    Capital y sociedad: aproximaciones teóricas y empíricas

    Under capital concept lies multiple meanings undercovered result of multiple semantic references that have the concept. This work will attempt to prove that capital, collecting the distinctions at the present day that provide it, become to the mechanism that make possible the structural coupling among interactions and functional systems and whose evolution run in parallel to the society's evolution established a theoretical passing through, with diverse empirical manifestations, between the differentiation and integration of society. It is posible in view the intrincate relationships that establish the communicative mechanism capital with generalized symbolic communication media, features that allow design general communicative frameworks of intersystemical relationships, allow likewise that will named communication assymetry.Bajo el concepto de capital se cobijan múltiples acepciones, fruto de las múltiples referencias con que cuenta semánticamente el concepto. Intentaremos demostrar que el capital, recogiendo las distinciones que provee en la actualidad, corresponde al mecanismo que posibilita el acoplamiento estructural entre los sistemas de interacción y los sistemas funcionales y cuya evolución corre en paralelo a la evolución de la sociedad estableciendo un tránsito teórico, con diversas manifestaciones empíricas, entre la diferenciación y la integración de la sociedad. Ello es posible dadas las intrincadas relaciones que establece el mecanismo comunicativo capital con los medios de comunicación simbólicamente generalizados, características que permiten diseñar comunicativamente marcos generales de relaciones intersistémicas, posibilitando asimismo lo que denominaremos asimetrías en la comunicación

    El artículo buscará proponer una discusión teórica que apunte a la descripción de los acoplamientos estructurales entre los sistemas de interacción y los sistemas funcionales.  Partiendo de las premisas de la Teoría de sistemas autopoiéticos, se buscará establecer algunos parámetros de observación de las operaciones de los sistemas funcionales, las cuales serán susceptibles de ser observadas por la interacción, siendo esta observación un antecedente directo del acoplamiento entre los sistemas de interacción y los sistemas funcionales. El artículo propondrá que el vínculo observable entre las operaciones de los sistemas funcionales y las operaciones de los sistemas de interacción corresponde al concepto de capital desarrollado por la teoría sociológica a lo largo de su historia, fenómeno que ocurre con matices y diferencias para todos los sistemas funcionales

    Biomarkers and polymorphisms in pancreatic neuroendocrine tumors treated with sunitinib

    Several circulating biomarkers and single nucleotide polymorphisms (SNPs) have been correlated with efficacy and tolerability to antiangiogenic agents. These associations remain unexplored in well-differentiated, metastatic pancreatic neuroendocrine tumors treated with the multitargeted tyrosine kinase inhibitor sunitinib. We have assessed the effect on tumor response at 6 months, overall survival, progression-free survival and safety of 14 SNPs, and 6 soluble proteins. Forty-three patients were recruited. Two SNPs in the vascular endothelial growth factor receptor 3 (VEGFR-3) gene predicted lower overall survival: rs307826 with hazard ratio (HR) 3.67 (confidence interval [CI] 95%, 1.35-10.00) and rs307821 with HR 3.84 (CI 95%, 1.47-10.0). Interleukin-6 was associated with increased mortality: HR 1.06 (CI 95%, 1.01-1.12), and osteopontin was associated with shorter PFS: HR 1.087 (1.01-1.16), independently of Ki-67. Furthermore, levels of osteopontin remained higher at the end of the study in patients considered non-responders: 38.5 ng/mL vs. responders: 18.7 ng/mL, p-value=0.039. Dynamic upward variations were also observed with respect to IL-8 levels in sunitinib-refractory individuals: 28.5 pg/mL at baseline vs. 38.3 pg/mL at 3 months, p-value=0.024. In conclusion, two VEGFR-3 SNPs as well as various serum biomarkers were associated with diverse clinical outcomes in patients with well-differentiated pancreatic neuroendocrine tumors treated with sunitinib

    The Courtyard of TT 209 (Areas C1 and C2) Seasons 2012 to 2014

    In 2012, the Archaeological Mission of the University of La Laguna began its work in TT 209 (Luxor, Egypt) under the name Proyecto dos cero nueve [Project two zero nine]. The objective of this article is to present a synthesis of the processes of excavation and documentation in the area of the tomb where activities began, the courtyard (areas C1 and C2), during the first three seasons (2012, 2013/14 and 2014). The working method is explained, the two main formation processes of the sedimentary deposits that the archaeological investigation recognised—geogonic and anthropic—are described, and the different kinds of materials found there are examined, which correspond to a broad chronological and cultural framework. The analysis of this dataset has made it possible to obtain a historical interpretation of the activities that were carried out in this external part of the funerary complex, from the moment of its construction to the present.En 2012 la Misión Arqueológica de la Universidad de La Laguna comenzó sus trabajos en la TT 209 (Luxor, Egipto) en el marco del denominado Proyecto dos cero nueve. El objetivo del presente artículo es presentar una síntesis del proceso de excavación y documentación de la zona de la tumba donde se iniciaron las actividades, el patio (áreas C1 y C2), durante las tres primeras campañas (2012, 2013-2014 y 2014). Se expone el método de trabajo, se explican los dos principales procesos de formación del depósito sedimentario, tanto geogénicos como antrópicos, reconocidos a través de la intervención arqueológica y se exponen las diferentes clases de materiales hallados, que corresponden a un amplio marco cronológico y cultural. El análisis de este conjunto de informaciones ha permitido obtener una interpretación desde la perspectiva histórica de las actividades desarrolladas en esta parte exterior del complejo funerario desde el momento de su construcción hasta la actualidad

