Children’s and adults’ use of fictional discourse and semantic knowledge for prediction in language processing
Using real-time eye-movement measures, we asked how a fantastical discourse context competes with stored representations of real-world events to influence the moment-by-moment interpretation of a story by 7-year-old children and adults. Seven-year-olds were less effective at bypassing stored real-world knowledge during real-time interpretation than adults. Our results suggest that children privilege stored semantic knowledge over situation-specific information presented in a fictional story context. We suggest that 7-year-olds’ canonical semantic and conceptual relations are sufficiently strongly rooted in statistical patterns in language that have consolidated over time that they overwhelm new and unexpected information even when the latter is fantastical and highly salient

Effect of monthly vitamin D3 supplementation in healthy adults on adverse effects of earthquakes: randomised controlled trial
Objective: To determine whether supplementation with vitamin D improves resilience to the adverse effects of earthquakes. Design: Opportunistic addition to an established randomised double blind placebo controlled trial. Setting: Christchurch, New Zealand, where a prolonged series of catastrophic earthquakes beginning on 4 September 2010 occurred, which caused widespread destruction, fatalities, and extensive psychological damage. Participants: 322 healthy adults (241 women; 81 men) aged 18-67 who were already participating in the vitamin D and acute respiratory infections study (VIDARIS) between February 2010 and November 2011. Intervention Participants were randomised to receive an oral dose of either 200 000 IU vitamin D3 monthly for two months then 100 000 IU monthly (n=161) or placebo (n=161) for a total of 18 months. Main outcome measure This is a post hoc analysis from the previously published VIDARIS trial. The primary endpoint in the current analysis was the self reported effects and overall adverse impact of the Christchurch earthquakes as assessed by questionnaire four months after the most destructive earthquake on 22 February 2011, which was used as the index event. The secondary end point was the number of “psychological” adverse events that participants reported at their usual monthly appointments as part of the original VIDARIS trial. Results: 308 participants completed the earthquake impact questionnaire (n=152 in the vitamin D group and 156 in the placebo group). There was no significant difference in the number of self reported adverse effects between those receiving vitamin D supplementation and those receiving placebo. There was also no difference in the overall adverse impact score between treatment groups (χ2 P=0.44). The exception was that those in the vitamin D group experienced more adverse effects on family relationships (22% v 13%; χ2 P=0.03). The number of psychological adverse events—such as fatigue, stress, anxiety, and insomnia—that participants reported at their usual monthly appointments was significantly higher after the earthquake (χ2 P=0.007) but did not differ between treatment groups. Conclusion: In this trial, vitamin D supplementation did not reduce the adverse impact of earthquakes in healthy adults. Nutrient enrichment effects are conditional on upstream nutrient concentrations: Implications for bioassessment in multi-use catchments.
Human impacts on stream ecosystems are expected to intensify with population growth and climate change. Decisive information on how stream communities respond to cumulative human impacts is therefore integral for protecting streams draining multi-use catchments. To determine cumulative influences of nutrient enrichment and assess more nuanced approaches for the evaluation of human impacts, we present results from one factorial and two gradient assessment designs applied to benthic algae and macroinvertebrate data from 14 mid-order streams in southern Ontario, Canada with pre-existing human impacts (i.e., sewage effluent and agriculture). We found that among stream variability in ecological indicators measured downstream of sewage effluent outfalls confounded our generalized factorial assessment and provided inconclusive information on a known human impact. Despite our gradient assessment also not having strong statistical support, accounting for the extent of nutrient enrichment associated with differences in sewage effluent and agricultural inputs revealed that larger longitudinal changes in stream communities were associated with increased nutrient enrichment. However, re-weighting our nutrient enrichment gradient based on upstream nutrient concentrations to account for nonlinearities in the response of stream communities to nutrient enrichment produced more robust assessment results that were consistent with predicted effects of nutrients on stream ecosystems. Thus, while our factorial assessment suggests that the communities are resistant to nutrients from cumulative human impacts, our targeted gradient assessment demonstrates that the effects of nutrient enrichment are highly conditional on upstream ecosystem conditions. Future assessments may need to go beyond traditional approaches (i.e., impact presence/absence) and more explicitly consider the environmental stressors and their associated complexities related to the impact under investigation
World distribution, diversity and endemism of aquatic macrophytes
To test the hitherto generally-accepted hypothesis that most aquatic macrophytes have broad world distributions, we investigated the global distribution, diversity and endemism patterns of 3457 macrophyte species that occur in permanent, temporary or ephemeral inland freshwater and brackish waterbodies worldwide. At a resolution of 10 × 10° latitude x longitude, most macrophyte species were found to have narrow global distributions: 78% have ranges (measured using an approach broadly following the IUCN-defined concept “extent of occurrence”) that individually occupy <10% of the world area present within the six global ecozones which primarily provide habitat for macrophytes. We found evidence of non-linear relationships between latitude and macrophyte α- and γ-diversity, with diversity highest in sub-tropical to low tropical latitudes, declining slightly towards the Equator, and also declining strongly towards higher latitudes. Landscape aridity and, to a lesser extent, altitude and land area present per gridcell also influence macrophyte diversity and species assemblage worldwide. The Neotropics and Orient have the richest ecozone species-pools for macrophytes, depending on γ-diversity metric used. The region around Brasilia/Goiás (Brazil: gridcell 10–20 °S; 40–50 °W) is the richest global hotspot for macrophyte α-diversity (total species α-diversity, ST: 625 species/gridcell, 350 of them Neotropical endemics). In contrast, the Sahara/Arabian Deserts, and some Arctic areas, have the lowest macrophyte α-diversity (ST <20 species/gridcell). At ecozone scale, macrophyte species endemism is pronounced, though with a>5-fold difference between the most species-rich (Neotropics) and species-poor (Palaearctic) ecozones. Our findings strongly support the assertion that small-ranged species constitute most of Earth’s species diversity
