While many people are overtreated in hospitals with families and communities relegated to the margins, still more remain undertreated, dying of preventable conditions and without access to basic pain relief. Genesis and growth of extracellular vesicle-derived microcalcification in atherosclerotic plaques
Clinical evidence links arterial calcification and cardiovascular risk. Finite-element modelling of the stress distribution within atherosclerotic plaques has suggested that subcellular microcalcifications in the fibrous cap may promote material failure of the plaque, but that large calcifications can stabilize it. Yet the physicochemical mechanisms underlying such mineral formation and growth in atheromata remain unknown. Here, by using three-dimensional collagen hydrogels that mimic structural features of the atherosclerotic fibrous cap, and high-resolution microscopic and spectroscopic analyses of both the hydrogels and of calcified human plaques, we demonstrate that calcific mineral formation and maturation results from a series of events involving the aggregation of calcifying extracellular vesicles, and the formation of microcalcifications and ultimately large calcification zones. We also show that calcification morphology and the plaque’s collagen content – two determinants of atherosclerotic plaque stability - are interlinked
Mental simulation as substitute for experience
People spend a considerable amount of their time mentally simulating experiences other than the one in which they are presently engaged, as a means of distraction, coping, or preparation for the future. In this integrative review, we examine four (non-exhaustive) cases in which mentally simulating an experience serves a different function, as a substitute for the corresponding experience. In each case, mentally simulating an experience evokes similar cognitive, physiological, and/or behavioral consequences as having the corresponding experience in reality: (1) imagined experiences are attributed evidentiary value like physical evidence, (2) mental practice instantiates the same performance benefits as physical practice, (3) imagined consumption of a food reduces its actual consumption, and (4) imagined goal achievement reduces motivation for actual goal achievement. We organize these cases under a common superordinate category and discuss their different methodological approaches and explanatory accounts. Our integration yields theoretical and practical insights into when and why mentally simulating an experience serves as its substitute

Searching the World Wide Web in Low-Connectivity Communities
The Internet has the potential to deliver information to communities around the world that have no other information resources. High telephone and ISP fees - in combination with low-bandwidth connections - make it unaffordable for many people to browse the Web online. We are developing the TEK system to enable users to search the Web using only email. TEK stands for "Time Equals Knowledge," since the user exchanges time (waiting for email) for knowledge. The system contains three components: 1) the client, which provides a graphical interface for the end user, 2) the server, which performs the searches from MIT, and 3) a reliable email-based communication protocol between the client and the server. The TEK search engine differs from others in that it is designed to return low-bandwidth results, which are achieved by special filtering, analysis, and compression on the server side. We believe that TEK will bring Web resources to people who otherwise would not be able to afford them
