    Implications of quality deterioration for public foodgrain stock management and consumers in Bangladesh

    In the late 1990s, government policy in Bangladesh shifted in favor of increased public foodgrain stocks, setting official minimum stock targets of 1.0 to 1.2 million tons, as compared to operational targets of about 700 to 800 thousand metric tons in the early 1990s. Because no mechanism for stock rotation involving simultaneous buying and selling grain at a wholesale level exists, higher stock levels with no increase in distribution led to an increase in average age of stocks and problems of stock quality deterioration. This paper extends earlier analyses of stock policy by focusing on a key aspect of stock management in Bangladesh: the economic costs of stock quality deterioration in storage, including the implicit costs to recipients of Public Food Distribution System (PFDS) foodgrain. Using market prices to value procurement and distribution of rice and wheat, consumer and producer subsidies accounted for 57.4 and 20.9 percent, respectively, of net outlay in 2000/01. Implicit losses to rice consumers due to quality deterioration were significant in 2000/01: about 1.05 billion Taka (about 19 million dollars), equal to 10.9 percent of total net outlay on rice of the PFDS. Analysis of the costs and benefits of alternative stock targets based on calculations of the minimum age of stock on a monthly basis indicates that moderate increases in the size of stock (e.g. 200 thousand tons), lead to only small net marginal outlays. However, unless procurement and distribution are also raised, the age and quality of the stock for distribution deteriorates, resulting in significant losses to program recipients.Grain production ,Bangladesh ,Food supply ,

    Food aid has played a useful role in Government of Bangladesh efforts to increase food security in the last three decades, adding to foodgrain availability, supplying wheat for targeted distribution to poor households, and helping to finance development projects and programs. However, sustained increases in domestic production of both rice and wheat have increased the likelihood of disincentive effects arising from continued large inflows of food aid. The analysis shows that if good rice harvests continue so that real rice prices remain at their levels of 2000, and if international wheat prices return to their average 1995-99 levels, then public wheat distribution may need to be cut to levels below the current amount of food aid received (650 thousand tons in 2000/2001) to avoid reducing domestic prices below import parity. However, resources will continue to be required for programs that increase access to food by the poor, contribute to increased utilization of food and result in improved nutritional outcomes, even if the need for food aid to increase availability of foodgrains diminishes.Food relief Bangladesh. ,Food security. ,Development projects. ,Rice Prices Bangladesh. ,Wheat Prices. ,

Salah satu masalah di dalam ormas FPI ini adalah tindakan yang mengandung sifat anarkis tidak berkeperimanusiaan sehingga membuat bangsa ini terkukung oleh sebuah ideologi dari tindakan Front Pembela Islam tersebut,tindakan semacam ini seharusnya berkordinasi dengan tokoh-tokoh keagamaan seperti para kyai, ulama ustaz dan tokoh-tokoh yang berpengaruh terhadap negara Indonesia ini. Di sini penulis ingin mengkritiki dengan menggunakan pisau analis kritik ideologi Jurgen Habermas dengan di dukung oleh salah satu tindakan komunikatif di dalam mengedepankan emansipatoris dan terujudnya masyarakat tanpa paksa. Sistem kerja kritik ideologi Jurgen Habermas ini menggunakan tiga bentuk tindakan yaitu, konstruktif, dekonstruktif, dan destruktif, dengan tiga tindakan ini bisa kita ketahui dimana letak kekurangan dari tindakan Front Pembela Islam.     Development and Evaluation of a Novel Pellet-Based Tablet System for Potential Colon Delivery of Budesonide

    Budesonide, a potent glucocorticoid, is used for the treatment of inflammatory bowel diseases. Current available oral formulations of budesonide have low efficacy against ulcerative colitis because of the premature drug release in the upper part of the gastrointestinal tract. In this paper a pH- and time-controlled colon-targeted pellet-based tablet of budesonide was established. Pellet cores were prepared by extrusion-spheronization method and further coated with xanthan gum (barrier layer), Eudragit NE30D and L30D55 combination (inner layer), and Eudragit FS30 (as enteric layer) sequentially to achieve the required release profile. The coated pellets then compressed into tablets using inert tabletting granules of Cellactose or Pearlitol. Release studies, performed in simulated gastric, intestinal, and colon pH were used in sequence to mimic the gastrointestinal transit. The influence of formulation variables like barrier layer thickness, inner layer composition, and enteric coat thickness on drug release were investigated and the coated pellets that contained 12% weight gain in xanthan gum layer, Eudragit L30D55 and Eudragit NE30D with a ratio of 3 : 7 in inner layer with 30% weight gain and 25% weight gain in Eudragit FS layer were found to protect the drug release in stomach and small intestine and 83.35 ± 2.4 of budesonide was released at 24 h. The drug release from the tablets prepared using 40% Cellactose 80 as tableting excipient was found to be closely similar to that of uncompressed pellets

