    Paroxysmal nocturnal hemoglobinuria in the differential diagnosis of thrombocytopenia

    Paroxysmal nocturnal hemoglobinuria (PNH) is a disease which diagnosis may be delayed due to variable clinical findings. We describe herein a case of PNH in a 21 year old woman who admitted with complaints of chronic weakness, intermittent spontaneous ecchymoses, and an intermittent abdominal pain. On laboratory tests thrombocytopenia and iron deficiency anemia without any clinical findings were found. Flow cytometric evaluations showed a PNH clone of 15% for erythrocytes, 64% for monocytes, and 60% for granulocytes. The patient was diagnosed with PNH and an eculizumab therapy was initiated. Following initiation of eculizumab therapy, the frequency of abdominal pain attacks decreased, hemoglobin level normalized, and platelet values increased slightly. In patients submitting with a triad of symptoms such as thrombocytopenia, iron deficiency anemia, and abdominal pain attacks of unknown etiology we suggest considering PNH. We also encourage physicians to share their similar observations in order to raise the knowledge on infrequent presentations of PNH

    Eruptive xanthomas are benign lesions which are found in extensor surfaces of the extremities and are often associated with hypertriglyceridemia and/or uncontrolled diabetes. In this case report, we present a patient with hypertriglyceridemia, type 2 diabetes mellitus and eruptive xanthomas who recovers fully after treatment. A 37-year-old male patient presented to our clinic with reddish yellow lesions on his elbows. His body mass index was 30 kg/m(2) and his laboratory results showed high serum triglyceride and glucose levels (triglyceride 6548 mg/dL, glucose 245 mg/dL), his hemoglobin A1c was 11.2%. Although eruptive xanthomas have a benign nature, they are associated with disease which often need lifelong treatment

    The proximate composition of male and female Leuciscus lepidus in Beyşehir Lake was investigated. The fatty acid profiles of total lipid, phospholipid, and triacylglycerol in muscle and liver of male and female L. lepidus were evaluated by gas chromatography. Proximate analyses showed that meat of male and female L. lepidus had 15.13 ± 0.04 and 18.75 ± 0.11% fat, 20.42 ± 0.45 and 22.21 ± 0.56% protein, 65.47 ± 1.37 and 61.28 ± 1.03% moisture, and 1.51 ± 0.05 and 1.50 ± 0.03% ash, respectively. The percentage of total saturated fatty acids was higher in liver than in muscle, whereas the total polyunsaturated fatty acid (PUFA) content was the lowest in all fatty acid profiles. The phospholipids contained more PUFAs than triacylglycerol. Analysis of variance indicated significant differences ( < 0.05) between male (47.51%) and female (49.98%) muscle PUFAs in total lipid. The proportion of omega 3 ( 3) to omega 6 ( 6) fatty acids of total lipid was 3.15 in male and 3.68 in female. The ratio is an important indicator for comparing the value of fish oil. Therefore, it was concluded that L. lepidus was considered to be a high quality product for healthy food choice. Additionally, female L. lepidus may especially be used to produce fish oil supplements from freshwater fish combined with vegetable oils

An experimental study was performed to evaluate the effect of PRF on growth factor levels in urethral repair. Materials and Methods: Eighteen Wistar albino rats were included in the study. Rats were allocated in three groups (n:6): control (CG), sham (SG), and PRF (PRFG). In SG, a 5 mm vertical incision was performed in the penile urethra and repaired with 10/0 Vicryl (R) under a microscope. In PRFG, during the urethral repair as described in SG, 1 cc of blood was sampled from each rat and centrifuged for 10 minutes at 2400 rpm. PRF obtained from the centrifugation was placed on the repair site during closure. Penile urethras were sampled 24 hours after PRF application in PRFG and after urethral repair in SG. Transforming growth factor beta receptor (TGF-beta-R-CD105), vascular endothelial growth factor (VEGF) and its receptor (VEGF-R), as well as endothelial growth factor receptor (EGFR), were evaluated in subepithelia of the penile skin and urethra. Groups were compared for growth factor levels and growth factor receptor expression with the Kruskal Wallis test. Results: TGF-beta-R levels were significantly decreased in SG when compared to CG (p 0.05). Conclusion: Use of PRF after urethral repair increases TGF-beta-R and VEGF expressions in urethral tissue. PRF can be considered as an alternative measure to improve the success of urethral repair. (C) 2013 Elsevier Inc.     Caffeic acid phenethyl ester (CAPE), an active component of propolis, has been attracting the attention of different medical and pharmaceutical disciplines in recent years because of its antioxidant, anti-inflammatory, antiproliferative, cytotoxic, antiviral, antifungal, and antineoplastic properties. One of the most studied organs for the effects of CAPE is the kidney, particularly in the capacity of this ester to decrease the nephrotoxicity induced by several drugs and the oxidative injury after ischemia/reperfusion (I/R).
