    Lagrangian and ALE Formulations For Soil Structure Coupling with Explosive Detonation

    Simulation of Soil-Structure Interaction becomes more and more the focus of computational engineering in civil and mechanical engineering, where FEM (Finite element Methods) for soil and structural mechanics and Finite Volume for CFD (Computational Fluid Dynamics) are dominant. New advanced formulations have been developed for FSI (Fluid Structure Interaction) applications using ALE (Arbitrary Lagrangian Eulerian), mesh free and SPH (Smooth Particle Hydrodynamic) methods. In defence industry, engineers have been developing protection systems for many years to reduce the vulnerability of light armoured vehicles (LAV) against mine blast using classical Lagrangian FEM methods. To improve simulations and assist in the development of these protections, experimental tests and new numerical techniques are performed. Initial conditions such as the loading prescribed by a mine on a structure should be simulated adequately in order to conduct these numerical calculations. The effects of blast on structures often depend on how the initial conditions are estimated and applied. This article uses two methods to simulate a mine blast, namely the classical Lagrangian as well as the ALE formulations. The comparison was carried out for a simple and also a more complex target.     First identification of an Escherichia coli clinical isolate producing both metalloβ-lactamase VIM-2 and extended-spectrum β-lactamase IBC-1

    AbstractAn Escherichia coli strain with decreased susceptibility to carbapenems was isolated from a hospitalised patient in Athens, Greece. The strain was resistant to all β-lactams, including aztreonam, whereas the MIC of imipenem and meropenem was 0.5 mg/L. A positive EDTA-disk synergy test suggested the production of a metallo-β-lactamase. PCR experiments revealed the presence of the blaVIM-2, blaIBC-1 and blaTEM-1 genes. Resistance to β-lactams was not transferable by conjugation. This is the first report of a clinical isolate of E. coli producing VIM-2, and the first report of the coexistence of blaVIM-2 and blaIBC-1 in a single clinical isolate

    Morphological and Biological Characterization of Monosporascus cannonballus isolates, responsible of watermelon decline in Kairouan's area

    Watermelon grown in Kairouan's region is infected by vine decline disease caused by M. cannonballus. In vitro tests showed that this pathogen produces perithecia with a diameter of 495 μm releasing ascospores with a diameter of 44 μm. The mycelium is very fine, rarely visible in the media KOMADA and TANAKA. The colony of different isolates on substrate culture such as Malt, MS and S, appears very dense. On PDA, the grayish brown color characteristic of this ascomycete was observed. The perithecia production in vitro test for this fungus requires relatively long period of incubation (45 days). The effect of culture media showed that the PDA, MS and Malt are the best for the mycelia development of M. cannonballus. The most favorable culture media for fructification arein descending order MS, S, PDA, Malt. M. cannonballus isolates tested in this study showed an optimum temperature of mycelial growth and reproduction of 30°C. MT15 andMT12 isolates originated from Chebika (Kairouan government) showed a significantmycelial growth at all temperatures of incubation. MT7 and MT14 isolates from the sameregion were the most fertile. The results of pH effect on M. cannonballus developmenthave revealed that the maximum mycelial growth occurred at pH 6 for most of isolates andfor the fructification occurred at pH 4 and pH 8. For the osmotic pression with the addition of NaCl and KCl, the optimal development for majority of isolates is registeredat -0.5 MPa and -2 MPa, with a minimum at -4MPa. The inhibition of the fructification of the M. cannonballus isolates occurred beyond -2MPa

    Composición bioquímica y potencial antioxidante del pepino del mar Mediterráneo comestible Holothuria tubulosa

