Reflections on the documentary abstract and it?s practice: revision and systematization of theoreticalpractical approaches
[Resumen]
Se realiza una categorización de los estudios del resumen documental a partir de la revisión de los diferentes
modelos y propuestas de los investigadores en este campo. Se señala la evolución de la investigación
en esta actividad de tratamiento documental de contenido, atendiendo a las aproximaciones de la
textolingüística y el estructuralismo lingüístico, los enfoques semióticos, las orientaciones de la psicología
cognitiva, o el funcionalismo sociointeraccional. Para ello, se establecen distintas etapas, que van desde
los postulados precientíficos iniciales hacia avances metateóricos y procedimentales más complejos y
dinámicos. Las nuevas orientaciones y tendencias son clasificadas en base a tres dimensiones: el enfoque
del texto/producto, el enfoque del proceso-actividad resumidora y la aproximación comunicativo-sociocultural,
como puente integrador entre ambos. Finalmente, se alude a una fusión necesaria de las teorías
resumidoras existentes para configurar un mapa consolidado de modelos resumidores en distintos ámbitos
de especialidad.[Abstract]
We elaborate a study of the documentary abstract´s categorisation based on the revision of the different
models the investigators in this field´s proposal. In this paper it is indicated the evolution of the investigation about the activity of the documentary treatment focusing on content. This research is developing
through various interdisciplinary approaches, such as, textolingüístique approache, linguistic estructuralism
approach, semiotic approach, including mental psychology orientation, and sociointeractional
functionalism. In order to achieve the purpose, different stages are established: starting from the initial
pre-scientific proposals which postulated towards the more complex and dynamic metatheoretical and procedural
advances. The new directions and tendencies are classified on the basis of three dimensions: text/
product approach, abstracting process-activity approach and the communicative-sociocultural approach,
as the integrating bridge between both. Finally, it guide to a necessary fusion of the existing summarizing
theories in order to form a consolidated map of abstracting models in different scopes of specialty
Therapeutic communication does not burn you out; it is invigorating
El objetivo de este trabajo es ayudar a clarificar
y modificar la idea que tienen ciertos
profesionales de que la "relación de ayuda
quema". Es decir, tratar de precisar, partiendo de
cómo la Relación de Ayuda si la entendemos en su
justo valor, no sólo no quema, no nos lleva al síndrome
del burn-out, sino que ésta es vivificante
tanto a nivel personal como profesional. Si ciertos
profesionales han llegado a esa triste experiencia,
(de que la relación de ayuda quema) pensamos se
debe, entre otras razones: por una parte, a no tener
una idea clara de lo que es la comunicación y la
relación terapéutica o de ayuda; por otra parte, a
habernos olvidado que no podemos dedicarnos a
esta noble labor, sin antes haber trabajado un mínimo
sobre nosotros mismos, es decir, sobre nuestro crecimiento personal, autoestima, problemas, conflictos,
etc. Si esto no se hace, cabe el peligro de no
saber dónde nos situamos, transgrediendo ciertos
principios o límites que van anejos a la Relación de
Ayuda.
Asi mismo, intentamos exponer algunas de las
fases por las que se llega al síndrome del burn-out,
con el fin de que nos sirvan de reflexión, y poder
así restaurar la "vida" personal y profesional que
hayamos podido haber dejado en este caminar
acompañando a los pacientes.The aim of this work is to help to clarify and
modify the perception of some professionals
that therapeutic communication is
something that leads to "burn out". That is to say,
to show how therapeutic communication not only
does not burn you out, but can be invigorating at
both a personal and professional level. The fact
that some professionals have been through this sad
experience (burnt out by therapeutic communication)
may be due, among other reasons, to not
having a clear concept of communication and the therapeutic relationship, and having forgotten that
we cannot dedicate ourselves to this noble task without
working first on ourselves, our personal
growth, self-esteem, problems, conflicts, etc. If
this is not done, there is a risk of not knowing
where we stand—transgressing certain limits or
principles that go along with therapeutic communication.
Moreover, we try to state some of the stages of
"burn-out syndrome", in order to help us to reflect,
and to restore the professional and personal "life"
that we may have left behind while caring for our
patients
Respuesta inmune en mucosas de piel y branquias de Solea senegalensis tras la alimentación con una dieta conteniendo Ulva ohnoi (5%)
Abstract
In the search of the improvement of the feeding of the fish in the aquaculture, macroalgae seem to be a promising source of nutrients and bioactive substances. In the case of the genus Ulva, the effect of its inclusion in feed on the growth, tissues composition and the stress response of some fish species has been evaluated. However, it is necessary to evaluate the effects on the immune system, as its inclusion could modify the resistance to pathogens. In the present work, we have studied the response of the immune system at lysozyme expression level, cellular immune response genes, and inflammatory response genes in mucous of skin and sole gills (Solea senegalensis) fed with a diet supplemented in Ulva ohnoi (5%). Results obtained indicate absence of significant changes in the expression of assayed immune genes.
Resumen
En la búsqueda de la mejora de la alimentación de los peces en la acuicultura, las algas se presentan como una prometedora fuente de nutrientes y sustancias bioactivas. En el caso del género Ulva, se ha publicado el efecto de su inclusión en piensos sobre el crecimiento, la composición de los tejidos y la respuesta al estrés de algunas especies de interés acuícola. Sin embargo, es necesario evaluar los efectos en el sistema inmune, puesto que su inclusión podría modificar la resistencia a patógenos. En el presente trabajo se ha estudiado la respuesta del sistema inmune a nivel de expresión de lisozimas, genes de respuesta inmune celular, y genes de la respuesta inflamatoria en piel y branquias de lenguados (Solea senegalensis) alimentados con una dieta suplementada en Ulva ohnoi (5%). Influence of the Cumulative Incidence of COVID-19 Cases on the Mental Health of the Spanish Out-of-Hospital Professionals
This study aimed to analyze the psychological affectation of health professionals (HPs) of
Spanish Emergency Medical Services (EMSs) according to the cumulative incidence (CI) of COVID19 cases in the regions in which they worked. A cross-sectional descriptive study was designed,
including all HPs working in any EMS of the Spanish geography between 1 February 2021 and 30 April
2021. Their level of stress, anxiety and depression (DASS-21) and the perception of self-efficacy (GSES) were the study’s main results. A 2-factor analysis of covariance was used to determine if the CI
regions of COVID-19 cases determined the psychological impact on each of the studied variables. A
total of 1710 HPs were included. A third presented psychological impairment classified as severe.
The interaction of CI regions with the studied variables did not influence their levels of stress, anxiety,
depression or self-efficacy. Women, younger HPs or those with less EMS work experience, emergency
medical technicians (EMT), workers who had to modify their working conditions or those who lived
with minors or dependents suffered a greater impact from the COVID-19 pandemic in certain regions.
These HPs have shown high levels of stress, anxiety, depression and medium levels of self-efficacy,
with similar data in the different geographical areas. Psychological support is essential to mitigate