    Mesoporous silica nanoparticles functionalized with folic acid/methionine for active targeted delivery of docetaxel

    Abstract: Mesoporous silica nanoparticles (MSNs) are known as carriers with high loading capacity and large functionalizable surface area for target-directed delivery. In this study, a series of docetaxel-loaded folic acid- or methionine-functionalized mesoporous silica nanoparticles (DTX/MSN-FA or DTX/MSN-Met) with large pores and amine groups at inner pore surface properties were prepared. The results showed that the MSNs were successfully synthesized, having good pay load and pH-sensitive drug release kinetics. The cellular investigation on MCF-7 cells showed better performance of cytotoxicity and cell apoptosis and an increase in cellular uptake of targeted nanoparticles. In vivo fluorescent imaging on healthy BALB/c mice proved that bare MSN-NH2 are mostly accumulated in the liver but MSN-FA or MSN-Met are more concentrated in the kidney. Importantly, ex vivo fluorescent images of tumor-induced BALB/c mice organs revealed the ability of MSN-FA to reach the tumor tissues. In conclusion, DTX/MSNs exhibited a good anticancer activity and enhanced the possibility of targeted drug delivery for breast cancer

    Strength and weakness of the National Nutrition Improvement Program in Rural and Nomadic Women: findings from a policy triangle framework

    IntroductionNutrition-sensitive agriculture (NSA) is a comprehensive, inter-sectoral approach to improve food security and nutrition. In Iran, “National Nutrition Improvement Program in Rural and Nomadic Women” has been developed and implemented as a NSA program. The main purpose of this study was analysis of this program using the Policy Triangle Framework.MethodsThis was a qualitative policy analysis study, which was conducted retrospectively. The study population included policymakers and executors at macro (Tehran Province) and micro levels (County and village) from the two involved ministries, as well as rural women in Tehran province. The collected data included the program document (N = 210), in-depth semi-structured interviews (N = 40), as well as focus group discussions (N = 8). Data was analyzed using MAXQDA 2010 software with a deductive approach.ResultsThe findings of this study indicated that the underlying factors including social, economic and cultural status, health, structural-environmental and political are associated to the development of the program. The current program is a multi-faceted, in accordance with the existing needs, which provides opportunities to improve nutrition and community health, empower women, strengthen socio-economic status at the micro and macro levels and pave the way for other projects, by connecting the nutrition sector to agriculture. The analysis has also shown that the content of this program requires more consideration in budget and motivational measures. Although developing an NSA program based on inter-sectoral collaboration is a valuable step, it needs to be improved in the areas of sustainability, inter-sectoral collaboration, resources and facilities, monitoring and evaluation, as well as needs assessment.ConclusionThe findings of the present study can be used as evidence by policymakers and planners in redesigning and implementing the program, or developing other NSA programs

    Prediction of sleep quality and insomnia severity by psychological disorders and acute stress among earthquake survivors in Sarpol-e Zahab, Iran, 2017

    Background: Psychological and health outcomes of natural disasters such as earthquakes affect survivors and health services for a long time. In the present study, posttraumatic stress disorders (PTSD), symptoms of psychopathology, sleep quality, and insomnia disorder were investigated among survivors of earthquake occurred at the Western Iran on November 12, 2017. Materials and Methods: This study was conducted on 1031 adult participants from rural and urban areas of Sarpol-e Zahab, a city in Kermanshah Province (Western Iran), who suffered from mental health problems due to the earthquake, a magnitude-7.3 quake, occurred in the Western Iran in 2017. Participants completed the Symptom Checklist 90, Insomnia Severity Index, Pittsburgh Sleep Quality Index, and Self-Rating Scale for PTSD. Data were analyzed using Pearson correlation and multivariate regression analysis by Statistical Package for Social Sciences Software (Version 21). Results: A positive correlation was observed between insomnia severity and all psychopathological symptoms. There was a positive correlation between sleep quality and somatization, obsessive compulsive disorder, depression, and psychoticism. In addition, acute stress was positively correlated with insomnia severity and sleep quality. Conclusion: According to the results of the present study, it is recommended that psychological disorders in earthquake victims be considered to enhance their sleep quality

