    O setido de parasitoses intestinais em populações americanas, identificando dilemas bioéticos

    Objective: Identifying meanings that American populations assign to parasitic intestinal diseases and raising potential conflicts or dilemmas in making-decision in various health activities result of the contact between traditional and biomedical perspectives. Methodology: Bibliographic review on intestinal parasites that met the inclusion criteria and supply insights for hermeneutics understanding of intestinal parasites and allow identify bioethical dilemmas for health activities resulting from contact between the emic and ethical visions (of biomedicine). Results: The social construction of parasitic intestinal diseases influences their perception of causality, harmfulness and as a health problem. This affects actions that people undertake to these diseases. People’s perception of the diseases can be different to the health professionals. Sometimes ethnoanatomy is related to beneficial effects from the parasites. People recognize risk factors but this does not necessarily lead to preventive behaviors. Some researches recognize informative education without community participation is not successful because people do not feel motivated to transform your reality. There were established various bioethical dilemmas arising from contact between emic and biomedical perspectives. Conclusions: Emic approach allows us to understand people´s logic about intestinal parasites. Although people recognize infection risks from an etic perspective, health officers’ lack of understanding of emic perspective of parasitic intestinal diseases, and the poor living conditions cannot produce changes that affect the reduction on prevalence rates. Those facts will affect the success of care, prevention, promotion and health education programs. The emic perspective is not definitely. There are a lot of infrastructural conditions that developed different conflict situations for the work of the health professionals. They can provide elements for prevention and promotion strategies and programs, but depending on the socio-economical (poverty, community participation and empowerment) and political (i.e. leadership, formal commitments) conditions can be used or not. The last conclusion is that many ethical and moral problems must be analyzed by bioethics.Objetivo: Identificar sentidos que pueblos del continente americano le asignan a las parasitosis intestinales y plantear posibles conflictos o dilemas en las tomas de decisiones en diferentes actividades de salud producto del encuentro de valores distintos, campo de acción de la bioética. Metodología: Revisión bibliográfica de artículos sobre parasitosis intestinales que cumplieran los criterios de inclusión establecidos y suministraran aportes para la comprensión hermenéutica de las parasitosis intestinales y permitieran identificar dilemas bioéticos para actividades en salud resultantes del contacto entre las visiones émica y ética (de la biomedicina). Resultados: La construcción social de las parasitosis intestinales influye en su percepción de causalidad, nocividad y como problema de salud, lo cual afecta las acciones que la gente emprenda ante ellas. Se advierte que tales construcciones sociales pueden no coincidir con la perspectiva biomédica. Se observó que en algunas poblaciones la construcción social del cuerpo se relaciona con la percepción de efectos benéficos de los parásitos. La gente reconoce factores de riesgo pero esto no necesariamente conduce a comportamientos preventivos. Algunos estudios reconocen que la educación ha sido de tipo informativo, sin participación comunitaria, lo cual impide que la gente se identifique con ella y se sienta motivada a transformar su realidad. Se identificaron diversos dilemas bioéticos producto del contacto entre visiones de las poblaciones, y la biomedicina. Conclusiones: La aproximación émica permite entender la lógica de la población sobre las parasitosis intestinales e identificar disyuntivas bioéticas. Aunque la gente reconozca los riesgos de infección, la falta de comprensión de la perspectiva émica de los parasitismos por parte del personal de salud y las deficientes condiciones de vida permiten que no se produzcan transformaciones que incidan en la disminución de sus prevalencias. Eso mismo afectará el éxito de programas de atención, prevención, promoción y educación en la salud. Pero el análisis de la visión émica, aunque es fundamental, no lo es todo. En el problema de las parasitosis intestinales intervienen también condiciones infraestructurales, que conllevan situaciones conflictivas para los profesionales de la salud, como agentes externos a las comunidades. Se pueden hacer campañas de educación que efectivamente mejoren la prevención, pero realidades socio-económicas (por ejemplo, pobreza, participación comunitaria, empoderamiento,) y políticas (compromisos y acciones de líderes y gobiernos a diferentes niveles) pueden incidir para que lo que se aprendió no surta el efecto deseado o que se modifiquen las condiciones estructurales que generan los problemas. En este punto se generan problemas morales que deberían ser analizados por la bioética.Objetivo: Identificar sentidos que povos das Américas atribuem às parasitoses intestinais e formular potenciais conflitos ou dilemas nas tomada de decisões em várias atividades de saúde produto do encontro de valores diferentes, o escopo da bioética. Metodologia: pesquisa de artigos sobre parasitoses intestinais que atenderam aos critérios de inclusão e forneceram contribuições para a compreensão hermenêutica das parasitoses intestinais e permitiram identificar dilemas bioé- ticos para atividades de saúde decorrentes do contato entre as visões émicas e éticas (da biomedicina) . Resultados: A construção social das parasitoses intestinais influencia na sua percepção de causalidade, nocividade e como um problema de saúde, o que afeta as ações que as pessoas empreendem diante deles. No problema das parasitoses intestinais também interferem condições de infraestrutura, que implicam situações conflitivas para os profissionais de saúde, e para agentes de fora para as comunidades.     Percepción del medio ambiente y del trabajo campesino : un relato de experiencia en La Virgen (Colombia)