    The sea cucumber or holothurian is a marine species which has been prized in some Asian coun­tries for its nutritional qualities. The purpose of this work was to study the biochemical composition and free radical scavenging and antioxidant activities of Holothuria tubulosa tegument from the Bizerta lagoon in north­ern Tunisia. The obtained data demonstrated that the extract of sea cucumber teguments exhibited high bio­chemical levels (such as moisture 80.77%, protein 7.07%, lipids 10.21%, energy value 13.64 Kcal/g ww), and an important nutritional value (including n-3/n-6: 2.11, EPA+DHA: 20.96, AI: 1.38 and TI: 0.54). High anti­oxidant activities were recorded in the integument by the radical scavenging tests of ABTS and DPPH as well as by the total antioxidant capacity and the FRAP in comparison with the BHT standard. Our results showed that H. Tubulosa tegument has high nutritional value with high antioxidant activities and could be considered a nutraceutical product.El pepino de mar o la holoturia es una especie marina apreciada en algunos países asiáti­cos por sus cualidades nutricionales. El propósito de este trabajo fue estudiar la composición bioquímica y las actividades antioxidantes y de eliminación de radicales libres del tegumento de Holothuria tubulosa de la laguna de Bizerta, en el norte de Túnez. Los datos obtenidos demuestran que el extracto de tegumentos de pepino de mar mostró altos niveles bioquímicos (como humedad 80,77%, proteína 7,07%, lípidos 10,21%, valor energético 13,64 Kcal/gww) y un valor nutricional importante (incluyendo n-3/ n-6: 2,11, EPA+DHA: 20,96, AI: 1,38 y TI: 0,54). Se registraron altas actividades antioxidantes en el tegumento mediante las pruebas de eliminación de radicales de ABTS y DPPH, así como por la capacidad antioxidante total y el FRAP, y esto, en comparación con el estándar BHT. Nuestros resultados mostraron que el tegumento de H. Tubulosa tiene un valor nutricional importante con una alta actividad antioxidante y podría considerarse un producto nutracéutico

    L’acupuncture : comme alternative dans la prise en charge de la paralysie faciale peripherique idiopathique : experience tunisienne

    La paralysie faciale périphérique idiopathique ou à frigoré (PfPi) est la plus fréquente des paralysies faciales. Elle pose un problème thérapeutique malgré la diversité des traitements proposés. L’acupuncture est une des alternatives proposées parmi cet arsenal thérapeutique. Le but de ce travail est d’évaluer l’apport de l’acupuncture dans le traitement de la PfPi et de dégager les différents facteurs pronostiques pouvant influencer la récupération de la fonction faciale. Notre étude a regroupé 40 patients porteurs de PfPi recrutés aux services d’ORL de l’hôpital régional de Jendouba et de l’hôpital de Charles Nicolle sur une période de 14 mois et pris en charge aux centres d’acupuncture à l’hôpital Mongi Slim et à l’hôpital régional de Jendouba. Tous les patients ont été traités, auparavant, selon le concept de la médecine occidentale, et n’ont été adressés qu’après échec du traitement médical conventionnel. L’évaluation de notre travail est basée sur deux tests : le testing musculaire de freyss et le grading de House et Brackman. L’étude statistique s’est basée sur le logiciel « SPSS ». Les résultats de cette étude montrent que le traitement par acupuncture trouve son indication lorsque le traitement médical conventionnel a échoué, même après un long délai de consultation.Mots-clés : paralysie faciale périphérique, acupuncture, traitement.The Bell’s palsy is the most common peripheral facial paralysis. it poses a therapeutic problem despite the diversity of treatments. Acupuncture is an alternative among the proposed therapeutic arsenal. The purpose of this study was to evaluate the contribution of acupuncture in the treatment of facial paralysis and to identify different prognostic factors that may influence the recovery of facial function. Our study included 40 patients with Bell’s palsy recruited in the services of ENT in Jendouba hospital and Charles Nicolle hospital over a period of 14 months and supported in centers of acupuncture in Mongi Slim hospital and Jendouba hospital. All patients were previously under the concept of medical treatment, and were sent after failure of this treatment. The evaluation of our work is based on two tests: muscle testing of freyss and grading of House and Brackman. The study is based on statistical software "SPSS". The results of this study show that treatment with acupuncture is indicated in Bell’s palsy when conventional medical treatment has failed and even after a long period of consultation.Keywords: facial palsy, acupuncture, treatment

    blaKPC and rmtB on a single plasmid in Enterobacter amnigenus and Klebsiella pneumoniae isolates from the same patient