Uncovering treatment burden as a key concept for stroke care: a systematic review of qualitative research
<b>Background</b> Patients with chronic disease may experience complicated management plans requiring significant personal investment. This has been termed ‘treatment burden’ and has been associated with unfavourable outcomes. The aim of this systematic review is to examine the qualitative literature on treatment burden in stroke from the patient perspective.<p></p>
<b>Methods and findings</b> The search strategy centred on: stroke, treatment burden, patient experience, and qualitative methods. We searched: Scopus, CINAHL, Embase, Medline, and PsycINFO. We tracked references, footnotes, and citations. Restrictions included: English language, date of publication January 2000 until February 2013. Two reviewers independently carried out the following: paper screening, data extraction, and data analysis. Data were analysed using framework synthesis, as informed by Normalization Process Theory. Sixty-nine papers were included. Treatment burden includes: (1) making sense of stroke management and planning care, (2) interacting with others, (3) enacting management strategies, and (4) reflecting on management. Health care is fragmented, with poor communication between patient and health care providers. Patients report inadequate information provision. Inpatient care is unsatisfactory, with a perceived lack of empathy from professionals and a shortage of stimulating activities on the ward. Discharge services are poorly coordinated, and accessing health and social care in the community is difficult. The study has potential limitations because it was restricted to studies published in English only and data from low-income countries were scarce.<p></p>
<b>Conclusions</b> Stroke management is extremely demanding for patients, and treatment burden is influenced by micro and macro organisation of health services. Knowledge deficits mean patients are ill equipped to organise their care and develop coping strategies, making adherence less likely. There is a need to transform the approach to care provision so that services are configured to prioritise patient needs rather than those of health care systems
Genetic association study of QT interval highlights role for calcium signaling pathways in myocardial repolarization.
The QT interval, an electrocardiographic measure reflecting myocardial repolarization, is a heritable trait. QT prolongation is a risk factor for ventricular arrhythmias and sudden cardiac death (SCD) and could indicate the presence of the potentially lethal mendelian long-QT syndrome (LQTS). Using a genome-wide association and replication study in up to 100,000 individuals, we identified 35 common variant loci associated with QT interval that collectively explain ∼8-10% of QT-interval variation and highlight the importance of calcium regulation in myocardial repolarization. Rare variant analysis of 6 new QT interval-associated loci in 298 unrelated probands with LQTS identified coding variants not found in controls but of uncertain causality and therefore requiring validation. Several newly identified loci encode proteins that physically interact with other recognized repolarization proteins. Our integration of common variant association, expression and orthogonal protein-protein interaction screens provides new insights into cardiac electrophysiology and identifies new candidate genes for ventricular arrhythmias, LQTS and SCD
Community-Centered Responses to Ebola in Urban Liberia: The View from Below
The West African Ebola epidemic has demonstrated that the existing range of medical and epidemiological responses to emerging disease outbreaks is insufficient, especially in post-conflict contexts with exceedingly poor healthcare infrastructures. This study provides baseline information on community-based epidemic control priorities and identifies innovative local strategies for containing EVD in Liberia.In this study the authors analyzed data from the 2014 Ebola outbreak in Monrovia and Montserrado County, Liberia. The data were collected for the purposes of program design and evaluation by the World Health Organization (WHO) and the Government of Liberia (GOL), in order to identify: (1) local knowledge about EVD, (2) local responses to the outbreak, and (3) community based innovations to contain the virus. At the time of data collection, the international Ebola response had little insight into how much local Liberian communities knew about Ebola, and how communities managed the epidemic when they could not get access to care due to widespread hospital and clinic closures. Methods included 15 focus group discussions with community leaders from areas with active Ebola cases. Participants were asked about best practices and what they were currently doing to manage EVD in their respective communities, with the goal of developing conceptual models of local responses informed by local narratives. Findings reveal that communities responded to the outbreak in numerous ways that both supported and discouraged formal efforts to contain the spread of the disease. This research will inform global health policy for both this, and future, epidemic and pandemic responses
Global variation in the beta diversity of lake macrophytes is driven by environmental heterogeneity rather than latitude
Aim: We studied global variation in beta diversity patterns of lake macrophytes using regional data from across the world. Specifically, we examined 1) how beta diversity of aquatic macrophytes is partitioned between species turnover and nestedness within each study region, and 2) which environmental characteristics structure variation in these beta diversity components. Location: Global Methods: We used presence-absence data for aquatic macrophytes from 21 regions distributed around the world. We calculated pairwise-site and multiple-site beta diversity among lakes within each region using Sørensen dissimilarity index and partitioned it into turnover and nestedness coefficients. Beta regression was used to correlate the diversity coefficients with regional environmental characteristics. Results: Aquatic macrophytes showed different levels of beta diversity within each of the 21 study regions, with species turnover typically accounting for the majority of beta diversity, especially in high-diversity regions. However, nestedness contributed 30-50% of total variation in macrophyte beta diversity in low-diversity regions. The most important environmental factor explaining the three beta diversity coefficients (total, species turnover and nestedness) was altitudinal range, followed by relative areal extent of freshwater, latitude and water alkalinity range. Main conclusions: Our findings show that global patterns in beta diversity of lake macrophytes are caused by species turnover rather than by nestedness. These patterns in beta diversity were driven by natural environmental heterogeneity, notably variability in altitudinal range (also related to temperature variation) among regions. In addition, a greater range in alkalinity within a region, likely amplified by human activities, was also correlated with increased macrophyte beta diversity. These findings suggest that efforts to conserve aquatic macrophyte diversity should primarily focus on regions with large numbers of lakes that exhibit broad environmental gradients.
