    Sulfassalazine use in Treatment of Chronic Idiopathic Urticaria – Experience of an Outpatient Immunoallergy Clinic

    Os autores apresentam seis casos clínicos de doentes com urticária crónica idiopática (UCI) que não respondiam à terapêutica anti-histamínica convencional, necessitando de corticoterapia sistémica frequente para controlar as queixas. Após várias tentativas com outras terapêuticas, foram medicados com sulfassalazina. Efectuou-se uma análise deste grupo de doentes, procurando estabelecer uma avaliação do risco-benefício para a utilização da sulfassalazina na UCI, concluindo-se favoravelmente da sua utilização.     Frequência da Anafilaxia Induzida pelo Exercício numa Consulta de Imunoalergologia

    Introdução: A anafilaxia induzida pelo exercício (AIE) é uma forma rara de alergia física que ocorre na sequência de esforços físicos. A verdadeira incidência e prevalência da AIE permanecem por esclarecer, não existindo até à data dados publicados a nível nacional. Objectivos: Estimar a frequência da AIE no ambulatório de um serviço de Imunoalergologia e incrementar o conhecimento em relação a esta patologia. Métodos: De 7699 doentes observados na consulta de Imunoalergologia durante o período de um ano, incluímos os correspondentes a quadros de anafilaxia notificados pelo corpo clínico (“pelo menos um episódio de reacção sistémica grave”). Resultados: A AIE foi reportada em 5 de 103 doentes com história de anafilaxia; correspondendo a uma frequência de 0,06% na população observada na consulta. A média etária destes doentes era de 20,2 ± 10,3 anos (entre 10 e 37 anos) e a distribuição por sexo masculino/feminino de 4:1. Todos tinham história pessoal de atopia e de rinite alérgica; dois doentes (40%) tinham asma. As actividades desencadeantes das crises foram a corrida, o futebol, a natação e a dança. Todos os doentes tinham sintomas com o exercício dependente da ingestão prévia de alimentos: cereais em três doentes (trigo – dois, cevada – um), leguminosas em dois (amendoim – um, feijão -frade e feijão -verde – um); com teste cutâneo por picada positivo para os referidos alimentos. Conclusões: A AIE representa 5% dos casos de anafi laxia reportados. Todos os casos identifi cados apresentavam AIE dependente de alimentos, encontrando-se os doentes controlados com a evicção dos alimentos referidos 6 horas antes da prática de exercício e sendo portadores de dispositivo para autoadministração de adrenalina

    Moving from information and collaboration to action: report from the 3rd International Dog Health Workshop, Paris in April 2017

    Abstract Background Breed-related health problems in dogs have received increased focus over the last decade. Responsibility for causing and/or solving these problems has been variously directed towards dog breeders and kennel clubs, the veterinary profession, welfare scientists, owners, regulators, insurance companies and the media. In reality, all these stakeholders are likely to share some responsibility and optimal progress on resolving these challenges requires all key stakeholders to work together. The International Partnership for Dogs (IPFD), together with an alternating host organization, holds biennial meetings called the International Dog Health Workshops (IDHW). The Société Centrale Canine (French Kennel Club) hosted the 3rd IDHW, in Paris, in April, 2017. These meetings bring together a wide range of stakeholders in dog health, science and welfare to improve international sharing of information and resources, to provide a forum for ongoing collaboration, and to identify specific needs and actions to improve health, well-being and welfare in dogs. Results The workshop included 140 participants from 23 countries and was structured around six important issues facing those who work to improve dog health. These included individualized breed-specific strategies for health and breeding, extreme conformations, education and communication in relation to antimicrobial resistance, behavior and welfare, genetic testing and population-based evidence. A number of exciting actions were agreed during the meeting. These included setting up working groups to create tools to help breed clubs accelerate the implementation of breed-health strategies, review aspects of extreme conformation and share useful information on behavior. The meeting also heralded the development of an online resource of relevant information describing quality measures for DNA testing. A demand for more and better data and evidence was a recurring message stressed across all themes. Conclusions The meeting confirmed the benefits from inclusion of a diverse range of stakeholders who all play relevant and collaborative parts to improve future canine health. Firm actions were set for progress towards improving breed-related welfare. The next international workshop will be in the UK in 2019 and will be organized by the UK Kennel Club

