    A inserção das empresas multinacionais na economia mundial

O objetivo da pesquisa se volta ao ramo do comércio internacional, onde são estudadas as transferências dos fatores produtivos entre países, integrando mundialmente as relações comercias e produtivas através das corporações multinacionais. A importância da abordagem neste tema se transmite através da compreensão de como o sistema capitalista alterou suas relações comerciais e produtivas entre as fronteiras nacionais. As empresas multinacionais estabeleceram um marco histórico no capitalismo, pois superaram em poder os Estados-nação e deram início a uma nova configuração da economia mundial, em que a dimensão predominante passa a ser a da mobilidade da produção dos bens e serviços, além de contribuírem para amadurecimento da globalização financeira. O estudo se volta para os fatores históricos que proporcionaram estas empresas, até então nacionais, a instalarem bases produtivas em países estrangeiros, se tornando assim empresas multinacionais. As estratégias de expansão utilizadas pelas empresas multinacionais apareceram de forma a favorecer a produção das empresas através do investimento direto externo, por razões como a redução dos custos de produção, baixos níveis salariais dos países receptores e até sob o pretexto de investimento de longo prazo, através da inserção em um potencial mercado regional. São vistas algumas características dos principais países originários das empresas multinacionais, notavelmente a tríade, composta por Estados Unidos, Japão e União Européia. Através das características das grandes empresas nacionais de origem triádica, que viriam a se tornar multinacionais, pode-se observar as razões históricas que favoreceram essa nova dinâmica da produção mundial. Além disso, são estudadas as principais vantagens que as empresas originadas nos países centrais obtém sobre as concorrentes estrangeiras. Posteriormente, a atenção se volta para as empresas multinacionais nos dias atuais, através de alguns gráficos e dados, onde podem ser observadas as tendências crescentes de acumulação das empresas, além das estratégias utilizadas por estas grandes organizações

    CONTEXTO: Relatada desde a Antiguidade, a tricotilomania (TTM) somente na última década despertou maior interesse clínico, sendo incluída no DSM-IV-TR (The Diagnostic and Statistical Manual of Mental Disorders 4th Text Revision) como um transtorno do controle dos impulsos não especificado. Dados recentes estimam uma prevalência em torno de 3%, indicando uma incidência mais comum do que se imaginava. Aspectos clínicos e terapêuticos ainda não estão totalmente definidos. OBJETIVO: Revisão sistemática da literatura de vários aspectos da tricotilomania pertinentes à teoria clínica e prática. MÉTODO: Os termos "trichotillomania", "epidemiology", "clinical characteristics", "etiology", "comorbidity" e "treatment" foram consultados nas bases de dados Medline/PubMed, Lilacs, PsycINFO e Cochrane Library. RESULTADOS: Pesquisas com populações não clínicas sugerem que a TTM é mais comum do que se acreditava. Aspectos fenomenológicos, taxonômicos, comorbidades e possibilidades terapêuticas são discutidos. CONCLUSÃO: Apesar de um crescente número de estudos recentes, questões clínicas e terapêuticas permanecem em aberto.     Behavioral symptoms related to cognitive impairment

    Neuropsychiatric symptoms (NPS) are core features of Alzheimer's disease and related dementias. On one hand, behavioral symptoms in patients with mild cognitive impairment (MCI) can indicate an increased risk of progressing to dementia. On the other hand, mild behavioral impairment (MBI) in patients who usually have normal cognition indicates an increased risk of developing dementia. Whatever the cause, all dementias carry a high rate of NPI. These symptoms can be observed at any stage of the disease, may fluctuate over its course, are a leading cause of stress and overload for caregivers, and increase rates of hospitalization and early institutionalization for patients with dementia. The clinician should be able to promptly recognize NPI through the use of instruments capable of measuring their frequency and severity to support diagnosis, and to help monitor the treatment of behavioral symptoms.     Geriatric Depression and Cognitive Impairment: A Follow up Study