    The Effect of Group Counselling on Body Image Coping Strategy among Adolescent Girls

    Background Adolescents, pay particular attention to their body image. Dissatisfaction with body image in people can lead to stress. Accordingly, this study aimed to investigate the effectiveness of group counselling on body image and coping strategies among adolescent girls. Materials and Methods This clinical trial study was conducted on 60 adolescent girls in Karaj City, Iran. The samples were selected using a multi stage sampling technique. For the intervention group, four counseling sessions were held weekly and each session lasted 60 to 90 minutes. The control group received an educational body image package at the end. The Multidimensional Body-Self Relations questionnaire and Body Image Coping Strategy Inventory were completed by participants in both groups before and two weeks after the intervention. The collected data were analyzed using the SPSS-19.0 software.   Results The mean score of the positive rational acceptance before the intervention in intervention and control groups were 43.541±2.798 and 41.875±13.146, respectively. These values after the intervention were 62.708±2.484 and 46.972±16.545 in the intervention and control groups, respectively. There was a significant difference between the intervention and control groups in the mean score of body image and the positive rational acceptance two weeks after the completion of the intervention (P = 0.0001). Conclusion The overall results of this study indicated the effectiveness of intervention (Group Counseling) in improving the body image score and increasing the positive strategic skills

    Mapping 123 million neonatal, infant and child deaths between 2000 and 2017

    Since 2000, many countries have achieved considerable success in improving child survival, but localized progress remains unclear. To inform efforts towards United Nations Sustainable Development Goal 3.2—to end preventable child deaths by 2030—we need consistently estimated data at the subnational level regarding child mortality rates and trends. Here we quantified, for the period 2000–2017, the subnational variation in mortality rates and number of deaths of neonates, infants and children under 5 years of age within 99 low- and middle-income countries using a geostatistical survival model. We estimated that 32% of children under 5 in these countries lived in districts that had attained rates of 25 or fewer child deaths per 1,000 live births by 2017, and that 58% of child deaths between 2000 and 2017 in these countries could have been averted in the absence of geographical inequality. This study enables the identification of high-mortality clusters, patterns of progress and geographical inequalities to inform appropriate investments and implementations that will help to improve the health of all populations

    Burden of injury along the development spectrum : associations between the Socio-demographic Index and disability-adjusted life year estimates from the Global Burden of Disease Study 2017

    Background The epidemiological transition of non-communicable diseases replacing infectious diseases as the main contributors to disease burden has been well documented in global health literature. Less focus, however, has been given to the relationship between sociodemographic changes and injury. The aim of this study was to examine the association between disability-adjusted life years (DALYs) from injury for 195 countries and territories at different levels along the development spectrum between 1990 and 2017 based on the Global Burden of Disease (GBD) 2017 estimates. Methods Injury mortality was estimated using the GBD mortality database, corrections for garbage coding and CODEm-the cause of death ensemble modelling tool. Morbidity estimation was based on surveys and inpatient and outpatient data sets for 30 cause-of-injury with 47 nature-of-injury categories each. The Socio-demographic Index (SDI) is a composite indicator that includes lagged income per capita, average educational attainment over age 15 years and total fertility rate. Results For many causes of injury, age-standardised DALY rates declined with increasing SDI, although road injury, interpersonal violence and self-harm did not follow this pattern. Particularly for self-harm opposing patterns were observed in regions with similar SDI levels. For road injuries, this effect was less pronounced. Conclusions The overall global pattern is that of declining injury burden with increasing SDI. However, not all injuries follow this pattern, which suggests multiple underlying mechanisms influencing injury DALYs. There is a need for a detailed understanding of these patterns to help to inform national and global efforts to address injury-related health outcomes across the development spectrum.Peer reviewe
