Molecular mechanisms of cytokine and ubiquinone induced otoprotection against cisplatin
Als Nebenwirkung des Zytostatikums Cisplatin wurde neben Neuro- und
NephrotoxizitÀt auch OtotoxizitÀt beschrieben. Die SchÀdigung ist irreversibel
und fĂŒhrt zu Tinnitus sowie Hör-verlust und damit zu kommunikativen EinbuĂen
oder Entwicklungsdefiziten. Ziel der vorliegen-den Arbeit ist es,
otoprotektive Substanzen zu identifizieren und zugrunde liegende Signalkaska-
den zu charakterisieren. FĂŒr das Zytokin Interleukin-6 (IL-6) konnte in vitro
bereits limitiertes otoprotektives Potential beschrieben werden. Neben den
Charakteristika der IL-6 vermittelten Protektion soll das protektive Potential
des IL-6-Verwandten Oncostatin M (OSM) untersucht werden. Weiterhin soll der
Einfluss von SignalmolekĂŒlen wie des Signal Transducer and Activa-tor of
Transcription 3 (STAT3), der durch beide Zytokine aktiviert wird, untersucht
werden. Als Ursache Cisplatin-induzierter SchÀden werden erhöhte Spiegel
reaktiver Sauerstoffspezies (ROS) diskutiert, deshalb wird das otoprotektive
Potential des antioxidativen Ubichinon sowie die Rolle des STAT3, das auch
mitochondriale Funktionen aufweist, untersucht. DarĂŒber hinaus wird der
Einfluss des IL-6, OSM und Ubichinon auf die Apoptoseregulation untersucht.
Explantate Cortiâscher Organe neonataler Ratten wurden fĂŒr 24h mit IL-6, OSM
oder bzw. simultan mit Ubichinon prĂ€- und fĂŒr weitere 24h simultan mit
Cisplatin inkubiert. Haarzellen und NeuronenfortsÀtze wurden fluoreszenz-
markiert, morphologisch evaluiert und quantifiziert. Andere Explantate wurden
parallel zu IL-6, OSM und Ubichinon mit dem STAT3 Inhibitor WP1066 kultiviert,
im Anschluss gleichermaĂen weiterbehandelt und ausgewertet. Zudem wurden durch
die Behandlungen induzierte Protein-SpiegelÀnderungen mittels Western Blot
untersucht: STAT3, seine Serin- und Tyrosin-phosphorylierte Form wurden im
Nukleus und Zytoplasma analysiert. BCL-X, BCL-2, AIF, Survivin und Bax wurden
im Zytoplasma untersucht. Gruppen wurden mittels Rangsummen-ANOVA auf
Unterschiede geprĂŒft, diese wurden als statistisch signifikant angesehen, wenn
der p-Wert 0,05 unterschritt. IL-6, OSM und Ubichinon schĂŒtzen beide
Haarzelltypen statistisch signifikant vor Cisplatin-induzierter ToxizitÀt
entlang der gesamten Cochlea. Ubichinon schĂŒtzt die Neuronen-FortsĂ€tze
statistisch signifikant entlang des gesamten Cortiâschen Organs, die Zytokine
limitiert auf einzelne Abschnitte. Die IL-6- und OSM-induzierte Otoprotektion
wurde durch WP1066 aufgehoben, die durch Ubichinon-induzierte nicht. Durch
IL-6-Behandlung stieg Tyrosin-phosphoryliertes STAT3 im Zytoplasma statistisch
signifikant, durch OSM-Behandlung stiegen zytoplasmatisches und nukleÀres,
Tyrosin-phosphoryliertes STAT3 sowie zytoplasmatisches, Serin-phosphoryliertes
STAT3 statistisch signifikant. Die Apoptose-regulierenden Proteine zeig-ten
durch die Behandlung keine signifikanten SpiegelÀnderungen. Mitglieder der
IL-6-Zytokin-Familie weisen neben ihren proinflammatorischen Funktio-nen
otoprotektives Potential auf, das durch Tyrosin-phosphoryliertes STAT3
vermittelt wird. Ubichinon wirkt ebenfalls otoprotektiv, eine Beteiligung des
STAT3 abseits seiner klassischen Rolle als Transkriptionsfaktor kann nicht
ausgeschlossen werden. Die Schlussfolgerungen dieser Arbeit sind limitiert,
dennoch stellt die vorliegende in vitro Studie einen Baustein auf dem Weg zum
