    Intraoperative Administration of Intravenous Acetaminophen at Induction or End of Surgery and its Relationship with Opioid Consumption and Pain Scores

    Obesity has become increasingly prevalent in the United States. Bariatric surgeries have increased as the prevalence of obesity has risen, providing an effective alternative to weight loss. Intravenous acetaminophen (IVA) is a safe and effective non-opioid medication that can be given without the risk of respiratory or cardiac complications. Research has shown that the use of IVA during surgery can improve postoperative pain scores, reduce opioid requirements, and improve patient satisfaction. A retrospective cohort study of 200 patients (100 per group) undergoing laparoscopic bariatric surgery (LBS) was performed. Statistical analysis was used to determine the relationship between those who received IVA near anesthesia induction or near end of surgery and intraoperative/postoperative opioid consumption and pain scores. Other study variables included: age, gender, American Society of Anesthesiologists (ASA) classification, Body Mass Index (BMI), and length of anesthesia (LOA). The results of the study determined there was no significant relationship between the administration time of IVA and intraoperative/postoperative opioid consumption or pain scores. There were significant relationships found between IVA administration time and BMI, age, and LOA. While no significant relationships were found related to the administration timing of IVA and opioid consumption and pain scores, many studies have found efficacy in the use of IVA in reducing opioid consumption and pain scores in a variety or procedures and populations. Although this study did not provide results influencing the administration timing of IVA, it is recommended that other studies follow a similar study design in further investigation into the use of IVA to enhance anesthesia care and improve patient safety

    Molecular characterization of Trichomonas gallinae isolates recovered from the Canadian Maritime provinces’ wild avifauna reveals the presence of the genotype responsible for the European finch trichomonosis epidemic and additional strains

    Finch trichomonosis, caused by Trichomonas gallinae, emerged in the Canadian Maritime provinces in 2007 and has since caused ongoing mortality in regional purple finch (Carpodacus purpureus) and American goldfinch (Carduelis tristis) populations. Trichomonas gallinae was isolated from (1) finches and rock pigeons (Columbia livia) submitted for post-mortem or live-captured at bird feeding sites experiencing trichomonosis mortality; (2) bird seed at these same sites; and (3) rock pigeons live-captured at known roosts or humanely killed. Isolates were characterized using internal transcribed spacer (ITS) region and iron hydrogenase (Fe-hyd) gene sequences. Two distinct ITS types were found. Type A was identical to the UK finch epidemic strain and was isolated from finches and a rock pigeon with trichomonosis; apparently healthy rock pigeons and finches; and bird seed at an outbreak site. Type B was obtained from apparently healthy rock pigeons. Fe-hyd sequencing revealed six distinct subtypes. The predominant subtype in both finches and the rock pigeon with trichomonosis was identical to the UK finch epidemic strain A1. Single nucleotide polymorphisms in Fe-hyd sequences suggest there is fine-scale variation amongst isolates and that finch trichomonosis emergence in this region may not have been caused by a single spill-over event

    In 2005, trichomonosis emerged as a new cause of substantial mortality in United Kingdom (UK) greenfinch (Carduelis chloris) and chaffinch (Fringella coelebs) populations. Similar to the UK, in the summer and fall of 2007–2009 and 2011, mortalities due to trichomonosis occurred in the Canadian Maritime provinces’ purple finch (Carpodacus purpureus) and American goldfinch (Cardeuelis tristis) populations consistent with emergence of trichomonosis in this region. Trichomonosis is caused by Trichomonas gallinae, a protozoan parasite infecting the upper digestive tract of birds. Trichomonosis causes slow death from emaciation and dehydration due to lesions preventing ingestion of food and water. This disease is reported by the public at their backyard feeding and watering stations. Therefore, this study’s goal was to better understand the epidemiology of trichomonosis in this region through collaboration with the Maritime bird feeding public. Through use of molecular tools and field techniques this study was able to conclude that (1) rock pigeons (Columba livia) can act as a reservoir for T. gallinae in the Canadian Maritime provinces, (2) the same T. gallinae strain that caused the United Kingdom epidemic is present in Canadian Maritime wild avifauna, (3) moist birdseed can possibly act as a fomite in the transmission of T. gallinae and (4) climate is a potential contributing factor in the emergence of T. gallinae in the Canadian Maritime provinces. Finally, this study provided the first scientific approach to assess the role of birdfeeders and watering stations as fomites facilitating transmission of trichomonosis. The information obtained from this project resulted in general recommendations on trichomonosis prevention for the concerned bird feeding public, thus protecting the health and welfare of birds frequenting birdfeeders