    Identification and characterization of Cardiac Glycosides as senolytic compounds

    Compounds with specific cytotoxic activity in senescent cells, or senolytics, support the causal involvement of senescence in aging and offer therapeutic interventions. Here we report the identification of Cardiac Glycosides (CGs) as a family of compounds with senolytic activity. CGs, by targeting the Na+/K+ATPase pump, cause a disbalanced electrochemical gradient within the cell causing depolarization and acidification. Senescent cells present a slightly depolarized plasma membrane and higher concentrations of H+, making them more susceptible to the action of CGs. These vulnerabilities can be exploited for therapeutic purposes as evidenced by the in vivo eradication of tumors xenografted in mice after treatment with the combination of a senogenic and a senolytic drug. The senolytic effect of CGs is also effective in the elimination of senescence-induced lung fibrosis. This experimental approach allows the identification of compounds with senolytic activity that could potentially be used to develop effective treatments against age-related diseases.We thank Matthias Drosten, Alejo Efeyan and Sean Morrison for plasmids.     Nutrición enteral domiciliaria en España: registro Nadya del año 2011-2012

    Objective: To describe the results of the home enteral nutrition (HEN) registry of the NADYA-SENPE group in 2011 and 2012. Material and methods: We retrieved the data of the patients recorded from January 1st 2011 to December 31st 2012. Results: There were 3021 patients in the registry during the period from 29 hospitals, which gives 65.39 per million inhabitants. 97.95% were adults, 51.4% male. Mean age was 67.64 ± 19.1, median age was 72 years for adults and 7 months for children. Median duration with HEN was 351 days and for 97.5% was their first event with HEN. Most patients had HEN because of neurological disease (57.8%). Access route was nasogastric tube for 43.5% and gastrostomy for 33.5%. Most patients had limited activity level and, concerning autonomy, 54.8% needed total help. Nutritional formula was supplied from chemist’s office to 73.8% of patients and disposables, when necessary, was supplied from hospitals to 53.8% of patients. HEN was finished for 1,031 patients (34.1%) during the period of study, 56.6% due to decease and 22.2% due to recovery of oral intake. Conclusions: Data from NADYA-SENPE registry must be explained cautiously because it is a non-compulsory registry. In spite of the change in the methodology of the registry in 2010, tendencies regarding HEN have been maintained, other than oral routeObjetivos: Describir los resultados del registro de nutrición enteral domiciliaria (NED) del grupo NADYASENPE de los años 2011 y 12. Material y métodos: Se recopilaron los datos introducidos en el registro desde el 1 de enero de 2011 al 31 de diciembre de 2012. Resultados: Hubo 3021 pacientes en el registro durante el periodo, procedentes de 29 hospitales, lo que da una prevalencia de 65,39 casos por millón de habitantes. 97.95% fueron adultos, 51,4% varones. La edad media fue 67,64 ± 19,1 años y la mediana 72 años para los adultos y 7 meses para los niños. La duración media de la NED fue 351 días y para el 97,5% fue el primer episodio con NED. La mayoría de pacientes tenían NED por una enfermedad neurológica (57,8%). La vía de acceso fue sonda nasogástrica para el 43,5% y gastrostomía para el 33,5%. La mayoría de pacientes tuvieron un nivel de actividad física limitado y, respecto a la autonomía, 54,8% necesitaba ayuda total. La fórmula de nutrición se suministró desde las oficinas de farmacia para el 73,8% y los fungibles, cuando fueron necesarios, desde los hospitales para el 53,8%. La NED se suspendió en 1.031 pacientes (34,1%) durante el periodo de estudio, 56,6% debido a fallecimiento y 22,2% debido a recuperación de la vía oral. Conclusiones: Los datos del registro NADYA-SENPE deben ser interpretados con precaución ya que se trata de un registro voluntario. A pesar del cambio de metodología del registro en 2010, las tendencias en NED se han mantenido, salvo la importancia cuantitativa de la vía ora

    En este trabajo presentamos el proceso llevado a cabo en la innovación docente en dos grupos del Grado de Educación Social (Palencia y Valladolid) durante el primer cuatrimestre. Con este proyecto queremos indagar acerca de las habilidades blandas que consigue nuestro alumnado a través de aprendizajes ubiquos e invisibles, así como en los procesos de construcción de la ciudadanía que realizan. Para ello se elaboró una guía de trabajo en el aula alrededor de la construcción de una tecnobiografía de cada estudiante. Los procesos de trabajo propuestos han sido individuales y colectivos y ha permitido a los estudiantes reflexionar sobre su identidad digital y su relación con las TIC en los dos aspectos focos de nuestra innovación. Los instrumentos de recogida de datos han sido variados: un cuestionario, observaciones, dos plataformas digitales donde se han desarrollado la docencia y los propios productos textuales y audiovisuales que han generado los estudiantes.     Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.