Design eines klinisch-protektiven Protokolls fĂŒr betroffene Patienten dar und
ermöglicht die Ableitung neuer wissenschaftlicher Fragenstellungen.Cisplatin, a chemotherapeutic drug, can cause neuro-, nephro- and ototoxic
side effects. Cisplatin-induced ototoxicity is irreversible and leads to
hearing loss and tinnitus, resulting in loss of communicative skills. The aim
of this study was to identify otoprotective agents and to reveal protective
signaling pathways. Cytokine interleukin-6 (IL-6) is known to exert limited
otoprotective potential in vitro. Next to further characterizing this
protection it was to be determined, if oncostatin M (OSM), a close relative of
IL-6, also induces otoprotection. Both cytokines are known to activate Signal-
Transducer-and-Activator-of-Transcription-3 (STAT3), and the role of STAT3 in
otoprotection was going to be investigated. Cisplatin-induced toxicity is said
to be mediated via upregulation of reactive oxygen species (ROS), therefore
otoprotective potential of antioxidant ubiquinone is going to be analyzed, as
well as the role of STAT3 in this context, which is known to exert functions
in mitochondria next to its classic role as a transcription factor.
Furthermore, the influence of IL-6, OSM and ubiquinone on the regulation of
apoptosis is going to be investigated. Organ of Corti explants dissected from
neonatal rats were preincubated for 24h with either IL-6, OSM or ubiquinone,
followed by concurrent incubation with cisplatin for another 24h. Hair cells
and spiral ganglion neurons were visualized with immunofluorescence, the
morphology and amount were evaluated. In other experiments, the explants were
simultaneously incubated with STAT3 inhibitor WP1066 and then evaluated as
above. Changes in the target protein levels following the treatment were
investigated via Western blot: Total levels of STAT3 and its serine- or
tyrosine-phosphorylated forms were analyzed in cytoplasmic and nuclear
fractions of explant tissues. Levels of BCL-x, BCL-2, AIF, survivin and Bax
were investigated in the cytoplasm. Treatment groups were examined for
significant differences via ANOVA on ranks, differences were considered as
significant when p was below 0.05. IL-6, OSM and ubiquinone significantly
protect both hair cell types from the cisplatin-induced toxicity along the
whole length of the cochlea. Ubiquinone significantly protects neurites along
the whole cochlea whereas the cytokine-induced protection was limited to
certain parts of the organ of Corti. Cytokine-mediated protection was
abolished by WP1066, whereas ubiquinone-induced protection was not. Tyrosine-
phosphorylated STAT3 increased significantly in the cytoplasm following IL-6
treatment or OSM treatment. OSM induced additional increase of nuclear
translocation and of serine-phosphorylation. There were no significant changes
in the level of apoptosis-regulating proteins in response to the cytokines or
ubiquinone. IL-6-cytokine-family members and Ubiquinone induce otoprotective
mechanisms. The mechanisms involve phosphorylation of STAT3 on one or two
domains and in case of IL-6 and OSM can be blocked by STAT3 inhibitor WP1066.
The mechanism induced by Ubiquinone seems to be insensitive to WP1066,
suggesting involvement of other than JAK2 kinase. This study is an attempt
toward designing a novel clinical protocol for patients scheduled to
chemotherapy involving cisplatin. In addition, it allows to pose new
scientific questions