    El objetivo del artículo es compartir una experiencia que exploró las percepciones sobre medio ambiente y trabajo campesino para estimular un proceso de reflexión sobre la relación entre ambos, que a su vez incentivara el empoderamiento comunitario en la Inspección de La Virgen (Colombia). Mediante 2 talleres participativos se indagaron las percepciones de campesinos/as (adultos y jóvenes), para conocerlas desde la visión emic y así producir una reflexión sobre sus problemas ambientales, sus posibles soluciones e incentivar un proceso de concientización que incida sobre el empoderamiento. La participación fue de carácter voluntario y las personas tuvieron previo conocimiento del propósito de los talleres. Se utilizaron dibujos, inspirados en cartografía social, para representar las percepciones de los participantes porque son elementos lúdicos que facilitan la comunicación, la reflexión y motivan el trabajo en equipo. Uno de los principales resultados fue la preocupación por la contaminación y disponibilidad del agua. Ambos grupos identificaron el ambiente con los recursos físicos. Además, el ambiente y la naturaleza fueron asociados con tranquilidad, pureza del aire y no contaminación. Las soluciones planteadas se relacionaron con acciones comportamentales y conductuales de los individuos, con la construcción de pozos sépticos, emplear abonos orgánicos y buscar la intervención de las instituciones del orden local y nacional en la solución de los problemas sentidos por la comunidad.The aim of the article is to share an experience that explored the perceptions of the environment and peasant work to stimulate a process of reflection on the relationship between the two that in turn will encourage community in the Inspection of La Virgen (Colombia). Perceptions of young and adult farmers were investigated through two participatory workshops, in order to know them from the emic perspective and thus produce a reflection on their environmental problems, possible solutions and encourage a process of awareness that impinges on empowerment. Participation in the workshops was voluntary and individuals had prior knowledge of their purpose. Drawings, inspired by social cartography, were used to represent the perceptions of the participants because they are playful elements that facilitate communication, reflection, and motivate teamwork. One of the main results was the concern about pollution and water availability. Both groups identified the environment with physical resources, such as water, wildlife, rivers and streams, trees, mountains and the sun. In addition, the environment and nature were associated with tranquility, clean air and no pollution. The solutions proposed are related to behavioral actions of individuals such as recycling, restrict chemicals use, avoid burning and logging, construct septic tanks, use organic fertilizers and seek the intervention of national and local institutions to solve problems felt by the community

    Contaminación del aire y enfermedad respiratoria en menores de cinco años de bogotá, 2007

    Objetivos Determinar la relación entre contaminación del aire extra e intramurospor material particulado PM10 con síntomas respiratorios en niños menores de 5años en tres localidades del Distrito Capital.Métodos Estudio de cohortes. 315 niños y niñas de jardines ubicados en zonade mayor exposición a material particulado PM10, y 304 de jardines en zona demenor exposición. Se siguieron las cohortes por 19 semanas, teniendo en cuentasíntomas respiratorios. Se midió material particulado intramuros y extramuros enlos jardines. Se determinaron prevalencias y asociación entre variables.Resultados Hubo una diferencia de promedios de MP intramuros entre mayory menor exposición de 85.6 μg/m3vs. 61.8 μg/m3 (p and lt;0,05). 74.3 % de los niñospresentaron sibilancias en el último año, con diferencia entre mayor y menorexposición de 79,6 % vs 69,0 % (p and lt;0,05). Sibilancias y expectoración estuvieronasociados a mayor exposición a PM10 (RR 1,39 y 1,30). Factores asociados asíntomas respiratorios: tabaquismo pasivo, el antecedente de bajo peso al nacer,el antecedente de sibilancias, la cercanía del jardín o vivienda a menos de 100metros de chimeneas, vías de alto flujo vehicular, vías en mal estado y colecho.Discusión Un niño menor de 5 años expuesto a PM10 tiene 1,70 veces másriesgo de presentar ausentismo escolar por enfermedad respiratoria aguda, unadisminución del PM10 reduciría en un 41,1 % este ausentismo escolar

