    An investigation to determine the perceived social support of professional nurses working in a psychiatric hospital in the Western Cape

    Magister Curationis - McurGlobally, workplace stress has been reported as having deleterious effects on workers. Professional nurses, working in a psychiatric hospital with patients suffering from mental illnesses, may present with signs and symptoms related to anxiety, depression, persistent chronic stress and burnout. Social support plays an important role in the lives of professional nurses, who face challenges while working within a stressful environment. In addition, social support enhances the level of job performance, reduces the level of job stress and improves work commitment among mental health nurses. Without adequate social support in the workplace, stress has negative effects on both professional nurses and patients. The aim of this study was to investigate the perceived social support of professional nurses, working in a psychiatric hospital, in the Western Cape. The objective formulated to achieve the aim, was to determine the social support received by professional nurses from family, significant others and friends.     Safety and pharmacokinetics of MM-302, a HER2-targeted antibody–liposomal doxorubicin conjugate, in patients with advanced HER2-positive breast cancer: A phase 1 dose-escalation study

    BackgroundThis phase 1 dose-escalation trial studied MM-302, a novel HER2-targeted PEGylated antibody-liposomal doxorubicin conjugate, in HER2-positive locally advanced/metastatic breast cancer.MethodsPatients were enrolled in four cohorts: MM-302 monotherapy (8, 16, 30, 40, and 50 mg/m2 every 4 weeks [q4w]); MM-302 (30 or 40 mg/m2 q4w) plus trastuzumab (4 mg/kg q2w); MM-302 (30 mg/m2) plus trastuzumab (6 mg/kg) q3w; MM-302 (30 mg/m2) plus trastuzumab (6 mg/kg) and cyclophosphamide (450 mg/m2) q3w.ResultsSixty-nine patients were treated. The most common adverse events (AEs) were fatigue and nausea. Grade 3/4 AEs of special interest included neutropenia, fatigue, mucosal inflammation, anemia, thrombocytopenia, febrile neutropenia, and palmar-plantar erythrodysesthesia. The MTD was not reached. With MM-302 ≥ 30 mg/m2, overall response rate (ORR) was 13% and median progression-free survival (mPFS) 7.4 months (95% CI: 3·5-10·9) in all arms. In 25 anthracycline-naïve patients, ORR was 28·0% and mPFS 10·9 months (95% CI: 1·8-15·3). Imaging with 64Cu-labeled MM-302 visualized tumor-drug penetrance in tumors throughout the body, including the brain.ConclusionMM-302 monotherapy, in combination with trastuzumab, or trastuzumab plus cyclophosphamide, was well tolerated and showed promising efficacy.     Neuroscience-informed classification of prevention interventions in substance use disorders : an RDoC-based approach

    Neuroscience has contributed to uncover the mechanisms underpinning substance use disorders (SUD). The next frontier is to leverage these mechanisms as active targets to create more effective interventions for SUD treatment and prevention. Recent large-scale cohort studies from early childhood are generating multiple levels of neuroscience-based information with the potential to inform the development and refinement of future preventive strategies. However, there are still no available well-recognized frameworks to guide the integration of these multi-level datasets into prevention interventions. The Research Domain Criteria (RDoC) provides a neuroscience-based multi-system framework that is well suited to facilitate translation of neurobiological mechanisms into behavioral domains amenable to preventative interventions. We propose a novel RDoC-based framework for prevention science and adapted the framework for the existing preventive interventions. From a systematic review of randomized controlled trials using a person-centered drug/alcohol preventive approach for adolescents, we identified 22 unique preventive interventions. By teasing apart these 22 interventions into the RDoC domains, we proposed distinct neurocognitive trajectories which have been recognized as precursors or risk factors for SUDs, to be targeted, engaged and modified for effective addiction prevention.Peer reviewe

    64Cu-MM-302 Positron Emission Tomography Quantifies Variability of Enhanced Permeability and Retention of Nanoparticles in Relation to Treatment Response in Patients with Metastatic Breast Cancer

