    Human chitotriosidase helps <i>Plasmodium falciparum</i> in the <i>Anopheles</i> midgut

    High levels of plasma chitotriosidase (Chit) represents a marker of macrophage activation in human malaria infection. Plasmodium falciparum during its maturation cycle in the Anopheles midgut produces an analogue of Chit for the digestion of peritrophic matrix (PM). Our results confirm the hypothesis that Chit contained in blood of malaria patients could help P. falciparum to complete its cycle in the Anopheles midgut and to produce a bigger number of oocysts/sporozoites. This could balance the different genetic protection in humans conferred by the heterozygous Hb beta (S) gene, which seems to be associated with an increasing effect on P. falciparum transmission from humans to mosquitoes

    Facies created by the yellow coral Dendrophyllia cornigera (Lamarck, 1816): Origin, substrate preferences and habitat complexity

    The yellow coral Dendrophyllia cornigera (Lamarck, 1816) is a NE Atlantic-Mediterranean scleractinian. It is considered a typical hard bottom species, generally reported on outcropping rocks from mesophotic to upper bathyal depths. Several evidences suggest that this species is able to tolerate a broad range of temperatures, which allows it to colonize numerous environments in a wide depth range. In the present study, we first provided a detailed ecological characterization of the D. cornigera dense aggregation thriving on the Mantice Shoal (NW Ligurian Sea, Mediterranean Sea). Information on substrate type and inclination, average extension and density, colonies size-class frequency distribution and associated fauna are reported. Then, we presented an extensive review of the available information on the ecology of this species, including 142 new ROV records from the Italian coast (40–1820 m). Results indicated that D. cornigera occurs on a wide range of substrates, including soft bottoms and hardgrounds (outcropping rocks, coralligenous rock and dead cold-water coral frameworks), with significant differences in colony density and size among different substrates. Dendrophyllia cornigera creates three main facies, each characterized by a specific combination of substrate, inclination, depth, and associated fauna. Scattered living colonies, as well as large thanatocoenoses, display a wide geographical and bathymetric distribution. Differently, the facies represented by dense meadows on horizontal soft-bottoms results rare, being reported only from the Mantice Shoal and the Amendolara Bank (Ionian Sea). The radiocarbon age of the thanatocoenoses varies between 400 (Corsica Channel) and 13000 (Vercelli Seamount) years before present. This study highlights the wide adaptability of D. cornigera in terms of environmental settings, changing the current view on the ecology of this species and providing essential insights for the implementation of international deep-sea habitat classification schemes and conservation measures

    Interference mitigation in wideband radios using spectrum correlation and neural network

    Technologies such as cognitive radio and dynamic spectrum access rely on spectrum sensing which provides wireless devices with information about the radio spectrum in the surrounding environment. One of the main challenges in wireless communications is the interference caused by malicious users on the shared spectrum. In this manuscript, an artificial intelligence enabled cognitive radio framework is proposed at system-level as part of a cyclic spectrum intelligence algorithm for interference mitigation in wideband radios. It exploits the cyclostationary feature of signals to differentiate users with different modulation schemes and an artificial neural network as classifier to detect potential malicious users. A dataset consisting of experimental modulated and dynamic signals is recorded by spectrum measurements with an in-house software defined radio testbed and then processed. Cyclostationary features are extracted for each detected signal and fed to a neural network classifier as training and testing data in a complex and dynamic scenario. Results highlight a classification rate of 3c1 3c1 1 in most of cases, even at low transmission power. A comparison with two previous works with hand-crafted features, which employ an energy detector-based classifier and a naive Bayes-based classifier, respectively, is discussed

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     The IDENTIFY study: the investigation and detection of urological neoplasia in patients referred with suspected urinary tract cancer - a multicentre observational study

    Objective To evaluate the contemporary prevalence of urinary tract cancer (bladder cancer, upper tract urothelial cancer [UTUC] and renal cancer) in patients referred to secondary care with haematuria, adjusted for established patient risk markers and geographical variation. Patients and Methods This was an international multicentre prospective observational study. We included patients aged ≥16 years, referred to secondary care with suspected urinary tract cancer. Patients with a known or previous urological malignancy were excluded. We estimated the prevalence of bladder cancer, UTUC, renal cancer and prostate cancer; stratified by age, type of haematuria, sex, and smoking. We used a multivariable mixed-effects logistic regression to adjust cancer prevalence for age, type of haematuria, sex, smoking, hospitals, and countries. Results Of the 11 059 patients assessed for eligibility, 10 896 were included from 110 hospitals across 26 countries. The overall adjusted cancer prevalence (n = 2257) was 28.2% (95% confidence interval [CI] 22.3–34.1), bladder cancer (n = 1951) 24.7% (95% CI 19.1–30.2), UTUC (n = 128) 1.14% (95% CI 0.77–1.52), renal cancer (n = 107) 1.05% (95% CI 0.80–1.29), and prostate cancer (n = 124) 1.75% (95% CI 1.32–2.18). The odds ratios for patient risk markers in the model for all cancers were: age 1.04 (95% CI 1.03–1.05; P < 0.001), visible haematuria 3.47 (95% CI 2.90–4.15; P < 0.001), male sex 1.30 (95% CI 1.14–1.50; P < 0.001), and smoking 2.70 (95% CI 2.30–3.18; P < 0.001). Conclusions A better understanding of cancer prevalence across an international population is required to inform clinical guidelines. We are the first to report urinary tract cancer prevalence across an international population in patients referred to secondary care, adjusted for patient risk markers and geographical variation. Bladder cancer was the most prevalent disease. Visible haematuria was the strongest predictor for urinary tract cancer