    Increased Oxygen Recovery from Sabatier Systems Using Plasma Pyrolysis Technology and Metal Hydride Separation

    State-of-the-art life support carbon dioxide (CO2) reduction technology is based on the Sabatier reaction where less than 50% of the oxygen required for the crew is recovered from metabolic CO2. The reaction produces water as the primary product and methane as a byproduct. Oxygen recovery is constrained by the limited availability of reactant hydrogen. This is further exacerbated when Sabatier methane (CH4) is vented as a waste product resulting in a continuous loss of reactant hydrogen. Post-processing methane with the Plasma Pyrolysis Assembly (PPA) to recover hydrogen has the potential to dramatically increase oxygen recovery and thus drastically reduce the logistical challenges associated with oxygen resupply. The PPA decomposes methane into predominantly hydrogen and acetylene. Due to the highly unstable nature of acetylene, a separation system is necessary to purify hydrogen before it is recycled back to the Sabatier reactor. Testing and evaluation of a full-scale Third Generation PPA is reported and investigations into metal hydride hydrogen separation technology is discussed

    A combined analytical-experimental investigation of friction in cylinder liner inserts under mixed and boundary regimes of lubrication

    It is necessary to develop an analytical solution in order to combine predictions with measured tribological parameters and fundamentally understand the mechanism of lubrication in a typical region of engine cycle, using tribometric studies. This paper deals with the development of such a representative approach. An analytical, rather than a numerical approach is expounded, as it is shown to suffice for the purpose of precise time-efficient predictions, which conform well to the measurements. The effect of surface topography, material and operating conditions are ascertained for the representative case of top compression ring—cylinder liner contact at the top dead centre reversal in transition from the compression to power stroke. Stainless steel uncoated surface used as press fit cylinder liners for niche original equipment manufacturer applications are compared with those furnished with a Nickel-Silicon Carbide wear-resistant coating of choice in high performance motorsport

    Life path analysis: scaling indicates priming effects of social and habitat factors on dispersal distances

    1. Movements of many animals along a life-path can be separated into repetitive ones within home ranges and transitions between home ranges. We sought relationships of social and environmental factors with initiation and distance of transition movements in 114 buzzards Buteo buteo that were marked as nestlings with long-life radio tags. 2. Ex-natal dispersal movements of 51 buzzards in autumn were longer than for 30 later in their first year and than 35 extra-natal movements between home ranges after leaving nest areas. In the second and third springs, distances moved from winter focal points by birds that paired were the same or less than for unpaired birds. No post-nuptial movement exceeded 2 km. 3. Initiation of early ex-natal dispersal was enhanced by presence of many sibs, but also by lack of worm-rich loam soils. Distances travelled were greatest for birds from small broods and with relatively little short grass-feeding habitat near the nest. Later movements were generally enhanced by the absence of loam soils and short grassland, especially with abundance of other buzzards and probable poor feeding habitats (heathland, long grass). 4. Buzzards tended to persist in their first autumn where arable land was abundant, but subsequently showed a strong tendency to move from this habitat. 5. Factors that acted most strongly in ½-km buffers round nests, or round subsequent focal points, usually promoted movement compared with factors acting at a larger scale. Strong relationships between movement distances and environmental characteristics in ½-km buffers, especially during early ex-natal dispersal, suggested that buzzards became primed by these factors to travel far. 6. Movements were also farthest for buzzards that had already moved far from their natal nests, perhaps reflecting genetic predisposition, long-term priming or poor habitat beyond the study area

    Human African trypanosomiasis

    Human African trypanosomiasis (sleeping sickness) occurs in sub-Saharan Africa. It is caused by the protozoan parasite Trypanosoma brucei, transmitted by tsetse flies. Almost all cases are due to Trypanosoma brucei gambiense, which is indigenous to west and central Africa. Prevalence is strongly dependent on control measures, which are often neglected during periods of political instability, thus leading to resurgence. With fewer than 12 000 cases of this disabling and fatal disease reported per year, trypanosomiasis belongs to the most neglected tropical diseases. The clinical presentation is complex, and diagnosis and treatment difficult. The available drugs are old, complicated to administer, and can cause severe adverse reactions. New diagnostic methods and safe and effective drugs are urgently needed. Vector control, to reduce the number of flies in existing foci, needs to be organised on a pan-African basis. WHO has stated that if national control programmes, international organisations, research institutes, and philanthropic partners engage in concerted action, elimination of this disease might even be possible