    Purpose: Therapeutic nanoparticles are designed to deliver their drug payloads through enhanced permeability and retention (EPR) in solid tumors. The extent of EPR and its variability in human tumors is highly debated and has been proposed as an explanation for variable responses to therapeutic nanoparticles in clinical studies. Experimental Design: We assessed the EPR effect in patients using a 64Cu-labeled nanoparticle, 64Cu-MM-302 (64Cu-labeled HER2-targeted PEGylated liposomal doxorubicin), and imaging by PET/CT. Nineteen patients with HER2-positive metastatic breast cancer underwent 2 to 3 PET/CT scans postadministration of 64Cu-MM-302 as part of a clinical trial of MM-302 plus trastuzumab with and without cyclophosphamide (NCT01304797). Results: Significant background uptake of 64Cu-MM-302 was observed in liver and spleen. Tumor accumulation of 64Cu-MM-302 at 24 to 48 hours varied 35-fold (0.52–18.5 %ID/kg), including deposition in bone and brain lesions, and was independent of systemic plasma exposure. Computational analysis quantified rates of deposition and washout, indicating peak liposome deposition at 24 to 48 hours. Patients were classified on the basis of 64Cu-MM-302 lesion deposition using a cut-off point that is comparable with a response threshold in preclinical studies. In a retrospective exploratory analysis of patient outcomes relating to drug levels in tumor lesions, high 64Cu-MM-302 deposition was associated with more favorable treatment outcomes (HR = 0.42). Conclusions: These findings provide important evidence and quantification of the EPR effect in human metastatic tumors and support imaging nanoparticle deposition in tumors as a potential means to identify patients well suited for treatment with therapeutic nanoparticles

    Recruitment Disruption and the Role of Unaffected Populations for Potential Recovery After the Pinna nobilis Mass Mortality Event

    A devastating mass mortality event (MME) very likely caused by the protozoan Haplosporidium pinnae first detected in 2016 in the Western Mediterranean Sea, is pushing the endemic bivalve Pinna nobilis to near extinction. Populations recovery, if possible, will rely on larval dispersal from unaffected sites and potential recolonization through recruitment of resistant juveniles. To assess the impact of the MME on the species’ larval recruitment, an unprecedented network of larval collector stations was implemented over several thousands of kilometers along the Western Mediterranean coasts during the 3 years after the onset of the MME. The findings of this network showed a generalized disruption in recruitment with dramatic consequences for the recovery of the species. However, there were exceptions to this pattern and recruits were recorded in a few sites where the resident population had been decimated. This hints to the importance of unaffected populations as larval exporting sources and the role of oceanographic currents in larval transport in the area, representing a beacon of hope in the current extremely worrying scenario for this emblematic species.En prens

    Tracking a mass mortality outbreak of pen shell Pinna nobilis populations: A collaborative effort of scientists and citizens

    A mass mortality event is devastating the populations of the endemic bivalve Pinna nobilis in the Mediterranean Sea from early autumn 2016. A newly described Haplosporidian endoparasite (Haplosporidium pinnae) is the most probable cause of this ecological catastrophe placing one of the largest bivalves of the world on the brink of extinction. As a pivotal step towards Pinna nobilis conservation, this contribution combines scientists and citizens’ data to address the fast- and vast-dispersion and prevalence outbreaks of the pathogen. Therefore, the potential role of currents on parasite expansion was addressed by means of drift simulations of virtual particles in a high-resolution regional currents model. A generalized additive model was implemented to test if environmental factors could modulate the infection of Pinna nobilis populations. The results strongly suggest that the parasite has probably dispersed regionally by surface currents, and that the disease expression seems to be closely related to temperatures above 13.5 °C and to a salinity range between 36.5–39.7 psu. The most likely spread of the disease along the Mediterranean basin associated with scattered survival spots and very few survivors (potentially resistant individuals), point to a challenging scenario for conservation of the emblematic Pinna nobilis, which will require fast and strategic management measures and should make use of the essential role citizen science projects can play.info:eu-repo/semantics/publishedVersio

    Adult spawning and early larval development of the endangered bivalve Pinna nobilis

    [EN] The development of aquaculture activities has posed an alternative solution for the preservation of some overexploited shell¿sh ¿sheries worldwide. In the same way, endemic Mediterranean bivalves such as Pinna nobilis, highly threatened by habitat loss and coastal pollution, could found in aquaculture a solution for preserving the continuity of the species. Given the endangered status of the species, the biological and ecological processes regulating natural populations have been well studied, but there are still important knowledge gaps preventing the development of viable arti¿cial cultures. This study describes for the ¿rst time the larval development of P. nobilis (from fertilization until pediveliger larval stages) in captivity conditions. Moreover, di¿erent rearing tanks of 5, 16 and 80 L, larvae density from 1 to 600 larvae mL¿1, light conditions, food doses, were tested in order to establish the bases for the optimal rearing of the species and provide a source of individuals for restoring ¿eld populations. Results showed that 16 L tanks with a concentration of 2 larvae mL¿1, constant temperature of 21 °C, 12/12 h photoperiod and fed with an ¿optimal¿ mixture of 25 cells per ¿L of Chaetoceros calcitrans + 33.3 cells per¿L ofPavlova lutheri + 100 cells per¿L ofIsochrysis galbana¿ appear to be the best conditions to rearlarvae ofP. nobilis.Di¿erentcaptivity conditions such as loweror highertank volume, larvae density, or food doses; light privation did not report better results for larval development.The present study was financed by the Caisse d'Epargne Cote d'Azur.
