The effect of malnutrition on patients undergoing elective joint arthroplasty.
Malnutrition has been linked to serious complications in patients undergoing elective total joint arthroplasty (TJA). This study prospectively evaluated 2,161 patients undergoing elective TJA for malnutrition as defined by either an abnormal serum albumin or transferrin. The overall incidence of malnutrition was 8.5% (184 of 2,161) and the rate of overall complications in the malnourished group was 12% as compared to 2.9% in patients with normal parameters (P55years-old undergoing TJA and is associated with a significant increase in post-operative complications

DLocalMotif: a discriminative approach for discovering local motifs in protein sequences
Motivation: Local motifs are patterns of DNA or protein sequences that occur within a sequence interval relative to a biologically defined anchor or landmark. Current protein motif discovery methods do not adequately consider such constraints to identify biologically significant motifs that are only weakly over-represented but spatially confined. Using negatives, i.e. sequences known to not contain a local motif, can further increase the specificity of their discovery
The concurrent validity of a rugby-specific Yo-Yo Intermittent Recovery Test (Level 1) for assessing match-related running performance

The concurrent validity of a rugby-specific Yo-Yo Intermittent Recovery Test (Level 1) for assessing match-related running performance. Journal of Strength and Conditioning Research, 35(1), 176-182. http://doi.org/10.1519/JSC.0000000000002621This study investigated the concurrent validity of a rugby-specific high-intensity intermittent running test (HIIR) against the internal, external and perceptual responses to simulated match-play. Thirty-six rugby league players (age 18.5 ± 1.8 years; stature 181.4 ± 7.6 cm; body mass 83.5 ± 9.8 kg) completed the prone Yo-Yo IR1, of which sixteen also completed the Yo-Yo IR1, and 2 x ~20 min bouts of a simulated match-play (RLMSP-i). Most likely reductions in relative total, low-speed and high-speed distance, mean speed and time above 20 W·kg-1 (HMP) were observed between bouts of the RLMSP-i. Likewise, rating of perceived exertion (RPE) and percentage of peak heart rate (%HRpeak) were very likely and likely higher during the second bout. Pearsonâs correlations revealed a large relationship for the change in relative distance (r = 0.57-0.61) between bouts with both Yo-Yo IR1 tests. The prone Yo-Yo IR1 was more strongly related to the RLMSP-i for change in repeated sprint speed (r = 0.78 cf. 0.56), mean speed (r = 0.64 cf. 0.36), HMP (r = 0.48 cf. 0.25), fatigue index (r = 0.71 cf. 0.63), %HRpeak (r = -0.56 cf. -0.35), RPEbout1 (r = -0.44 cf. -0.14), and RPEbout2 (r = -0.68 cf. -0.41) than the Yo-Yo IR1, but not for blood lactate concentration (r = -0.20 to -0.28 cf. -0.35 to -0.49). The relationships between prone Yo-Yo IR1 distance and measure of load during the RLMSP-i suggests it possesses concurrent validity and is more strongly associated with measures of training or match load than the Yo-Yo IR1 using rugby league players
Towards a common methodology for developing logistic tree mortality models based on ring-width data
Tree mortality is a key process shaping forest dynamics. Thus, there is a growing need for indicators of the likelihood of tree death. During the last decades, an increasing number of tree-ring based studies have aimed to derive growth-mortality functions, mostly using logistic models. The results of these studies, however, are difficult to compare and synthesize due to the diversity of approaches used for the sampling strategy (number and characteristics of âaliveâ and âdeathâ observations), the type of explanatory growth variables included (level, trend, etc.), and the length of the time-window (number of years preceding the alive/death observation) that maximized the discrimination ability of each growth variable.
Here, we assess the implications of key methodological decisions when developing tree-ring based growth-mortality relationships using logistic mixed-effects regression models. As examples we use published tree-ring datasets from Abies alba (13 different sites), Nothofagus dombeyi (one site) and Quercus petraea (one site). Our approach is based on a constant sampling size and aims at (1) assessing the dependency of growth-mortality relationships on the statistical sampling scheme used; (2) determining the type of explanatory growth variables that should be considered; and (3) identifying the best length of the time window used to calculate them.
The performance of tree-ring based mortality models was reasonably high for all three species (Area Under the receiving operator characteristics Curve: AUC > 0.7). Growth level variables were the most important predictors of mortality probability for two species (A. alba, N. dombeyi), while growth-trend variables need to be considered for Q. petraea. In addition, the length of the time window used to calculate each growth variable was highly uncertain and depended on the sampling scheme, as some growth-mortality relationships varied with tree age.
The present study accounts for the main sampling-related biases to determine reliable species-specific growth-mortality relationships. Our results highlight the importance of using a sampling strategy that is consistent with the research question. Moving towards a common methodology for developing reliable growth-mortality relationships is an important step towards improving our understanding of tree mortality across species and its representation in dynamic vegetation models
Choice-Disability and HIV Infection: A Cross Sectional Study of HIV Status in Botswana, Namibia and Swaziland
Interpersonal power gradients may prevent people implementing HIV prevention decisions. Among 7,464 youth aged 15â29Â years in Botswana, Namibia and Swaziland we documented indicators of choice-disability (low education, educational disparity with partner, experience of sexual violence, experience of intimate partner violence (IPV), poverty, partner income disparity, willingness to have sex without a condom despite believing partner at risk of HIV), and risk behaviours like inconsistent use of condoms and multiple partners. In Botswana, Namibia and Swaziland, 22.9, 9.1, and 26.1% women, and 8.3, 2.8, and 9.3% men, were HIV positive. Among both women and men, experience of IPV, IPV interacted with age, and partner income disparity interacted with age were associated with HIV positivity in multivariate analysis. Additional factors were low education (for women) and poverty (for men). Choice disability may be an important driver of the AIDS epidemic. New strategies are needed that favour the choice-disabled
The effect of a supplementary ('Gist-based') information leaflet on colorectal cancer knowledge and screening intention: a randomized controlled trial.
Guided by Fuzzy Trace Theory, this study examined the impact of a 'Gist-based' leaflet on colorectal cancer screening knowledge and intentions; and tested the interaction with participants' numerical ability. Adults aged 45-59 years from four UK general practices were randomly assigned to receive standard information ('The Facts', n = 2,216) versus standard information plus 'The Gist' leaflet (Gist + Facts, n = 2,236). Questionnaires were returned by 964/4,452 individuals (22 %). 82 % of respondents reported having read the information, but those with poor numeracy were less likely (74 vs. 88 %, p < .001). The 'Gist + Facts' group were more likely to reach the criterion for adequate knowledge (95 vs. 91 %; p < .01), but this was not moderated by numeracy. Most respondents (98 %) intended to participate in screening, with no group differences and no interaction with numeracy. The improved levels of knowledge and self-reported reading suggest 'The Gist' leaflet may increase engagement with colorectal cancer screening, but ceiling effects reduced the likelihood that screening intentions would be affected