Climate reanalysis represents an alternative option that provide complete spatio-temporal exposure coverage, and yet are to be systematically explored for their suitability in assessing temperature-related health risks at a global scale. Here we provide the first comprehensive analysis over multiple regions to assess the suitability of the most recent generation of reanalysis datasets for health impact assessments and evaluate their comparative performance against traditional station-based data. Our findings show that reanalysis temperature from the last ERA5 products generally compare well to station observations, with similar non-optimal temperature-related risk estimates. However, the analysis offers some indication of lower performance in tropical regions, with a likely underestimation of heat-related excess mortality.     Association Between Preexisting Versus Newly Identified Atrial Fibrillation and Outcomes of Patients With Acute Pulmonary Embolism

    Background Atrial fibrillation (AF) may exist before or occur early in the course of pulmonary embolism (PE). We determined the PE outcomes based on the presence and timing of AF. Methods and Results Using the data from a multicenter PE registry, we identified 3 groups: (1) those with preexisting AF, (2) patients with new AF within 2 days from acute PE (incident AF), and (3) patients without AF. We assessed the 90-day and 1-year risk of mortality and stroke in patients with AF, compared with those without AF (reference group). Among 16 497 patients with PE, 792 had preexisting AF. These patients had increased odds of 90-day all-cause (odds ratio [OR], 2.81; 95% CI, 2.33-3.38) and PE-related mortality (OR, 2.38; 95% CI, 1.37-4.14) and increased 1-year hazard for ischemic stroke (hazard ratio, 5.48; 95% CI, 3.10-9.69) compared with those without AF. After multivariable adjustment, preexisting AF was associated with significantly increased odds of all-cause mortality (OR, 1.91; 95% CI, 1.57-2.32) but not PE-related mortality (OR, 1.50; 95% CI, 0.85-2.66). Among 16 497 patients with PE, 445 developed new incident AF within 2 days of acute PE. Incident AF was associated with increased odds of 90-day all-cause (OR, 2.28; 95% CI, 1.75-2.97) and PE-related (OR, 3.64; 95% CI, 2.01-6.59) mortality but not stroke. Findings were similar in multivariable analyses. Conclusions In patients with acute symptomatic PE, both preexisting AF and incident AF predict adverse clinical outcomes.     Development and Validation of the Gene Expression Predictor of High-grade Serous Ovarian Carcinoma Molecular SubTYPE (PrOTYPE).

    PURPOSE: Gene expression-based molecular subtypes of high-grade serous tubo-ovarian cancer (HGSOC), demonstrated across multiple studies, may provide improved stratification for molecularly targeted trials. However, evaluation of clinical utility has been hindered by nonstandardized methods, which are not applicable in a clinical setting. We sought to generate a clinical grade minimal gene set assay for classification of individual tumor specimens into HGSOC subtypes and confirm previously reported subtype-associated features. EXPERIMENTAL DESIGN: Adopting two independent approaches, we derived and internally validated algorithms for subtype prediction using published gene expression data from 1,650 tumors. We applied resulting models to NanoString data on 3,829 HGSOCs from the Ovarian Tumor Tissue Analysis consortium. We further developed, confirmed, and validated a reduced, minimal gene set predictor, with methods suitable for a single-patient setting. RESULTS: Gene expression data were used to derive the predictor of high-grade serous ovarian carcinoma molecular subtype (PrOTYPE) assay. We established a de facto standard as a consensus of two parallel approaches. PrOTYPE subtypes are significantly associated with age, stage, residual disease, tumor-infiltrating lymphocytes, and outcome. The locked-down clinical grade PrOTYPE test includes a model with 55 genes that predicted gene expression subtype with >95% accuracy that was maintained in all analytic and biological validations. CONCLUSIONS: We validated the PrOTYPE assay following the Institute of Medicine guidelines for the development of omics-based tests.     The application of omics in ruminant production: a review in the tropical and sub-tropical animal production context

    The demand for animal products (e.g. dairy and beef) in tropical regions is expected to increase in parallel with the public demand for sustainable practices, due to factors such as population growth and climate change. The necessity to increase animal production output must be achieved with better management and production technologies. For this to happen, novel research methodologies, animal selection and postgenomic tools play a pivotal role. Indeed, improving breeder selection programs, the quality of meat and dairy products as well as animal health will contribute to higher sustainability and productivity. This would surely benefit regions where resource quality and quantity are increasingly unstable, and research is still very incipient, which is the case of many regions in the tropics.     Angular analysis of the B-0 -> K*(0) e(+) e(-) decay in the low-q(2) region

