The proposal of workshops it had been elaborated thinking about the adolescents and in your interests with respect to the topic of the process of the communication and specially the assertive that permit establish you appropriate interpersonal relations, the satisfaction with the life and the self-confidence.La evolución del pensamiento, la realidad sociocultural actual y su interdependencia comunicativa abren paso a nuevas y sugestivas perspectivas de observación y explicación de los hechos de la comunicación.  Los adolescentes interesados en dominar nuevas estrategias para lograr una mejor comunicación asertiva que les permitan construir relaciones afectivas entre ellos, y fomentar el desarrollo de la comunicación como expresión y símbolo de la identidad cultural. La orientación para el desarrollo de la comunicación asertiva en los adolescentes requiere de diversas herramientas que le garanticen un cambio totalmente ascendente. De modo que la presente investigación declara como objetivo: elaborar talleres de orientación para favorecer la comunicación asertiva en adolescentes del 9no grado de la ESBU Mario Carbó Calzada. La propuesta de talleres se ha elaborado pensando en los adolescentes y en sus intereses con respecto al tema del proceso de la comunicación y en particular la asertiva que les permitan establecer adecuadas relaciones interpersonales, la satisfacción con la vida y la confianza en sí mismo

    Antitumor Necrosis Factor Agents to Treat EndoscopicPostoperative Recurrence of Crohn’s Disease: A Nationwide Study With Propensity-Matched Score Analysis

    INTRODUCTION:Patients with Crohn's disease experiencing endoscopic postoperative recurrence (POR) may benefit from antitumor necrosis factor (TNF) agents but scarce data on this are available. Our aim was to assess the efficacy of anti-TNF in improving mucosal lesions in patients with endoscopic POR.METHODS:Multicenter, retrospective, study of patients with Crohn's disease who underwent therapy with anti-TNF agents for endoscopic POR (Rutgeerts score > i1). Treatment outcomes were assessed by the findings in the last ileocolonoscopy performed after anti-TNF therapy was initiated. Endoscopic improvement and remission were defined as any reduction in the baseline Rutgeerts score and by a Rutgeerts score < i2, respectively.RESULTS:A total of 179 patients were included, 83 were treated with infliximab and 96 with adalimumab. Median time on anti-TNF therapy at the last endoscopic assessment was 31 months (interquartile range, 13-54). Endoscopic improvement was observed in 61%, including 42% who achieved endoscopic remission. Concomitant use of thiopurines and treatment with infliximab were associated with endoscopic improvement (odds ratio [OR] 2.15, 95% confidence interval [CI] 1.04-4.46; P = 0.03, and OR 2.34, 95% CI 1.18-4.62; P < 0.01, respectively) and endoscopic remission (OR 3.16, 95% CI 1.65-6.05; P < 0.01, and OR 2.01, 95% CI 1.05-3.88; P = 0.04, respectively) in the multivariable logistic regression analysis. These results were confirmed in a propensity-matched score analysis.DISCUSSION:In patients with endoscopic POR, anti-TNF agents improve mucosal lesions in almost two-thirds of the patients. Aparecen en las investigaciones no solo respuestas y experiencias, sino nuevos planteamientos para una perspectiva futura, necesarios en las transformaciones para las próximas décadas que deberán vivirse en los planes y programas de estudio, los sistemas de evaluación, la formación docente, el perfil y las competencias digitales de los estudiantes, entre otros aspectos.     Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Determination of phenols, flavones, and lignans in virgin olive oils by solid-phase extraction and high-performance liquid chromatography with diode array ultraviolet detection

    A simple analytical method for the quantitative determination of phenols, flavones, and lignans in virgin olive oils was developed. The polar fraction was isolated from small amounts of oil sample (2.5 g) by solid-phase extraction (SPE) using diol-phase cartridges, and the extract was analyzed by reversed-phase HPLC coupled with diode array UV detection. Chromatographic separation of pinoresinol, cinnamic acid, and 1-acetoxypinoresinol was achieved. Repeatability (RSD 90%), and response factors for each identified component were determined. SPE on amino-phase cartridges was used for isolating acidic phenols and as an aid for phenol identification. For the first time, 2-(4-hydroxyphenyl)ethyl acetate was detected in olive oils. The aldehydic structure of the ligstroside aglycon was confirmed by NMR spectroscopy. The colorimetric determination of total o-diphenolic compounds by reaction with molybdate was consistent with their HPLC determination. Differences between results obtained by liquid−liquid extraction and SPE were not statistically significant.Peer reviewe

    Assessment of the Technique of Breastfeeding in Babies with Down Syndrome

    Valoración de la técnica de amamantamiento en bebés con síndrome de DownAvaliação da técnica de amamentação em bebês com síndrome de DownObjective: To evaluate the technique and duration of breastfeeding in healthy children and children with Down Syndrome (DS) using the breastfeeding observation form. Materials and methods: An observational study of a prospective cohort was carried out at the Clinical Hospital of Granada during 2015. The Study Group consisted of 40 children with DS and the control group was formed by each new-born with DS and a healthy new-born with the same characteristics of weight and gestational age was selected. The new-borns evaluated shared housing with the mother where the technique was valued during the first 5 days postpartum by a health professional. A bivariable analysis was performed to compare the groups using Student’s “T” test for numerical variables and chi-square for categorical variables. Results: Lactogenesis onset was earlier in the DS group (92.5 % in the first 24 hours vs 20 %; p &lt;0.001). It was observed that 60 % of the healthy children were breastfed for more than three months while in the group of babies with DS this time period was 47.5 %. Resultados: la lactogénesis se produjo primero en el grupo sin SD (92,5 % en las primeras 24 horas vs 20 %; p&lt;0,001). Se observó que el 60 % de los niños sanos mantuvieron la LM por más de tres meses, mientras que el grupo de bebés con SD logró en este tiempo el 47,5%. Conclusiones: la LM presentó, al inicio, más dificultades en las madres de niños con SD. Resultados: a lactogênese ocorreu primeiramente no grupo sem SD (92,5 % nas primeiras 24 horas vs 20 %; p &lt;0,001). Tornou-se evidente que 60 % das crianças saudáveis mantiveram a amamentação por mais de três meses, enquanto no grupo de bebês com SD, esse tempo foi de 47,5 %. Method: Randomized clinical trial, with an intervention group and a control group. Forty-three patients were operated of bariatric surgery. The intervention group was applied a program of moderate physical activity during six months. Grip strength and bioimpedance were measured at their baseline form, at one month, and six months after surgery. A bivariate study was conducted to observe variable changes. Results: Upon conducting Student´s t for independent samples in all the variables (weight, body mass index, grip strength, lean mass, fat mass, and metabolism), the result is that no significant differences exist between the intervention group and the control group with p &gt; 0.05. Materiais e método: ensaio clínico randomizado, com um grupo de intervenção e um grupo controle. Quarenta e três pacientes foram operados de cirurgia bariátrica. Ao grupo de intervenção, foi aplicado um programa de atividade física moderada durante seis meses. A força muscular e a bioimpedância foram medidas em sua forma basal em um mês e aos seis meses depois da cirurgia. Foi realizado um estudo de bivariante para observar as mudanças das variáveis. Resultados: ao realizar o teste t de Student para amostras independentes em todas as variáveis (peso, índice de massa corporal, força muscular, massa magra, massa gordurosa e metabolismo), não se verificam diferenças significativas entre o grupo de intervenção e o grupo controle com p &gt; 0,05.