    Introduction: Depression in older adults has become a major problem for public health. A high percentage of this population is under-diagnosed in primary care. The objectives of this work were firstly, to investigate the causes, risk factors, cognitive profile, functional status and quality of life of patients with geriatric depression, and secondly, to make a follow up of these patients. Materials and methods: Patients who consulted for memory problems associated with depression were recruited during the years 2005-2007. A semi-structured neuropsychiatric interview, an extensive neuropsychological battery, and complementary studies were performed. Results: One hundred and one depressive patients and 25 normal controls were evaluated. There was a significant prevalence and incidence of depression in the geriatric population. Significant differences (p<0.05) were found between depressive patients and controls in dyslipidemia, heart disease, cerebrovascular disease, inadequate family support, family history of depression and inactivity (OR 6.5). A global cognitive impairment was frequently associated with depression. Depression caused an alteration in functional status. Follow up results: From the 101 patients evaluated only 61 attended to the follow up visit (61.4%). All patients were indicated antidepressant treatment. Of these, only 36 patients continue with the treatment indicated in the baseline visit. Of the patients who were in antidepressant treatment (n=36) 46.6% had an excellent to good response, and 13.3% had a response from fair to poor. The main causes of poor response were adverse effects, low-dose and treatment neglect. Of the reevaluated patients, 56.6% improved in cognition or mood. The greatest improvement was observed in depression and anxiety affective symptoms. Within the cognitive profile, memory and attention trend to improve with medical treatment. Conclusion: Depression is a prevalent disease in the elderly population. It is important to implement health policies to inform the community, prevent associated risk factors, and promote appropriate treatments and rehabilitation.     Risk of Conversion to Dementia in a Mild Behavioral Impairment Group Compared to a Psychiatric Group and to a Mild Cognitive Impairment Group

    Background: There is insufficient available information on behavioral changes in the absence of cognitive impairment as factors increasing the risk of conversion to dementia. Objective: To observe and analyze patients with mild behavioral impairment (MBI), mild cognitive impairment (MCI), and a psychiatry group (PG) to compare the risk of progression to dementia. Methods: From 677 initially assessed =60-year-old patients, a series of 348 patients was studied for a five-year period until censoring or conversion to dementia: 96 with MBI, 87 with MCI, and 165 with general psychiatry disorders, including 4 subgroups: Anxiety, Depression, Psychosis and Others. All patients were assessed with clinical, psychiatric, neurological, neuropsychological, and neuroimaging studies. Results: From 348 patients, 126 evolved to dementia (36.2%). Conversion was significantly higher in MBI (71.5%), followed by the MCI-MBI overlap (59.6%) and MCI (37.8%) groups, compared to PG (13.9%) (Log-rank p < 0.001). MCI patients mostly converted to Alzheimer's dementia, while MBI converted to frontotemporal dementia and Lewy body dementia. Patients in PG converted to Lewy body dementia and frontotemporal dementia. Conclusion: Conversion to dementia is significantly higher in patients with neuropsychiatric symptoms. The MBI concept generates a new milestone in the refining of diagnosis of neurodegenerative diseases and the possibility of creating neuropsychiatric profiles.     Interventions for treating depression after stroke