    An angular analysis of the B0K0e+eB^0 \rightarrow K^{*0} e^+ e^- decay is performed using a data sample, corresponding to an integrated luminosity of 3.0 {\mbox{fb}^{-1}}, collected by the LHCb experiment in pppp collisions at centre-of-mass energies of 7 and 8 TeV during 2011 and 2012. For the first time several observables are measured in the dielectron mass squared (q2q^2) interval between 0.002 and 1.120GeV2 ⁣/c4{\mathrm{\,Ge\kern -0.1em V^2\!/}c^4}. The angular observables FLF_{\mathrm{L}} and ATReA_{\mathrm{T}}^{\mathrm{Re}} which are related to the K0K^{*0} polarisation and to the lepton forward-backward asymmetry, are measured to be FL=0.16±0.06±0.03F_{\mathrm{L}}= 0.16 \pm 0.06 \pm0.03 and ATRe=0.10±0.18±0.05A_{\mathrm{T}}^{\mathrm{Re}} = 0.10 \pm 0.18 \pm 0.05, where the first uncertainty is statistical and the second systematic. The angular observables AT(2)A_{\mathrm{T}}^{(2)} and ATImA_{\mathrm{T}}^{\mathrm{Im}} which are sensitive to the photon polarisation in this q2q^2 range, are found to be AT(2)=0.23±0.23±0.05A_{\mathrm{T}}^{(2)} = -0.23 \pm 0.23 \pm 0.05 and ATIm=0.14±0.22±0.05A_{\mathrm{T}}^{\mathrm{Im}} =0.14 \pm 0.22 \pm 0.05. The angular observables AT(2)A_{\mathrm{T}}^{(2)} and ATImA_{\mathrm{T}}^{\mathrm{Im}} which are sensitive to the photon polarisation in this q2q^2 range, are found to be AT(2)=0.23±0.23±0.05A_{\mathrm{T}}^{(2)} = -0.23 \pm 0.23 \pm 0.05 and ATIm=0.14±0.22±0.05A_{\mathrm{T}}^{\mathrm{Im}} =0.14 \pm 0.22 \pm 0.05. The results are consistent with Standard Model predictions

    Study of the rare B-s(0) and B-0 decays into the pi(+) pi(-) mu(+) mu(-) final state

    A search for the rare decays Bs0π+πμ+μB_s^0 \to \pi^+\pi^-\mu^+\mu^- and B0π+πμ+μB^0 \to \pi^+\pi^-\mu^+\mu^- is performed in a data set corresponding to an integrated luminosity of 3.0 fb1^{-1} collected by the LHCb detector in proton-proton collisions at centre-of-mass energies of 7 and 8 TeV. Decay candidates with pion pairs that have invariant mass in the range 0.5-1.3 GeV/c2c^2 and with muon pairs that do not originate from a resonance are considered. The first observation of the decay Bs0π+πμ+μB_s^0 \to \pi^+\pi^-\mu^+\mu^- and the first evidence of the decay B0π+πμ+μB^0 \to \pi^+\pi^-\mu^+\mu^- are obtained and the branching fractions are measured to be B(Bs0π+πμ+μ)=(8.6±1.5(stat)±0.7(syst)±0.7(norm))×108\mathcal{B}(B_s^0 \to \pi^+\pi^-\mu^+\mu^-)=(8.6\pm 1.5\,({\rm stat}) \pm 0.7\,({\rm syst})\pm 0.7\,({\rm norm}))\times 10^{-8} and B(B0π+πμ+μ)=(2.11±0.51(stat)±0.15(syst)±0.16(norm))×108\mathcal{B}(B^0 \to \pi^+\pi^-\mu^+\mu^-)=(2.11\pm 0.51\,({\rm stat}) \pm 0.15\,({\rm syst})\pm 0.16\,({\rm norm}) )\times 10^{-8}, where the third uncertainty is due to the branching fraction of the decay B0J/ψ(μ+μ)K(890)0(K+π)B^0\to J/\psi(\to \mu^+\mu^-)K^*(890)^0(\to K^+\pi^-), used as a normalisation.A search for the rare decays Bs0→π+π−μ+μ− and B0→π+π−μ+μ− is performed in a data set corresponding to an integrated luminosity of 3.0 fb−1 collected by the LHCb detector in proton–proton collisions at centre-of-mass energies of 7 and 8 TeV .     Observation of the B0 → ρ0ρ0 decay from an amplitude analysis of B0 → (π+π−)(π+π−) decays

    Proton–proton collision data recorded in 2011 and 2012 by the LHCb experiment, corresponding to an integrated luminosity of 3.0 fb−1 , are analysed to search for the charmless B0→ρ0ρ0 decay. More than 600 B0→(π+π−)(π+π−) signal decays are selected and used to perform an amplitude analysis, under the assumption of no CP violation in the decay, from which the B0→ρ0ρ0 decay is observed for the first time with 7.1 standard deviations significance. The fraction of B0→ρ0ρ0 decays yielding a longitudinally polarised final state is measured to be fL=0.745−0.058+0.048(stat)±0.034(syst) . The B0→ρ0ρ0 branching fraction, using the B0→ϕK⁎(892)0 decay as reference, is also reported as B(B0→ρ0ρ0)=(0.94±0.17(stat)±0.09(syst)±0.06(BF))×10−6