Characteristics and outcomes of adult patients in the PETHEMA registry with relapsed or refractory FLT3-ITD mutation-positive acute myeloid leukemia
This retrospective study investigated outcomes of 404 patients with relapsed/refractory (R/R) FMS-like tyrosine kinase 3 (FLT3)-internal tandem duplication (ITD) acute myeloid leukemia (AML) enrolled in the PETHEMA registry, pre-approval of tyrosine kinase inhibitors. Most patients (63%) had received first-line intensive therapy with 3 + 7. Subsequently, patients received salvage with intensive therapy (n = 261), non-intensive therapy (n = 63) or supportive care only (n = 80). Active salvage therapy (i.e., intensive or non-intensive therapy) resulted in a complete remission (CR) or CR without hematological recovery (CRi) rate of 42%. More patients achieved a CR/CRi with intensive (48%) compared with non-intensive (19%) salvage therapy (p < 0.001). In the overall population, median overall survival (OS) was 5.5 months; 1- and 5-year OS rates were 25% and 7%. OS was significantly (p < 0.001) prolonged with intensive or non-intensive salvage therapy compared with supportive therapy, and in those achieving CR/CRi versus no responders. Of 280 evaluable patients, 61 (22%) had an allogeneic stem-cell transplant after they had achieved CR/CRi. In conclusion, in this large cohort study, salvage treatment approaches for patients with FLT3-ITD mutated R/R AML were heterogeneous. Median OS was poor with both non-intensive and intensive salvage therapy, with best long-term outcomes obtained in patients who achieved CR/CRi and subsequently underwent allogeneic stem-cell transplant.This study was supported by Centro de Investigación Biomédica en Red Cáncer (CIBERONC), Valencia, Spain [CB16/12/00284]

The aim of our study is to assess the relationship between fruit and vegetable (F&V) consumption and plasma FAs and
their subtypes.
Methods and Results: Plasma FAs are assessed in a cross-sectional analysis of a subsample of 240 subjects from the
PREDIMED-Plus study. Participants are categorized into four groups of fruit, vegetable, and fat intake according to the
food frequency questionnaire. Plasma FA analysis is performed using gas chromatography. Associations between FAs
and F&V consumption are adjusted for age, sex, physical activity, bodymass index (BMI), total energy intake, and alcohol
consumption. Plasma saturated FAs are lower in groups with high F&V consumption (-1.20 mg cL−1 [95% CI: [-2.22, -
0.18], p-value = 0.021), especially when fat intake is high (-1.74 mg cL−1 [95% CI: [-3.41, -0.06], p-value = 0.042). Total
FAs and n-6 polyunsaturated FAs tend to be lower in high consumers of F&V only in the high-fat intake groups.
Conclusions: F&V consumption is associated with lower plasma saturated FAs when fat intake is high. These findings
suggest that F&V consumption may have different associations with plasma FAs depending on their subtype and on the

Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)
This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes. Role of age and comorbidities in mortality of patients with infective endocarditis
[Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality.
[Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk.
[Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality.
[Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group
CIBERER : Spanish national network for research on rare diseases: A highly productive collaborative initiative

This innovative research structure comprises 11 different specific areas dedicated to the main public health priorities in the National Health System. CIBERER, the thematic area of CIBER focused on rare diseases (RDs) currently consists of 75 research groups belonging to universities, research centers, and hospitals of the entire country. CIBERER's mission is to be a center prioritizing and favoring collaboration and cooperation between biomedical and clinical research groups, with special emphasis on the aspects of genetic, molecular, biochemical, and cellular research of RDs. This research is the basis for providing new tools for the diagnosis and therapy of low-prevalence diseases, in line with the International Rare Diseases Research Consortium (IRDiRC) objectives, thus favoring translational research between the scientific environment of the laboratory and the clinical setting of health centers. In this article, we intend to review CIBERER's 15-year journey and summarize the main results obtained in terms of internationalization, scientific production, contributions toward the discovery of new therapies and novel genes associated to diseases, cooperation with patients' associations and many other topics related to RD research