    Background: Depression is an important consequence of stroke that impacts on recovery yet is often not detected or inadequately treated. This is an update of a Cochrane review first published in 2004. Objectives: To determine whether pharmaceutical, psychological, or electroconvulsive treatment (ECT) of depression in patients with stroke can improve outcome. Search strategy: We searched the trials registers of the Cochrane Stroke Group (last searched October 2007) and the Cochrane Depression Anxiety and Neurosis Group (last searched February 2008). In addition, we searched the Cochrane Central Register of Controlled Trials (The Cochrane Library, Issue 1, 2008), MEDLINE (1966 to May 2006), EMBASE (1980 to May 2006), CINAHL (1982 to May 2006), PsycINFO (1967 to May 2006) and other databases. We also searched reference lists, clinical trials registers, conference proceedings and dissertation abstracts, and contacted authors, researchers and pharmaceutical companies. Selection criteria: Randomised controlled trials comparing pharmaceutical agents with placebo, or various forms of psychotherapy or ECT with standard care (or attention control), in patients with stroke, with the intention of treating depression. Data collection and analysis: Two review authors selected trials for inclusion and assessed methodological quality; three review authors extracted, cross-checked and entered data. Primary analyses were the prevalence of diagnosable depressive disorder at the end of treatment. Secondary outcomes included depression scores on standard scales, physical function, death, recurrent stroke and adverse effects. Main results: Sixteen trials (17 interventions), with 1655 participants, were included in the review. Data were available for 13 pharmaceutical agents, and four trials of psychotherapy. There were no trials of ECT. The analyses were complicated by the lack of standardised diagnostic and outcome criteria, and differing analytic methods. There was some evidence of benefit of pharmacotherapy in terms of a complete remission of depression and a reduction (improvement) in scores on depression rating scales, but there was also evidence of an associated increase in adverse events. There was no evidence of benefit of psychotherapy. Authors' conclusions: A small but significant effect of pharmacotherapy (not psychotherapy) on treating depression and reducing depressive symptoms was found, as was a significant increase in adverse events. More research is required before recommendations can be made about the routine use of such treatments

    What is the therapeutic value of antidepressants in dementia? A narrative review

    Objectives: Antidepressants are commonly used in dementia. Depression is a frequent and important co-morbidity in dementia and antidepressants are often used to treat depression and more widely. However there are questions about their utility in depression in dementia and other behavioural and psychological symptoms of dementia (BPSD). The aim of this narrative review is to summarise the evidence on whether there is therapeutic value in prescribing antidepressants to people with dementia. Methods: A PubMed search was performed to identify RCTs that prescribed antidepressants to people with dementia, either in the treatment of BPSD (depression, anxiety, agitation/aggression, psychosis, apathy) or for secondary outcomes (quality of life, carer burden, activities of daily living, cognition, clinical severity, adverse events). Results: Thirty-six RCTs were identified (participant n=3,386). A consistent finding in well-designed blinded placebo controlled trials in dementia is the lack of positive effect of antidepressants on outcomes of interest including depression. One large well-designed study has reported a significant reduction in agitation in people with dementia, but at the expense of clinically significant adverse events. Otherwise change observed in open trials is also seen in the placebo group, suggesting any effect is not attributable to the prescription of antidepressants. Conclusions: It is striking how few data there are on indications other than depression. We should question the use of antidepressants in dementia. Definitive trials of clinical effectiveness of specific indications such as anxiety and agitation in dementia and discontinuation of antidepressants in dementia are needed

    Introduction: Progress in understanding and management of vascular cognitive impairment (VCI) has been hampered by lack of consensus on diagnosis, reflecting the use of multiple different assessment protocols. A large multinational group of clinicians and researchers participated in a two-phase Vascular Impairment of Cognition Classification Consensus Study (VICCCS) to agree on principles (VICCCS-1) and protocols (VICCCS-2) for diagnosis of VCI. We present VICCCS-2. Methods: We used VICCCS-1 principles and published diagnostic guidelines as points of reference for an online Delphi survey aimed at achieving consensus on clinical diagnosis of VCI. Results: Six survey rounds comprising 65–79 participants agreed guidelines for diagnosis of VICCCS-revised mild and major forms of VCI and endorsed the National Institute of Neurological Disorders–Canadian Stroke Network neuropsychological assessment protocols and recommendations for imaging. Discussion: The VICCCS-2 suggests standardized use of the National Institute of Neurological Disorders–Canadian Stroke Network recommendations on neuropsychological and imaging assessment for diagnosis of VCI so as to promote research collaboration