    O ethos e o pathos na sala de parto

    RESUMO Objetivo Compreender o ethos e o pathos presentes nos discursos de mulheres parturientes e profissionais de saúde no contexto da sala de parto. Método Pesquisa qualitativa do tipo interpretativa. Utilizou-se o método da Análise do Discurso. Participaram das entrevistas 36 mulheres e 24 profissionais de saúde de maternidades do interior de Minas Gerais. Resultados Os discursos sinalizam a concepção que as mulheres têm do parto, centrada no olhar do outro e nas suas representações; as dificuldades das enfermeiras para operacionalizar o cuidado e construir uma imagem profissional autônoma. O discurso médico enfatiza a especialidade, a capacidade de intervir em situações de risco e as mudanças no status da profissão. Conclusões A construção do ethos de médicos e enfermeiros é fundamental para o delineamento de campos de saberes mais flexíveis e parauma atuação profissional condizente com os seus papéis e comprometida com os preceitos éticos e legais do cuidado obstétrico

    Array-based comparative genomic hybridization has been assumed to be the first genetic test offered to detect genomic imbalances in patients with unexplained intellectual disability with or without dysmorphisms, multiple congenital anomalies, learning difficulties and autism spectrum disorders. Our study contributes to the genotype/phenotype correlation with the delineation of laboratory criteria which help to classify the different copy number variants (CNVs) detected.     Effect of 6-months of physical exercise on the nitrate/nitrite levels in hypertensive postmenopausal women

    <p>Abstract</p> <p>Background</p> <p>Evidences have showed that the incidence of arterial hypertension is greater in postmenopausal women as compared to premenopausal. Physical inactivity has been implicated as a major contributor to weight gain and abdominal obesity in postmenopausal women and the incidence of cardiovascular disease increases dramatically after menopause. Additionally, more women than men die each year of coronary heart disease and are twice as likely as men to die within the first year after a heart attack. A healthy lifestyle has been strongly associated with the regular physical activity and evidences have shown that physically active subjects have more longevity with reduction of morbidity and mortality. Nitric oxide (NO) produced by endothelial cells has been implicated in this beneficial effect with improvement of vascular relaxing and reduction in blood pressure in both laboratory animals and human. Although the effect of exercise training in the human cardiovascular system has been largely studied, the majority of these studies were predominantly conducted in men or young volunteers. Therefore, the aim of this work was to investigate the effects of 6 months of dynamic exercise training (ET) on blood pressure and plasma nitrate/nitrite concentration (NOx<sup>-</sup>) in hypertensive postmenopausal women.</p> <p>Methods</p> <p>Eleven volunteers were submitted to the ET consisting in 3 days a week, each session of 60 minutes during 6 months at moderate intensity (50% of heart rate reserve). Anthropometric parameters, blood pressure, NOx<sup>- </sup>concentration were measured at initial time and after ET.</p> <p>Results</p> <p>A significant reduction in both systolic and diastolic blood pressure values was seen after ET which was accompanied by markedly increase of NOx<sup>- </sup>levels (basal: 10 ± 0.9; ET: 16 ± 2 μM). Total cholesterol was significantly reduced (basal: 220 ± 38 and ET: 178 ± 22 mg/dl), whereas triglycerides levels were not modified after ET (basal: 141 ± 89 and ET: 147 ± 8 mg/dl).</p> <p>Conclusion</p> <p>Our study shows that changing in lifestyle promotes reduction of arterial pressure which was accompanied by increase in nitrite/nitrate concentration. Therefore, 6-months of exercise training are an important approach in management arterial hypertension and play a protective effect in postmenopausal women.</p

    Search for direct pair production of the top squark in all-hadronic final states in proton-proton collisions at s√=8 TeV with the ATLAS detector

    The results of a search for direct pair production of the scalar partner to the top quark using an integrated luminosity of 20.1fb−1 of proton–proton collision data at √s = 8 TeV recorded with the ATLAS detector at the LHC are reported. The top squark is assumed to decay via t˜→tχ˜01 or t˜→ bχ˜±1 →bW(∗)χ˜01 , where χ˜01 (χ˜±1 ) denotes the lightest neutralino (chargino) in supersymmetric models. The search targets a fully-hadronic final state in events with four or more jets and large missing transverse momentum. No significant excess over the Standard Model background prediction is observed, and exclusion limits are reported in terms of the top squark and neutralino masses and as a function of the branching fraction of t˜ → tχ˜01 . For a branching fraction of 100%, top squark masses in the range 270–645 GeV are excluded for χ˜01 masses below 30 GeV. For a branching fraction of 50% to either t˜ → tχ˜01 or t˜ → bχ˜±1 , and assuming the χ˜±1 mass to be twice the χ˜01 mass, top squark masses in the range 250–550 GeV are excluded for χ˜01 masses below 60 GeV
