelectron spectroscopy was applied to identify the oxidation states of As in laden Mn/Mg/Fe-LDH. The key removal mechanism of As(III) by Mn/Mg/Fe-LDH was oxidation-coupled adsorption, and that of As(V) was reduction-coupled adsorption. The As(V) mechanism adsorption mainly involved: (1) the inner-sphere and outer-sphere complexation with OH groups of Mn/Mg/Fe-LDH; and (2) anion exchange with host anions (NO3-) in its interlayer. The primary mechanism adsorption of As(III) was the inner-sphere complexation. The redox reactions made Mn/Mg/Fe-LDH loss its original layer structure after adsorbing As(V) or As(III). The adsorption process was highly irreversible. Mn/Mg/Fe-LDH can decontaminate As from real groundwater samples from 45-92 ppb to 0.35-7.9 ppb (using 1.0 g/L). Therefore, Mn/Mg/Fe-LDH has great potential as a material for removing As

    We investigated antimicrobial residues, non-typhoidal Salmonella (NTS), Vibrio spp. and their associated antimicrobial resistance (AMR), in shrimps locally purchased in Ho Chi Minh City (Vietnam). In addition, we investigated the relationship between AMR in NTS, Vibrio spp. and antimicrobial residue in the same sample. A total of 40 samples of shrimp heads/shells from different retail sources was cultured using ISO 6579-1:2017 (NTS) and ISO/TS 21872-1:2007 (Vibrio spp.). Phenotypic antimicrobial susceptibility was investigated using Vitek (NTS, 34 antimicrobials) and disk diffusion (Vibrio spp., 12 antimicrobials). A total of 9 (22.5%) samples contained antimicrobial residue, including tetracyclines, fluoroquinolones, sulfonamides and macrolides (in 7.5%, 7.5%, 2.5% and 2.5% of samples, respectively). Shrimp samples from supermarkets had a higher prevalence of antimicrobial residue than those purchased in street markets (50% vs. 13.3%) (p = 0.049). A total of 30 (75%) samples were contaminated with NTS. All samples contained Vibrio spp., with V. parahaemolyticus being most common (87.5% samples). A total of 58.9% NTS isolates were multidrug resistant. With regards to the highest priority, critically important antimicrobials, the highest resistance corresponded to quinolones (14.4-47.8%), followed by 3rd and 4th generation cephalosporins (3.3-7.8%). Vibrio spp. isolates were characterised by their high resistance against ampicillin (82.7%) and 3rd generation cephalosporins (8.3-16.5%). Extended Spectrum Beta-Lactamase (ESBL) activity was detected in 28.1% V. parahaemolyticus isolates. Half of ESBL-positive V. parahaemolyticus strains harboured bla CTX-M1. We found an association between the presence of residues and the number of resistances for NTS (p = 0.075) and Vibrio spp. isolates (p = 0.093) from the same sample. These findings suggest that the presence of residues may contribute to the selection of AMR in foodborne pathogens in shrimps. Authorities should strengthen policies aiming at restricting inappropriate antimicrobial usage in shrimp farming, and step up monitoring of antimicrobial residues and food-borne pathogens at retail in Vietnam

    Multilocus sequence typing of Cryptococcus neoformans var. grubii from Laos in a regional and global context

    Cryptococcosis causes approximately 180 000 deaths each year in patients with human immunodeficiency virus (HIV). Patients with other forms of immunosuppression are also at risk, and disease is increasingly recognized in apparently immunocompetent individuals. Cryptococcus neoformans var. grubii, responsible for the majority of cases, is distributed globally. We used the consensus ISHAM Multilocus sequence typing (MLST) scheme to define the population structure of clinical C. neoformans var. grubii isolates from Laos (n = 81), which we placed into the global context using published MLST data from other countries (total N = 1047), including a reanalysis of 136 Vietnamese isolates previously reported. We observed a phylogeographical relationship in which the Laotian population was similar to its neighbor Thailand, being dominated (83%) by Sequence Types (ST) 4 and 6. This phylogeographical structure changed moving eastwards, with Vietnam's population consisting of an admixture of isolates dominated by the ST4/ST6 (35%) and ST5 (48%) lineages. The ST5 lineage is the predominant ST reported from China and East Asia, where it accounts for >90% of isolates. Analysis of genetic distance (Fst) between different populations of C. neoformans var. grubii supports this intermediate structure of the Vietnamese population. The pathogen and host diversity reported from Vietnam provide the strongest epidemiological evidence of the association between ST5 and HIV-uninfected patients. Regional anthropological genetic distances suggest diversity in the C. neoformans var. grubii population across Southeast Asia is driven by ecological rather than human host factors. Where the ST5 lineage is present, disease in HIV-uninfected patients is to be expected