    Enterobacter amnigenus (EA76) and Klebsiella pneumoniae (KP76) isolates with multidrug-resistant (MDR) patterns were identified from the same patient in the neurosurgery department of our hospital. An outbreak of MDR K. pneumoniae had also occurred in this department. To characterize the resistance mechanism and molecular epidemiology of these isolates, sequential experiments including antimicrobial susceptibility testing, polymerase chain reaction (PCR), plasmid analysis, pulsed field gel electrophoresis (PFGE), and multilocus sequence typing (MLST) were performed. EA76 and KP76 were resistant to all of the antibiotics tested, except colistin and tigecycline. blaKPC-2, blaTEM-1, blaSHV-12, blaCTX-M-3, blaCTX-M-14, and rmtB genes were identified in both isolates, with blaKPC-2, blaTEM-1, blaCTX-M-14, and rmtB being co-carried on one plasmid in each isolate. Further analysis showed different restriction patterns between the two KPC-carrying plasmids. Of the 11 carbapenem-resistant isolates found in the outbreak, all were resistant to all of the β-lactams tested, with 63.64% (7/11) also exhibiting resistance to aminoglycosides and 72.73% (8/11) exhibiting resistance to quinolones. PCR analysis and molecular typing of the 11 K. pneumoniae strains revealed that the seven aminoglycoside-resistant isolates shared the same antibiotic-resistant gene pattern and identical or one-band-difference PFGE profiles relative to KP76. In addition, all of the eight aminoglycoside-resistant isolates, including KP76, belonged to the national epidemic clone ST11. The overall results indicate the emergence of E. amnigenus and outbreak of ST11 K. pneumoniae, with both co-harboring blaKPC and rmtB genes on a single plasmid in our neurosurgery wards

    Geographical variation in therapy for bloodstream infections due to multidrug-resistant enterobacteriaceae: a post hoc analysis of the INCREMENT study

    We aimed to describe regional differences in therapy for bloodstream infection (BSI) caused by extended-spectrum ?-lactamase-producing Enterobacteriaceae (ESBL-E) or carbapenemase-producing Enterobacteriaceae (CPE). 1,482 patients in 12 countries were included from an observational study of BSI caused by ESBL-E or CPE. Multivariate logistic regression was used to calculate adjusted odds ratios (aORs) for the influence of country of recruitment on empirical use of ?-lactam/?-lactamase inhibitors (BLBLI) or carbapenems, targeted use of BLBLI for ESBL-E and use of targeted combination therapy for CPE. The use of BLBLI for empirical therapy was least likely in sites from Israel (aOR 0.34, 95% CI 0.14-0.81), Greece (aOR 0.49, 95% CI 0.26-0.94) and Canada (aOR 0.31, 95% CI 0.11-0.88) but more likely in Italy (aOR 1.58, 95% CI 1.11-2.2) and Turkey (aOR 2.09, 95% CI 1.14-3.81), compared to Spain as a reference. Empirical carbapenems were more likely to be used in sites from Taiwan (aOR 1.73, 95% CI 1.03-2.92) and USA (aOR 1.89; 95% CI 1.05-3.39), and less likely in Italy (aOR 0.44, 95% CI 0.28-0.69) and Canada (aOR 0.10, 95% CI 0.01-0.74). Targeted BLBLI for ESBL-E was more likely in sites from Italy. Treatment at sites within Israel, Taiwan, Turkey and Brazil was associated with less combination therapy for CPE. Although this study does not provide precise data on the relative prevalence of ESBL-E or CPE, significant variation in therapy exists across countries even after adjustment for patient factors. A better understanding of what influences therapeutic choices for these infections will aid antimicrobial stewardship efforts.PH is supported by an Australian Postgraduate Award from the University of Queensland.     Novel β-lactam-β-lactamase inhibitor combinations: expectations for the treatment of carbapenem-resistant Gram-negative pathogens

    Introduction: The burden of antimicrobial resistance among Gram-negative bacteria is increasing and growing into a major threat of public health. Treatment options for carbapenem-resistant Enterobacteriaceae are limited and resistance rates to existing compounds are mounting. The pipeline includes only a small number of novel anti-infective agents in development or in the market with promising results against multidrug-resistant (MDR) Gram-negative. Areas covered: Herein the authors present the modern available knowledge regarding novel β-lactam-β-lactamase inhibitors, i.e. mechanisms of action, in vitro activity, current PK/PDs, clinical trials and clinical efficacy against MDR and XDR Gram-negatives, as well as toxicity issues. Expert opinion: Ceftazidime-avibactam and meropenem-vaborbactam are promising therapeutic options as both are active against Enterobacteriaceae producing ESBL, AmpC, and KPC, whereas only avibactam inhibits certain class D β-lactamases, mainly OXA-48. New drugs active against Gram-negative MDR isolates including imipenem/cilastatin with relebactam and avibactam combined with aztreonam or ceftaroline are in different stages of development. However, the disadvantage to be seriously considered by the clinician is that β-lactam/β-lactamase inhibitors are ineffective against metallo-β-lactamases (with the exception of aztreonam-avibactam) as well as Acinetobacter baumannii. © 2019, © 2019 Informa UK Limited, trading as Taylor & Francis Group
